Objectives More and more countries introduce a total ban on smoking tobacco in public places. The aim of this work was to evaluate the effectiveness of “The Act of 8 April 2010 on amendment of the act on protection of health against the consequences of consumption of tobacco and tobacco products and act on National Sanitary Inspectorate” and assess the frequency of complying with the smoking bans by the students of the Medical University of Lodz, Poland. Material and Methods Between 2007–2011, at the Social Medicine Institute of the Medical University of Lodz, a study using random survey was carried out involving students who were starting their studies at the Health Department of the Medical University of Lodz. The analysis of the collected material showed that 1038 people reported being smokers at the time of the study. Among that group, 530 students were included in the study prior to, and 508 after the introduction of the amendment. In order to verify their compliance with the smoking ban, the respondents were asked whether they smoked only in designated areas or wherever they wanted to. Results The ratio of people claiming they smoked anywhere they wanted to, disregarding the smoking ban, was 60% (N = 318) and after the amendment had been introduced, this ratio was 62.2% (N = 316), it increased by 2.2 percentage points. The observed difference was statistically irrelevant (Chi² = 0.530, p > 0.05). Conclusions The Act ”On amendment of the act on protection of health against the consequences of consumption of tobacco and tobacco products and Act on National Sanitary Inspectorate” in Poland did not result in the expected changes in the frequency of complying with the smoking ban by the 1st year students.

W XVIII w. nałóg palenia tytoniu był powszechny we wszystkich kręgach społeczeństwa. Zwyczaj palenia tytoniu w towarzystwie był również notowany na kartach ksiąg miejskich z okresu staropolskiego. Kilka zapisów z księgi miejskiej Uniejowa z 1763 r. dotyczy sporu o fajkę z tytoniem. W domu jednego z mieszczan zebrało się wówczas grono znajomych z lokalnej elity. Znajdujący się w nim burmistrz Uniejowa wypalił pożyczoną od kupca ze Zdun fajkę z tytoniem. Kupiec zażądał zwrotu tytoniu, a na ofertę zapłaty za tytoń pieniędzmi zareagował lekceważąco. W efekcie poirytowany burmistrz uderzył kupca w twarz. Doszło do rękoczynów, burmistrz i kupiec odnieśli szereg ran oraz potłuczeń na ciele. Impulsywny burmistrz zastał skazany za zainicjowanie bójki na okres tygodnia pozbawienia wolności. Swego rodzaju scenka rodzajowa zapisana na kartach księgi wójtowsko-ławniczej wiele mówi o ówczesnej mentalności, tj. popędliwości krewkich mieszczan w sposobie rozwiązywania nawet takich drobnych nieporozumień.
Smoking tobacco was very popular in the 18th century among all social groups. The custom of smoking in company was also noted in municipal books from the Old Polish period. Several entries in municipal records from the year 1763 address a dispute over a pipe with tobacco. A group of representatives of local elite gathered in a house of one of the burghers. The mayor of Uniejów smoked a pipe borrowed from a merchant of Zduńska Wola. The merchant demanded that he return the tobacco, and disrespectfully dismissed the offer to pay for the tobacco, whereupon the irritated mayor slapped him on the face. A fight ensued, in which both of them sustained numerous wounds and bruises. The mayor was sentenced to a week’s imprisonment. This scene reflects the mentality of those times, i.e. impulsiveness of hot-tempered burghers in resolving even such minor disputes.

The purpose of the research is analysis of a model of influence of excise tax rate increase for tobacco products in Ukraine on financial indices of state funds, prevalence of tobacco smoking, and indices of society health. Due to the results of research, the author determined the opportunities of the excise tax for tobacco smoking prevalence regulation in Ukraine. The dynamics of changes of specific and ad valorem rates of the excise tax and the dynamics of its contribution to the revenues of the State Budget of Ukraine were analyzed. The author estimated the changes of tobacco smoking prevalence in Ukraine on the basis of an adapted imitation model under conditions of increase of excise tax rates to the level of rates of the European Union countries. The research shows the excise tax is an effective financial regulator of tobacco smoking prevalence in Ukraine and it influences a population health condition. Increase of the excise tax rates causes reduction of amounts of tobacco product consumption and decrease of the share of smoking adults. Gradual increase of the Ukrainian rates to the level of the EU before 2018 may allow pulling from 1.0 to 1.9 million of people through premature death. Aside from a positive social effect of health enhancement, increase of the share of excise tax within a structure of a retail price of tobacco products will positively influence the growth of tax incomings to the State Budget. Consequently, under the Ukrainian contemporary economic environment the excise tax is an effective financial regulator of tobacco smoking prevalence.

