Background. Polycystic ovary syndrome (PCOS) is a disorder often occurring in women at reproductive age. An important factor in PCOS pathogenesis is insulin resistance, which pronounces hyperandrogenism and leads to the development of various metabolic disorders. Objectives. The aim of this study is to determine the effect of reduction diet with low glycemic index (GI) on anthropometric parameters of women with PCOS and the assessment of the effectiveness of the diet on body mass and adipose tissue reduction. Material and methods. The study was performed on 24 women with PCOS diagnosed with Rotterdam’s criteria. Anthropometric measurements and bioelectrical impedance were performed. All participants received 7-day diet and recommendations relating to the change in lifestyle. After three months of using the dietary recommendations the measurements were repeated. Results. Statistical significance was observed for body mass (↓on average by 5.93 kg±2.95), BMI (↓2.14 kg/m2±1.2), circumference of: waist (↓7.7 cm±5.9), hip (↓4.8 cm±5.4), arm (↓1.9 cm±3.7) and measurements of skin fold under the shoulder blade (↓4.8 mm±4,6), above iliac crest (↓6.76 mm±5.7) and above triceps brachii muscle (↓5.25 mm±7.4). Considering body composition measurements, statistically significant were differences in the measurements of BCMI (↑18.042±8.8), TBW expressed in percentage (↑2.729±2.75) and in litres (↑0.071±5.15), FM in percentage (↓3.291±5.6) and in kg (3.354 kg±4.9). Conclusions. Body mass reduction using a rational diet with low GI is an effective method to support of PCOS treatment. Using reduction diet for three months together with increased physical activity enables to reduce body weight by on average 5.93 kg, which increases the chances to treat infertility in women. This should be the suggested type of diet in PCOS treatment.
Wprowadzenie. Zespół policystycznych jajników (PCOS) to zaburzenie coraz częściej pojawiające się wśród kobiet w wieku reprodukcyjnym. Ważnym czynnikiem w patogenezie PCOS jest insulinooporność, która nasila hiperandrogenizm oraz prowadzi do rozwoju zaburzeń metabolicznych, w tym zespołu metabolicznego. Cel. Celem badań jest ocena wpływu stosowania diety redukcyjnej o niskim indeksie glikemicznym na parametry antropometryczne kobiet z PCOS oraz ocena skuteczności diety na redukcję masy ciała i tkanki tłuszczowej. Materiał i metody. Badanie przeprowadzono wśród 24 kobiet ze stwierdzonym zespołem policystycznych jajników, według kryteriów Rotterdamskich. Wykonano pomiary antropometryczne metodą bioimpedancji elektrycznej. Każda z uczestniczek badania otrzymała siedmiodniowy jadłospis oraz zalecenia odnoszące się do zmiany stylu życia. Po trzech miesiącach stosowania zaleceń dietetycznych pomiary powtórzono. Wyniki: Statystyczną istotność wykazano w zmianie masy ciała (↓średnio o 5,93 kg ±2,95), BMI (↓2,14 kg/m2± 1,2), obwodach: talii (↓7,7cm ± 5,9), bioder (↓4,8 cm ± 5,4), ramienia (↓1,9 cm ± 3,7) oraz pomiarach fałdów skórno-tłuszczowych pod łopatką (↓4,8 mm ± 4,6), nad grzebieniem kości biodrowej (↓6,76 mm ± 5,7), nad mięśniem trójgłowym ramienia (↓5,25 mm ± 7,4). Z pomiarów składu ciała, statystycznie istotne okazały się różnice w pomiarach BCMI (↑18,042 ± 8,8), TBW w organizmie wyrażonej procentowo (↑2,729 ± 2,75) i w litrach (↑0,071 ± 5,15), odsetka FM (↓3,291 ± 5,6) oraz wyrażonej w kilogramach (3,354kg ± 4,9). Wnioski. Redukcja masy ciała przy zastosowaniu racjonalnej diety opartej na piramidzie zdrowego żywienia i niskim IG jest skuteczną metodą wspomagającą leczenie PCOS. Stosowanie przez okres trzech miesięcy diety redukcyjnej, połączonej ze zwiększoną aktywnością fizyczną, pozwala na redukcję masy ciała średnio o 5,93 kg co zwiększa szansę powodzenia w leczeniu bezpłodności kobiet. Ten typ diety powinien być sugerowany w leczeniu kobiet z PCOS.

