Environmental regulations instigated the technological and procedural revolution in shipping. One of the challenges has been sulfur emission control areas (SECA) and requirement of fuel changeover. Initially, many reports anticipated that new grades of low sulfur fuels might increase various technical problems in ship operation. This research develops a simple and easy to use method of the failure severity and intensity assessment in relation to fuel changeover. The scale of failure rate in the ship’s fuel system was evaluated qualitatively and quantitively, using developed failure frequency indicator and the time between failure. Based on 77 records of fuel system failures collected on seven ships, it has been found that frequency of failures related to SECA fuel changeover is on average nearly three times higher compared to the rest of sailing time. Their severity did not significantly change, but the structure of failures changed considerably. The method and presented results may help in improvement of ship’s systems design and on-board operational procedures.

The aim of this study was to examine the relationship between the occurrence of selected types of failures and the time and season of their occurrence in public transport buses in the city of Lublin. The analysis was carried out for buses operated in the Municipal Transport Company within the past 3 years. The chi-squared analysis showed that during operation, there are significant differences between the failures and the annual season, but there are no differences between the frequency of failures and the time of day.

Opis:
Among the risk assessment methods, failure modes and effects analysis (FMEA) is a popular, widely used engineering technique in many areas. It can be used to identify and eliminate known or potential failure modes to enhance reliability and safety of complex systems. In practice, risk estimations encounter difficulties connected with shortage of data. In such cases, we have to rely on subjective estimations made by persons with practical knowledge in the field of interest, i.e. experts. However, in some realistic situations, the decision makers might be unable to assign the exact values to the evaluation judgments due to his/her limited knowledge. In other words, there is a certain degree of hesitancy in human cognition and his/her judgment, who may have insufficient knowledge of the problem domain or uncertainty in assigning the evaluation values to the objects considered. In order to deal with ambiguity and uncertainty in the imperfect information, there have been recently proposed many various such theories as fuzzy sets, interval-valued fuzzy sets, type-2 fuzzy sets, hesitant sets, grey sets, rough sets and intuitionistic fuzzy sets. They have drawn more and more attention of scholars and been adopted in many applications This article addresses the Atanassov’s interval-valued intuitionistic fuzzy sets and FMEA methods in the risk estimation of the system failures based on the expert judgments.

Probabilistic models have been developed to evaluate the relationship between reliability measures and the performance of a repairable network with built in redundancy. Networks with built in redun-dancy have been considered and explicit expressions have been derived for three characteristics related to such systems including steady-state availability, period of repair, and a profit function. Various graphs have been plotted to discover the impact of availability and mean time to system failure on net profit, as well as the impact of the failure and service rate on the steady-state availability, net profit and mean time to system failure. The system was analysed using first order linear differential equations.

It also presents the possibilities and the ways to use each form as well as their impact on the quality of the use of safety maneuvers. Investigations were carried out by simulation methods with utilisation of the computer software.

The topic of failures and water losses in the urban-rural water supply network was discussed. The purpose of the work was to assess the level of real water losses and the failure rate of the urban-rural water supply network in 2010-2016. The article’s introduction contains the information on the types and causes of breakdowns in the water supply network. The life cycle of the leakage from its inception to its removal was also described. Next, the methodology and indicators for assessing actual losses in the water supply network were presented according to the guidelines of the International Water Association (IWA), i.e. ILI, CARL, UARL, RLB and water balance. The material and methods chapter describes how to calculate the failure rate. On the basis of the literature review, a method of assessing the failure of the water supply network and the level of actual water losses was presented. The research part presents the results of the water losses indicators values and assesses them against the literature data for the entire urban-rural system. The results of the failure of the water supply of the urban area and the rural area were also presented and compared with the literature data. A method of calculating limit values of the failure intensity index for the entire water supply system was also proposed. The changes in the intensity of failure occurrence during the year were presented. The object described in the studies achieved the A rating according to the classification of ILI and RLB values throughout the entire analysis period (2010–2016). Failure intensity in the analyzed period exceeded the average values calculated for water supply networks in Poland many times. The values in individual cases exceeded the limit values specified in the literature.

