Transplantation, one of the most intensively developing fields of medicine, is giving patients hope for the chance of "second life" and health. The 21st century medical achievements heralded to achieve more difficult challenges, such as face transplant or lower extremity transplant. Among the futuristic dreams of the scientists, one of the projects - the Head to Body Transplant, stands out as one of the most controversial surgeries of the century. The aim of this study was to present the current state of art in medical aspect on cephalic anastomosis, as well as ethical implications. Among articles in PubMed Medline database related to human cephalic anastomosis, head transplant, head to body transplant keywords, 15 articles were selected for further analysis. Reviewed research presents significant development in conduction of the human cephalic anastomosis project. Most of the crucial parts of the surgery, such as deep profound hypothermia, spinal cord anastomosis - the detachment and fusion with the fusiogenes particles, have been practiced on animal models. The more newest revelations are revealed to the public, the more ethical and technical concerns are raised, primarily in terms of one-center study aprroach, infavourable benefit-cost ratio and futuristic apprehensions of infertility, transsexual or even unnatural life prolongation reasons for performing this type of surgery.

Around 25,000 people annually worldwide are subject to the procedure of allogeneic hematopoietic stem cell transplantation (HSCT). As the number of procedures has been on the rise, post-transplant care is becoming an essential part of work at transplant centres and general practitioners’ offices. Advances in technology and supportive therapies extend long-term survival of patients after HSCT, which increases the risk of complications resulting from exposure before, during and after transplant procedures. The complications may lead to significant morbidity, deteriorated quality of life, and late mortality in patients who have received hematopoietic stem cells. Statistical analyses have shown that the survival rate of post-HSCT patients is ca. 30% lower than expected, and that infections, organ failure and secondary cancers lead to mortality in this population of patients.

Back pain and the accompanying complications isthe most common disorder observed in the contemporary world. They relate mainly to middle-aged people, adversely affect the quality of life of patients, therefore they were considered as a civilization disease. Undoubtedly, back pain is inseparably bound up with the states of depression, because the physical sensations have a huge impact on the psyche. The nursing practice aiming at the improvement of the quality of life can be very difficult, because it must be directed at the same time towards mitigating the effects of the disease in physical and emotional terms. It includes learning to cope with problems of everyday life, stress and suffering. This should include all the needs and values significant for the patient in question in such a way as to regain as far as possible previous active life.
Bóle kręgosłupa oraz powikłania im towarzyszące to najczęstsze spotykane schorzenie we współczesnym świecie. Dotyczą głównie osób w średnim wieku, wpływają niekorzystnie na jakość życia pacjentów, dlatego uznano ją za chorobę cywilizacyjną. Niewątpliwie ból kręgosłupa wiąże się nierozłącznie ze stanami depresyjnymi, gdyż doznania fizyczne mają ogromny wpływ na psychikę. Postępowanie pielęgniarskie dążące do poprawy jakości życia bywa bardzo trudne, gdyż musi być ukierunkowane na jednoczesne łagodzenie skutków choroby pod względem fizycznym, jak i emocjonalnym. Zawiera uczenie radzenia sobie z problemami życia codziennego, ze stresem i cierpieniem. Powinno obejmować wszystkie istotne dla opisywanej pacjentki potrzeby i wartości w taki sposób, aby odzyskać w miarę możliwości dotychczasowe aktywne życie.

