Opis:
Fungal infections of the ear and paranasal sinus, are recognised quite rarely, but they make sometimes, for many reasons, very important diagnostic-therapy issue. Cantagion may developes near every paranasal sinus. Facilities to invasion give every states, which weaken general and punctual immunity (like immunosupression, chemo-sterydotherapy, blood disease, pregnancy and HIV). After penetrated organism, course of infections, in case of type of patogens, trim of the patient and localization, can have without symptoms, sharp, chronic or fulminant shape. The hardest course with the highest mortality occur in the cases of mucormycosis and aspergillosis. Actually curiosity occur cases of fungal infections, since this times consider to be unpathogenic for humans. For treatment of sinus mycosis in majority chirurgical treatment is required. In addition or the sake of morphological differentiation fungal, in their developing cycle, treatment mycosis recommends serious diffi culty, extra factor, which impeds therapy, is a must of prolongely antifungal treatment and repeated (to total elimination) remove hyphae from sinus, which stay sometimes even for week. Progress of the mycotic infections in paranasal sinus, remain bacterial infections and is often reason of bad diagnosis and incorrect treatment.

Skin mycoses in animals are mostly caused by Microsporum canis, Microsporum persicolor, Microsporum gypseum and in rare cases by Trichophyton mentagrophyres. As a rule only one dermatophyte species is isolated from skin lesions, but at times mixed infection with bacteria occur. In the described case in a cat with typical dermatomycosis, Microsporum canis and Trichophyton mentagrophytes were isolated simultaneously. To the best of our knowledge this is the first case of mixed dermatophyte infection to be descibed in the cat. This paralell infection may suggests, that primary infection of one dermatophyte does not prevent the animal from a secondary infection.

Over the last years, systemic fungal infections have dramatically increased in hospitalized patients. The Candida is the main pathogen caused nosocomial fungal infections. The aim of the study: The aim was to analyze frequency of occurrence of the yeast-like fungi in different biological materials isolated from the patients of an Intensive Care Unit of the University Hospital of Lodz in the period of 2000-2003. Material and methods: 123 strains of fungi were analyzed with the use of API 20 C AUX® test (bioMarieux). Results: Among all the investigated Candida strains C. albicans accounted for 52.0%. Samples from respiratory system and urine most often contained the strains of C. albicans (56.3 and 60.5%, respectively); blood samples contained C. parapsilosis (44.8%). In patients who were untreated by bacterial antibiotics C. albicans was the most frequent species, whereas in patients who were ordered bacterial antibiotics it was C. parapsilosis that dominated. Conclusions: (1) Candida is the most frequent cause of fungal infections in patients hospitalized in an intensive care unit. (2) C. parapsilosis is the main pathogen caused bloodstream infections. This species is also more frequent in patients who were ordered antibacterial antibiotics over five days. (3). Species other than C. albicans become more and more frequent and dangerous.

Prevalence of multifocal fungal infections in patients undergoing permanent immunosuppresion alter renal transplantation. The aim of the study was to estimate the prevalence of fungi in monofocal and multifocal infections in renal transplant recipients. 32 renal transplant recipients were examined for presence of fungi in genital organs, oral cavity and rectum. Fungal strains were recovered in 66 out of 96 samples (68.2% of all renal transplant recipients) in oral cavity (65.6%), rectum (37.5%) and genital organs (25.0%). Monofocal mycosis was found only in 21.9% of the patients. Multifocal infections occurred in 68.2% and contained ontocenoses of oral cavity and rectum (34.4%) the most frequently. Trifocal infection (genital organs - oral cavity - rectum) occurred in 12.5% of all examined renal transplant recipients. The following fungi were found: Candida albicans (31 strains), C. glabrata (5), C. guilliermondii (2), C. krusei (20, Saccharomyces cerevisiae (1).

