Introduction: Giant cell tumor of the tendon sheath is the most common benign proliferative lesion involving the upper limb, characterized by relatively high recurrence rate after surgery. Aim: The objective of the study was a retrospective analysis of outcomes of the operative treatment of these tumors, in a longterm (a mean of 4,2 year) follow-up. Material and methods: Preoperative examination was performed in 58 patients, 36 females (62%) and 22 males (38%), in a mean age of 41 years, and treatment outcomes were assessed in 47 persons (81% of the operated patients), at a mean of 4.2-year follow-up. The final assessment was performed in a form of phone interview. Results: The most common site of the tumors was the fingers – 42 cases (72%). In 31 patients (53%) the lesion had a well-defined capsule, and in 11 (19%) a satellite nodule was found around the main tumor. A total of 9 relapses (21%) occurred, all within the first 2 years following surgery. Two patients had a next episode of recurrence after the second operation. In 8 out of the 9 patients with a recurrence, the primary lesion did not have a well-defined capsule. In 38 patients who had no relapse, 31 were completely symptom-free, whereas 7 complained of mild pain of the scar and/or numbness of a part of the involved finger. Conclusions: The main factor that impacted the high rate of recurrence was incomplete tumor excision, which resulted from inadequately accurate surgery and the tumor morphology (having no well-defend capsule). The role of operating with the use of magnifying devices and keeping a greater surgical margin at resection of the non-capsulated lesions was emphasized, as it could translate into reducing the recurrence rate.

In this project the anterior cruciate ligament (ACL) replacement in a rabbit model was performed for the preclinical phase of investigations of a new form of biomaterial i.e. polymer beads (granules). This material, based on resorbable aliphatic polyester (polylactide - PLA) was used as a filler of the bone tunnels to enhance tendon-to-bone healing. The high specific surface area and well known biocompatibility of this material designated it as an osteoconductive agent in reconstructive ligament surgery. Additionally, a new surgical concept was proposed which retains the natural elasticity of the harvested tendon. This method ensures fixation of the grafted tendon within the bone tunnels in their entire length.

Delayed tendon healing is still found to be among the complications that occur most often after tendon repair. The role of local injection of Artemisia absinthium was evaluated in healing of experimental Achilles tendon injury in rabbits. In nine adult New Zealand rabbits a partial thickness tenotomy was created on both hindlimbs. A. absinthium extract and normal saline were respectively injected daily to treatment and control groups for three days. On the day 7 after injury, the tendon sections showed that healing rate in A. absinthium treated group was higher than that in control group. Furthermore, at days 14 and 28, comparison between A. absinthium treated group and control group demonstrated that A. absinthium increased the healing rate but with no significance. Results of this study have showed that application of A. absinthium extract can improve healing process of damaged Achilles tendon.
Powolne gojenie jest jedną z komplikacji pojawiających się najczęściej po operacji odbudowy ścięgna. Badano wpływ miejscowego podania Artemisia absinthium w iniekcji na gojenie doświadczalnego uszkodzenia ścięgna Achillesa u królików. U dziewięciu dorosłych królików nowozelandzkich przeprowadzono częściową tenotomię ścięgien obu kończyn tylnych. Następnie podawano w iniekcji wyciąg z A. absinthium lub sól fizjologiczną odpowiednio w grupie doświadczalnej i w grupie kontrolnej przez trzy dni.Siódmego dnia po uszkodzeniu okazało się, że tempo gojenia w grupie leczonej wyciągiem z A. absinthium było wyższe niż w grupie kontrolnej. Co więcej, porównanie przeprowadzone 14. i 28. dnia wykazało, że podanie A. absinthium przyspieszyło gojenie, ale nieistotnie.Wyniki tego badania pokazują, że podanie wyciągu z A. absinthium może przyspieszyć process gojenia uszkodzonego ścięgna Achillesa.