Purpose: To evaluate the effect of family support and family conflict on treatment compliance of type 2 diabetes patients. Materials/Methods: The study was conducted in the endocrinology department of a university hospital. Data were collected from 174 individuals diagnosed with Type 2 DM. Patient Identification Form, Family Support and Conflict Scale in Type 2 Diabetes, and Patient Compliance Scale in Type 2 DM Treatment were used for data collection. Results: It was found that the increase in family support increased compliance with the treatment, and the increase in family conflict decreased the compliance to the treatment. A statistically significant difference was found between gender (p = 0.037), place of residence (p <0.001), income level (p <0.001) and compliance. A statistically significant difference was found between marital status (p <0.001) and family support. Conclusions: The increase in family support increases compliance to the treatment, the increase in family conflict decreases the compliance to the treatment.

The purpose of the article is to show the life situation of drug addicts before the start of the withdrawal treatment in an addiction treatment centre and after all stages of the treatment have been completed. It is important to show the difference in perception of a person’s life status before and after treatment. Despite similar potential problems, for example in the housing, professional, health and family situation, there is a fundamental change in the way they are assessed and how to solve them. The reason is the confrontation with the disease that occurs during the stay in the centre. Thanks to it, it is possible to think about what to change in the person’s life. The broadly understood change is possible thanks to appropriate support after leaving the centre. This support, provided, among others, by social workers, consists in accompanying such a person in the implementation of subsequent challenges and in solving emerging problems. It is associated with motivating, indicating resources and strengthening the sense of agency in life.

W artykule przedstawiono strukturę opracowanego i wykonanego systemu komputerowego BSA Analysis Tool, wspomagającego procesy obliczeniowe wybranych parametrów organizmu człowieka: BSA, TBSA i BMI. W materiale, poza opisem systemu, wskazano na utylitarny charakter opracowania, szczególnie w przypadku: ośrodków medycznych - jako narzędzie wspomagające procesy leczenia oparzeń i chemioterapii oraz w ośrodkach naukowo-dydaktycznych jako narzędzie wspomagające procesy dydaktyczne.
The paper describes in detail capabilities and a structure of the BSA Analysis Tool system which was developed for supporting the calculation and analysis process of human BSA (Body Surface Area), TBSA (Total Body Surface Area) and BMI (Body Mass Index). In some practical applications, the BSA coefficient can be determined using online calculators, but they are not that strongly developed. Moreover, the program allows calculating automatically the TBSA, replacing the hand-filled tables procedure. Advanced functions of the program allow for its use in medical centers as a tool assisting the treatment of burns or chemotherapy, as well as centers of research and teaching. The complex structure of the application consists of two main layers: clinical and scientific (Fig. 1). The BSA coefficient can be calculated on the basis of ten mostly used methods, while the value of the TBSA coefficient can be calculated on the basis of three methods. For calculating the BSA, only the most necessary data about the patients is required: body weight, height, age and gender, whereas the TBSA value in addition depends on the size of the body damaged area. The scientific layer structure is composed of three modules: comparison of methods for calculating the BSA, comparison of the BSA and BMI coefficients and analysis of the BSA surfaces. These tools enable a thorough analysis of the BSA coefficient (Figs. 2 and 3) as well as fast and accurate calculations of the BSA and TBSA.