Nail plate susceptibility 10 Scopulariopsis brevicaulis infection. Experimental Scopulariopsis brevicaulis infection of toe and finger nails collected from 79 persons of both sexes and at various age is presented. Crumbled nail fragments were incubated with a S. brevicaulis strain isolated from toenail acauliosis. The culture was inspected and evaluated under light microscope after seven days of incubalion. Hyphe intensively penetrating nail fragments were found in 67.1% of toenails and 38% of fingernails. The infection was most intensive in the nails collected from males and elderly persons.

For the past twenty years, invasive fungal infections have dramatically increased resulting in the high morbidity and mortality. The main reason of these infections is the development of invasive diagnostic and therapeutic technologies. Immunosuppression, neutropenia, broad-spectrum antibacterial agents, corticosteroids, chemotherapy, central intravascular catheter, total parenteral nutrition are the major risk factors of the invasive fungal infections. Most of these factors are very common for hospitalized patients. For example, patients with fungemia are more likely to die during hospitalization then those with bloodstream infection caused by nonfungal pathogens. Unfortunately, diagnosis and diagnostic methods of fungal infection are still difficult.

The aim of this study was to analyse, taking into consideration the infection risk factors, the incidcnce of fungal infections occurrence in Medical Intensive Care Units. Yeast-like fungi strains isolated from various clinical materials underwent mycological examination. Mycological diagnosis was performed in compliance with compulsory laboratory methods. The detailed observation concerned patients who were admitted to the intensive care unit by urgent reasons or because of basie disease aggravation, trauma, surgical operations and those with diabetes mellitus. The main etiological fungal infections factor were C. albicans strains. The increased incidence of C. glabrata and C. parapsilosis participation in yeast-like fungi infection pathogenesis was observed. The results presented in this study confirm, that intensive care units patient,. for the reason of fungal infections, make the increased risk group.

Mycotic complications of shunt infection in children with primary hydrocephalus. Recently, the incidence of fungal infections in children, including cbildren with shunt-dependent hydrocephalus, has increased. The analysis comprised 8 cbildren treated in the III Clinic of Pediatrics of ICZMP during the period of 12 months (12% of all infectious complications of the shunt system). The clinical picture of fungal infection included Symptoms of shunt dysfunction: febrile conditions, vomiting, distress and loss of appetite. The most common pathogens isolated from the cerebro-spinal fluid were fungi from the Candida species. Mean value of pleocytosis in the cerebro-spinal fluid was 812 cell/μ, and mean protein concentration was 311 mg/dl. Treatment consisted of monotherapy with Dillucan, monotherapy with Ancotil or combined treatment with Ancotil and Amphotericine B. The drugs were administered intravenously and intraventricolarly after removal of the shunt and application of external drainage. Sterility of cerebro-spinal fluid was obtained in the shortest time with the use of Ancotil. Propbylactic application of antifungal drugs decreases the frequency of infections in children with shunt-dependent hydrocepbalus.

Molecular method B and techniques used in diagnosis and epidemiology or infections caused by pathogenic fungi. In this paper we reviewed the latest literature on molecular techniques used in diagnosis and epidemiology of infections caused by pathogenic fungi. Traditional methods used for the identification and typing of medically relevant fungi include morphological and biochemical analysis. These methods are time-consuming and base on phenotypic features what makes them unreliable. We described the usefulness in mycological studies of fast and very sensitive molecujar methods which rely on PCR and hybridization techniques.

Opis:
Molecular method B and techniques used in diagnosis and epidemiology or infections caused by pathogenic fungi. In this paper we reviewed the latest literature on molecular techniques used in diagnosis and epidemiology of infections caused by pathogenic fungi. Traditional methods used for the identification and typing of medically relevant fungi include morphological and biochemical analysis. These methods are time-consuming and base on phenotypic features what makes them unreliable. We described the usefulness in mycological studies of fast and very sensitive molecujar methods which rely on PCR and hybridization techniques.