One of the greatest achievements of modern medicine is the development of transplantology. It improved survival and quality of life of people with organ failure. It gives them hope for a new and better life. This issue of is controversial among the public as most organs are obtained from deceased donors. The procedure of organ procurement from a donor requires commitment of many individuals working in various institutions throughout the country. Hospitals have a legal obligation to report potential donors whoare usually managed in the intensive care units. Organ donors include mostly young people whose death is a shock to relatives, for whom it is hard to comment on the will of the patient regarding organ donation. A Local transplant coordinator is a person monitoring institutional organ donation within the administered area. They supervise activities related to the confirmation of brain death, notify the Local Organ Procurement Organisation (Poltransplant) about potential donors.They support colleagues in the care of the donors, organise and authorise taking organs from deceased donors in their institutions. Coordinators have the qualifications, knowledge and skills necessary to perform those tasks. They are the people who create the image of transplantology in society.
Jednym z największych osiągnięć współczesnej medycyny jest rozwój transplantologii. Dzięki niej poprawiła się jakość i przeżycie wielu osób, które nie mogą funkcjonować z niewydolnym narządem. Transplantacja jest dla nich nadzieją na nowe, lepsze życie. Tematyka ta wywołuje wiele kontrowersji wśród społeczeństwa, gdyż większość narządów pozyskiwana jest od osób zmarłych. Procedura pobrania narządu od dawcy wymaga zaangażowania w pracę wielu osób, pracujących w różnych instytucjach w całej Polsce. Szpitale mają prawny obowiązek zgłaszania potencjalnych dawców, którzy najczęściej przebywają w Oddziałach Intensywnej Terapii, gdzie roztacza się opiekę nad zmarłym potencjalnym dawcą narządów. Dawcy są to najczęściej osoby młode, a ich śmierć jest szokiem dla osób bliskich, którym ciężko wypowiedzieć się na temat woli zmarłego na dawstwo narządów. Lokalny koordynator transplantacyjny jest osobą, która zajmuje się monitorowaniem potencjału dawstwa na obszarze swojego działania. Czuwa nad sprawnym przebiegiem całości działań związanych z wysunięciem podejrzenia śmierci mózgu i jej potwierdzenia. Jego zadaniem jest zgłoszenie potencjalnego dawcy do Centrum Organizacyjno-Koordynacyjnego ds. transplantacji “Poltransplant” i dokonanie autoryzacji pobrania. Wspomaga współpracowników w opiece nad dawcą do momentu pobrania narządów. Organizuje pobranie wielonarządowe w swoim szpitalu. Posiada kwalifikacje, wiedzę i umiejętności niezbędne w realizacji wyznaczonych zadań. Jest to osoba, która kreuje wizerunek transplantologii w społeczeństwie.

The occurrence of primary ureteral carcinoma of the remnant ureter after nephrectomy for benign disease is very rare, and such cases in a kidney transplanted patient are even rarer. The case of a 62-year-old female patient is presented who had end stage autosomal dominant polycystic kidney disease and in 1991 underwent cadaveric kidney transplantation. Immunosuppression therapy was maintained with evorolimus and prednisolone. Normal serum creatinine levels were maintained and no signs of allograft rejection were detected. In 2010, the patient developed intermittent macroscopic haematuria and was admitted for further evaluation. Computer tomography, cystoscopy, laboratory tests and urine cytology were performed and no apparent reason for the haematuria was found. After more episodes of uncontrollable haematuria, in 2011 she underwent bilateral nephrectomy of her native kidneys. In 2015, after another episode of haematuria, a CT was performed on the patient. She was diagnosed with a solid enhancing mass in the right ureteral stump. Ureterectomy with bladder cuff excision was performed with lower midline laparotomy. The specimen was verified as high grade urothelial carcinoma, in stage T1 according to the 2004 WHO grading system. To the best of the authors’ knowledge, this is first case in the availabe literature of the occurrence of transitional cell carcinoma developing in the remnant ureter in a kidney transplanted patient after bilateral prophylactic nephrectomy for uncontrollable haematuria – 24 years after transplantation.

The aim of the research paper was to identify the relationship between a chronic cardiac disease and heart transplantation, and the level of patients’ selfcare. The notion of self-care and its levels has been defined, and the circumstances in which it becomes activate specified. Research findings showed that there is no difference in the level of self-care between the group of patients with a chronic heart condition and the group of patients with a heart transplant, and between cardiac patients and healthy individuals. The difference showed in the higher global level of self-care in patients after heart transplantation compared to healthy individuals. The research revealed that a heart transplantation treatment has a direct effect on the scope of self-care in patients. It can therefore be concluded that in the context of changes following a cardiac transplantation, self-care is activated as a buffering mechanism, facilitating a better adaptation of a patient. Yet, it cannot be stated that self-care is a rational mechanism consciously used in order to cope with the situation.

Introduction and aim. In patients waiting for organ transplantation, increased waiting time can increase anxiety by creating negative automatic thoughts. The aim of this study is to examine the relationship between corneal transplant recipients’ negative automatic thoughts and anxiety before organ transplantation. Material and methods. The population of this correlational descriptive study consisted of 108 patients waiting for cornea transplantation in the Eye Bank unit of a hospital in the south east region of Türkiye. The data were obtained from the patients presented to the ophthalmology clinic for transplantation between September and December 2022. A patient information form, the Negative Automatic Thoughts Scale, and the Trait Anxiety Scale were used to collect the data. Percentage distribution, arithmetic mean, logistic regression and correlation analysis were used in the data analysis. Results. It was determined that the mean score of the negative automatic thoughts of the transplant patients participating in the study was 100.69±47.83, and the trait anxiety mean score was 53.06±6.07. There was a positive relationship between negative automatic thoughts and trait anxiety at a moderate level, which was statistically significant (p<0.05, r=0.53). Conclusion. It was determined in the study that the patients’ negative automatic thoughts and trait anxiety were high, and their anxiety increased as the negative automatic thoughts increased.

