Objectives: The aim of this study was to assess the exposure of patients to organic substances produced and identified in surgical smoke formed in the abdominal cavity during laparoscopic cholecystectomy. Material and Methods: Identification of these substances in surgical smoke was performed by the use of gas chromatography-mass spectrometry (GC-MS) with selective ion monitoring (SIM). The selected biomarkers of exposure to surgical smoke included benzene, toluene, ethylbenzene and xylene. Their concentrations in the urine samples collected from each patient before and after the surgery were determined by SPME-GC/MS. Results: Qualitative analysis of the smoke produced during laparoscopic procedures revealed the presence of a wide variety of potentially toxic chemicals such as benzene, toluene, xylene, dioxins and other substances. The average concentrations of benzene and toluene in the urine of the patients who underwent laparoscopic cholecystectomy, in contrast to the other determined compounds, were significantly higher after the surgery than before it, which indicates that they were absorbed. Conclusions: The source of the compounds produced in the abdominal cavity during the surgery is tissue pyrolysis in the presence of carbon dioxide atmosphere. All patients undergoing laparoscopic procedures are at risk of absorbing and excreting smoke by-products. Exposure of the patient to emerging chemical compounds is usually a one-time and short-term incident, yet concentrations of benzene and toluene found in the urine were significantly higher after the surgery than before it.

The aim of the study was to examine the possibility of determination and optimization of methodology for analysis of three selected drugs: trimethoprim, sulfamethoxazole and diclofenac in urine using high-performance liquid chromatography, without extraction of the pharmaceuticals to the solid phase. The chromatographic separation conditions were determined using a gradient elution system and mobile phase, which was a mixture of acetonitrile and a phosphate buffer, allowing detection of trimethoprim, sulfamethoxazole and diclofenac from 260, 210 and 310 μg/L respectively, and simultaneous determination of these analytes in the presence of urine components.
Celem pracy było sprawdzenie możliwości i opracowanie metodyki oznaczania trzech wybranych leków: trimetoprimu, sulfametoksazolu i diklofenaku w moczu z wykorzystaniem wysokosprawnej chromatografii cieczowej (ang. high performance liquid chromatography, HPLC) bez uprzedniej ekstrakcji analitów do fazy stałej. Określono warunki rozdzielania chromatograficznego z zastosowaniem elucji gradientowej fazy ruchomej, którą była mieszanina acetonitrylu i buforu fosforanowego, co pozwoliło na detekcję trimetoprimu, sulfametoksazolu i diklofenaku odpowiednio na poziomie od 260, 210 i 310 μg/l i równoczesne oznaczenie tych analitów w obecności składników moczu.

Objectives: Benzene is commonly emitted in several industries, leading to widespread environmental and occupational exposure hazards. While less toxic solvents have been substituted for benzene, it is still a component of petroleum products and is a trace impurity in industrial products resulting in continued higher occupational exposures in industrial settings in developing countries. Materials and Methods: We investigated the potential use of an electronic nose (e-nose) to monitor the headspace volatiles in biological samples from benzene-exposed Egyptian workers and non-exposed controls. The study population comprised 150 non-smoking male workers exposed to benzene and an equal number of matching non-exposed controls. We determined biomarkers of benzene used to estimate exposure and risk including: benzene in exhaled air and blood; and its urinary metabolites such as phenol and muconic acid using gas chromatography technique and a portable e-nose. Results: The average benzene concentration measured in the ambient air of the workplace of all studied industrial settings in Alexandria, Egypt; was 97.56±88.12 μg/m³ (range: 4.69–260.86 μg/m³). Levels of phenol and muconic acid were signifi cantly (p < 0.001) higher in both blood and urine of benzene-exposed workers as compared to non-exposed controls. Conclusions: The e-nose technology has successfully classifi ed and distinguished benzene-exposed workers from non-exposed controls for all measured samples of blood, urine and the exhaled air with a very high degree of precision. Thus, it will be a very useful tool for the low-cost mass screening and early detection of health hazards associated with the exposure to benzene in the industry.