In this article suggest some new approaches to solving the problems of medical data analysis and their personalization. To accomplish this was proposed to create a decision support system to the execution of sequence stages of analysis of patient's data. The main stages of development and design of decision support systems that enable to make decomposition of control process and describe the relationship between input and output control flows. Applying the theory of decision trees during construction of decision trees of decision support system is due to the formation of a sequence of questions asked by the doctor when searching an individual approach when choosing a treatment. Decision tree creates a hierarchical structure of rules. This approach allows you to present the logic of sequence issues by doctor in solving the medical problem history and it makes possible to simulate decision making process by physician when selecting treatment scheme. Search the target value of output of decision medical support system makes it possible to select top of graph system that is located with more probability on the best way to the target. Important step in addressing the process of personalizing treatment schemes is estimated function that is based on Bayes theorem. Weight of occurrence next event corresponds to the highest value of the posterior probability of occurrence of the next state, given the time-dependent input parameters. Proposed improved method of decision-making for personalization standard schemes by modifying the method of decision-making based on decision trees considering relationship between the input parameters and evaluation function and result of its works is a personalized therapeutic scheme of treatment. It analyzed the quantitative results of applying the proposed method and existing for determining personalized schemes.

In the paper, the problems of devices supporting functional rehabilitation of a human wrist were addressed. A literature review and a description of selected devices together with an indication of their advantages and disadvantages were conducted. The biomechanical structure of a human wrist was analyzed. On this basis and after taking into consideration ranges of motion of the selected joints the concept of a new mechanism was developed. A 3D model of the device was built in the Autodesk Inventor system. For the purpose of simulations another model was developed in the MSC Adams system. Issues of drives and sensors selection, as well as requirements for the control system, were examined.

W artykule przedstawiono system wspomagania decyzji odnośnie do oceny stanu ogólnego pacjenta, ze szczególnym naciskiem położonym na choroby układu krążenia i układu oddechowego. Działanie tego systemu jest oparte na danych wejściowych ze standardowego monitora funkcji życiowych (w fazie rozwoju algorytmów dane są również symulowane). Sygnały są przetwarzane i analizowane za pomocą algorytmów opartych, przede wszystkim, na sieciach Bayesa. System na wyjściu powinien generować informację na temat wykrytych problemów, przyszłych zagrożeń, przyczyn pogorszenia zdrowia i propozycji dalszego leczenia.
This paper presents decision support system for estimation of patient's general state, with particular stress laid on cardiovascular and pulmonary diseases. It works in conjunction with a standard medical monitor, which provides asset of input signals (for development stage signals are also simulated). The signals are preprocessed and analysed by a set of algorithms, the core of which is based on Bayesian networks. As an output the system should give information about detected problems, eventual future threats, possible causes and suggestions for further treatment.

The article has considered the general principles and capabilities of the medical support of NATO troops, confirmed the expediency of further transformation of tasks and functions of the state in terms of medical support of troops (forces) of the security and defense sector of Ukraine in crises through the approximation to the relevant NATO standards, justified the need to implement the Euro-Atlantic experience of rendering medical care, use the general framework of categories and concepts, principles of the evaluation of capabilities (assets) of medical support which will contribute to the development of integrated medical space for medical defense of troops (forces) and elaboration of individual crisis response model taking into account the basic international requirements. The authors have determined the priority tasks of further cooperation between Ukraine and the North Atlantic Treaty Organization in the context of the reforms of medical logistics. It has been substantiated the need to keep reforming military medicine in conformity with NATO standards and recommendations that will promote an active involvement of medical services of troops (forces) in medical support of national and multi-domestic contingents during crises and adaptation of interoperable medical capabilities of joint missions under the auspices of NATO and the UN. The research has analyzed the mechanisms of interdepartmental coordination on medical support of troops (forces) and proved the feasibility of using the experience of NATO member states in the organization and functioning of the system of medical support of the military contingent with the adaptation of NATO doctrinal documents to the realities of modern Ukraine.