Doxorubicin chemotherapeutic agent is widely utilized for many types of cancers since the late 1960s. Cardiomyopathy is a well-known side effect of doxorubicin often limiting its use. In many cases doxorubicin cardiomyopathy can lead to end stage cardiac failure requiring heart transplantation. The quality of life of heart transplant patients is exceptional with most patients being able to continue normal activities following recovery. There has been significant advancement in cardiac transplantation since it was first attempted in 1967 in Cape Town, South Africa. Drugs such as cyclosporine played an important role in preventing graft failure and prolonging patient survival. Cardiac transplant can extend a patients’ life by over a decade. The patient in this case, Mr. Glen Frank Spurling, has survived 30 years following his cardiac transplant surgery. In this article an overview of doxorubicin cardiotoxicity, cardiac transplantation, and an interview with Mr. Glen Frank Spurling is presented.

Objectives The aim of the study is to describe both professional and social activities of patients after heart transplant. Material and Methods Ninety-five heart transplant patients treated at the Silesian Center for Heart Diseases in Zabrze were surveyed, comprising 29 women (30.5%) and 66 men (69.5%). The average age of respondents was 54.3 years old (standard deviation (SD) = 15 years); the average period that had elapsed since the heart transplant was 7.1 years (SD = 4 years). We designed a questionnaire as a tool for collecting information from patients. Results Twenty-five percent of patients worked at the time of completion of the questionnaire. Eighty percent of those patients were working before and after the transplant, 20% – only after transplantation (p < 0.05). A different job position at a new workplace had 47.8% of patients, 34.8% of them had the same job position at the same work place as they had had before, 63.4% of the heart transplant respondents were pensioners. Eighty-two percent of patients had a certificate with a designated degree of disability – among them: 69% had a certificate for a significant degree of disability, 22% – for a moderate degree of disability. Among those surveyed, 52.5% said that their financial situation had not changed whereas 34.5% of those surveyed reported a change for the worse. Thirty-seven percent of respondents reported changes in family relationships. Seventy-seven percent reported that they received help from family members, as compared with 19% who did not. Conclusions Only 25.3% of the patients treated at the Silesian Centre for Heart Diseases after heart transplant are employed and it is one of the lowest employment rates in this category of patients in Europe. One third of working patients have the same work place as they had before their operation. Heart transplant is a cause of changes in family relationships. Most often family bonds are strengthened but sometimes family members become nervous, impatient and unwilling to talk about the transplant.

Transplantology is constantly evolving. New challenges are emerging, including those related to transplants after suicide. Concerned for the welfare of donor families who have experienced the trauma of a loved one’s suicide, it is important that the donation decision process be as minimally burdensome as possible. Therefore, it is important to raise public awareness of organ donation in the context of suicide. Educating the public can help break down the barriers and false beliefs about the process. Although suicide is a profoundly tragic event, the opportunity to donate an organ to save another person’s life is extremely valuable. The purpose of this paper is to show the legal and ethical treatment of organ transplantation from suicidal patients. The article presents a case report of a person who committed suicide, but for whom resuscitation was undertaken in the prehospital setting, despite severe craniocerebral trauma, and a return of spontaneous circulation was achieved. Subsequent in-hospital efforts made it possible to successfully maintain vital functions and declare brain death. The commitment of the medical personnel and the professional discussions with the patient's family made it possible to obtain consent for the donation of seven organs.

Introduction. Drug–induced gingival overgrowth is a condition caused by side effects of treatment with one of three types of drugs: phenytoin (used in epilepsy treatment), cyclosporin A (used in transplantology after allogeneic organ transplants) and calcium channel blockers (in the treatment of hypertension). Gingival overgrowth leads to the development of inflammation within the gums and periodontium, reduced comfort in a patient’s life, and consequently even loss of teeth. Aim. The aim of this study was to present the issue of drug–induced gingival overgrowth based on a review of the literature and observations of patients treated in the Clinical Department of Maxillo-Facial Surgery, Frederic Chopin Provincial Specialist Hospital in Rzeszów. Case description. Massive gingival overgrowth requires surgical management. Attention should be paid to multidisciplinary cooperation in case of patients qualified for a transplant. It is also import_ant to qualify and evaluate the state of the oral cavity prior to the implementation of immunosuppressive medication, instruction of patients on oral hygiene and removal of the outbreaks of infection.
