Introduction. Infliximab is a monoclonal antibody that acts against tumor necrosis factor TNF-α. The drug is used in the treatment of autoimmune diseases. Aim. This article reviewed the efficacy and safety of infliximab for the treatment in severe ulcerative colitis. This review included studies that evaluated the clinical use of infliximab. Material and methods. This meta-analysis was performed according to systematic literature search of three major bibliographic databases (Scopus, PubMed, and Cochran). Results. Infliximab has been approved by the US Food and Drug Administration (FDA) as a medicine to treat Leśniowski and Crohn’s disease, ulcerative colitis, psoriasis, psoriatic arthritis, ankylosing spondylitis, and rheumatoid arthritis. However, further trials are required to compare other parameters of efficacy such as the clinical response with infliximab. Conclusion. In patients suffering from Crohn’s disease or ulcerative colitis under infliximab maintenance therapy, sustained good trough levels are associated with: better response and remission rates, more mucosal healing and less loss of response.

Regulatory effect of CD25, an activation antigen the α subunit of interleukin 2 receptor (IL2R) on the activity of natural killer (NK) cells was studied in fifty elderly (57-70 years old) and fifty young people (19-35 years old). Cytotoxic NK activity was assessed by 51Cr release assay, the levels of interleukin 2 (IL2) and tumour necrosis factors α (TNFα) were measured using bioassays and expression of CD16 and CD25 proteins by flow cytometry. Low NK activity in the elderly was associated with decline of full health, lowered serum concentration of IL2 and increased production of TNFα during NK reaction. Inhibition of TNFα activity by anti-TNF monoclonal antibody suppressed exclusively NK activity of low NK responders. Moreover, stimulation in vitro of blood mononuclear cells, with TNFα induced in the elderly low NK responders a significantly higher increase of the CD25 expression on the surface of NK cells as compared with that in the elderly high responders. Since the CD25 molecule constitutes a subunit of the high affinity receptor, binding IL2 to immunocompetent cells, its increased expression on NK cells of low NK responders would enable them to bind even low amounts of the endogenous IL2 available in this group of the elderly. Thus, an overproduction of TNFα seems to be a mechanism compensating, in the non-fully healthy elderly, for the decreased IL2 production, promoting efficient cytotoxic reaction.

Nieinfekcyjne zapalenia błony naczyniowej (NZBN) stanowią problem diagnostyczny i terapeutyczny, mają przewlekły i kumulacyjny charakter, a także są przyczyną znaczącej utraty funkcji wzroku u młodych osób, uniemożliwiającej aktywność zawodową. Diagnostyka i leczenie wymagają współpracy specjalistów z różnych dziedzin i opierają się na licznych badaniach laboratoryjnych i obrazowych. Praca przedstawia wskazania, efektywność i bezpieczeństwo terapii pierwszym lekiem biologicznym stosowanym w okulistyce, jakim jest inhibitor czynnika martwicy guza α, adalimumab. Może być on uznany za istotną opcję terapeutyczną w leczeniu zapalenia błony naczyniowej części pośredniej i tylnej, a także zapalenia wszystkich części błony naczyniowej, ponieważ pozwala pacjentom na osiągnięcie i utrzymanie długotrwałej kontroli nad chorobą przy jednoczesnej terapii glikokortykosteroidami czy lekami immunomodulującymi lub jej braku.
Noninfectious uveitis is known for its chronic and cumulative nature. It is often associated with diagnostic and therapeutic difficulties. It causes significant vision loss in young patients, making them incapable of maintaining an active work life. Diagnosis and treatment require cooperation between different specialists and are based on multiple blood tests and other diagnostic tests. This article presents medical indications and the safety and efficacy of adalimumab, a tumor necrosis factor inhibitor and the first biological drug used in ophthalmology. Adalimumab is an important therapeutic option for the treatment of intermediate, posterior, and pan-uveitis, enabling patients to obtain and maintain long-term disease control during simultaneous corticosteroid/immunomodulatory therapy or without it.

The studFigy aims to determine the TNF-α single-nucleotide polymorphism TNF-α (-308) and assess the association of TNF-a(-308) SNP with the risk of silicosis among workers directly exposed to silica dust in Vietnam. A study was undertaken among 78 cases with silicosis and 103 controls without silicosis in Vietnam. Blood samples were collected for genomic DNA extraction from each subject. The phenotyping of TNF-α(-308) was performed using polymerase chain reaction‐based restriction fragment length polymorphism (PCR‐RFLP) and dye termination sequencing. Results: The average exposure time of the case group was slightly higher than that of the control group (12.46 ± 6.732 years vs. 12.09 ± 7.854 years). The majority of genotypes in both silicosis and non-silicosis was GG. When analyzing the concentration of TNF-α in the study participants' blood, it is shown that the average concentration of TNFα in the case group was higher than that in the control group. The genotype AG in the case group was 1.368 times higher than that in the control group. The percentage of all A alleles in the case group with silicosis was 1.342 times higher than the control group without the disease, similar to previous studies. Conclusion: The majority of genotypes in both groups was GG. The average concentration of TNF-α in blood, genotype AG, and the percentage of all A alleles in the case group was higher than that in the control group.

