The subject of the article is the identification of factors behind the incorrect operation of a car turbocharger that, in longer perspective, lead to the damage of the device. Due to the operating principle and design features, the device works with many engine systems – intake, exhaust, lubrication systems and also, increasingly more often, with the cooling system. A multitude of relations, complexity, and working conditions are reasons due to which the device is sensitive to disturbances of quality parameters of the working media (e.g. oil). In the article based on the selected causes of damage of turbochargers, we focused on the following initiating event, concerning the bearings lubrication system, which is clogged oil strainer. The authors aims at performing an analysis allowing to increase the detectability of emerging malfunctions that lead to deterioration of operating conditions of the device. For this task tool such as ETA (Event Tree Analysis) was used. It is a tool for assessing system and process safety. It facilitates detecting potential risks, as well as relations existing between actions or events. This method explores the path from the initiating event to the outcome (most often representing the immobilization of the tested object), with particular emphasis on intermediate stages affecting the technical condition of the object. Unlike the FTA analyses, which have a deductive nature, the ETA analysis is inductive. The analysis of the obtained results, supported with validation by means of ETA, allowed proving that, given the current state of knowledge and advancements in technology, it is possible to apply additional sensors for monitoring the operation of a turbocharger. The proposed modifications were considered minor design changes that significantly increase the reliability of the device.

Water supply system is complicated technical system which belongs to underground urban infrastructure. Its accurate functioning determines everyday live stability of citizens. Risk analysis is a key phase of the process of water supply safety management. The aim of this paper is to present new propose of methods for risk of the first type (associated with quantity of supplied water) and the risk of the second type (associated with quality of supplied water) analyses in water supply system. The paper contains the description of matrix methods for risk analyses and methods describing cause-and-effect relationship of failure events (fault tree analysis).

In this research study, we focus on the tree test results as well as the first-order kinetic model to evaluate flotation test data to propose a conceptual design of a flotation circuit for a specific coal sample. Results from the tree test showed it was possible to achieve a product with ash content less than 10% with 8% as combustible recovery and indicated for this coal sample, to obtain low ash – low recovery condition. Kinetic test results showed some of the streams had the same constant, so it could combine streams with similar rates according to configuration aspects. The proposed circuit includes stages (1- rougher, 2- rougher -scavenger, 3- cleaner, 4- cleaner -scavenger, and 5-recleaner) and recleaner concentrate indicated as the final product and rougher -scavenger tailings and cleaner -scavenger tailings also indicated as a final tailing. It is worth noting the proposed circuit is a conceptual design, so the validation of data on a larger scale for the obtainment of the optimized circuit is crucial.

Many lives and aircrafts have been lost due to human errors associated with mental workload overload (MWLOL). Human errors are successfully considered in existing Fault Tree Analysis (FTA) methods. However, MWLOL is considered through Performance Shaping Factors indirectly and its information is hidden in FT construction, which is not conducive to analyze the root causes of human errors and risks. To overcome this difficulty, we develop a risk analysis method where Multiple Resources Model (MRM) is incorporated into FTA methods. MRM analyzes mental workload by estimating the resources used during performing concurrent tasks, probably including abnormal situation handling tasks introduced by basic events in FT. Such basic events may cause MWLOL and then trigger corresponding human error events. A MWLOL gate is proposed to describe MWLOL explicitly and add these new relationships to traditional FT. This new method extends previous FTA methods and provides a more in-depth risk analysis. An accident, a helicopter crash in Maryland, is analyzed by the proposed method.

Technological progress, as well as the development of the sciences on occupational health and safety, increased safety and improved working conditions in enterprises of even the most onerous industries. To maintain the required level of safety, preventive actions are taken to identify all hazards in the workplace. In practice, many methods are used to identify hazards depending on the type of technological processes and the types of workstations analyzed. The article presents the advantages of the fault tree analysis method as a tool to support the process safety management in the enterprise. The article presents features of the fault tree analysis method as a tool for better security management in an enterprise. Cause and effect relationships between events in the fault tree schema on selected examples were examined. Indirect and direct causes of accidents and failures in the enterprise have been identified, including human errors (human factor). The use of the fault tree analysis method to support accident and failure risk assessment in an industrial plant was also analyzed.

