Opis:
The aim of this study was to present the current situation of trichinellosis, basing on the results of examination of the tissues of animals hunted in Poland. Investigation was carried out from 1st January to 30th June 2022 in the Department of Parasitology and Parasitic Diseases of National Veterinary Research Institute in Poland. Samples were collected out of 103 wild boars (Sus scrofa) and one wolf (Canis lupus), originating from 14 voivodeships. The study of wild boars muscle tissues revealed 89 positive samples, including 60 T. spiralis, 21 T. britovi, two T. pseudospiralis, and five mixed invasions with T. spiralis and T. britovi. In one case no amplification products were obtained. T. britovi was also found in the sample of wolf muscle tissue. This study emphasize the fact that trichinellosis in hunting animals is still a zoonotic threat, and should be closely monitored.

Opis:
Research into changes in the activity and kinetics of the reaction catalysed by the important detoxificating enzyme, glutathione-S-transferase (GST), in the intestinal phase of trichinellosis as presented in the present study is a pioneering achievement. In every instance of infection – or if infection is only suspected – the patient receives anthelmintics that are effective against mature intestinal forms, newborn larvae and encysted larvae of Trichinella spiralis. This group of drugs includes benzimidazoles such as albendazole. The primary action of these drugs concerns β-tubuline. The functions of microtubules are damaged by bonding β-tubuline with benzimidazoles, which inhibits the polymerisation of microtubules. Newly considered fields of action of benzimidazoles on the pathological changes occurring in the host during the course of parasitic diseases are the following elements taking part in the immunological defence against a parasitic infection: for example, detoxificating enzymes such as glutathione-Stransferase (GST), and Toll-like receptors (TLRs). Occurrence of cell resistance to drugs is attributed to certain enzymes, (such as GST), which perform the detoxification of these drugs and facilitate their removal from cells through a pump dependent on compounds coupled with glutathione. In the case of the intestinal phase of trichinellosis, GST may remove toxins secreted by both the adult and newborn larvae of T. spiralis. The structure of a dimeric enzyme of the host, such as GST, may be impacted by various factors, for example excretory-secretory products (ES), as well as drugs used. Over the past few years, the interest of researchers in the innate immune response as the first line of defense of vertebrates against of pathogens, was increased. The number of works devoted to the role of Toll-like receptors, which enable the immediate commencement of protective action by the host against various infections without engaging lymphocytes, is on the noticeable. The experimental material consisted of small and large intestine from BALB/c mice infected with T. spiralis strain MSUS/PO/60/ISS3. Mice were inoculated orally with about 400 T. spiralis larvae per mouse. On the 2nd day post-infection, the mice were treated with albendazole, in a dose of 0.5 mg/mouse. The activity and kinetics of the GST were determined spectrophotometrically. An evaluation of the expression of TLRs (TLR2 and TLR4 on the mRNA level) was examined using the RT-PCR technique with the appropriate starter sequences for TLR2 and TLR4. The presence of Toll-like receptors and their expression on the protein level was confirmed using the immunohistochemical method (Santa Cruz ABC Stainig system sc-2023 test) with specific polyclonal antibodies antiTLR2 and anti-TLR4. My results pointed to the participation of GST, as well as Toll-like receptors, in defence processes of the host during the intestinal phase of trichinellosis in mice. The adult and newborn larvae of T. spiralis present in the intestines of infected mice are a source of excretory-secretory (ES) products, which appear to stimulate the activity and change catalytic properties of conformers GST. It is supposed that ES products secreted by the parasite acts on GST as an allosteric activator. Additionally, the presence of Toll-like receptors (TLR2 and TLR4) in the intestinal phase of trichinellosis was investigated. Thus, the infection with T. spiralis leads to strong biochemical reactions with the involvement of GST, and immunological reactions with the involvement of TLR2 and TLR4 receptors. Administration of albendazole, resulted in 2-fold decrease of the total activity of GST in comparison with its activity in infected and untreated animals. Also, drug administration had weakened expression of gene of TLR4 and TLR2 in the large intestine, and expression of gene of TLR2 in the small intestine. My results confirmed destructively action of albendazole on T. spiralis (resulted in 4-fold decrease in the intensity of infection measured in muscles). The destruction by the drug of a adult and newborn larvae of T. spiralis lowers the quantities of their ES products stimulated the activation of the host’s response. This is apparent as weakening the innate immune response, in which take part Toll-like receptors play a role, and in biochemical processes with the participation of the GST enzyme. In summary my results contribute new data on the participation of Toll-like receptors and GST in the defence mechanisms of the host during in the intestinal phase of experimental trichinellosis in mice.