Objectives: The aim of the study was to evaluate the prevalence and socio-demographic correlates of daily cigarette smoking among adults in Poland. Materials and Methods: A nationally representative household study was implemented between 2009 and 2010 to explore smoking pattern among the population aged 15 years and older. The smoking status and socio-demographic data were determined based on the Global Adult Tobacco Survey (GATS) questionnaire. Out of the 14 000 households selected for the survey, 7840 sampled persons completed the interviews, including 2162 respondents who declared daily smoking of cigarettes. Logistic regression models were applied to assess factors related to daily cigarette smoking. Results: Over 33% of men and 21% of women (p < 0.01) reported tobacco smoking on a daily basis. The significantly higher risk of smoking on a daily basis was observed among the male and female 20–59 years of age compared to the 60 or older population (p < 0.05). For men and women with the lower educational (primary/vocational/secondary) level, the risk of smoking was significantly higher than for the subjects with university degree (p < 0.05). The unemployed men smoked daily significantly more frequently than the employed ones (OR = 1.8; 95% CI: 1.4–2.4). This association was not observed among women (p > 0.05). The residents of urban areas smoked significantly more frequently than people living in rural settings (p < 0.05). Conclusions: Actions to tackle socio-economic inequalities in smoking need to be intensified. The antismoking efforts should be focused on the population of Poles at large and people with lower educational levels, unemployed men, and residents of large urban settings in particular. The tobacco control interventions should also address the population of women at reproductive age.

Tobacco smoking and smoking-attributable mortality in Central and Eastern EuropeTobacco is the major determinant of the health gap between the east (EU10) and the west part (EU15) of European Union. Over half of the excess mortality among middle-aged men in the EU10 countries might have been avoided if the tobacco consumption of these men had been the same as that of equivalent men in the EU15. The role of tobacco in shaping mortality differences between the EU10 and EU15 countries in women is less prominent than in men. Nevertheless, about one fourth of the excess mortality among middle-aged women in EU10 countries can be attributed to their higher tobacco consumption. 41% of male deaths between 35 and 64 years were attributable to tobacco smoking in the EU10 countries compared with 33% in the EU15 countries. The figures for women were 17% in the EU10 and 14% in the EU15 countries. It is worth emphasizing that, in some EU10 countries, the tobacco control policies and programmes have already produced notable results on tobacco- attributable mortality, in particular among men.

The Catholic Church with Regard to a Family at Risk from Cigarette SmokeFamily is the value of particular people, societies, the world and the Church. Smoking of tobacco is one among numerous threats to functions of families. The threat afflicts human physical, mental and spiritual health as well as his environment suffering from being forced to passive smoking. The problem of smoking also concerns interpersonal relations and it disturbs sexual and procreative, tutelary-educational and economical functions. Smoking parents do not take into account their child’s well-being, health and the conditions for his or her development. They frequently give priority to their own pleasures over the child’s needs. They expose their child to the threat of the loss of health or even life and set a poor example to follow. Smoking of tobacco violates the Fifth Commandment (‘thou shalt not kill’ [You shall not murder]) and the Commandment of Love; it is a sin against God, other person and smokers themselves. The Catholic Church does not act indifferent about such behaviour. It encourages us in bearing testimony to our own lives, to be an example of behaviour free from addiction, by religious instruction lessons, sermons, retreats, courses for fiancées and fiancés and TV and radio broadcasts. In the time of struggling of our freedom and enslavement, it is good to notice, appreciate and utilize the potentiality of the Roman Catholic Church.

Introduction. Cessation of tobacco use has the potential to provide the greatest immediate benefits for tobacco control. Understanding the social determinants of smoking cessation is an essential requirement for increasing smoking cessation at the population level. The purpose of this study was to analyze the socio-economic dimensions associated with cessation success among adults in Argentina and Uruguay. Materials and methods. Data from the Global Adult Tobacco Survey (GATS), a cross-sectional, population-based, nationally representative survey conducted in Argentina (n=5,383) and Uruguay (n=4,833) was utilized. Univariable and multivariable logistic regression analyses with results being presented as odds ratios (OR) with 95% confidence intervals were applied to study differences among those respondents who sustained smoking abstinence (≥1 year) and those who continued smoking. Results. The GATS study revealed that social gradients in tobacco quitting exist in Argentina and Uruguay. Being aged 25–34, particularly men in Uruguay, women in Argentina, low educated men in Argentina and having a lower asset index were associated with reduced odds for quitting. Conclusion. Factors that are driving differences in smoking cessation between diverse social groups in Latin America countries need to be considered when implementing relevant interventions to ensure tobacco control strategies work effectively for all population segments.