A numerical analysis has been designed to study internal flow phenomena in a diagonal rotor. A calculated diagonal rotor was designed by a quasi-three-dimensional method. Its hub and casing walls were inclined 45o and 25o, respectively. The numerical simulation was based on the Navier-Stokes equations coupled with a k-ε turbulence model. We found that the rotor's wake was stronger near the hub and in the casing end wall region. The wake at a lower flow rate was stronger than that at a higher flow rate. Static pressure gradually increased from the hub to the casing along the height of a blade, on the rotor pressure surface and in the front 60% of the chord region of the suction surface. In the back 40% of the chord region of the suction surface, static pressure gradually decreased. A passage vortex formed in the stator flow passage and an 80% axial chord plane. It was located near the hub end-wall. The passage vortex developed into a large vortex centered near the midspan at a 99% axial chord plane of the stator. The casing wall boundary layer downstream of the rotor occupied approximately 10% of the flow passage. Along the height of a blade, the meridian velocity gradually increased upstream of rotor and decreased downstream. The calculated aerodynamic characteristic curve, the meridian velocity distribution upstream and downstream of the rotor, and the streamline distribution on the meridian surface were consistent with experimental results and design data. Our findings proved that the present numerical method is reliable and practicable. It can be used to design and analyze swept diagonal rotors in order to improve their surging and rotation stall state. The present results also provide comparative data for the design of highly-loaded swept diagonal rotors in future studies.

Due to the unique geographic location, complex navigation environment and intense vessel traffic, a considerable number of maritime accidents occurred in estuarine waters during recent years which caused serious loss of life, property and environmental contamination. Based on the historical data of maritime accidents from 2003 to 2012, which is collected from Shenzhen Maritime Safety Administration, this paper conducted a risk analysis of maritime accidents by applying Bayesian network and fault tree analysis. First a Bayesian network model was introduced to describe the consequence of accidents based on the accident investigation report. Then fault tree analysis was applied to estimate the probability on the basis of accident statistics and ship traffic flow. Finally the risk of maritime accidents in Shenzhen Waters was depicted through the consequence multiplied by the probability of an accident.

W artykule przedstawiono analizę zagrożeń metodą drzewa błędów (FTA) dla wybranych stanowisk pracy. Zawarte w artykule schematy drzewa błędów ilustrują dwa typy zagrożeń: wypadkiem podczas pracy z prasą hydrauliczną oraz zniszczeniem maszyny na skutek przegrzania. W obu przypadkach przeprowadzona analiza wskazuje przyczyny zagrożenia wynikające z faktu awaryjności elementów maszyny lub stanowiska pracy oraz możliwe do popełnienia błędy operatora. Za pomocą drzewa błędów wskazano wzajemne związki między zdarzeniami prowadzącymi do zagrożenia w analizowanych przykładach. Zidentyfikowano również działania korygujące dla analizowanych zagrożeń w celu zminimalizowania ryzyka zawodowego.
The paper presents an analysis of the Fault Tree Analysis (FTA) approach for selected workplaces. The fault tree diagrams included in this article illustrate two types of hazards: an accident when working with a hydraulic press and damage to a machine due to overheating. In both cases, the analysis indicates the cause of the hazard resulting from the failure of the machine or workpiece, and the operator's possible errors. The fault tree identifies the relationship between events leading to a threat in the analyzed examples. The corrective actions were also identified for the hazard analyzed to minimize occupational risk.

Urządzenia transportu podziemnego ze względu na pracę w ograniczonych przestrzeniach wyrobisk górniczych niosą specyficzne zagrożenia dla pracującej załogi. W artykule przeanalizowano pracę maszyn transportowych takich jak: taśmociągi, kolej szynowa oraz transport pionowy w szybach górniczych. Do analizy zagrożeń wykorzystano metodę drzewa błędów wyznaczając bezpośrednie i pośrednie przyczyny zagrożeń w transporcie podziemnym. Konstrukcję drzewa błędów przedstawiono dla trzech rodzajów zdarzeń: transport taśmociągiem, kolejką szynową oraz transport pionowy szybem kopalnianym.
Underground transport equipment due to work in limited spaces of mining excavations carry specific hazards to the working crew. The article analyzes the operation of transport machines such as conveyor belts, underground railway and vertical transport in mining shafts. The fault tree method was used to analyze the hazards by determining the direct and indirect causes of hazards in underground transport. The construction of the fault tree was presented for three types of events: transport by conveyor, underground railway and vertical transport by a mine shaft.