Opis:
Mycological investigations were carried out in 20 patients due to the delayed wound healing, following orthopaedic surgery at the Department of Orthopaedics, Medical University of Łódź. The material samples for fungal assessment were taken from the wound, surrounding skin and oral cavity. In all patients, the fungi were isolated at least from one ontocenosis; from the wound in 40.0% of subjects studied, from the wound-surrounding skin in 55.0% and from the oral cavity in 85.0%. It should be emphasized that the occurrence of fungi was more frequently observed simultaneously in two or three ontocenoses (70.0%) than in one (30.0%). Unifocal infection was found only in the oral cavity, while the presence of fungi in the wound was accompanied by their appearance in the region of wound-surrounding skin and/or in the oral cavity.

Relationship between activity of hydrolases in fungi isolated from vagina of pregnant women and selected symptoms. Pregnancy is considered a factor of vulvovaginal mycosis. Secretion of hydrolases is an important determinant of Candida virulence. Thus, the aim of the study was to found the relationsbip between activity of 19 hydrolases in fungi isolated from vagina of pregnant women and symptoms of mycosis. 251 pregnant women were examined. Samples were collected from vagina and cultured on Sabouraud media. Activity of hydrolytic enzymes was evaluated using API ZYM (bioMerieux) test. Fungi were found in 20.1% of vaginal samples. Symptoms were detected in 45.8% of wornen. Only 32.1% of women with discharge and 26.4% with pruritus had mycosis. Out of the 19 examined hydrolazes, 13 active enzymes were detected in fungal strains. We found for the first time the relationsbip between activity of fungal esterase lipase and the presence of vaginal discharge in pregnant women.

The aim of this study was to detect fungi in three ontocenoses of 947 women (aged 20-45). The prevalence of multifocal fungal infections in women varied from 62.2 + 2.99% to 81.0 + 3.92% in different ontocenoses. They assumed the following mean values: 38.4 + 1.77% for the vagina, 42.2 + 1.80% for the oral cavity, and 25.0 + 1.58% for the anus. The clinical manifestations of sexual organs inflammations were statistically significantly (p < 0.01) more frequent in women infected with fungi than in those without fungi in the vaginal ontocenosis. The highest convergence of the fungi presence with subjective and objective symptoms (assessed using the Yule Q index value) concerned: pruritus of the vulva or/and vaginal walls, burning sensation or painful urethra, as well as oedema of the external opening of the urethra, vulva or/and the vagina (Q from +0.808 to +0.970). However, there was no statistically significant convergence between occurrence of the fungi and vaginal discharge and painful hypogastrium. Based on more than 40 phenotypical features (morphological, biochemical) of the axenic strains originating from vaginal, oral, and anal ontocenoses, a total of 12 species of fungi were identified: Candida albicans, C. guilliermondii, C. krusei, C. tropicalis, C. kefyr, C. utilis, C. humicola, C. viswanathii, C. glabrata, C. parapsilosis, Rhodotorula mucilaginosa, and Geotrichum candidum. For intraspecific features, depending on the profile of the compounds used for auxanogram, 9 codes were read for Candida albicans and only one code for each of other species was detected. In API ZYM test based on assessment of 19 hydrolase activities the enzymograms of species included from 6 to 11 enzymes.

The aim was looking for correlation between hydrolytic activity and multifocal Candida invasions in 33 lung cancer patients (LC) before treatment. Materials used in this study were pharyngeal and rectal swab, sputum, bronchial secretion and urine. Isolation and identification of fungi were performed using ID C 32 test and the hydrolytic activity using APIZYM test. Results: 1/ there were only Candida strains, especially C. albicans (78,8 %) isolated. 2/ Candida in more than one sample were present in 74%, in three or four in 27,7% of LC. 3/ Candida strains produced 17 among 19 investigated enzymes. 4/ there were statistically significant correlations between the activity of some hydrolytic enzymes and number of Candida foci observed. Conclusions: 1/ the results confirmed the influence of Candida hydrolytic activity on the course of infcction. 2/ hydrolytic enzymes activity is one of important Candida pathogenicity factors.