Opis:
An accurate determination of the system failure threshold is an essential requirement in achieving an appropriate system residual life prediction and a reasonable planned maintenance strategy optimization afterward for degradation systems. This paper proposes a failure threshold determination method based on quantitative measurement of the similarity between the operating system and the historical systems. The similarity is formulated by a weighted average function and then calculated by a convex quadratic formulation to minimizing the variance between the operating system and the historical systems. With an accurate determination of the system failure threshold in real-time, a better prediction of the residual life for the operating system is achieved. Finally, a real case study for several power-shift steering transmission systems monitored using oil spectral analysis is adopted to illustrate and numerically compare the improved performance of the proposed method.
W przypadku systemów podlegających degradacji, dokładne określenie progu awarii systemu stanowi niezbędny warunek dokonania trafnej prognozy jego trwałości resztkowej oraz późniejszej optymalizacji strategii konserwacji rutynowych. W artykule zaproponowano metodę wyznaczania progu awarii opartą na ilościowym pomiarze podobieństwa między systemem użytkowanym obecnie a systemami użytkowanymi uprzednio. Podobieństwo formułuje się na podstawie funkcji średniej ważonej, a następnie oblicza na podstawie wypukłej formy kwadratowej w celu zminimalizowania wariancji między obecnie użytkowanym systemem a uprzednimi systemami. Dzięki dokładnemu określeniu progu awarii systemu w czasie rzeczywistym uzyskuje się lepszą prognostykę trwałości resztkowej obecnie użytkowanego systemu. W końcowej części pracy, w celu zilustrowania i numerycznego porównania ulepszonej wydajności proponowanej metody, zaprezentowano studium przypadku obejmujące kilka układów przeniesienia napędu monitorowanych przy użyciu analizy spektralnej oleju.

A "multi-state linear k-within-(m,s)-of-(m,n):F lattice system" (MS L(k,m,s,n:F)) comprises of m×n components, which are ordered in m rows and n columns. The state of system and components may be one of the following states: 0, 1, 2, …, H. The state of MS L(k,m,s,n:F) is less than j whenever there is at least one sub-matrix of the size m×s which contains kl or more components that are in state less than l for all j ≤ l ≤ H. This system is a model for many applications, for example, tele communication, radar detection, oil pipeline, mobile communications, inspection procedures and series of microwave towers systems. In this paper, we propose new bounds of increasing MS L(k,m,s,n:F) reliability using second and third orders of Boole-Bonferroni bounds with i.i.d components. The new bounds are examined by previously published numerical examples for some special cases of increasing MS L(k,m,s,n:F). Also, illustration examples of modelling the system and numerical examples of new bounds are presented. Further, comparisons between the results of second and third orders of Boole-Bonferroni bounds are given.
"Wielostanowy system liniowy k-w- ( m, s ) -z- ( m, n ):F o strukturze kratowej" (MS L(k, m, s, n:F)) składa się z m × n elementów, uporządkowanych w m wierszach i n kolumnach. Stan systemu i elementów może być jednym z następujących stanów: 0, 1, 2, ..., H. Stan MS L (k, m, s, n: F) jest mniejszy niż j, gdy istnieje co najmniej jedna pod-matryca o rozmiarze m × s, która zawiera kl lub więcej elementów, które znajdują się w stanie mniejszym niż l dla wszystkich j ≤ l ≤ H. System ten stanowi model dla wielu zastosowań, na przykład w telekomunikacji, detekcji radarowej, rurociągach naftowych, komunikacji mobilnej, procedurach przeglądu oraz systemach wież radiolinii. W niniejszym artykule proponujemy nowe granice zwiększania niezawodności MS L ( k, m, s, n: F) z wykorzystaniem drugiego i trzeciego stopnia nierówności Boole'a–Bonferroniego z niezależnymi elementami o jednakowym rozkładzie. Nowe granice omówiono na podstawie poprzednio publikowanych przykładów numerycznych dla niektórych szczególnych przypadków zwiększania MS L ( k, m, s, n: F). Przedstawiono także przykłady ilustrujące modelowanie systemu oraz numeryczne przykłady nowych granic. Ponadto porównano wyniki uzyskane dla drugiego i trzeciego stopnia nierówności Boole'a–Bonferroniego.