Artykuł przedstawia wybrane problemy projektowe i wykonawcze prefabrykowanych zbiorników cylindrycznych sprężonych zewnętrznymi cięgnami bez przyczepności. Analizowano zbiorniki na ciecze, których ściany stanowią prefabrykowane elementy żelbetowe użebrowane wzdłuż zewnętrznych pionowych krawędzi. W rozwiązaniu takim pionowe styki prefabrykatów wypełniane są betonem ekspansywnym lub materiałami żywicznymi. W pracy poruszono problem szczelności ścian takich zbiorników, połączenia ściennych elementów prefabrykowanych z dnem, kolejność montażu ścian i kolejność ich sprężania. W podsumowaniu, na podstawie wiedzy literaturowej i doświadczenia autorów, podano wskazówki praktyczne do projektowania i wykonawstwa takich obiektów.
The paper presents selected problems of designing and executing precast cylindrical tanks prestressed with external unbonded tendons. Tanks for fluids with walls made of precast reinforced concrete elements ribbed along external longitudinal edges were analyzed. In such solution vertical joints of precast elements are filled with expanding concrete or resin materials. In the paper as problems as leaktightness of tanks walls, joints of precast wall elements with bottom, sequence of walls executing and sequence of prestressing are brought up. In a summary practical recommendations based on literature and authors' experience for designing and executing that kind of structures are given.

The mechanisms of pain in tendinopathy are unclear. Current theories implicate tendon structural changes, neovascularisation, inflammation or changes in central pain processing. As with other types of musculoskeletal pain, tendon pain has high interindividual variability and, as with other types of pain, this could be attributed to genetic variation. Notably, the association between certain genetic polymorphisms and susceptibility to tendinopathy is well established in the literature. Therefore, the investigation of the mechanisms of tendon pain should also extend to include genetic variation as a possible explanation for the clinical features of tendon pain. This review summarises the current knowledge on genetic contributors to chronic pain and highlights findings that are relevant to chronic tendon pain. In particular, based on the current hypotheses on the possible sources of tendon pain, it focuses on findings that relate to genes that encode structural connective tissue components, inflammatory markers, ion channels and catecholamines and how they may relate to chronic tendon pain. In the absence of a definitive mechanism of tendon pain, an a priori genetic approach that is guided by these current hypotheses may help elucidate the mechanisms of tendon pain which may allow a more rational approach to research and treatment.

Fibroma of the tendon sheath (FTS) is a rare, benign, soft tissue lesion. Clinically, FTS presents similarly to the more common giant cell tumor of the tendon sheath. It is distinguished histologically by the lack of giant cells, foamy histiocytes and synovial cells. We presented a case of FTS involving the common tendon sheath surrounding the flexor tendons leading to the third metacarpal. A 63-year-old man presented with a 3-month history of a painless mass in his right palm that had recently tripled in size. Examination demonstrated a 5x4 cm firm, nodular, superficial mass that was adherent to the overlying skin. Radiographs of the hand revealed a soft tissue mass without bony abnormality. Ultrasound demonstrated a solid, heterogeneous and hypoechoic mass and computed tomography demonstrated that the mass centered predominantly at the mid and distal portions of the third metacarpal. The patient underwent excisional biopsy of the lesion and a palmar, longitudinal incision was made from the wrist to the third metacarpal. Submitted histologic sections revealed a well-circumscribed lesion closely resembling hyalinized collagen. Neither vascular proliferations, necrosis, nor mitoses were observed. Similarly, multinucleated giant cells, pigment-laden macrophages, and inflammatory cells were also not identified. A diagnosis of FTS was rendered. We provided an additional rare case to the literature of a FTS and highlight the need to consider this entity in the differential diagnosis for any soft tissue lesion in the hand. Three months post surgery the patient demonstrated full range of motion of the hand

Giant cell tumor of tendon sheath is a rare and benign so-fttissue tumor, which is even less common in the pediatric population. It may be connected with chromosomal translocations, oeftn the 1p11-13. Symptoms include pain, swelling, palpable mass or limited range of motion. Magnetic resonance is the diagnostic method of choice. Histopathological image obtained from biopsy should be also evaluated. The gold standard of treatment is total surgical resection of the tumor, as incomplete excision may result in recurrence. The aim of this study was to evaluate the results of early and delayed surgical treatment and the rehabilitation of patients with traumatic injury in zone I of the extensor tendon of the fingers II–V. 47 patients after traumatic, closed damage of the extensor tendons of the fingers II–V of the hand were treated and examined. 17 women (36.2%) and 30 men (63.8%) aged 14–80 years were included in the study. Patients with a delayed first degree damage of the extensors tendon, as well as fourth degree damage, according to the Doleyle scale qualified for surgical treatment. Surgical treatment consisted of suturing the tendon band or restoring its attachment to the phalanx bone, as well as the arthrodesis of the distal interphalangeal (DIP) joint with Kirschner wire in extension. The wire was removed after 6 weeks. Rehabilitation treatment was carried out in order to restore a full range of motion of the fingers. Patients were under constant supervision of the medical team. The examination of the patients took place before and 3 months after the surgery. The presence of the pain was assessed by means of a 10-point VAS scale (Visual Analog Scale). A goniometer was used to measure the range of motion of patient’s fingers. Crawford’s scale was used to assess the results of treatment of injuries to the extensor tendons of the fingers. The early stages of treatment to these yielded excellent results in 84.2% of patients, 14.3% good results, 17.8% of satisfactory results and 3.6% of patients had poor results. The differences in the results were not statistically significant. Further operative intervention should be considered for patients with extensive damage to the tendon of the extensor finger with a greater detachment of a fragment of phalanx bone shortly after the injury. In patients with extensive damage to the tendon of the extensor finger with greater detachment of bone fragment of phalanx further surgery in the early period after injury should be considered.

