Introduction. Traumatic brain injuries (TBI) are a real social problem, with an upward trend worldwide. TBI is the leading cause of death and disability, especially among young men. Each year in Europe, and also in Poland, 243 per 100,000 individuals suffer from TBI. Assessing prognosis after traumatic brain injury is very important in order to help clinicians to make a decision about the implementation of specific methods of treatment, and to make communication with the patient and the patient’s family easier. Objective. The objective of this review was to present prognostic factors, to assess outcomes within a short time after a moderate to severe traumatic brain injury, as well as to predict functional outcome. The most important independent variables were: age, Glasgow Coma Scale (GCS) motor score, pupil response, Marshall CT classification and traumatic subarachnoid haemorrhage. Other important prognostic factors included hypotension, hypoxia, glucose, coagulopathy, haemoglobin and category of CT characteristic, such as midline shift, mass lesion, basal cistern. Conclusions. Gender and intraventricular haemorrhage did not have predictive value. This subject needs much more research in the area of new prognostic factors which would be better associated with outcome after traumatic brain injury.

Introduction. In the literature there are some discrepancies concerning the occurrence of vertigo and disequilibrium, as well as the type of vestibular dysfunction in patients after mild traumatic brain injury (MTBI). Objective. The aim of the study was to assess the type of vestibular system dysfunction in MTBI patients after injury at baseline and 6-month follow-up. Materials and method. From a group of 188 patients with vertigo/dizziness and balance instability after MTBI, prospective analysis in 50 patients with abnormal videonystagmography (VNG) were conducted. Anamnesis, otoneurological examination, self-assessed Dizziness Handicap Inventory (DHI) and VNG test were carried in each patient twice – at baseline and after 6 months. Results. A significant improvement in the DHI total score (9.8 points, p<0.001) and in the subscales was found after 6 months. At baseline, VNG test showed the peripheral vestibular system dysfunction in 19 cases (38%), central vestibular dysfunction in 20 patients (40%) and mixed vestibular dysfunction in 11 cases (44%). After 6 months, the VNG records normalization was observed in 19 patients (38%), an improvement was significantly more frequently related to peripheral disorders, compared to central ones (p<0.001). Conclusions. In patients after mild traumatic brain injury, about one-fourth of them had vestibular system dysfunction with the same frequency of peripheral, central and mixed lesions in videonystagmography. Subjective post-traumatic symptoms and vestibular system dysfunction in patients after MTBI decrease after 6-month follow-up. However, more than half of patients still have abnormal VNG results, which indicate dysfunction of the central vestibular system.

Introduction. In spite of the rapid development in various communication-support technologies for those waking up from a coma, studies describing the sole process of reconstructing communication in this group of patients are scarce. Objective. The aim of this study was to analyze communication reactions in a minimal state of consciousness and describe the nonverbal behaviours characteristic for each stages significant for the therapy of communication. Materials and method. 18 severely brain-injured patients in a minimal state of consciousness participated in the half-year observation study, which included people experiencing at least 4 weeks of consciousness disorder/coma. Age of patients 25±5 years. Psychological assessment included: observation of various attempts of communications undertaken by patients, caregivers and family interview, the Glasgow Coma Scale (GCS) and Individual Communication Sheet. Results. Data analysis showed a significant increase in preverbal communication, both in primal and sensory areas when compared between Stage II (GCS=6–8 points) and Stage III (GCS=9–12 points). After a time, primary communication reached a high level. Patients produced communication attempts from the behaviour organization level, and an increase in the nonverbal communication level was noted. Based on observations, nonverbal communication profiles for each stage of waking up from a coma were introduced. Conclusions. It was found that in the process of waking up from a coma the patients communicate with the use of the preverbal level of primal communication, the sensory and behaviour organization activities. The characteristics of the communication reactions show that in Stage III there is a significant increase in two preverbal communication areas: primal and sensory acts, when compared with Stage II.

These mechanisms are enlarging from the first to the seventh day of the posttraumatic period and significantly higher against the background of combined cranio-skeletal injuries on the 3rd and on the 7th day of post-traumatic period.

As a result of using a multi-modality neurological/mental assessment and a multifaceted rehabilitation program (including group therapy), 108 patients with sequelae of mine blast closed traumatic brain injury (MB CTBI) and post-traumatic stress disorder (PTSD) have demonstrated positive trends concerning their neurological and psychological  status. From the standpoint of the pathogenetic concept of neurotic disorders, there are four principal categories of neurotic disorders, namely asthenic, anxiety-depressive, hysteria-like and phobic. A conclusion was made that using the methods of group psychotherapy as a constituent of rehabilitation program was effective.

