In Africa, few studies are interested in unilateral spatial neglect (NSU) in Parkinson's disease. However, this syndrome is a deficit to detect, respond to or orientate towards meaningful stimuli (Heil- man, KM 1973), observable after an injury affecting the non-dominant hemisphere for language. The significant handicap it entails justifies the need for early diagnosis and care. The NSU study is mo- tivated by its link with neurocognitive phenomena that are important on the theoretical level (attention, visuospatial and perceptual awareness). The objective is to study USN in Parkinson's pa tients, followed and hospitalized at the Neurology Department of Hassan II University Hospital in Fez. The visual-graphic test that has been used to detect this pathology is that of Bell's test. The test focuses on the detection of targets placed among several stimuli on a sheet of A4 paper. The material included 120 people: 60 Parkinsonian patients: 34 men (56,7%), and 26 women (43,3%) and 60 control subjects: 34 men (56,7%), and 26 women (43,3%). The groups were matched by age and sex. Different aspects of neglect have been observed throughout the Bell's test. It was found that total omission of bell figures was significantly influenced by age, being less frequent in the 35-49 age group in both groups, and higher in the elderly (50-80 years), as well the level of education. It have been reduced considerably with the increase in education. The hand used and the laterality had no effect; t = 3.76 degrees of freedom (df) = 108.27 and p = 0.000. Unilateral spatial neglect has a negative effect in subjects with Parkin- son's disease. It deserves to be systematically sought for a better clinical evaluation and therapeutic management of the patients.

One of the overriding principles of modern economic policy is promotion of innovation. It also turns out, however, that innovations may work against competition, thus becoming a major challenge for antitrust law. The importance and the degree of diffi culty surrounding this challenge as well as some controversies related to it can be evidenced by the antitrust enforcement policy referring to innovations. This policy has thus far been highly enigmatic and labile. It seems that no consensus can be reached on the relation between competition and innovations as much as it cannot be achieved on the validity of including the latter in the antitrust enforcement regime, particularly when it comes to applying the prohibition of the abuse of a dominant position. While it is true that innovations compound the risk of antitrust errors, especially those consisting in de-legalisation of innovations which are not anticompetitive, it would nevertheless be wrong to underestimate, and even more so to ignore, the risk of errors amounting to an unjustifi ed tolerance of anticompetitive innovations and an overstated belief in the self-regulatory ability of the market. In any case, considering the priority objective of the economic policy which should be to support and promote innovations, as well as taking into account that the goal of antitrust law is not only to settle real confl icts arising between innovations and the protection of competition, but also to provide for an innovation-friendly environment, the antitrust intervention into innovations should be sensible, farsighted and predictable. Otherwise, antitrust law may become a burden for the actual and potential innovators – like the mythical sword above the head of Damocles.

Opis:
Unilateral neglect syndrome, a difficulty in reporting, responding, or orienting to novel or meaningful stimuli presented within the contralesional hemispace, is a common consequence of brain injury that cannot be explained by primary sensorimotor defects. It is a heterogeneous cluster of cognitive deficits usually resulting from right hemisphere damage affecting the left part of the body and space. Right hemineglect also occurs, but is much less frequent. Even though about half of patients tend to recover from neglect relatively quickly and spontaneously, chronic forms of the disorder are associated with slowed motor function recovery, poor response to rehabilitation, and difficulties in many everyday activities. This article reviews the theoretical approaches and efficacy of different strategies of neurorehabilitation including such groups of therapeuticmethods as: psychological techniques of compensation (scanning training, lateralized cueing), techniques of sensory modulation (optokinetic stimulation, vestibular stimulation, transcutaneous nerve and muscle stimulation), techniques of sensorimotor plasticity activation (eye patching, adaptation to prism lenses, trunk rotation), and techniques of balancing interhemispheric interactions (limb activation therapy, transcranial magnetic stimulation). Despite years-long clinical observations and numerous experimental studies verifying usefulness of different interventions, it is not clear yet whether one rehabilitation technique is appropriate for all hemineglect patients and diverse manifestations of neglect. Some authors claim that combination of different techniques may produce additive benefits (e.g. linking of specific psychological interventions with sensory manipulation, improving of attention and visuospatial abilities, self-awareness training or pharmacological treatment).

