Opis:
Ultrasound imaging of the musculoskeletal system is an important element of the diagnostic and therapeutic protocol. Clinical decisions, including those regarding surgical procedures, are often based solely on ultrasound imaging. However, detailed knowledge on the anatomy and a correct scanning technique are crucial for an accurate diagnosis. Modern ultrasonographic equipment allows obtaining detailed anatomical images of muscle tendons, ligaments, nerves and vessels of the carpal area. Ventral wrist ultrasound is one of the most common diagnostic procedures in patients with suspected carpal tunnel syndrome. Ventral wrist evaluation is also often performed in patients with wrist pain of unclear etiology, rheumatic diseases, wrist injuries or symptoms of ulnar neuropathy. The aim of this paper is to present ultrasound images with corresponding anatomical schemes. The technique of ultrasound examination of the ventral wrist along with practical guidance to help obtain highly diagnostic images is also discussed. The present paper is the second part of an article devoted to ultrasound anatomy and wrist ultrasound technique – the part discussing the dorsal side of the wrist was published in the Journal of Ultrasonography, Vol. 15, No 61. The following anatomical structures should be visualized during an ultrasound examination of the ventral wrist, both in the carpal tunnel as well as proximally and distally to it: four flexor digitorum superficialis tendons, four flexor digitorum profundus tendons, flexor pollicis longus, flexor carpi radialis tendon, median nerve and flexor retinaculum; in the carpal tunnel as well as proximally and distally to it: the ulnar nerve, ulnar artery and veins; the tendon of the flexor carpi ulnaris muscle; carpal joints.
Niniejsza praca jest drugą częścią publikacji poświęconej anatomii ultrasonograficznej i technice badania nadgarstka – część o stronie grzbietowej nadgarstka opublikowano w „Journal of Ultrasonography” Vol. 15, No. 61. W trakcie oceny strony dłoniowej nadgarstka należy uwidocznić struktury anatomiczne – w kanale nadgarstka oraz proksymalnie i dystalnie od niego: cztery ścięgna zginacza powierzchownego palców, cztery ścięgna zginacza głębokiego palców, ścięgno zginacza długiego kciuka, ścięgno mięśnia zginacza promieniowego nadgarstka, nerw pośrodkowy i troczek zginaczy; w kanale Guyona proksymalnie i dystalnie od niego: nerw łokciowy, tętnica i żyły łokciowe; ścięgno mięśnia zginacza łokciowego nadgarstka; stawy nadgarstka. Opis:
Objectives: Subchondral carpometacarpal cysts are classic and almost pathognomonic lesions found in workers using vibrating instruments over prolonged periods of time. Materials and Methods: We present the case of a 53-year-old woman who worked for 30 years sewing shoe uppers, a task which required grasping fi rmly a pear-shaped handle awl and pushing it through the leather upper and the sole of the shoe, with combined fl exion and supination movement of the wrist. After approximately 20 years of working, the patient noted gradual onset of paresthesias in the dominant (right) hand, with increasing diffi culty in grasping the awl. Subsequent diagnosis of carpal tunnel syndrome was confi rmed by electrophysiologic testing and its surgical release was performed. Nevertheless, hand pain, paresthesias and weakness persisted. Results: Ultrasound of the snuffbox tendons excluded DeQuervain tenosynovitis. Radiographic imaging of the symptomatic hand showed carpometacarpal subchondral cystic formations. Conclusions: In addition to demonstrating the usefulness of radiographic imaging in patients with persistent hand pain post-carpal release, this case is important in illustrating that repetitive movements with high pressure over the palmar carpal area may cause bone cysts, even if the subjects do not use vibrating tools.