The study focuses on achieving the proper degree of daily patient activation, the minimization of the manifestations, the consequences of post-coma immobilization syndrome, especially the syndromes of repressed consciousness, highlighting the key importance of the type of individual rehabilitation programs’ content. The main aim of this study was to develop and justify a system of differentiated medical (physical) rehabilitation of patients with post-traumatic post-coma long-term disorders of consciousness, identify key principles for the functioning of this system, and highlight the basic guidelines for specialized rehabilitation medical institutions. The work is based on the results of diagnosis, rehabilitation and restorative treatment of 220 patients with traumatic post-coma disorders of consciousness, who were examined and underwent neurorehabilitation (NR) treatment at the State Institution Romodanov Neurosurgery Institute of National Academy of Medical Sciences of Ukraine and the "Nodus" Practical Center for Neuro Rehabilitation from January 2007 to July 2018 inclusive. All patients after courses of rehabilitation treatment were discharged for outpatient monitoring or continued treatment of somatic complications of the underlying disease at their place of residence. During the rehabilitation treatment conducted over the study period, none of the patients died. The conclusion as to the applied effectiveness of the established system of rehabilitation is made after comparing the expected results of the rehabilitation and rehabilitation with the real achievements, carrying out a detailed analysis of the implemented individual programs at each stage of the restoration of consciousness, as well as clarifying the design of daily exercises and the principles of the distribution of loads in order to solve the set clinical problems, and both general and current goals.

Background. Severe traumatic brain injury stays one of the main reasons of humans’ mortality until the age of 40. Ternopil University Hospital delivers medical care to the patients with neurosurgical pathology of Ternopil region, which is comprised of 1 107 000 citizens. Material and methods. In this paper we analyzed the treatment results of 301 patients with severe traumatic brain injury (STBI), who were treated at intensive care unit of Ternopil University Hospital during 2013-2014. Microbiological research and evaluation of results were accomplished according to generally accepted principles, based on the Bergey’s classification. Results. In 72 patients it was complicated by severe sepsis. The mortality rate in clinic reached 29.7% in 2013-2014, or equals 1.9 person per 100 000 of population. Among the intensive care unit patients with STBI, who had the surgery based on this pathological condition, sepsis developed in 23.9% of patients. Acute respiratory distress syndrome (ARDS), which complicated sepsis, was successfully cured in 73.3% of cases, by additional usage of traditional ventilation, kinesiotherapy, oxygen therapy and pulse therapy with glucocortisteroids (GS). 22.2% of patients obtained acute renal failure, 9.72% of patients were treated by implementing hemodialysis. Concomitant polytrauma was present in 23.9% of patients. Conclusions. Theusageof pulse therapy with glucocortisteroids additionally to traditional mechanical ventilation, kinesiotherapy, and oxygen therapy for the treatment of acute respiratory distress syndrome helps to improve the life expectancy of patients.
Wprowadzenie. Silne urazowe uszkodzenie mózgu pozostaje jedną z głównych przyczyn zgonów pacjentów w wieku poniżej 40 lat. Uniwersytecki Szpital w Tarnopolu zapewnia opiekę medyczną pacjentom z patologią neurologiczną z regionu Tarnopola, który zamieszkuje 1,107,000 mieszkańców. Materiał i metody. W tym artykule zostały przeanalizowane wyniki leczenia 301 pacjentów z silnym urazowym uszkodzeniem mózgu (STBI), którzy byli leczenie na oddziale intensywnej terapii w Uniwersyteckim Szpitalu w Tarnopolu w latach 2013-2014. Badania mikrobiologiczne oraz ocena wyników przeprowadzono zgodnie z ogólnie przyjętymi zasadami w obdarciu o klasyfikację Bergeya. Wyniki. W przypadku 72 pacjentów komplikacją była ciężka sepsa. Śmiertelność zanotowana przez szpital była na poziomie 29.7% w latach 2013-2014, lub też była równa 1.9 osób na 100,000 populacji. Wśród pacjentów z STBI przebywających na oddziale intensywnej terapii, którzy byli poddani operacji z powodu wystąpienia u nich tego stanu patologicznego sepsa rozwinęła się w przypadku 23,9% z nich. Zespół ostrej niewydolności oddechowej (ARDS) wraz ze skomplikowaną postacią sepsy został pomyślnie wyleczony w przypadku 73,3% pacjentów dzięki dodatkowemu użyciu tradycyjnej wentylacji, kinezyterapii, terapii tlenowej oraz leczenia pulsacyjnego glikokortykosteroidami (GS). U 22,2% pacjentów wystąpiła ostra niewydolność nerek. 9,72% pacjentów poddano leczeniu za pomocą hemodializą. Towarzyszący uraz mnogi wystąpił w przypadku 23,9% pacjentów. Wnioski. Użycie leczenia pulsacyjnego glikokortykosteroidami stosowane dodatkowo oprócz tradycyjnej wentylacji mechanicznej, terapii tlenowej w leczeniu zespołu ostrej niewydolności oddechowej pozwala zwiększyć przeżywalność pacjentów.