Laryngeal reinnervation is the method of treatment in unilateral vocal fold paralysis. During the surgery an anastomosis between the main branch of ansa cervicalis and recurrent laryngeal nerve is formed. Regeneration of the nerve’s functional abilities is a long process, lasting from 4 to 6 months, up to a year. A 35 year-old male was admitted to the Department of Otolaryngology at Warsaw Medical University in order to undergo surgical removal of reoccurrence of Schwannoma in left cerebellopontine angle by translabyrinthine approach. Post-operative period was complicated with vagal nerve paralysis. The patient reported hoarseness, dysphagia and choking. Physical examination revealed left vocal fold immobilization in abducted position, saliva in piriform fossa and left-sided soft palate paralysis yet the patient was discharged home. 4 weeks after the surgery, the patient was admitted to the hospital again due to developing aspiration pneumonia. Augmentation of the left vocal fold with calcium hydroxyapatite was performed 2 months later. After this intervention temporary vocal improvement and choking frequency reduction was observed. Finally, the patient underwent laryngeal reinnervation with formation of ansa cervicalis main branch to recurrent laryngeal nerve anastomosis. Left vocal fold regained its functional abilities and the quality of the voice improved. Laryngovideostroboscopy performed 6 months later revealed mobilization of left vocal fold sufficient for proper glottis closure. The voice measures normalized and the swallowing improved. Conclusions This case report and analyzed data confirm that laryngeal reinnervation surgery in unilateral vocal fold paralysis helps achieve satisfactory effects in voice function and life quality.

The problem to find an optimal thickness of a three-layered plate (ignoring shears in the middle plate) in a set of bounded Lipschitz continuous functions is considered. The variable thickness of the exterior layer of the plate is to be optimized to reach the minimal weight under some constraints for maximal stresses. The cost functionals represent: 1) weight of the three-layered plate, 2) positive distribution (a non-negative Radon measure). The state problem is represented by a variational inequality and the design variables influence both the coefficients and the set of admissible functions. The existence of the optimal thickness is proved and some convergence analysis for an approximate penalized optimal control problem is presented. We prove the existence of a solution to the weight minimization problem or minimization the work of interaction forces on the basis of a general theorem on the control of variational inequalities.
W pracy zajęto się problemem doboru optymalnej grubości trójwarstwowej płyty (z pominięciem naprężeń stycznych w warstwie środkowej) w zbierze ograniczonych, ciągłych funkcji Lipschitza. Zmienna grubość warstwy zewnętrznej jest optymalizowana poprzez minimalizację ciężaru przy pewnych ograniczeniach narzuconych na maksymalne naprężenia. zastosowane funkcjonały kosztu reprezentują: 1) ciężar trójwarstwowej płyty, 2) dodatni rozkład (nieujemna miara Radona). Zagadnienie stanu opisano nierównością wariacyjną oraz uwzględniono wpływ zmiennych konstrukcyjnych na współczynniki i zbiór dopuszczalnych funkcji. Udowodniono istnienie optymalnej grubości warstwy i przedstawiono analizę zbieżności przybliżonego problemu optymalizacji metodą funkcji kary. Wykazano istnienie rozwiązania przy minimalizacji ciężaru płyty i minimalizacji pracy sił oddziaływania pomiędzy płytą a podporą na podstawie ogólnego twierdzenia o doborze parametrów w układach opisanych nierównościami wariacyjnymi.

A model of a flat triple pendulum with obstacles imposed on its position is used for the modelling of a piston-connecting rod-crankshaft system of a mono-cylinder four-stroke combustion engine. The introduced selfexcited system can be only treated as the first step in more advanced modelling of real processes, but some examples of numerical simulations exhibit well known six stages of the piston sliding along the cylinder surface per cycle.
Model płaskiego potrójnego wahadła fizycznego z przeszkodami ograniczającymi jego ruch został użyty do zamodelowania mechanizmu korbowego silnika spalinowego. Wprowadzony samowzbudny model może być traktowany jedynie jako bardzo zgrabne przybliżenie rzeczywistych zjawisk zachodzących w cylindrze silnika spalinowego. Pomimo tego, kilka zaprezentowanych przykładów symulacji numerycznych wykazuje bardzo dobrą zgodność z danymi doświadczalnymi prezentowanymi w literaturze.