ObjectivesThis study aimed to investigate the prevalence of musculoskeletal disorders in the hands and wrists of fishermen working as oyster shuckers, and to identify the risk factors associated with oyster shucking. Material and Methods A total of 590 fishery workers including those working as oyster shuckers in 2015–2017 were included in the study analysis. Hand osteoarthritis (OA), carpal tunnel syndrome (CTS), and ulnar impaction syndrome were diagnosed using X-ray, magnetic resonance imaging, and physical examinations according to each diagnostic criterion. The Rapid Entire Body Assessment was used as a measurement tool for analyzing the occupation-related burden factors.ResultsThe prevalence of hand OA, CTS, and ulnar impaction syndrome was higher in oyster shuckers than in aquaculture/offshore fishery workers. The prevalence of upper limb disease was higher in oyster shuckers than in other fishery workers, even in the prevalence of exposure to occupational burden factors. In oyster shuckers, the incidence of hand and wrist OA was 84%, that of CTS was 72%, and that of ulnar impaction syndrome was 41%. In addition, oyster shuckers were more likely to be exposed to occupational burden factors than were other types of fishermen. Conclusions The prevalence of hand OA, CTS, and ulnar impaction syndrome was higher in oyster shuckers. Therefore, proper stretching or exercises for rest and musculoskeletal relaxation, along with health-care education and promotion, will be needed for the fishermen’s repeated posture and impact work.

The effects of Ergorest arm supports on wrist angles and musculoskeletal strain in the neck-shoulder-arm region and electrical activity in the shoulder and arm muscles were studied during typing or the use of the mouse in work with a visual display unit (VDU). Twenty-one women were randomized into 3 groups (1 arm support, 2 arm supports, and control). Measurements were carried out before and after the 6-week intervention. The wrist extension of the mouse hand, the muscle activity of the trapezius muscle, and the subjective discomfort ratings indicated that 2 arm supports were better than 1 in work with a mouse. The Ergorest arm support alleviates muscle and joint strain in VDU work when used for both arms.

Opis:
This study determined flexion and extension angles of resting fingers and wrist in terms of forearm posture (neutral, pronation and supination) and shoulder flexion (0°, 45°, 90° and 135°). The participants participated in 12 angle measurements for 16 finger joints and wrist. The finger joints flexed more in supination than in neutral posture and pronation and the thumb flexed more than the other fingers because of the gravity and skin tension. This phenomenon became more apparent as the shoulder flexed. The carpometacarpal joint had the largest flexion angle in the thumb joints, whereas the proximal interphalangeal joints had the largest flexion angles in the other finger joints. The resting posture of the wrist extended of ~16° in any forearm postures when the shoulder was at 0°. The results of this study could be useful for rehabilitation tool and product designs.

rehabilitation treatment after surgical removal of the ganglion of the wrist
Opis:
The aim of the study was to evaluate the effect of rehabilitation treatment to improve the treatment results of patients operated for ganglions of the wrist. We studied the hands of 77 patients with ganglions of the wrist. 43 women and 34 men aged between 16 and 98 years participated in the study. The open method was used in the surgical treatment to remove the wrist ganglions in all the patients. In the post-operative treatment 46 patients (group I) underwent hand rehabilitation treatment under the supervision of the authors and 31 patients (group II) did not undergo such a treatment. The examination was carried out on all the patients before surgery and at weeks 1 and 4 after surgery. The patients with ganglions of the wrist and pain, impaired hand mobility and impaired hand efficiency were qualified for the surgical treatment. In the rehabilitation treatment stretching exercises were performed as well as loosening and stabilizing the hand and teaching the patients the correct positioning of the upper limb during work and physical activity. The results were statistically analyzed using non-parametric tests such as the chi-square test, signs test and Mann-Whitney test. It was found that in patients after surgical removal of the wrist ganglion rehabilitation treatment has led to the resolution of pain syndromes, movement disorders and efficiency of hand.