This paper describes the past situation and analyses current changes in the epidemiological pattern of trichinellosis in Poland. Epidemiological data from the last 60 years, point out that the number of human cases as well as the number of deaths caused by trichinellosis has decreased significantly. Up to 90s the main source of Trichinella infection for people was pork. Among other implemented control measures, the introduction of the artificial digestion method in the early 80s to detect trichinellosis in pigs resulted in a shift in the sources of Trichinella infection in humans - pork was replaced with wild boar meat. In the years 1990-1995 the number of outbreaks due to pork consumption was 3.5-times higher than in the years 2000-2005. In the early nineties pork was the source of infection causing about 71% of all outbreaks; in 2000-2005 that number has fallen to only 12%. On the other hand wild boar meat was responsible for 23% of the outbreaks in 1990-1995 and as many as 88% of all outbreaks in the years 2000-2005. Moreover the number of persons infected in the outbreaks significantly decreased. The study of wild animals demonstrated that wild boars in Poland are infected not only with T. spiralis but also with Trichinella britovi. These results and EU recommendations indicate a requirement of determining the Trichinella species which cause infections in outbreaks. In the 3 trichinellosis outbreaks in 2005 the infected meat products were examined with molecular tools. T. spiralis species larvae were the etiological agents of infection in all these outbreaks. The current epidemiological situation of trichinellosis in Poland indicates a need of increasing the awareness of risks related to wild boar meat consumption among the general public. Introducing the artificial digestion method as an obligatory method for wild boar meat examination is also necessary.

The main purpose of this article was to analyse the effectiveness of two methods: trichinoscopy and digestion method for control of animal trichinellosis. It was established that the using of magnetic stirrer improved the digestion of meat and shortened the digestion to 90-100 minutes. It is worthy of notice that the digestion method is three to four times more sensitive than the traditional trichinoscopy. In the years 1999-2004 Trichinella larvae were found in pork meat 6 times rarely comparing with the years 1990-1994. The introduction of digestion method resulted directly in the reduction (4 times) of the Trichinella infection in humans in the years 2000-2004 comparing with the years 1990-1994. The ELISA test appeared to be also very useful for epidemiological studies on Trichinella infection in animals and humans.

Opis:
ELISA can be used to measure produced antibodies or Trichinella spp. antigens in the samples. They are detected with antibodies linked to an enzyme that reacts with a substrate and generate a colour reaction. The optical density (OD) of the reaction is measured spectrophotometrically. ELISA assays can be done in several different procedures called „direct”, "indirect", "sandwich", and "competition" ELISA. Since the 1970s, the studies have been done on improving or replacing direct methods of Trichinella diagnosis with serological methods based on the ELISA. When somatic antigens of L1 T. spiralis were used, the specificity of the ELISA was poor due to a high probability of cross-reactions with other pathogens. During the 1980s the specificity of the ELISA was improved by excretory-secretory (E/S) antigens obtained during Trichinella muscle larvae incubation in vitro. Recently a synthetic glycan antigen has been developed and the increasing of ELISA specificity and sensitivity was noticed. The sensitivity of the ELISA using an E/S antigen ranging from 93.1 to 99.2% but the specificity from 90.6 to 99.4%. The ELISA method is relatively simple to apply, reliable, readily standardized and provides an acceptable balance of sensitivity and specificity. But all modified procedures should be validated. In Poland, the studies on the usefulness of ELISA for antibodies detection against T. spiralis in pigs and wild animals are limited. Own ELISA procedure was prepared in Pathophysiology Lab. in W. Stefański Institute of Parasitology of PAS. ELISA was used to examine IgG level against L1 T. spiralis in pigs and wild boars serum samples. Of 1474 pig samples, only 12 were positive. Of 1880 wild boar samples only 14 were positive. The results of this study are comparable with performance obtained using commercial sets. The results showed the usefulness of ELISA for T. spiralis diagnosis in pigs and wild boars and confirmed the possibility of use the ELISA test for application in the slaughterhouse.

Since the 1970s, the studies have been done on improving or replacing direct methods of Trichinella diagnosis with serological methods based on the ELISA. When somatic antigens of L1 T. spiralis were used, the specificity of the ELISA was poor due to a high probability of cross-reactions with other pathogens. During the 1980s the specificity of the ELISA was improved by excretory-secretory (E/S) antigens obtained during Trichinella muscle larvae incubation in vitro. Recently a synthetic glycan antigen has been developed and the increasing of ELISA specificity and sensitivity was noticed. The sensitivity of the ELISA using an E/S antigen ranging from 93.1 to 99.2% but the specificity from 90.6 to 99.4%. The ELISA method is relatively simple to apply, reliable, readily standardized and provides an acceptable balance of sensitivity and specificity. But all modified procedures should be validated. In Poland, the studies on the usefulness of ELISA for antibodies detection against T. spiralis in pigs and wild animals are limited. Own ELISA procedure was prepared in Pathophysiology Lab. in W. Stefański Institute of Parasitology of PAS. ELISA was used to examine IgG level against L1 T. spiralis in pigs and wild boars serum samples. Of 1474 pig samples, only 12 were positive. Of 1880 wild boar samples only 14 were positive. The results of this study are comparable with performance obtained using commercial sets. The results showed the usefulness of ELISA for T. spiralis diagnosis in pigs and wild boars and confirmed the possibility of use the ELISA test for application in the slaughterhouse.

In this review the pathomechanisms of human trichinellosis with particular relation to intestinal and muscular invasion are presented. The numerous factors initiated by antigen released by Trichinella, which play a role in the development of pathological process (including immunological, pathomorphological, metabolic and bioelectric disturbances) and short clinical characteristics of the disease are also presented. Pathology of late period of trichninellosis sequelae are discussed.