When patients return to the emergency department (ED) within 72 hours after their previous ED discharge, it is generally assumed that their initial evaluation or treatment had been somehow inadequate. Mining data related to unplanned ED revisits is one method to determine whether this problem can be overcome, and to generate useful guidelines in this regard. In this study, we use the receiver operating characteristic (ROC) curve to compare the data mining model by affinity set to other well known approaches. Some scholars have validated the affinity model for its simplicity and power in handling information systems especially when showing binary consequences. In experimental results, SVM showed the best performance, with the affinity model following only slightly behind. This study demonstrated that when patients visit the ED with normotensive status or smooth breath patterns, or when the physician-patient ratio is moderate, the frequency with which patients revisit the ED is significantly higher.

Operation of a sewage treatment plant is a complex task because it requires maintaining the parameters of its activities at the appropriate level in order to achieve the desired effect of reducing pollution and reduce the flow of sediment discharged from the biological reactor. The basis for predicting the amount of excess sludge and operational parameters WWTP can provide physical models describing the biochemical changes occurring in the reactor, in which the input parameters, ie. Indicators of effluent quality and quantity of wastewater are modeled in advance. However, due to numerous interactions and uncertainty of the data in the physical models and forecast errors parameters of the inlet to the treatment plant Simulation results may be affected by significant errors. Therefore, to minimize the prediction error parameters of operation of the technological objects deliberate use of a black box model. In these models at the stage of learning is generated model structure underlying the projections analyzed the operating parameters of the plant. This publication presents the possibility of the use of methods: support vector, k – nearest neighbour and trees reinforced to predict the amount of the resulting excess sludge during wastewater treatment in the WWTP located in Sitkówka – News with a capacity of 72,000 3/d with a load of 275,000 PE . Due to the fact that did not have the quality parameters of wastewater at the inlet to the activated sludge chambers it was not possible to verify the empirical relationships commonly used in engineering practice to determine the size of the daily flow of excess sludge. Due to the significant differences in the amount of excess sludge generated in the period (t = 1-7 days) the simulation of the amount of sludge into the time were performed. To assessment the compatibility of measurement results and simulations quantities of sludge the mean absolute error and relative error of prediction for the considered parameter of technology was used. The analyzes carried out revealed that the amount of generated excess sludge can be predicted on the basis of parameters describing the quantity and quality of influent waste water (slurry concentration of total nitrogen and total phosphorus, BOD5) and the operating parameters of the biological reactor (recirculation rate, concentration and temperature of the sludge, the dosed amount of methanol and PIX). On the basis of computations, it can be concluded that the most accurate forecasting results amounts of sediment were obtained by using a reinforced trees (t = 2 to 5 days) and Support Vector Machines methods (t = 1, 6, 7 days). While the highest values of forecast errors sediments was obtained using a k – nearest neighbor (t = 2 to 5 days) and reinforced trees (t = 1, 6, 7 days).

Cancer patient needs and deserves various forms of support. The more assistance he receives, the better his functioning during and after the illness gets. Both science and practice provide evidence that it is not only medicine that affects the quality of life in a disease. Psychological and social factors also play an important role, including professional activity, which can have not only a financial but also a therapeutic dimension. The article discusses the role of employment and occupational activity on the quality of life of cancer patients.It describes the challenges resulting from combining treatment with work It also contains examples of social activities and projects, that are aimed towards helping post-cancer patients to get back on their feet as well as regain confidence within the labour market.
Pacjent z chorobą nowotworową potrzebuje i zasługuje na zróżnicowane wsparcie. Im większe i szersze ono będzie, tym lepsze będzie funkcjonowanie w trakcie i po chorobie. Zarówno nauka, jak i praktyka dostarczają dowodów na to, że nie tylko medycyna ma wpływ na jakość życia w chorobie. Dużą rolę odgrywają również czynniki psychologiczne i społeczne, w tym aktywność zawodowa, która może mieć wymiar nie tylko finansowy, ale i terapeutyczny. Artykuł ten dotyczy roli jaką odgrywa praca w życiu osób z doświadczeniem choroby nowotworowej. Opisane zostały wyzwania związane z łączeniem pracy i leczenia oraz z powrotem do codzienności zawodowej po leczeniu, a także przykłady projektów pomocy w powrocie na rynek pracy po chorobie onkologicznej.