Tobacco smoking in male and female population, Poland 1974–2004On the basis of nation-wide randomized surveys, this paper analyzes patterns and trends of smoking prevalence in Polish adult (20+) population. In 1974, 62% of adult men and 19% of women smoked daily. In 1982, smoking rates reached the highest level ever observed in Poland (among Poles aged 20–39, 70-80% of men and  0-50% of women smoked daily). Percent of former smokers was low (15% for men, 6% for women). There was also big gender difference in prevalence of never smoking (15% in men, 70% in women). In the past two decades, substantial decline in smoking prevalence is observed. In men, decline in smoking rates (43% of daily smokers in 2000–2004), contributed to decrease in lung cancer mortality. In women, smoking rates have not changed since the mid-1980s and did not led to decrease in lung cancer mortality, however, there are three different age-specific time trends in smoking prevalence in female population.

Objectives To evaluate the health effects of benzene exposure among smoking subjects from a prolonged flaring incident that occurred at the British Petroleum (BP) refinery in Texas City, USA. Material and Methods The study included smoking subjects who had been exposed and unexposed to the benzene release. Using medical charts, clinical data including white blood cell (WBC) counts, platelet counts, hemoglobin, hematocrit, blood urea nitrogen (BUN), creatinine, alkaline phosphatase (ALP), aspartate aminotransferase (AST), and alanine aminotransferase (ALT) in the case of smoking subjects exposed to benzene was reviewed and analyzed. Results A total of 791 tobacco smoking subjects (benzene-exposed: N = 733, unexposed: N = 58) were included. Benzene-exposed subjects had significantly higher levels of WBC (×103/μl) counts (8±2.1 vs. 7.5±1.6, p = 0.003) and platelet (×103/μl) counts (263.7±69.7 vs. 222.9±44.3, p = 0.000) as compared with the unexposed subjects. The mean hemoglobin, hematocrit, BUN, and creatinine levels did not differ significantly between the benzene-exposed and -unexposed smoking subjects. Serum levels of ALP (IU/l) was significantly elevated in the benzene-exposed subjects compared with the unexposed subjects (84.5±16.9 vs. 73.8±15.9, p = 0.002). Similarly, benzene-exposed subjects had significantly higher levels of AST and ALT as compared with those unexposed subjects. Conclusions Despite a smoking history, residents exposed to benzene from the prolonged BP flaring incident experienced significant alterations in hematological and hepatic functions indicating their vulnerability to the risk of developing hepatic or blood related disorders. Background. Electronic cigarettes (e-cigarettes) are small battery-powered electronic devices, heating the liquid to produce vapour – in most cases the latter contains nicotine and several flavourings. E-cigarettes are highly advertised across the media, mainly as healthy substitute to conventional cigarettes, aid in quitting smoking addiction or way of circumventing ban on smoking in public places. Objective. The aim of study was obtaining epidemiological data on cigarette smoking and electronic cigarette usage among Polish universities students. Material and methods. Students of different Polish state universities were asked to fill a self-prepared survey on cigarettesmoking and electronic cigarette usage. 1068 fulfilled questionnaires were gathered. The population was divided into two subgroups – medical universities’ students (n=545) and non-medical universities students (n=523). Results. 23.78% of respondents declared current smoking while 57.0% admitted ever smoking. The mean duration of smoking among current smokers was 4.17 ± 2.53 years. 56.30% of current smokers tried quitting at least once. 31.46% of students declared ever using e-cigarettes (37.28% (n=195) among non-medical universities’ students and 25.87% (n=141) among medical universities’ students and 8.33% current usage. Among the latter 52.81% admitted simultaneous smoking. 26.97% of current e-cigarettes’ users declared having experienced side effects of e-cigarettes. 42.70% (n=456) of respondents viewed e-cigarettes as safer than conventional cigarettes, this group comprises of 40.54% (n=212) non-medical and 44.77% (n=244) medical universities’ students. 85.39% (n=912) of students viewed e-cigarettes as generally unhealthy, there were 83.56% (n=437) non-medical and 87.16% (n=475) medical universities’ students among this group. Conclusions. The frequency of e-cigarettes usage resembles current status in many Western countries. Collected data shows high frequency of e-cigarettes usage and conventional cigarettes smoking among students (also medical universities’ students). The situation requires intensive preventive measures to limit and reduce the popularity of tobacco products along with modern equivalents like electronic cigarettes.
