Phosphatase and tensin homolog deleted on chromosome ten (PTEN) is a human suppressor gene. Its protein product is a bispecific phosphatase playing the complex role in the cell cycle regulating processes and apoptosis by the mechanism of signal transduction into the cell via tyrosine kinase B signaling pathway (PI3K/Akt/mTOR). Reduction or loss of PTEN function is implicated in the pathogenesis of many malignancies, including colorectal cancer. A gradual decrease in the function of PTEN in the sequence of transformations: normal tissue–polyp–adenocarcinoma – disseminated cancer was indicated. The relation between the PTEN loss and the higher clinical severity of colorectal cancer was observed, i.a. higher TNM status and higher tendency to form metastases, leading in some of the studies to shortened patients survival during the observation period. The potential predictive value of the PTEN function loss for the EGFR-targeted therapy in patients with advanced colorectal cancer is the subject of controversy. The potential application of PTEN assessment in clinical practice as a prognostic and/or predictive factor requires further well-designed prospective studies on larger patient population, using the unified methodology. The aim of the study is to summarize the current knowledge on the role of PTEN gene and PTEN protein in the pathogenesis of colorectal cancer and the role of PTEN in clinical practice.

Objective: Results of second-line everolimus treatment in clear cell renal carcinoma. Material and methods: Clinical data of 32 patients of the Clinic of Systemic and Metastatic Malignancies, Centre of Oncology, Maria Sklodowska-Curie Memorial Institute (Krakow Branch) treated with second-line everolimus between 2011 and 2016 were analysed. Results: Median progression-free survival was 7 months. Stable disease was observed in 24 cases. Adverse events occurred in 59% of all patients, and the majority were mild and moderate (G1 and G2) according to CTCAE. The most frequent adverse events included anaemia, stomatitis, rash and fatigue. Conclusion: Molecularly targeted drugs offer the only effective and available therapy in Poland, which can be administered in patients with advanced renal cell carcinoma. Second-line everolimus is a valuable therapeutic option in the above mentioned group (following progression on first-line TKI therapy), especially when it is necessary to avoid the accumulation of similar adverse events after TKI treatment (different toxicity profiles).

Opis:
Combining beta-particle effect with therapeutic properties of anti-CD20 monoclonal antibody in lymphomas, Mabthera™ (rituximab) was targeted in this study. The antibody was labeled with 153Sm-samarium chloride (185 MBq) after conjugation with freshly prepared ccDTPA. Conjugated-rituximab was obtained by the addition of 1 ml of a rituximab pharmaceutical solution (5 mg/ml, in phosphate buffer, pH = 7.8) to a glass tube precoated with freshly prepared ccDTPA (0.01–0.1 mg) at 25 degrees centigrade. Sm-153 chloride was obtained by a thermal neutron flux (5 × 1013 nźcm–2źs–1) of an enriched 152Sm2O3 sample, dissolved in acidic media. Radiolabeling was performed in one hour by the addition of DTPA-rituximab conjugate at room temperature. Radiochemical purity of 96% (ITLC) and 98% (HPLC) were obtained for the final radioimmunoconjugate (specific activity = 120 TBq/mmol). The final isotonic 153Sm-rituximab complex was checked by gel electrophoresis for protein integrity retention. Biodistribution studies in normal rats were performed to determine radioimmunoconjugate distribution up to 24 h. SPECT images were also obtained using 103 keV photons up to 48 h.

This paper presents the physico-chemical characteristics of titanium dioxide/chitosan dispersion. The surface and electrostatic properties of TiO2 particles in the electrolyte solution (NaCl) and in the presence of a biopolymer were determined by static and dynamic light scattering. The stability measurements were made under different conditions, where the variable was the dispersion pH. This paper mainly includes discussion of dispersion behaviour in neutral and alkaline environments. At each pH value (6–9), the addition of chitosan caused significant improvement in stability compared to the systems of base titanium dioxide. Of the tested systems, the most stable values of diameter and zeta potential for dispersion were found at pH 7. The study allowed the conclusion that the addition of chitosan to TiO2 dispersion created the possibility of using such systems as a drug carrier for the controlled release of active substances sensitive to pH changes.

