Aim An ileal pouch anal anastomosis (IPAA) leak is one of the most severe complication after restorative proctocoletomy (RPC). We present a rare case of a successful management of IPAA leak after RPC without defunctionig stoma with the utility of endoscopic vacuum therapy. Methods A 57-year-old male with a ileal pouch anal anastomosis leak after RPC due to ulcerative colitis with presacral abscess was qualified for endoscopic vacuum therapy (EVT). The abscess of the left buttock was drained and secured with suction drain (redon drain). Due to the lack of defunctioning stoma, a system for contain and divert fecal matter was placed within afferent limb of the J-pouch and EVT was placed directly within IPAA dehiscence. EVT was changed every third day. Results The patient underwent a total of five EVT sessions. Improvement of patient’s general condition characterized with lack of pelvic pain, fever and reduction of inflammatory markers was achieved. Locally, anastomosis dehiscence was healed with prominent reduction in the defect’s dimension, contraction and revascularization. Based on imaging studies no chronic presacral sinus or any other perianal disturbances were revealed at the time of five months follow up. Conclusions EVT is a promising method for management of IPAA leak. Although, it remains extremely difficult, EVT may serve as a method of choice in early pouch-related septic complications after RPC performed without defunctioning stoma

Opis:
Cupping therapy belongs to methods of natural medicine. Cups have been used since ancient times, with the first mentions of cups coming from excavations in Mesopotamia dating to around 3300 BC. The primary mechanism of cup action is related to blood function, known as autohemotherapy. The aim of this work was to present the different types of medical cups used today, techniques for placing them on the skin and their mechanisms of action for various medical conditions as well as in regenerative sports medicine and cosmetology. Methods of placing cups on the skin depend on the type of cup used, the purpose for which they are being used for and the experience level of the therapist. Cupping is a safe form of therapy support, provided that the treatments are performed according to a strictly established procedure and take into account contraindications. This work is a review based on a query of the literature on the subject, as well as the experience and results of research studies conducted by the authors. An important component of the work involves the demonstration of the use of cups as part of physiotherapeutic procedures and regimens using manual therapy (mobilization) and acupuncture points. The discussion presents a review of publications that highlight the therapeutic effects of cup use for various pathological conditions. The therapeutic effects of medical cups largely depend on several factors, mainly vacuum pressure, but also skin temperature under the cup and other forms of energy/indicators, which depend on the specific elements or components involved in the cupping procedure (i.e. heat, acupuncture needles).
Terapia z wykorzystaniem baniek należy do metod medycyny naturalnej. Bańki wykorzy- stywane były już w starożytności, pierwsze wzmianki o bańkach pochodzą z wykopalisk od- krytych w Mezopotamii ok. 3300 roku p.n.e. Główny mechanizm działania baniek związany jest z funkcją krwi – autohemoterapią. Celem pracy było przedstawienie wykorzystywanych współcześnie typów baniek lekarskich, technik ich stawiania oraz mechanizmów działania w odniesieniu do różnych stanów chorobowych, a także w odnowie biologicznej sportowców i kosmetologii. Sposoby stawiania baniek zależą od ich typu, przeznaczenia i doświadczenia terapeuty. Stawianie baniek jest bezpieczną formą wspomagania terapii pod warunkiem, że za- biegi są wykonywane według ściśle ustalonej procedury i z uwzględnieniem przeciwwskazań. Praca ma charakter przeglądu narracyjnego dokonanego na podstawie kwerendy światowej literatury przedmiotu oraz doświadczeń i rezultatów eksperymentów badawczych przepro- wadzonych przez autorów pracy. Ważną składową pracy jest ukazanie zastosowania baniek w algorytmie postępowania fizjoterapeutycznego z wykorzystaniem środków terapii manualnej (mobilizacji) i punktów akupunkturowych. W dyskusji dokonano przeglądu publikacji dotyczących efektów terapeutycznych stosowania baniek w różnych stanach patologicznych organizmu. Można stwierdzić, że efekty terapeutyczne bańki lekarskiej zależne są od kilku czynników, głównie podciśnienia, ale także od temperatury skóry pod bańką i innych postaci energii w zależności od zastosowanego czynnika fizycznego (np. ciepło, igły akupunkturowe).

Multiple therapeutic approaches of the treatment of pilonidal sinuses have been described in the literature, but there are still controversies and lack of standardization. Vacuum Assisted Closure (VAC) therapy has potential beneficial effect on the wound healing after the sinus resection. The aim of the study was to analyze the results of VAC therapy in the treatment of pilonidal sinuses. Material and methods. After randomization in the control group (9 men) the simple excision of the pilonidal cyst was performed with the standard wound dressing. In the VAC group (10 men) the same surgical procedure was performed, but after the excision the VAC dressing with mobile VAC Freedom device was used. Both groups were treated in an outpatient setting under local anesthesia. The wound size, time of surgery, time of wound healing time of recovery and pain after the surgery (VAS score) were compared. Results. In VAC treated group the wound size and time of surgery were similar to control group. Time of wound healing, recovery and the pain after surgery in days 4-7 were reduced in comparison to the standard treated group. Conclusions. VAC therapy can be easily used in an outpatient setting, mobile device is highly accepted, operation of the equipment is simple. VAC therapy significantly decreases the time of wound healing and absenteeism from work as well as the postoperative late pain.

Opis:
Aim: To investigate the effects of vacuum-assisted closure (VAC) vs. relaparotomy on demand (ROD) onto the i) severity and course of disease, ii) surgical outcome, iii) intraperitoneal bacterial load as well as iv) local/systemic inflammatory and immune response in postoperative secondary peritonitis. Methods: Over a defined time period, all consecutive patients of the reporting surgical department with a secondary peritonitis (assessed by Mannheim’s Peritonitis Index [MPI] and APPACHE II score) were enrolled in this systematic unicenter clinical prospective observational pilot study. Patients were subclassified into VAC or ROD group according to surgeon’s individual decision at the time point of primary surgical intervention with the intent to sanitize the source of infection. Early postoperative result was assessed by 30-d and in-hospital mortality. Bacterial load was characterized by microbiological culture of intraperitoneal fluid collection obtained on postoperative days (POD) 0 (primary surgical intervention), 1,4,7,10,13 and following description of the microbial spectrum including semiquantitative assessment of bacterial load. Local/systemic inflammatory and immune response was determined by ELISA-based analysis of CrP, PCT and the representative cytokines such as TNF-α/IL-1α/IL-6/IL-8/IL-10 of serum and peritoneal fluid samples. Results: Over a 26-month investigation period, 18 patients (male:female=9:9) were eligible for study criteria: n=8 were enrolled in the VAC and n=10 in the ROD group. With regard to early postoperative results represented by mortality, there is no significant difference between both patient groups. Despite the relatively low number of cases enrolled, a tendency for more severe findings associated with the VAC group could be detected based on the MPI score. There was also a tendency for higher APACHE II scores in the VAC group from the 7th POD on and, in addition, patients of this group had a longer hospital stay. For patients with persisting infection, there were no relevant differences comparing VAC therapy and ROD. Cytokines released in particular at the beginning of the inflammation cascade with proinflammatory characteristics showed higher values within the peritoneal fluid whereas CrP and PCT were found to be higher within the serum samples. Conclusion: Comparing data of various local/systemic inflammatory and immune parameters, there were only a few correlations. This may indicate compartmentation of the inflammatory process within the abdominal cavity. Based on the observed inter-individual variation of this pilot study data, the clinically applicable benefit appears questionable. In this context, reliable effects of VAC therapy on the reduction of bacterial burden within the abdominal cavity could not clearly be detected.