Wprowadzenie. Elektroniczne papierosy (e-papierosy) są niewielkimi urządzeniami elektronicznymi, zasilanymi bateriami. Po ogrzaniu płynu zawartego w papierosie utworzona zostaje para, zawierającą określona ilość nikotyny i substancji zapachowych. E-papierosy są szeroko reklamowane na całym świecie, najczęściej jako zdrowsza alternatywa dla tradycyjnych papierosów, pomoc w rzucaniu nałogu tytoniowego lub sposób na ominięcie zakazu palenia w miejscach publicznych. Cel. Celem badania było uzyskanie danych epidemiologicznych dotyczących palenia papierosów oraz używania e-papierosów przez studentów polskich publicznych uczelni wyższych. Materiał i metody. Studenci wielu polskich publicznych uczelni wyższych zostali poproszeni o wypełnienie autorskiego formularza ankietowego na temat palenia papierosów oraz użytkowania e-papierosów. Do analizy użyto 1068 prawidłowo wypełnionych formularzy ankiety. Grupa respondentów została podzielona na dwie podgrupy – studentów uniwersytetów medycznych (n=545) oraz niemedycznych (n=523). Wyniki. Wśród ankietowanych 23.78% zadeklarowało aktywne palenie w momencie wypełniania ankiety, 57.0% przyznało się do palenia papierosów kiedykolwiek w przeszłości. Średnia długość nałogu obecnych palaczy wynosi 4.17 ± 2.53 lat. 56.30% obecnych palaczy próbowało przynajmniej raz rzucić nałóg. 31.46% ankietowanych deklarowało użycie e-papierosów przynajmniej jeden raz. Wśród nich 37.28% (n=195) stanowili studenci uczelni niemedycznych i 25.87% (n=141) studenci uczelni medycznych. 8.33% badanych używa ich obecnie, pośród nich 52.81% zadeklarowało jednoczasowe palenie tradycyjnych papierosów. 26.97% obecnych użytkowników e-papierosów deklarowało odczuwanie przynajmniej raz skutków ubocznych korzystania z tych urządzeń. 42.70% (n=456) badanych uważa e-papierosy za bezpieczniejsze dla zdrowia od tradycyjnych papierosów, na tą grupę przypada 40.54% (n=212) studentów uczelni niemedycznych oraz 44.77% (n=244) medycznych. 85.39% ankietowanych (n=912) uważa e-papierosy za niekorzystne dla zdrowia, z czego 83.56% (n=437) to studenci uczelni niemedycznych i 87.16% (n=475) medycznych. Wnioski. Odsetek populacji polskich studentów używających e-papierosy jest zbliżony do wyników uzyskanych przez badaczy w wielu krajach zachodnich. Dane te wskazują na częste używanie e-papierosów i papierosów tradycyjnych wśród studentów, w tym także studentów uczelni medycznych. Introduction: The article presents a selected part of an analysis of health- and lifestyle-related determinants of the longevity of male residents of the city of Łódź. The WHO strategy for the European region assumes that the term ‘lifestyle' refers to the way of life based on the association between conditions of life understood in a broad sense and individual patterns of behaviour determined by socio-cultural factors and individual features. On this basis, the aim of this study is to identify the multiple statistical relationships on the mortality of men living in Łódź, particularly the influence of the following variables on the probability of survival: age, subjective health assessment, nutritional habits, alcohol consumption and tobacco smoking. Materials and Methods: The study sample (1004 respondents) was selected, and the study was carried out, based on the standards of the CINDI WHO Programme. The investigations included socio-demographic data, health status, lifestyle and anthropometric measurements. Results: From the results of the first- study performed in Poland of the longevity of men with identified risk factors of cardiovascular diseases, it has been found that: 1) Tobacco smoking has a negative influence on male longevity; 2) Alcohol consumed in moderate amounts favours male longevity; 3) The level of physical activity observed among the studied men was too low to affect longevity; 4) A diet rich in fish and consumption of yellow cheese are positive predictors of longevity, while additional use of table salt at meals and consumption of sweets (cakes) are negative ones. Conclusions: Risky lifestyles and a reluctance to abandon them are responsible for a shorter lifespan among men in Poland. Reducing this difference between women and men is a real challenge for public health professionals and preventive medicine specialists.