Tendon injury is an increasing problem in medicine due to aging of the population and increased activity demands. Many rodent animal models are used in order to evaluate tendon reconstruction. Although tendon reruptures are a known problem, the outcomes of tendon repair in animal models are rarely discussed in the literature. The goal of the present experimental study was to compare the primary fixation stability of three suture techniques for repair of Achilles tendon defects in a rat model using a collagen scaffold. Methods: Cadaveric left hind limbs of Sprague-Dawley rats were prepared with an Achilles tendon defect of 3 mm and rejoined using a collagen scaffold. Three suture configurations (simple, simple stitch with additional framing suture, and modified Mason–Allen stitch; n = 5 each) underwent tensile testing until complete failure was observed. Results: Under a load of a mean value of 6.6 N, the failure load of simple stitches was the significantly lowest ( p < 0.01). Both, modified Mason–Allen stitches and simple stitches with additional framing suture showed a mean failure load of more than 14 N. Regardless of the suture technique, most of the samples showed failure of tendon due to suture tear-out. The suture material as well as the scaffold remained mostly intact. Conclusions: Although simple end-toend suture techniques are common in the literature, stitches with more suture strands should be preferred. Using techniques like an additional framing suture or modified Mason–Allen stitch, maximum failure load can be doubled and the risk of tendon rerupture may be decreased in vivo.

The paper discusses a biomechanical model reflecting deformation and tear of a tendon part, namely, ellipric in cross-section strand formed by stochastically located collagen fibers whose strain to stress relation obeys the exponential law. Recovery of the tendon by a plastic reinforcement is shown to result in elevated rigidity and shorter limiting elongation of the strand proportionally to the reinforced portion length, the strength of the restored strand being preserved almost fully. The limiting deformation of the recovered strand made with incisions for adaptation of the ends increases the more the deeper are the incisions, their number and the total length of the areas being adapted. This, nevertheless, decreases essentially the breaking load during further loading.

The thumb constitutes 40% of the whole hand function, and damage to the extensor pollicis longus (EPL) and/or the flexor pollicis longus (FPL) tendons of the thumb results in its significant limitation. The main factors contributing to damage of the EPL and/or FPL tendons are mechanical injuries - cuts and spontaneous ruptures. The aim of the study was to compare values of thumb and whole hand function loss and restoration as a result of the implemented physical therapy in patients with total damage to the EPL tendon with the values obtained by the study group 2 comprised of patients with damage to the FPL tendon. The study involved 25 patients of the Specialized Hand Therapy Center in Krakow. 15 of them had ruptured continuity of the EPL tendon (study group), and 10 suffered damage to the FPL tendons (control group). The study included measurements of active motion of the thumb and wrist and superficial sensation. On the basis of these tests, functional impairment was measured using the methodology according to Swanson. The study included assessment of muscle strength in terms of global and precision grips using a dynamometer. Statistically signifcant functional improvement was noted for the thumb and whole hand as well as muscle strength in both groups. The values of functional loss differed between the two groups. The indicators of functional improvement were greater in patients with damage to the EPL tendon. Wstęp: Kciuk stanowi o 40% funkcji całej ręki, a uszkodzenie ścięgna prostownika długiego kciuka (EPL) i/lub ścięgna zginacza długiego kciuka (FPL) prowadzi do jej znacznego ograniczenia. Głównymi czynnikami wpływającymi na uszkodzenie ścięgna EPL i/lub FPL są urazy mechaniczne – przecięcia i samoistne zerwania. Cel badań: Celem pracy było porównanie wartości ubytku oraz odbudowy funkcji kciuka i całej ręki w efekcie wdrożonej fizjoterapii w grupie pacjentów z całkowitym uszkodzeniem ścięgna EPL z grupą kontrolną pacjentów z uszkodzeniem ścięgna FPL. Materiał i metody: Badaniami objęto 25 pacjentów Specjalistycznego Ośrodka Rehabilitacji Ręki w Krakowie. U 15 z nich doszło do przerwania ciągłości ścięgna EPL (grupa badana 1), a u 10 uszkodzeniu uległo ścięgno FPL (grupa badana 2). Przeprowadzone badania obejmowały pomiary czynnej ruchomości kciuka i nadgarstka oraz czucia powierzchownego. Na podstawie tych badań obliczono ubytek funkcjonalny wg metodyki Swansona. Badania objęły również ocenę siły mięśniowej w zakresie ścisku globalnego i chwytów precyzyjnych przy użyciu dynamometru. Wyniki: Wykazano istotną statystycznie poprawę funkcjonalną kciuka i całej ręki oraz siły mięśniowej w obu badanych grupach. Wartości ubytku funkcjonalnego różniły się pomiędzy badanymi grupami. Wskaźniki poprawy funkcji były większe w grupie pacjentów z uszkodzeniem ścięgna EPL. Wnioski: Fizjoterapia funkcjonalna jest ważnym czynnikiem decydującym o powrocie funkcji kciuka i całej ręki po całkowitych uszkodzeniach ścięgien EPL i FPL.