Lung cancer is associated with one of the highest mortality rates among malignant tumors. It is the main death cause of men and women in Poland where over 15 000 men and 7 000 women die with this diagnosis annually. 1.7 million people die every year due to lung cancer in the world. Two main types of lung cancer are small cell lung cancer (SCLC) and non-small cell lung cancer (NSCLC). The history of lung cancer treatment begins with surgical approach followed by addition of chemotherapy and radiotherapy, which were used either separately or in combination depending on the stage of the lung cancer. Many somatic mutations were identified and molecularly targeted therapies could have been established. One of the oldest and the best known group of molecularly targeted drugs used in lung cancer treatment are tyrosine kinase inhibitors (TKIs) of epidermal growth factor receptor (EGFR). First EGFR-TKI was gefitinib, which has been examined in clinical trials before erlotinib, afatinib, dacomitinib and osimertinib. EGFR-TKIs increased overall survival (OS) with significantly less side effects when compared to standard chemotherapy. Another group of molecularly targeted drugs are anaplastic lymphoma (ALK) kinase inhibitors such as crizotinib, alectinib and ceritinib. Another innovation which was introduced in NSCLC treatment was immunotherapy. Its effect is based on modification of immune system leading to activation of cytotoxic T lymphocytes (CTL). Currently nivolumab and pembrolizumab (anti-PD1 antibodies) as well as atezoliumab (anti-PD-L1 antibody) are being used in NSCLC patients. The discovery of innovative therapies for NSCLC patients resulted in significant extension of patients' life expectancy while minimizing the side effects of such therapy. Moreover, the quality of patients’ life was significantly improved. However, important problems still remain to be solved: overcoming the resistance in the course of molecularly targeted therapies and the lack of predictive factors that determine the effectiveness of immunotherapy.

Osteosarcoma is the most common primary malignant bone tumor in adolescents and young adults. However, the involvement of serine/threonine phosphatase type 5 (PP5) in osteosarcoma remains unclear. The aim of this study was to evaluate the functional role of PP5 in osteosarcoma cells. Firstly, we found that PP5 is widely expressed in several human osteosarcoma cell lines. Then we used lentivirus-delivered siRNA to silence PP5 expression in Saos-2 and U2OS cell lines. Knockdown of endogenous PP5 expression by shRNA-expressing lentivirus significantly decreased the viability and proliferation of the osteosarcoma cells. Moreover, FACS analysis showed that knockdown of PP5 expression induced a significant arrest in the G0/G1 phase of the cell cycle, which was associated with the inhibition of cell proliferation. Therefore, knockdown of PP5 is likely to provide a novel alternative to targeted therapy of osteosarcoma and deserves further investigation.

Financing investments is one the most important issues in the policy of local governments. One of the ways of supporting local governments are targeted grants for investments from the state budget. Recent years have been characterized by a change in the policy of financing local governments in this area, which was mainly related to the change of priorities from investment to social policy support. The aim of the article is to present the legal possibilities and actual use of available sources of financing local and regional development by local governments units coming from targeted grants from the state budget of an investment character in the period of 2014-2018.
Finansowanie inwestycji jest jednym z najważniejszych zagadnień w polityce samorządów. Sposobem wsparcia dla samorządów są dotacje celowe z budżetu państwa, mające charakter inwestycyjny. Ostatnie lata charakteryzuje zmiana polityki finansowania samorządów w tym obszarze, co związane było głównie ze zmianą priorytetów z inwestycyjnych na wsparcie polityki socjalnej. Celem artykułu jest przedstawienie prawnych możliwości oraz faktycznego wykorzystania w okresie 2014-2018 dostępnych źródeł finansowania rozwoju lokalnego i regionalnego przez jednostki samorządu terytorialnego, pochodzących z dotacji z budżetu państwa, mających charakter inwestycyjny.