The introduction to the Fault Tree Analysis has been presented in this article. Selected publications on the Fault Tree Analysis have been listed. The usefulness of the method in marine engineering application has been indicated. Reliability testing facility has been presented, that is typical two-stroke, slow-speed ship crosshead Diesel engine intended to power the ship. The way of creating system model with the Fault Tree Analysis application has been presented. Reliability structure of tested engine has been modelled using Reliability Block Diagrams, as well as the Fault Tree Analysis. Main tree describing the object reliability structure analysis and the sub-tree structure modelling crank system, subsystem of the cam shaft, cylinder exemplary system and turbocharger have been built. Qualitative analysis of the tree has been conducted – the minimal cut sets of the system has been determined. The directions for further research have been indicated.

According to Polish nuclear law, newly emerging nuclear facilities require probabilistic safety assessment (PSA). This article is intended to present the PSA method and to present the error tree method by which the probability of unavailability of the gravity reactor cooling system (GDCS) of the ESBWR power plant designed by GE Hitachi was determined. This work includes creatiion process of a damage tree and performing a quantitative analysis in SAPHIRE tool and estimating uncertainty using the Monte Carlo method. As a part of the work, it was shown that in the probability of failure of a single GDCS P LINE-A line, the most important element are the basic events related in particular to the operation of service valves.

Merchant shipping has undergone a great transformation over the past three decades. The shipping market is highly competitive, which coupled with high crewing and fuel costs, leads to high operational costs. One of the paramount factor involved in vessel operation is the Maintenance cost and there is a dire need to keep this cost to a minimum. Fortunately the earlier policy of repair only maintenance in commercial shipping has been done away with, and was replaced by the policy of preventive maintenance. Planned Maintenance System was introduced by ship management companies in the early 90’s. Planned Maintenance offered benefits over the repair only policy, but has its own demerits. Many a time machinery equipment is opened up for routine maintenance after a specified time interval, irrespective of the need. This could lead to potential failures, which is explained by the fact that preventive maintenance resulted in meddling of a well set piece of machinery equipment, leading to its subsequent failure. This is where Condition based maintenance or CBM steps into prominence. CBM monitors the health of the machinery equipment, analyses the condition and helps you in decision making. The Main Propulsion system forms the heart of a vessel and we need to ensure its reliability, together with the reliability of its associated sub-systems. The entire system can be represented by reliability block diagrams, to show the interdependence of various components comprising the system. This helps in the decision making process of CBM whereby ship’s engineer may decide to stop the running machinery equipment, open and overhaul the same, else postpone the overhaul for a later safe date.

Any failure on the recovery system will cause a lot of environmental damage as well as energy loss. Hereof two types of alternatives; fast opening valve system (FOVS) and seal drum system (SDS) may be installed. The focus of this article is on the decision stage to choose the most preferred option in terms of reliability assessment. The major challenge in the research problem is on changing the pressure and temperature during operational cycles, which significantly affect the reliability. In addition, the lack of historical data complicates the reliability assessment method. Hence, we proposed a hybrid approach using fault tree analysis (FTA) and the Mamdani fuzzy inference to estimate reliability response as a function of a few frequently operating pressure and temperature. Also, discrete-event simulation helped us to evaluate the system reliability at different operating conditions. The comparisons reveals that the FOVs outperforms on average of 22.4% than the SDS and it is recommended for putting into practice for purchasing.

Sycamore maple (Acer pseudoplatanus L.) is a fast-growing tree species that produces economically attractive timber, provides ecological services and has high site adaptability, but it is only a minor component of European forests. This paper describes production, structure and diversity of sycamore-dominated forest stands that originated by succession on former agricultural lands in the Orlické hory Mts., the Czech Republic. The partial objectives were to determine impact of climatic factors on the radial growth of sycamore maple and evaluate the effect of game on natural regeneration. The number of sycamore seeds on the ground was on average 9–34 seeds m–2 and seedling mortality reached on average 89% caused by damage of hare and drought. Density of natural regeneration ranged from 1,532 to 4,244 recruits ha–1, but growth dynamic was strongly influenced by ungulate browsing (69%) with the largest losses in sycamore (96%). Analyses of climatic effects showed a significantly close relationship with growth of sycamore in submontane areas, being especially sensitive to extremely dry and cold winters or very high summer temperatures. Radial growth was more significantly influenced by monthly temperatures compared to precipitation, but increasing sum of annual precipitation was main positive driver factor of diameter increment. Radial growth had the strongest relationship with weather conditions in July and October of previous year and March of the current year. Stand volume ranged from 378 to 545 m3 ha–1 at age around 75 years, while sycamore accounted for 32–82%. The horizontal structure of the tree layer was mostly random to aggregated; resembling the clumped spatial pattern of natural regeneration. Besides their wood production, these stands fulfil many ecosystem functions that are associated with high ecological stability, high structural differentiation and medium to high species diversity.
