So far applanation tonometry has not worked out any theoretical basis for correcting the result of intraocular pressure measurement carried out on a cornea with noncalibration dimensions by means of the Goldmann tonometer. All the tables of instrument reading corrections for cornea thickness or cornea curvature radius are based exclusively on measurements. This paper represents an attempt at creating a mechanical description of corneal apex deformation in Goldmann applanation tonometry. The functional dependence between intraocular pressure and the pressure exerted on the corneal apex by the tonometer was determined from a biomechanical model. Numerical GAT simulations, in which this function was also interrelated with the cornea’s curvature radius and thickness were run and a constitutive equation for applanation tonometry, i.e. a full analytical description of intraocular pressure as a function of the above variables, was derived on this basis. The correction factors were defined and an algorithm for correcting the measured pressure was formulated. The presented formalism puts the results of experimental tonometry in new light. Analytical correction factors need not to come exclusively from measurements. A geometric interdependence between them and their dependence on pressure have been revealed. The theoretical description of applanation tonometry contained in the constitutive equation consists of a pressure function developed for a cornea with calibration dimensions and a coefficient correcting this calibration function, dependent exclusively on the cornea’s actual thickness and curvature radius. The calibration function is a generalization of the Imbert–Fick law.

The assessment of corneal biomechanics is essential for studying ophthalmological operations, such as refractive surgeries, and for more accurate estimation of intraocular pressure. The chief aim of the current study is to characterize corneal and fatty tissues in order to construct a model to predict eye globe behavior during dynamic tonometry tests. Methods: In the present study, images from corneal deformation, acquired from Corvis ST tonometer, were processed. Then, corneal pure displacement and eye globe retraction were calculated. Utilizing inverse finite element method, corneal material properties were calculated in order to predict pure deformation obtained from Corvis ST. Using a similar approach, material parameters of fatty tissue were estimated in order to predict the eye globe retraction. The model used for fatty tissue was considered as corneal boundary condition in a forward finite element model to create a joint model, which could simulate corneal behavior in dynamic tonometry tests. Results: It was shown that an isotropic material model is accurate enough to predict corneal deformation in dynamic tonometry tests. Moreover, effects of IOP on the estimated material properties were investigated. Finally, utilizing the joint model, it was demonstrated that there is strong correlation between corneal stiffness and the biomechanical parameter introduced by Corvis ST. Conclusions: An eye globe model was constructed and characterized by two distinct inverse models for corneal and fatty tissue. This model can be utilized for predicting eye globe behavior during dynamic tonometry tests besides other ophthalmological operations.

Nadciśnienie tętnicze jest poważnym i powszechnym problemem zdrowotnym i odpowiada za znaczną część udarów mózgu czy zawałów mięśnia sercowego. Podstawową metodą pomiaru ciśnienia jest pomiar ciśnienia obwodowego na tętnicy ramiennej. Jednak z ryzykiem możliwych powikłań choroby nadciśnieniowej lepiej koreluje ciśnienie centralne, czyli to panujące w aorcie i tętnicach szyjnych. Obecnie technologia pozwala na dokonanie pomiaru ciśnienia centralnego w sposób szybki i nieinwazyjny, lecz dostępność badania jest ograniczona. Określenie ciśnienia centralnego pozwala na precyzyjniejszy i bardziej adekwatny dobór leczenia hipotensyjnego u pacjenta, co przełoży się na dalszą redukcję ryzyka sercowo-naczyniowego.
Arterial hypertension is very important epidemiological issue, because it is mainly responsible for stroke and myocardial infarction. Blood pressure is commonly measured on the arm by means of pressure cuff. However, central blood pressure in aorta or carotid arteries, is better correlated with cardiovascular risk. Modern method of measurement of central blood pressure is applanation tonometry, which is not commonly available. The value of central blood pressure allows for better choice of effective hypotensive therapy, which decreases risk of important cardiovascular events.

Introduction and objective. Physical effort plays a positive role in reducing the risk of cardiovascular diseases. The aim of this study was to assess the cardiovascular status in postmenopausal women after several years of regular amateur training. Materials and method. A total of 55 generally healthy females aged 50–70 years, of whom 38 were members of a senior exercise group and 17 comprised a control group, were enrolled in the study. Parameters of blood flow, vascular resistance, myocardial contractility and thoracic fluid content were measured in a 10-minute supine resting test by impedance cardiography. Thereafter, central blood pressure, augmentation index and pulse wave velocity were measured by applanation tonometry. Results. Exercising women have a better outcome than the control group, when evaluated both with impedance cardiography and with applanation tonometry. They have a lower heart rate – HR (65.1 vs 71.5; p=0.033), higher blood flow (stroke index – SI, 58.6 vs 50.3; p=0.040), better myocardial contractility (acceleration index – ACI, 108.8 vs 88.1; p=0.027), higher preload (thoracic fluid content index – TFCI, 20.5 vs 18.1; p=0.002), lower afterload (systemic vascular resistance index – SVRI, 1972.9 vs 2110.5; p=0.026), lower central systolic blood pressure – cBPsys (119.0 vs 129.5; p=0.037), lower augmentation pressure – AP (10.3 vs 15.0; p=0.044) and lower pulse wave velocity – PWV (7.4 vs 8.4; p=0.001). Conclusions. Regular moderate continuous aerobic exercise training has a beneficial impact on the cardiovascular system in postmenopausal women.

Purpose: Presented study describes new parameters calculated from the Ocular Response Analyzer (ORA) raw data. Such an approach can increase the applicability of the ORA in ophthalmic diagnosis. Among many proposed and examined by us parameters from raw data of the air pressure and applanation curves, only a few were chosen and then applied for characterizing a selected group of patients. Methods: The study included healthy subjects in a control group and patients divided into 2 groups: suspect and glaucoma. A series of four ORA measurements were taken from each subject. The raw ORA data were numerically analyzed and new parameters were calculated from the ORA curves for each measurement. Comparative analysis was carried out for the newly proposed parameters (and original parameters from the ORA device). Results: This interesting finding is that the new parameters showed a statistically significant ability to distinguish the glaucoma suspect group from healthy and glaucomatous patients. Moreover comparable or higher repeatability than for IOPg and CH was obtained. Conclusion: Raw data from the ORA enables definition and numerical analysis of new parameters, characterizing every measurement, which can be successfully used for describing an individual eye and differentiating between some specific groups of patients.