We have shown before that the expression of ZNF334 gene, coding for a newly described zinc finger protein of as yet unknown function, is extremely reduced in CD4+ lymphocytes of rheumatoid arthritis (RA) patients regardless of their age, and thus can be considered a new molecular marker of the disease. Based on the promoter sequence of the gene we speculated that it might be regulated by TNFα. Here we have tested that hypothesis, studying the in vitro influence of TNFα on the ZNF334 gene expression and protein levels in resting and stimulated CD4+ cells of healthy volunteers. We have confirmed that treatment with TNFα modifies the levels of ZNF334 expression in the CD4+ cells ex vivo; however, the effect varied for different individuals and reduction of expression was seen only for those cell samples that initially exhibited high transcriptional activity of the gene, while for those exhibiting initially very low expression, some increase in the transcriptional activity was observed. Incubation with TNFα significantly reduced the amounts of two isoforms of ZNF334 protein (initially high in all subjects) in parallel to the reduced transcription. Finally, the expression of ZNF334 in CD4+ lymphocytes isolated after various periods of anti-CD3 stimulation generally increased with longer culture times, and the effect of TNFα treatment was negligible. Concluding, our results obtained in vitro for helper lymphocytes of healthy individuals seem to mimic the regulatory effect of TNFα on the expression of ZNF334 in the cells of RA patients.

TNF-α protein and caspase-3 in mudskipper muscle can be used as a biomarker to evaluate the environmental conditions. Therefore, this study aimed to analyze the heavy metal contents, specifically Cr and Fe, in the sediments from the mangrove area, as well as the expression of TNF-α and caspase-3 in mudskipper muscle. The samples used were the sediments and mudskippers from the waters of Poka, Waai, and Rutong Villages. The heavy metals were analyzed using Atomic Absorption Spectrophotometry (AAS), while the TNF-α and caspase-3 expressions were assessed with immunohistochemical staining. Observations were carried out using an optical microscope with 1000x magnification. The results showed that the proportion of Fe accumulated in mudskipper at the three locations was greater than Cr, while Cr was higher in the sediment compared to Fe. Moreover, the TNF-α and caspase-3 expressions appeared in muscle organs in varying amounts. This indicates that Cr and Fe under excessive conditions can be toxic to mudskipper causing muscle inflammation and cell death, which leads to the expression of TNF-α and caspase-3.

Opis:
Objectives To investigate the influence of the anti-tumor necrosis factor-α infliximab (IFX) in the case of rats with silicosis. Material and Methods Forty-eight Wistar rats were randomly divided into 3 groups. The study group (N = 16) – silicosis was induced by intratracheal instillation of 50 mg silica on day 1, and IFX was subcutaneously administered at a dose of 15 mg/kg of body weight from day 2 to day 6, the vehicle group (N = 16) – silica used as the study group but without IFX, the sham group (N = 16) – 1 ml of saline was intratracheal-used. Eight rats in each group were euthanized on day 7 and on day 14, respectively. Lung tissue sections were stained with hematoxylin and eosin or Masson’s trichromedye. The nuclear factor-κB p65 (NF-κB p65) positioning in the lung tissues were determined by immunohistochemical staining. Levels of tumor necrosis factor α (TNF-α) in rat serum and bronchoalveolar lavage fluid were measured with enzyme linked immunosorbent assay. The inducible nitric oxide synthase (iNOS) mRNA in the lung tissues was measured by quantitative real-time polymerase chain reaction, as well as inhibitor protein-κB (I-κB) and NF-κB p65 expression were measured quantitatively by western blotting. Results Silica installation increased the lung tissues inflammation reaction, oxidative stress and pulmonary fibrosis. Infliximab treatment significantly improved silica-induced lung pathological changes (inflammatory cells, collagen deposition), decreased the TNF-α inhibited NF-κB signaling (I-κB, NF-κB p65) as well as oxidant status (iNOS). Conclusions Infliximab may improve silica-induced pulmonary inflammation by decreasing the TNF-α, inhibiting NF-κB signaling (I-κB, NF-κB p65) as well as oxidant status (iNOS), which suggest that IFX has potential role in the treatment of silica-induced lung damage. Introduction and aim. Tumour necrosis factor-alpha (TNF-α) belongs to the cytokine family TNF/TNFR. As a multifunctional cytokine, TNF-α plays a significant role in diverse and a variety of cellular events such as cell survival, proliferation, differentiation, and death. As a pro-inflammatory cytokine, TNF-α acts as a bridge between inflammation and carcinogenesis. Receptor for advanced glycation end products (RAGE) are cellular receptors belonging to the immunoglobulin superfamily. As one of the primary mediators of innate immunity, acute and chronic inflammatory disorders, and certain cancers, RAGE signaling plays an important role. The aim of the present study is to analyze the prognostic significance of salivary TNF-α and RAGE in oral squamous cell carcinoma. Material and methods. A study was conducted testing saliva samples collected from ten patients with well-differentiated and moderately differentiated oral squamous cell carcinomas. To determine the levels of TNF-α and RAGE in unstimulated saliva from patients, an ELISA kit from RAY BIOTECH was used for the study, and the readings were read at 450 nm. Statistical analysis was performed using SPSS software. Version 23 of SPSS was used to plot the standard curve. Statistical comparisons were done using Mann-Whitney U test and ROC analysis. Results. Salivary TNF-α and RAGE in patients were considered to be induced by radiotherapy at a higher level in moderately differentiated squamous cell carcinoma when compared to well differentiated squamous cell carcinoma. Thus, there is an increase in the induced Salivary TNF-α and RAGE levels by radiotherapy with increase in the histological stages of oral squamous cell carcinoma. The statistical analysis also proved the same. Conclusion. Hence salivary TNF-α and RAGE may be used as a biomarker for oral cancer to predict the prognosis.