Background: Unilateral sinus disease (USD) occurs in 23 % of all cases. It is believed that it is mainly associated with cancer development. Retrospective data from large rhinological centers show that the most common USD is chronic rhinosinusitis (CRS), followed by mycosis, inverted papilloma and finally cancer, but only in a small percent of cases. The aim of the study: The analysis of USD in the group of patients who underwent FESS at the secondary referral center. Material and Method: The retrospective study of patients treated for USD in the Department of Otolaryngology in the Provincial Hospital in Poznan between June 2014 and June 2016. The analysis includes age, sex, the localization of lesions, histopathological and microbiological results, an extension of the surgery and treatment results. Results: Over the analyzed period of time, 415 FESS for chronic sinusitis were performed. In this group, 83 patients underwent surgery for USD. There were 35 women and 48 men.CRS was found in 48 cases, mycosis in nine cases, 12 patients were operated for non-malignant tumors, such as inverted papilloma (9), osteoma (2) and fibrosis tumor(1); seven patients had a choanal polyp and two of them had a foreign body in maxillary sinus – a tooth root lying loose. Four patients were diagnosed with a hypoplastic maxillary sinus and one patient suffered from frontal sinus pyocele. One side endoscopic opening of all sinuses was performed in the group with CRS, endoscopic medial maxillectomy was conducted in patients with inverted papilloma, and an isolated opening of the affected sinus was performed in the cases with mycosis. Conclusions: USD must be always suspected of malignant degeneration until proven otherwise. Endoscopic sinus surgery with the use of angled scope allows for the removal of even very extensive lesions. In our opinion, the extent of operation is determined by the nature of pathology. While extensive surgery is recommended in patients with inverted papilloma, a limited procedure should be performed in those with isolated mycosis.

The present paper is the second part of the contribution by Rojek and Telega (2001). An alternative adhesion law was used to the study of bone-implant interface. Numerical scheme was developed and applied to the knee joint after arthroplasty. Influence of wear debris on this interface and currently used wear models were investigated.
Zagadnienia kontaktowe z tarciem, adhezją i zużyciem w biomachanice ortopedycznej. Część II - Implementacja numeryczna i zastosowanie do stawów kolanowych po implementacji. Niniejsza praca stanowi część drugą pracy, której autorami są Rojek i Telega (2001). Do analizy interfazy kość-implant zastosowano alternatywny opis adhezji. Opracowano algorytm numeryczny, który zastosowano do analizy stawu kolanowego po endoprotezoplastyce. Zbadano wpływ produktów zużycia na analizowaną interfazę oraz przedstawiono aktualnie stosowane modele zużycia.

Crack and flaw identification problems in two-dimensional elastomechanics are numerically studied in this paper. The mechanical modelling is based on boundary element techiques, with special care of hypersingular issues for the cracks. The possibility of partially or totally closed cracks (unilateral contact effects) is taken into account by linear complementarity techniques. Backpropagation neural networks are used for the solution of the inverse problems. For dynamical problems, a suitable preprocessing of the input data enhances the effectiveness of the procedure. For the two-dimensional examples presented here, the proposed method has similar performance for classical crack and flaw identification problems. The identification of unilateral cracks is a considerably more difficult task, which nevertheless, can also be solved by the same method, provided that a suitable dynamical test loading is applied.
W pracy zajęto się problemem identyfikacji pęknięć i innych uszkodzeń strukturalnych w dwuwymiarowym stanie odkszałceń sprężystych za pomocą metod numerycznych. Modelowanie mechaniczne oparto na metodzie elementów brzegowych ze szczególnym uwzględnieniem kwestii osobliwości pęknięć. Możliwość powstawania częściowo lub całkowicie zamkniętych pęknięć wprowadzono poprzez wykorzystanie liniowej metody komplementarności. Dla rozwiązania zagadnień odwrotnych użyto sieci neuronowych ze wsteczną propagacją. W zgadnieniach dynamicznych efektywność zaproponowanej procedury zwiększono poprzez odpowiednią wstępną obróbkę danych wejściowych. Na przykładzie dwuwymiarowych modeli opisywanych w pracy stwierdzono podobną skuteczność metody, jak w przypadku klasycznego zagadnienia identyfikacji wad strukturalnych. Wykazano, że identyfikacja jednostronnych pęknięć, która jest znacznie trudniejszym zadaniem, jest możliwa za pomocą zaprezentowanej metody, jeśli do analizy modelu przyjąć odpowiednio dobrane obciążenia testowe.