Opis:
FT-Raman spectroscopy was used to investigate microstructural changes in the secondary protein structure of soft tissues subjected to increasing levels of macroscopic strain. Main protein bands at 938 cm-1 assigned as v(Cα–C), 1668 cm-1 — amide I and 1268 cm-1 — amide III are sensitive to applied strain and undergo wavenumber shifting. Other main vibrational modes at 1004 cm-1 assigned to the phenyl ring breathing mode and 2940 cm-1 (n(CH3,CH2)) remain unaltered. Spectroscopic results were compared with the mechanical relations obtained from the standard protocol of uniaxial tensile tests carried out in a testing machine. A clear correlation between Raman band shifting and the level of mechanical stress was established. Initially the load is transferred through elastin and then gradually also by collagen. It was proved that transferring loads by soft tissues involves changes in structural protein conformation. This process was described in detail for a tendon. It was also confirmed that mechanical properties of soft tissues depend on collagen and elastin fiers orientation.

Opis:
W pracy przedstawiono wyniki badań doświadczalnych przeprowadzonych na ścięgnach przednich kończyn świńskich. Badania przeprowadzono przy użyciu maszyny wytrzymałościowej MTS Insight. W ramach badań wyznaczono wartości modułu Younga, naprężeń oraz odkształceń ścięgien palców przednich jako preparatów do zastosowań ksenogenicznych.
In this paper it was presented the results of experimental investigations, performed on the swine tendons samples dissected from anterior limbs. The main aim of investigations was to determine the mechanical properties of swine tendons for xenogenic applications. The tests were carried out with the use of MTS Insight static machine. It was determined the Young's modulus values and also the stress and strains of tendons that occurs during the static tensile test.

Opis:
Achilles tendon rupture is a severe injury with poor curative effect due to its anatomical characteristic and mechanical peculiarity. Internal fixation of limited loop (IFLL) with steel-wire has been applied on patients with tendon rupture to fix the broken ends before physical rehabilitation. The purpose of this study is to investigate the biomechanical property and radiological characteristic of such suture technique for the repairment of tendon rupture. Methods: Tendons of pigs’ hint feet were separated for the biomechanical study. Suture surgery was performed according to the protocol of IFLL. Biomechanical Testing Machine was adopted to conduct the biomechanical tensile load examination. The maximal load, elastic modulus and tendon stiffness of the stitched tendons with or without reinforcement were examined. Results: The maximum tensile load of the stitched tendons using IFLL reached 1/4 of the uninjured tendon’s maximum tensile load, indicating that such suture technique is capable of providing enough tension for the ruptured tendon. Surprisingly, tendons fixed with titanium wire showed the highest load tension, which was comparable to the undamaged tendon. Therefore, we found the biomechanical basis of using IFLL in effectively connecting the rupture ends of tendons. Conclusions: In conclusion, we provide biomechanical evidence for the use of IFLL in treatment of Achilles tendon rupture, by providing enough strength for the ankle function. Such suture technique could help the patients with better rehabilitation and reduced in-hospital stay after Achilles tendon injury.