A growing number of targeted anticancer agents has shown the unexpected ability to induce QT interval prolongation. In addition, standard chemotherapeutics and a variety of conditions such as electrolyte abnormalities, endocrine disorders, cardiac diseases, nutritional disturbances and other factors may be associated with long QT syndrome in cancer patients. Prolongation of the QT interval can lead to life-threatening ventricular arrhythmias, including ‘torsade de pointes’ (TdP). The association between long QT interval and ventricular arrhythmias remains the subject of many controversies. The QT interval represents the time interval of both ventricular depolarization and repolarization. Not only abnormalities of ion channels, but also changes in the myocardial microarchitecture and other factors and disorders frequently seen in cancer patients may participate in its prolongation and potential risk of ventricular arrhythmias. The aim of this review was to summarize current knowledge about QT prolongation in cancer patients with the special focus on targeted therapy.

Introduction. Recently, monoclonal antibodies (mAbs) have become powerful human therapeutics in the diagnosis and treatment of many diseases. Drugs based on mAbs are approved for the treatment of cardiovascular, respiratory, hematology, autoimmunology, and oncology diseases. Aim. To present the current state of knowledge about the application of mAbs in the therapy of various diseases such as cancer, autoimmune and Alzheimer’s diseases. Material and methods. We conducted a thorough review of the scientific literature from the following databases: EBSCO, PubMed, Science Direct, and Springer Link. Results. Currently, the Food and Drug Administration (FDA) has approved more than 50 therapeutic mAbs which are applied in various clinical trials. Action of mAb are based on various mechanisms, including directly targeting the cells, modifying the host response, recognizing and degrading molecules as well as delivering cytotoxic moieties. Conclusion. Despite some limitations including side effects, and therapeutic challenges, monoclonal antibodies are an attractive option for the development of new therapies and molecular drug targets against a wide range of common diseases due to their specificity and flexibility. MAbs are considered as a great hope for medicine, and effective and safe drugs in the treatment of various diseases.y

Due to the anticancer properties of bleomycin (BLM) complexes, production of [103Pd]bleomycin ([103Pd]BLM) was targeted. Palladium-103 (T1/2 = 16.96 d) was produced via the 103Rh(p,n)103Pd nuclear reaction using a natural rhodium target. Proton energy was 18 MeV with 200 mA irradiation for 15 h (final activity 25.9 GBq of 103Pd2+, RCP > 95%, radionuclidic purity > 99%). 103Pd was separated from the irradiated target by anion exchange using a Dowex 1´8 (Cl-)/100-200 mesh resin in the form of Pd(NH3)2Cl2 in order to react with bleomycin to yield [103Pd]BLM. Chemical purity of the final product was in accordance to the accepted limits. [103Pd]BLM was prepared with a radiochemical yield of more than 98% at 80°C in 30 min. The labeling reaction was optimized for time, temperature and ligand concentration. Radiochemical purity of more than 99% was obtained using RTLC with specific activity of about 370 MBq/mmol. The stability of the tracer was checked in the final product and presence of human serum at up to 3 h. The complex was stable in human serum at 37°C up to 2 h of incubation. Biodistribution studies using a SPECT system performed in normal rats in the first 2-3 h.