ObjectivesHairstylists form an occupational group whose tasks involve repetitive and forced movements of hands and wrists, thus posing a risk of developing carpal tunnel syndrome (CTS). This study assessed the prevalence of and factors associated with CTS symptoms among hairstylists in Gaborone, Botswana.Material and MethodsA cross-sectional study was conducted using a self-administered questionnaire distributed among randomly selected hairstylists. The questionnaire gathered information on demographic characteristics, lifestyle, work-related characteristics and psychosocial factors. The Boston Carpal Tunnel Syndrome Questionnaire was used to determine the severity of CTS symptoms and its functional effects. Data were then analyzed using χ2 and logistic regression models. The level of significance was determined at p < 0.05.ResultsA total of 165 hairstylists took part in the study, with 92 (56%) of the respondents being females. The mean age (M±SD) of the respondents was 35.05±7.54 years with an age range of 22–63 years. Seventy-three (42.2%) hairstylists reported CTS symptoms, with the majority (73%) being females. Out of all the CTS cases, 53 (72.6%) and 16 (21.9%) had mild and moderate symptoms, respectively. Over 80% of the hairstylists did not know about CTS. Among individual factors, CTS symptoms were associated with being female (the odds ratio [OR] of 9.99, and the 95% confidence interval [CI]: 3.64–27.44), increasing age (OR 9.84, 95% CI: 2.74–35.36), the length of employment (OR 3.73, 95% CI: 1.39–9.95), hair washing (OR 2.88, 95% CI: 1.41–5.85), an awkward posture (OR 2.52, 95% CI: 1.03–6.19), and the use of a great muscular effort when performing a task (OR 2.39, 95% CI: 1.01–5.72). Perceived heavy workload and stressful work were also risk factors.ConclusionsThe results suggest a high prevalence of CTS among female hairstylists in Gaborone, and also point out that individual, work-related and psychosocial factors are associated with this syndrome. Future large-scale research is needed to establish the extent of CTS countrywide to influence policy-making. Currently, CTS is not listed amongst occupational health diseases in Botswana.

Opis:
Ultrasound imaging of the musculoskeletal system is superior to other imaging methods in many aspects, such as multidimensional character of imaging, possibility of dynamic evaluation and precise assessment of soft tissues. Moreover, it is a safe and relatively inexpensive method, broadly available and well-tolerated by patients. A correctly conducted ultrasound examination of the wrist delivers detailed information concerning the condition of tendons, muscles, ligaments, nerves and vessels. However, the knowledge of anatomy is crucial to establish a correct ultrasound diagnosis, also in wrist assessment. An ultrasound examination of the wrist is one of the most common US examinations conducted in patients with rheumatological diseases. Ultrasonographic signs depend on the advancement of the disease. The examination is equally frequently conducted in patients with pain or swelling of the wrist due to non-rheumatological causes. The aim of this publication was to present ultrasound images and anatomic schemes corresponding to them. The correct scanning technique of the dorsal part of the wrist was discussed and some practical tips, thanks to which highly diagnostic images can be obtained, were presented. The following anatomical structures should be visualized in an ultrasound examination of the dorsal wrist: distal radio-ulnar joint, radiocarpal joint, midcarpal joint, carpometacarpal joints, dorsal radiocarpal ligament, compartments of extensor tendons, radial artery, cephalic vein, two small branches of the radial nerve: superficial and deep, as well as certain midcarpal ligaments, particularly the scapholunate ligament and lunotriquetral ligament. The paper was distinguished in 2014 as the “poster of the month” (poster number C-1896) during the poster session of the European Congress of Radiology in Vienna.
Poprawnie wykonane badanie ultrasonograficzne nadgarstka dostarcza szczegółowych informacji o stanie ścięgien mięśni, więzadeł, nerwów i naczyń. W trakcie badania grzbietowej strony nadgarstka należy uwidocznić nas tępujące struktury anatomiczne: staw promieniowo-łokciowy dalszy, staw promieniowo-nadgarstkowy, staw śródnadgarstkowy, stawy śródręczno-nadgarstkowe, więzadło promieniowo-nadgarstkowe grzbietowe, przedziały ścięgien mięśni prostowników, tętnica promieniowa, żyła odpromieniowa oraz dwie małe gałązki nerwu promieniowego: powierzchowna i głęboka, niektóre więzadła śródnadgarstkowe, zwłaszcza więzadło łódeczkowato-księżycowate oraz więzadło księżycowato-trójgraniaste. Opis:
There has been an amplified focus on and benefit from the adoption of artificial intelligence (AI) in medical imaging applications. However, deep learning approaches involve training with massive amounts of annotated data in order to guarantee generalization and achieve high accuracies. Gathering and annotating large sets of training images require expertise which is both expensive and time-consuming, especially in the medical field. Furthermore, in health care systems where mistakes can have catastrophic consequences, there is a general mistrust in the black-box aspect of AI models. In this work, we focus on improving the performance of medical imaging applications when limited data is available while focusing on the interpretability aspect of the proposed AI model. This is achieved by employing a novel transfer learning framework, progressive transfer learning, an automated annotation technique and a correlation analysis experiment on the learned representations. Progressive transfer learning helps jump-start the training of deep neural networks while improving the performance by gradually transferring knowledge from two source tasks into the target task. It is empirically tested on the wrist fracture detection application by first training a general radiology network RadiNet and using its weights to initialize RadiNetwrist, that is trained on wrist images to detect fractures. Experiments show that RadiNetwrist achieves an accuracy of 87% and an AUC ROC of 94% as opposed to 83% and 92% when it is pre-trained on the ImageNet dataset. This improvement in performance is investigated within an explainable AI framework. More concretely, the learned deep representations of RadiNetwrist are compared to those learned by the baseline model by conducting a correlation analysis experiment. The results show that, when transfer learning is gradually applied, some features are learned earlier in the network. Moreover, the deep layers in the progressive transfer learning framework are shown to encode features that are not encountered when traditional transfer learning techniques are applied. In addition to the empirical results, a clinical study is conducted and the performance of RadiNetwrist is compared to that of an expert radiologist. We found that RadiNetwrist exhibited similar performance to that of radiologists with more than 20 years of experience. This motivates follow-up research to train on more data to feasibly surpass radiologists’ performance, and investigate the interpretability of AI models in the healthcare domain where the decision-making process needs to be credible and transparent.