We present a mesh adaptation method by node movement for two-dimensional linear elasticity problems with unilateral contact. The adaptation is based on a hierarchical estimator on finite element edges and the node displacement techniques use an analogy of the mesh topology with a spring network. We show, through numerical examples, the efficiency of the present adaptation method.

The paper deals with shape optimization of dynamic contact problem with Coulomb friction for viscoelastic bodies. The mass nonpenetrability condition is formulated in velocities. The friction coefficient is assumed to be bounded. Using material derivative method as well as the results concerning the regularity of solution to dynamic variational inequality the directional derivative of the cost functional is calculated and the necessary optimality condition is formulated.

Medialization thyroplasty (type I) is surgical procedure performed on the thyroid cartilage. The major indication for this surgery is significant glottis insufficiency due to unilateral vocal fold paresis. However the proce¬dure is also performed after vocal fold resections during cordectomy. The aim: The evaluation of voice results in patients after medialisation throplasty. Material and methods: In Otolaryngology Department of Medical University of Warsaw there were performed so far 8 thyroplasty procedures under local anaesthesia with implantation of medical silicon protesis. 6 patients had unilat¬eral vocal fold paresis and the rest two underwent in the past laser cordectomy due to T1a vocal carcinoma. Results: There were no complications during and post the surgery. The follow up examination in 1st , 3rd, 6th i 12th months postoperatively revealed for all patients significant improvement of glottal closure in laryngeal videostrobos¬copy. The voice quality improved both in perceptual evaluation (GRBAS scale) and acoustic analysis (F0, jitter, shim¬mer, NHR) in both patients groups. However the rate of improvement was much more significant in group with uni¬lateral vocal fold paresis. In all patients the maximum phonation time (MPT) increased. The self-evaluation of voice quality with Voice Handicap Index questionnaire confirmed also individual improvement. Conclusions: The speech rehabilitations is not successful in each patient with glottis insufficiency. The medialisation thyroplasty remains the standard procedure for permanent improvement of voice quality in those cases.

Introduction. Schizencephaly is one of the rare congenital defects of the central nervous system (CNS), known as neuronal migration disorders. The etiology of schizencecephaly is unequivocal. Established etiologies include in-utero infections (cytomegalovirus and herpes simplex virus, HSV type I), toxic abuse (cocaine, alcohol), as well as drug use (warfarin). Objectives. The aim of the study was to analyze the clinical presentation of schizencephaly with particular consideration of the course of epilepsy in paediatric patients. Materials and method. The study group consisted of 38 children with schizencephaly (20 of them had seizure) and was retrospectively assessed. Data were analyzed using SAS version 9.4. U Mann-Whitney and χ2 tests and logistic regression analysis were used in statistical analyses. Results. Epilepsy was the most frequent in bilateral type II schizencephaly (p=0.033). In logistic regression analysis, the presence of bilateral open schizencephaly significantly increased the risk of seizures (OR=11.67; 95%CI 2.44–55.83; p=0.002). Drug-resistant epilepsy was observed in 9 children (45% of the children with epilepsy). Prevalence of both epilepsy and drug-resistant epilepsy in schizencephaly did not significantly depend on gender, stage of development, type or localization of schizencephaly, and other coexisting CNS defects or clinical presentation of schizencephaly at follow-up in the study group of patients. Conclusions. The bilateral type of schizencephaly was identified as an independent risk factor for epilepsy in the analyzed children.