Increasing numbers of persons with traumatic brain injury (TBI) are being observed in highly motorized countries becau-se of the fact that (road) accidents constitute the most frequent cause of such events. For example, car accidents are the main cause of death in persons up to 44 years of age. Thanks to the progress in medical care, the chances of survival after traumatic brain injury have in-creased, which, however, is associated with an increasing number of patients with chronic neuro-psychiatric disturbances requiring tre-atment and rehabilitation. The majority of these patients suffer from post-traumatic emotional disturbances or personality changes. Among the emotional disturbances, depressive and anxiety syndromes as well as post-traumatic stress disorder (PTSD) are most fre-quent. A smaller number of patients demonstrate cognitive function disturbances. The incidence of chronic psychotic syndromes re-sulting from traumatic brain injury is much lower. It was demonstrated that TBI significantly increases the risk of occurrence of other mental disturbances, not directly related to TBI. Alcohol abuse and dependence is most frequent among these disturbances. This cre-ates a difficult challenge for psychiatric health care. Chronic post-TBI psychiatric disorders can significantly compromise the process of rehabilitation. Therefore, it is important to establish the diagnosis of neuro-psychiatric manifestations resulting from TBI in order to implement appropriate psychological and psycho-pharmacological therapy facilitating rehabilitation and reducing post-traumatic deficits in TBI patients. In this review, the most frequent psychiatric disturbances occurring in post-TBI patients, potentially compromi-sing the process of comprehensive rehabilitation, are described.

Urazy czaszkowo-mózgowe są uważane za główną przyczynę Śmiertelności w krajach Europy Zachodniej i USA. Istnieje ciągła potrzeba poszukiwania nowych, bardziej efektywnych metod terapeutycznych. W artykule przedstawiono stan aktualnej wiedzy na temat urazów czaszkowo-mózgowych związanych z pracą. Omówione zagadnienia wskazują na konieczność prowadzenia dalszych badań oceniających występowanie urazów czaszkowo-mózgowych związanych z pracą, czynników ryzyka oraz sposobów ich prewencji, a także powodowanych przez nie skutków długoterminowych.
Traumatic brain injury (TBI) is regarded a major cause of mortality in Western Europe and the USA. There is need for novel, more efficient therapeutic methods. This paper presents an overview of current knowledge on work-related traumatic brain injuries. It show the need to evaluate the incidence of work-related traumatic brain injuries, risk factors, ways of prevention, and long-term outcomes.

This article describes new innovations for people with neurocognitive disorders, which are implemented not in England only but also throughout all the UK. Following the publication of a report in 2007 by the Comptroller and Auditor General National Audit Office on „Improving Services and Support for Persons with Dementia,” set up in 2009, the first National Dementia Strategy was launched under the theme „Living better with dementia. At this time, ambitious goals have been set that aim to improve the quality of life of people with dementia and their carers (Bourn, 2007). In 2012 Prime Minister David Cameron launched Prime Minister’s Challenge on Dementia. In response, the Alzheimer Society has launched a program to create a friendly environment for people with dementia under the banner of Dementia Friends. The English Department of Health has identified tasks for NHS Health EducationEngland to deliver the necessary skills to provide the best quality care for people with dementia (Health Education England, 2017) and the NHS England, which launched the Well Pathway for Dementia.

Motor vehicle crashes are one of the leading causes of traumatic brain injuries. Restraint systems of cars are evaluated by crash tests based on human tolerance data, however, the reliability of data currently used has been questioned several times in the literature due to the neglect of certain types of effects, injury types and uncertainties. Our main goal was to re-evaluate the currently applied risk curve by taking the previously neglected effects into account. Methods: In this paper, the probability of traumatic brain injury was determined by reliability analysis where different types of uncertainties are taken into account. The tissue-level response of the human brain in the case of frontal crashes was calculated by finite element analyses and the injury probability is determined by Monte Carlo simulations. Sensitivity analysis was also performed to identify which effects have considerable contribution to the injury risk. Results: Our results indicate a significantly larger injury risk than it is predicted by current safety standards. Accordingly, a new risk curve was constructed which follows a lognormal distribution with the following parameters: μLN = 6.5445 and LN = 1.1993. Sensitivity analysis confirmed that this difference primarily can be attributed to the rotational effects and tissue-level uncertainties. Conclusions: Results of the tissue-level reliability analysis enhance the belief that rotational effects are the primary cause of brain injuries. Accordingly, the use of a solely translational acceleration based injury metric contains several uncertainties which can lead to relatively high injury probabilities even if relatively small translational effects occur.