The harsh environmental loads may lead to strength failure in the turbine in an aero-engine. To accurately assess the strength reliability of the turbine under multiple loads, the stress distributions of 41 danger sites of a turbine under thermal, centrifugal, and pneumatic loads were determined by the flow-thermal-solid coupling analysis using ANSYS. Second, based on the flow-thermal-solid coupling analysis and response surface method, the probabilistic analysis model of stress at the danger site was established. And the probabilistic distribution of stress was determined by sampling and hypothesis testing. Finally, the reliability model of the turbine with multi-site damage and failure dependency was established, by which a reliability of 0.99802 was calculated. And the actual reliability of the turbine was 0.99626 determined by the Monte Carlo simulations, which verified the model in precision. The results indicated that the reliability model has a high efficiency and higher precision than the traditional reliability model with failure independence.

Systems of public transport carry out their tasks on the territory of a given town and in the suburbs performing the same functions as in towns. A distinctive feature of this type of system is performance of its task for twenty four hours, in different traffic intensity conditions over an assigned administrative area. This type of variables has a large influence on occurrence of undesirable events (failure of vehicles and their devastation, collision and accidents). In the work, an attempt of identification of undesirable events occurring in the analyzed transport system has been made. Moreover, an analysis has been made which covers one day of the week, time of the day and weather conditions in which particular events took place. Another aspect of the work was to make an analysis of the vehicle driver's age, job experience and a given hour of work on the day of the event occurrence. Obtainment of such data allowed demonstrating the structure of events according to the criterion of the event type, and costs connected with bringing the damaged transport means to the state of serviceability. The obtained results of experimental tests provide a basis for taking rational decisions by decision makers employed in a given system. These decisions are supposed to reduce the number of undesirable events occurrence and costs connected with them.

The classic approach to evaluate the probability that an operational system is capable to operate satisfactorily and successfully perform the formulated tasks is based on availability that is coefficient which is determined based on the history of down-time and up-time occurring, while the risk-degree of down-time occurring strongly depends on the actual operational state of a system. The intelligence computational methods enable to create the diagnosis tools that allow to formulate the prognosis of operating time of a system and predict of failure occurring based on the past and actual information about system's operational state, especially genetic fuzzy systems (GFSs) that combine fuzzy approximate reasoning and capability to learn and adaptation. The paper presents the fuzzy rule-based inference system used to predict the operating time of exploitation system according to the specified operational conditions. The proposed algorithm was used to design the fuzzy model applied to estimate the operating time of a system between the actual time and predicted time of the next failure occurring under the stated operational parameters. The fuzzy system allows to prognoses the time of the predicted failure based on the operational parameters which are used to evaluate the actual operational state of the system. The attention in the paper is focused on the evolutionary computational techniques applied to design the fuzzy inference system. The paper proposes the genetic algorithm based on the Pittsburgh method and real-valued chromosomes used to optimize the knowledge base and parameters of antecedents and conclusions of the Takagi-Sugeno-Kang (TSK) fuzzy implications. The paper is the contribution to the GFSs, which aim is to find an appropriate balance between accuracy and interpretability, and also contribution to the research field on the diagnosis methods based on soft computing techniques. The evolutionary algorithm was tested for designing the fuzzy operating time predictor of material handling device.

The article presents untypical diagnostic of the slow-speed, marine main engine. The engine was newly made-up and checking during mounting process and during sea trial. However, a leak under piston chamber of second cylinder along the piston rod appeared few days after the sea trial. The stuffing box exchange (with the piston rod’s regeneration) did not give expected results − the failure happened again. All geometrical and exploitation parameters was in acceptable range. The ship with main engine was fifth in the series of sister ships. The authors were asked for urgent expert opinion. Mix of different measurements was planned after formulation several hypothesis. Displacements of piston rod and cylinder, a stress level of main engine body, and vibrations level in different points of main engine was performed during short sea voyage. Not a single hypothesis can be considered as the main cause of the failure. It turns out, that a serious failure need not be caused by a single reason. According to snowball theory a sum of small effects, each of them affecting slightly the engine operation, can be a cause of serious failure. In the authors’ opinion, the failure was caused by a sum of relatively slight effects. The probably scenario of the failure process was enunciated. The recommendation for the engine project was formulated: some of the geometrical tolerances should be changed.

The reliable operation of critical infrastructure has a direct impact on the energy security of country. Due to the complexity and vastness of such system it is exposed to various types of events that could lead to failure. These risks may result directly from the operation and also be the result of external factors. Especially dangerous are the undesirable events with incidental character or unlikely events that constitute a serious threat to people and the environment and resulting in significant loss.
