Opis:: Wound healing is a complex pathophysiological process, in which platelets play a crucial role. Platelet alpha-granules release growth factors to the wound bed; the factors are necessary in the healing process. In chronic wounds, such as poorly healing lower-leg ulcers of venous origin, there is decreased activity of multiple growth factors, so the concept of exogenous delivery of such factors seems a logical strategy. Platelet-rich plasma therapy in patients with lower-leg ulcers of venous origin combined with conventional treatment methods (previously ineffective in these patients) seems, based on our observation, an important adjunct leading to recovery. The aim of the study was to present an original method of autologous platelet-rich plasma application through the creation of a sort of “biological chamber” containing a concentrate of growth factors. Material and methods. The described therapy was implemented in 10 patients, who had been ineffectively treated for more than one year in the outpatient setting. Patients with exacerbation of inflammatory process, signs of wound infection and ankle brachial pressure index < 0.8 were excluded from the study. After the application of platelet-rich plasma, further treatment was continued with the use of moist therapy and compression therapy according to a uniform regimen. Results. Complete healing was achieved within 4-10 weeks from the beginning of the product administration in all patients. Conclusion. The presented method seems technically simple, effective and relatively inexpensive

Opis:: Nieregularność kształtu owrzodzeń oraz ich umiejscowienie na zakrzywionej powierzchni goleni są przyczyną dużych trudności pomiarowych. Celem pracy jest opracowanie własnej metody cyfrowej dokumentacji stanu miejscowego owrzodzeń żylnych goleni. Opracowana metoda obejmuje własną technikę fotografowania rany, obróbki cyfrowej uzyskanych obrazów oraz własną metodę pomiaru pola powierzchni z przetworzonego cyfrowo obrazu przy pomocy programu "Pole". W pracy dokonano kontrolnej analizy dokładności odwzorowania owrzodzeń oraz dokładności pomiaru pola ich powierzchni zaproponowaną metodą własną. Wyniki analizy wykazały wysoką precyzję pomiaru wielkości owrzodzenia naszą metodą oraz dowiodły, że umożliwia ona obiektywną ocenę zmian pola powierzchni i wyglądu rany.
Venous ulcers are usually flat wounds of varying size and irregular shape. One of the important elements included in the documentation of venous ulcer treatment is the size of their surface area. Consecutive measurements of the area are the basis for assessing progress in treatment. The irregular shape and location of venous ulcers on a curved surface are the main causes of difficulties in their measurement. The aim of the study was to elaborate the author's own method of digitally recording the local state of venous leg ulcers. The elaborated method involves the author's own method of photographing the wound, digitally processing the obtained images, and the author's own method of measuring the surface area directly from the digitally processed image using the computer program "Field". In the study an analysis of the imaging accuracy of a leg ulcer and the measuring accuracy of the surface area using the author's own method was carried out. Results of the analysis proved the high precision of the author's own method of venous ulcers size measurement and showed that the method enables an objective assessment of the appearance of the wound and changes to the surface area.

Opis:: The guideline titled “Prevention and treatment of pressure ulcers: clinical practice guideline” is the result of a collaborative effort between the National Pressure Ulcer Advisory Panel (NPUAP), European Pressure Ulcer Advisory Panel (EPUAP) and Pan Pacific Pressure Injury Alliance (PPPIA). A comprehensive literature review was conducted on pressure ulcer prevention and treatment. Rigorous scientific methodology was used to appraise available research and make evidence-based recommendations for the prevention and treatment of pressure ulcers. Draft guidelines were made available to 986 invited individual stakeholders and organizations or societies, and the feedback of the stakeholders was taken under consideration by the developers of the guideline. The guideline includes 575 explicit recommendations and/or research summaries for multidisciplinary pressure ulcer topics.

Opis:: This study was aimed to demonstrate the efficacy of interval hypoxic training (IHT) in complex treatment of Helicobacter pylori-associated duodenal peptic ulcer disease (DPUD) by parameters of aerobic metabolism and indexes of heart rate variability (HRV). Eighty patients with H. pylori-associated DPUD were included into the study, mean age 32±1.8 yrs, duration of the disease up to 10 years (66.3 %). IHT was modulated using Frolov's hypoxicator (TDI-01) for 30 days after standard eradication therapy. Daily hypoxic sessions consisted of three one-minute sessions, one two-minute, and one three-minute sessions separated by one-minute intervals of room-air breathing. Use of IHT resulted in more efficient elimination of clinical symptoms, histological hallmarks of inflammation and signs of oxidative stress in glandulocytes of the gastric mucosa as determined by 4-hydroxynonenal accumulation. Moderate prooxidant activity of IHT was demonstrated by the increased level of TBARS and oxidatively modified products, normalization of hydroperoxides, middle mass molecules and atherogenic beta-lipoproteins with simultaneous increase in catalase activity and mild decline of SOD activity. Therefore, IHT appeared to be accompanied by higher intensity of redox reactions and enhanced regeneratory processes in cells and tissues. Significant increase in HRV was also noted. Such changes were associated with reduction of inflammation signs and modulation of the autonomic homeostasis in DPUD patients. In general, use of IHT in complex treatment of H. pylori in DPUD patients can be recommended to increase resistance to oxidative stress and to modulate autonomic balance and oxidative homeostasis.

Opis:: The human microbiota has a tremendous effect on our health. In the last decades, our knowledge about interactions between bacteria and humans have grown greatly. Not only is it necessary for humans to synthesize vitamins, to have tight intestinal barriers or protect from pathogens, it also has an impact on our immune system and thus plays an important role in autoimmune diseases and prevention of excessive inflammatory response. The idea of probiotics is to restore the balance in humans digestive microbiota. There is a growing number of scientific papers that proves a positive impact of using probiotics in various diseases. However, there are still questions that need to be answered before probiotics play a bigger role in the treatment. This paper presents the information about the use of probiotics in most common diseases of gastrointestinal tract.