This paper gives the kinematic analysis of a 5-DOF industrial robotic manipulator while considering wrist in motion. Analytical solutions have been obtained for forward kinematics and inverse kinematics to accurately position the end-effector of robotic manipulator in three dimensional spaces. For the first time, a hybrid neuro-fuzzy intelligent technique with two different membership functions has been studied and their performances are comparatively evaluated with analytical solutions. An experiment has been performed for a desired trajectory. It is seen that the results for the intelligent technique are reasonably in agreement with experiment. Also, the results obtained highlight the importance of selection of a particular membership function for robotic manipulators of industrial use.

Złamania dystalnej części kości promieniowej należą do jednych z najczęstszych urazów kości. Materiały i metody: Analizie retrospektywnej poddano 208 losowo wybranych radiogramów stawu nadgarstkowego wykonanych w Zakładzie Radiologii Uniwersyteckiego Szpitala Klinicznego w Białymstoku aparatem RTG Axiom Aristos firmy Siemens.
Distal part radial bone fractures are one of the most common types of trauma. The basis of imaging examination of the injury is radiography. The advantages of the X-ray examination are low costs and widespread availability. Standard projections for the wrist are posteroanterior and lateral views. However, there are many ancillary views like a radiograms obtained with radial or ulnar deviation and carpal tunel. Aim: The aim of the study is retrospective analysis of dose area product and exposure index during radiography of the wrist. The analysis included value collimation size and exposure para¬meters depending on whether plaster immobilization was used or not. Materials and methods: We conduct retrospective analysis of randomly selected 208 radiographs of the wrist. X-ray photos were taken of patients at the Department of Radiology, Medi¬cal University Hospital in Bialystok, with Axiom Aristos Siemens. The analysis included value of the dose area product, exposure index, tube potential, tube current-time product and collima¬tion field. Statistical analysis was done using Statistica 12.5 and MS Excel 2013. Results: The most frequently radiographs were taken for wo¬men which average age was 63,79 while average age for men was 47,56. The average size of X-ray was 324,17 cm2. Assuming that the reference value of the collimation field is 234 cm2 (160x160), 160 (72.92%) images had collimation field greater than reference value. On the other hand when reference va¬lue is 432 cm2 (18 x 24 cm) only 35 (16.83%) of the radiographs were greater. The average exposure index value was 645,87, while the maximum value was 2980,00 and minimum value was 141,00. The average DAP in study group was 0,02 Gy∙cm2, SD = 0,01 Gy∙cm2. The largest recorded DAP was 0,07 Gy∙cm2, while the smallest was 0,01 Gy∙cm2. Statistic tests showed a signifi¬cant dependence between collimation field size value and DAP (rs = 0 ,78), c ollimation fi eld s ize v alue a nd E XI ( rs = 0 ,19); D AP (p = 0,00) and EXI (p = 0,00), and presence of plaster cast. Conclusions: In digital radiology (Siemens), plaster cast have a si¬gnificant influence on DAP, increasing it 0,01 Gy∙cm2 on average and EXI reduced its value almost by half. The increased surface area of the radiographs causes increased DAP and EXI.

