The aim of the study was to determine statistically significant factors with an impact on the early postoperative surgical outcome.Material and methods. The influence of applied fast-track components on surgical results and early postoperative outcome in 143 consecutive Kausch-Whipple procedure patients was evaluated in a single-center retrospective analysis of a prospective collection of patient-associated pre-, peri- and postoperative data from 1997-2006.Results. The in-hospital mortality rate was 2.8% (n=4). Fast-track measures were shown to have no effect on the morbidity rate in the multi-variate analysis. Over the study period, a decrease of intraoperative infusion volume from 14.2 mL/kg body weight/h in the first year to 10.7 mL/kg body weight/h in the last year was accompanied by an increase in patients requiring intraoperative catecholamines, up from 17% to 95%. The administration of ropivacain/sufentanil via thoracic peri-dural catheter injection initiated in 2000 and now considered the leading analgesic method, was used in 95% of the cases in 2006. Early extubation rate rose from 16.6% to 57.9%.Conclusions. Fast-track aspects in the perioperative management have become more important in several surgical procedure even in those with a greater invasiveness such as Kausch-Whipple. However, such techniques used in peri-operative management of Kausch-Whipple pancreatic-head resections had no impact on the morbidity rate. In addition, the low in-hospital mortality rate was particularly attributed to surgical competence.

Opis:
Celem oceny technologii medycznych (ang. Health Technology Assessment – HTA) jest wspieranie świadomego kształtowania polityki i oceny ekonomicznej w zakresie medycyny, a tym samym racjonalnej alokacji zasobów niezbędnych do rozwoju technologii medycznych. Przykładem zmian technologicznych w medycynie jest intensywny rozwój małoinwazyjnych technik chirurgicznych, w tym technik laparoskopowych. Biorąc pod uwagę korzyści uzyskiwane przez pacjenta w wyniku leczenia laparoskopowego, naturalną tendencją powinno być ich zastosowanie w miejsce metod otwartych. Niestety dla osób zarządzających placówkami zdrowia priorytetem jest minimalizacja kosztów wykonywanych procedur medycznych. Z tego względu, celem niniejszego artykułu jest analiza porównawcza kosztów wybranych zabiegów chirurgicznych wykonywanych metodą otwartą i laparoskopową. Uzyskane wyniki są częściowo zaskakujące, gdyż kwestionują przekonanie, że koszt wykonania zabiegów laparoskopowych przekracza koszt identycznych zabiegów wykonywanych metodą otwartą.
The aim of Health Technology Assessment is to support activities intended to form politics as well as economic analysis in medicine and hence rational allocation of resources which are necessary for developing medical technologies. An example of technological changes in medicine is an intensive development of minimally invasive surgical techniques including laparoscopic techniques. Taking into account the great benefits of laparoscopic treatment for patient the natural tendency should be the change of open surgeries to minimal invasive procedures. However the problem of conflicts costs-benefits is indicated by health managers, for whom the procedures’ costs are treated as a priority. Taking this as a background for considerations, there was formulated the research objective that is a comparative analysis of selected surgical procedures performed in open method and laparoscopy. The outcomes are partly surprising and questioned the hypothetical rule that the cost of performing laparoscopic procedures exceed the cost of identical procedures, but executed with open method.

The aim of the studywas to evaluate the cost of surgical care and short-term outcomes of Crohn’s disease(CD) in the era of laparoscopy and biologic therapy. Material and methods. Review of a prospective database identifiedCD patients that underwent surgical management. Patients were stratified into laparoscopic, open, and converted approaches. Main outcome measures were short-term patient outcomes and cost of care by approach. Results. 92 patients were analyzed- 63.1% laparoscopic, 32.6% open, 4.3% converted. The majority was elective (100% converted, 94.8% laparoscopic, 90% open) and segmental resections (75% converted, 70.7% laparoscopic, 43.3% open). Operative times were similar between laparoscopic and open (152 minutes and 138 minutes, respectively). More open patients required ICU care (20% versus 12.1% laparoscopic and 0% converted). The median LOS was 3 (1-25) days laparoscopic, 4 (1-29) open and 4 (3-8) converted. The laparoscopic complication rate was 15.5%, readmission rate 12.1%, and reoperation rate 8.6%. The mean total hospital cost was $9,702 laparoscopic, $10,782 open, and $13,293 for converted cases (US Dollars). Conclusions. Laparoscopy is appropriate for most CD cases. When necessary to combine with open surgery, this results in efficient and effective patient outcomes and healthcare utilization. These results are important when weighing the cost of ongoing medical therapy versus surgical intervention.

Introduction and aim. An extraoral cutaneous sinus tract of endodontic origin represents a skin condition that appears due to an infection that could be primary or secondary to trauma accompanied by alveolar bony changes. It may be easily misdiagnosed and inappropriately treated due to lack of inadequate expertise on part of medical professionals followed by faulty treatment which can complicate the case further. Description of case. This case report intends to highlight a case of a sixteen-year-old male patient referred for an extraoral cutaneous sinus tract misdiagnosed and insufficiently treated by a general physician followed by general dentists for more than a month due to lack of proficiency in the field of endodontics. The clinical and radiographic examinations confirmed the pulpal origin due to trauma related to permanent mandibular incisors. A combination of non-surgical root canal therapy followed by periapical surgery was performed leading to a significant healing of the lesion. Conclusion. Accurate diagnosis is the key to treat draining sinus tracts of endodontic origin. Root canal treatment and endodontic surgery should be used judiciously for effectively eliminating the pathogens thus providing healing and repair mechanisms a chance to achieve the desired result.

Background Job satisfaction is a major issue in work psychology among nurses, and in the surgical nursing context, important factors leading to the perception of job satisfaction have been suggested. Two European Union neighboring countries (Sweden and Poland) were chosen for the purpose of this study due to similar nursing education but different health care systems, employment regulations and salaries. Recognition of the factors which are related to nurse job satisfaction may lead to improvements in the nurses’ working conditions. The aim of this study was to explore and compare job satisfaction and various factors among Polish and Swedish nurses in a surgical ward context. Material and Methods The study had a cross-sectional survey design, with questionnaires among Polish and Swedish nurses in surgical care, and was conducted between April and December 2014. The main assessment tool was a Job Satisfaction Survey questionnaire. In total, 408 nurses returned the questionnaire (response rate – 59%). Results Swedish nurses rated job satisfaction significantly higher than Polish nurses. The possibilities for professional development at the current workplace correlated with job satisfaction in both groups. Higher values of exhaustion due to nurses’ working duties were correlated with general job satisfaction. Conclusions Swedish and Polish nurses showed ambivalence towards job satisfaction. Their job satisfaction increased when their exhaustion level was higher. The possibilities for achievements, developing professional skills, and promotion may be important factors affecting job satisfaction. Med Pr. 2019;70(2):155–67