Selektywna eliminacja (targeted killing) terrorystów z wykorzystaniem bezpilotowych statków latających (tzw. dronów) jest jedną z kluczowych metod stosowanych przez Stany Zjednoczone w ramach tzw. wojny z terroryzmem. Jej znaczenie jeszcze bardziej wzrosło od czasu, gdy USA zmniejszyły swoje zaangażowanie w wielkie kampanie w Afganistanie i Iraku. W świetle strat, jakie amerykańskie siły zbrojne poniosły w tych operacjach, drony jawią się jako wygodne, stosunkowo tanie i mało ryzykowne narzędzie. Z drugiej jednak strony, działania takie budzą ogromne kontrowersje, zarówno z uwagi na ofiary wśród ludności cywilnej, jak i z powodu samej natury ataków dokonywanych przez drony. Stanowią one bowiem ostatnie jak dotąd ogniwo typowej dla państw zachodnich tendencji do opierania działań wojennych na przewadze technologicznej, pozwalającej atakować wroga bez narażania własnych żołnierzy. Artykuł umieszcza selektywną eliminację w długiej tradycji dyplomacji opartej na sile zbrojnej. Przeanalizowano wpływ działań realizowanych z wykorzystaniem dronów na opinię publiczną, co jest kluczowe w warunkach prowadzenia współczesnej wojny, a także na charakter stosunków łączących Stany Zjednoczone z państwami, na terenach których dokonywane są ataki. Wreszcie odniesiono się do dylematów związanych z legalnością selektywnej eliminacji – a także, w szerszym ujęciu, działań podejmowanych w ramach wojny z terroryzmem – w kontekście prawa międzynarodowego.
Targeted killing of terrorists with the use of drones is one of the key methods used by the United States in the so-called war against terrorism. Its importance has further increased with the USA’s decreased engagement in big campaigns in Afghanistan and Iraq. In view of the losses suffered by the American armed forces during the operations in questions, drones seem to be a comfortable, relatively cheap and low-risk tool. However, such undertakings have generated a lot of controversy, both over casualties among civilians and the type of attacks performed by drones. Indeed, drones constitute the most recent example of the tendency, typical of the Western countries, for warfare to draw on technological advances, which allows for attacking the enemy without risking the lives of own soldiers. The article places targeted killing within the well-established tradition of diplomacy drawing on military force. The influence of drone-based actions on the public opinion has been analysed, which is crucial from the point of view of the conditions in which the war is being fought and of the nature of the relations between the United States and the countries whose territories have become the main arenas of attack. Then, dilemmas have been discussed related to the legality of targeted killing. Finally, the author has presented a broader perspective on the actions undertaken as part of the war on terrorism in the context of international law.

In only the past century, the landscape of breast cancer treatment has completely changed. The Halstedian hypothesis of the “contiguous spread” of breast cancer has been replaced by a consideration of its systemic nature. Today, patients with early-stage breast cancer are managed with breast-conserving therapy, which is as effective as mastectomy. Sentinel lymph node biopsy has largely replaced axillary lymph node dissection. Post-operative radiotherapy, chemotherapy and endocrine therapy have increased survival. Pre-operative cytotoxic therapy allows for less extensive surgery and for a curative resection even in more advanced stages. Rapid progress in molecular oncology revealed a large heterogeneity of breast cancer, resulting in a more personalized approach. Targeted therapies directed against epidermal growth factor receptor type 2 (HER2) have improved survival in HER2-positive breast cancer, which was once a poor-prognosis entity. Multi-gene prognostic signatures better predict prognosis and allow many patients to avoid chemotherapy. Personalized treatment has resulted in decreased toxicity and an improved quality of life. Within the past decades, breast cancer has become a good-prognosis malignancy with a five-year survival in the range of 80-85%. Future development of personalized medicine may further refine treatment based on the tumor’s molecular features.

Opis:
The article deals with the problem of adjusting the revenues of Polish local government units (LGUs) to their tasks. The problem was analysed both from the point of view of the possibility of increasing LGU revenues and designing an effective system of fiscal equalization. The authors discussed the external factors which limit the possibilities of introducing such a desirable system. Next, based on the presented analysis, they drew conclusions regarding the necessary revision of the existing scheme. The article concludes with specific recommendations for a new system of financing LGUs, assuming, in particular, far-reaching changes in the algorithms for determining and dividing general subsidies, as well as a fundamental reduction in the scope of the so-called commissioned tasks.