Opis:
Wrist stabilization is ensured by the complex of stabilizers, both static (intrinsic and extrinsic ligaments) and dynamic ones (flexor and extensor sinews of the wrist). Extrinsic carpal ligaments link both carpal rows with forearm, while the intrinsic ones connect individual carpal bones. From among intrinsic ligaments, the most significant from clinical point of view is scapholunate ligament (scapholunate lig. - SL). Instability of the SL resulting from SL ligament tear is the most frequent type of carpal instability. If untreated or undiagnosed, it may lead to destabilization of the carpal rows, and in the final stage to massive degenerative changes with SLAC (scapholunate advanced collapse). The aim of the study was to present a hypothesis of a possible influence of radial extensors of the wrist as dynamic stabilizers for SL complex, based on which it is possible to discern the partial ligament tear with accompanying damage of static stabilizers of the complex. Material and methods. This hypothesis was based on radiological tests carried out on 5 patients, with acute partial SL ligament tear, which was confirmed during wrist arthroscopy. The outcome was presented as well, with its visualization in radiological tests. Results. The picture of rotary subluxation of the scaphoid -signet ring sign in PA view and strengthening in stress position (clenched fist) was demonstrated in all 5 patients. In 2 patients who had wrist arthroscopy performed it was observed that the palmar and proximal part of SL ligament were injured, yet no damage of dorsal part was reported. In MRI tests of these patients, an additional injury of dorsal intercarpal ligament was revealed. Conclusions. Correction of the scaphoid reduction in position of clenched fist may result from influence of radial extensors of the wrist (both short and long) as dynamic stabilizers of SL complex. This imagining is a completion of the radiological symptoms so far occurring in this SL instability. In case of acute, partial scapholunate ligament tear, reduction of scapholunate subluxation and its internal fixation with the lunate and capitate for a 6-week period may be an optional procedure

Opis:
Objectives Association between carpal tunnel syndrome (CTS) and ulnar nerve entrapment at wrist remains controversial. The aim of the study has been to investigate the prevalence of Guyon’s canal syndrome amongst patients diagnosed with the CTS, occupationally exposed to repetitive wrist movements. Material and Methods The retrospective analysis of 310 patients (268 females, 42 males) representing the mean age of 52±7 years old hospitalized for the suspected occupational CTS was performed. Results In the analyzed cohort, 4 patients had undergone decompression of the Guyon’s canal in the right limbs. Nerve conduction studies (NCS) in the ulnar nerves performed during the hospitalization of those patients did not show any abnormalities. Nerve conduction studies revealed signs of the ulnar neuropathy (UN) at the wrist affecting exclusively sensory fibers for 6 patients. Only those 4 patients who had undergone the operation suffered from clinical symptoms of the UN before the surgery. In the case of the remaining patients, despite the NCS changes, signs suggestive of the UN at the wrist were not detected. In the case of the patients with the occupational CTS, no signs of the ulnar nerve dysfunction were recorded. Conclusions The frequency of ulnar nerve entrapment at the wrist among patients with the CTS is lower than that already reported. The low prevalence of ulnar involvement (3.2%) for the CTS patients in our study may be related to the relatively small number of the CTS hands with the severe changes in the NCS and/or other personal factor including anatomical variation of the Guyon’s canal borders and its contents.