Tobacco smoking is a major avoidable single cause of premature mortality in Poland. Almost one in three Polish males do not live to 65 years of age, and almost half of this premature mortality can be traced back to the much higher smoking prevalence in Poland than in Western Europe – every third Polish male and every fourth Polish female smokes daily. However, the current health situation in Poland is much better than two decades ago when the country entered a period of political and economic upheaval. In the early 1990s, the state of health of the Polish population was catastrophic and its tobacco consumption levels the highest in the world. In the early 1990s, the probability of a 15-year-old Polish boy living to the age of 60 was not just twice lower than in Western Europe, but also lower than in China or India. The health policy of limiting the health consequences of smoking conducted by the European Union and, in the last two decades, by the Polish parliament and government, helped to stop this health catastrophe. In Poland, cigarette consumption has decreased by 30% since 1990, as did lung cancer mortality among males. Despite this progress, tobacco smoking remains the most serious health problem in Poland. Therefore, comprehensive tobacco control policy should not only be continued, but expanded and accelerated. The EU Tobacco Products Directive proposes a package of actions for reducing tobacco-related health harm in Europe. The Directive proposal is rational, science-and-evidence based, and grounded on the best practice examples from other countries. Both the Polish tobacco control law and the WHO Framework Convention on Tobacco Control (FCTC), ratified by Poland in 2006, oblige our country to support tobacco control, including all the initiatives taken by the European Union.

Introduction: It is still uncertain if having at least moderate physical fitness is a necessary and sufficient condition for lowering the risk of death. The aforementioned statement constituted the basis for undertaking the study concerning the effect of particular health-related behaviours on the likelihood of survival in subjects with a moderate and high physical fitness. Materials and Methods: The study sample, i.e. 204 men aged 30-59 years living in Łódź, Poland, was selected within the CINDI WHO Programme and examined over the years 1980-1990 and 2003-2004. In each subject approximate values of one week energy expenditures associated with performing physical exercise of at least moderate intensity (> 1000 kcal/week) were estimated. Physical efficiency in the study group was evaluated basing on the results of the submaximal effort test. Information about selected socio-demographic characteristics, consumption of alcohol, cigarette smoking and diet was gathered. The vital status of the examined sample was checked in 2009. The Cox proportional hazards regression model was used to identify factors which influence the survival of examined population. Results: The probability of death was more than fourfold higher in the case of individuals who eat "beef or pork meat", as compared to those who consumed these products rarely or never. On the other hand, the subjects who declared regular consumption of yellow cheese had nearly fourfold lower death probability. Current smoking was found to be a significant negative risk factor while moderate consumption of beer a protective one. The level of physical fitness ($\text{V}_\text{O2max}$) did not significantly influence mortality. Conclusions: The results of the performed analyses indicate negative effect associated with consumption of beef/pork meat and smoking. Furthermore, the data pertaining to the consumption of yellow cheese as a potent protective factor for men's health requires further verification. The possibility that it is some lifestyle proxy rather than a causal factor can not be excluded.

Introduction: Longitudinal studies of multiple sclerosis (MS) demonstrated decrease of mortality rates (MR) throughout Europe and United States. The aim of this study was to explore possible a correlation between reduced MR in MS and prevalence rates (PR) of cigarette smoking (CS) in Poland. Method: The study was based on 10 272 MS patients (M – 4367, F – 5905) who died since 1982 thru 2008. Annual, average, crude MR (CMR) and sex-adjusted MR (SAMR) in MS per 100 000 population were correlated to PR of CS per 100 inhabitants in the general population and 2 subpopulations. Data were obtained from the Central Statistical Office and the Centre of Oncology in Warsaw. Results: CMR, SAMR in M and F were significantly higher in the years 1982–1995: 1.33 (SD 0.07), 1.19 (SD 0.13), 1.47 (SD 0.07) than in the years 1996–2008: p=0.000006, p=0.0002, p=0.0008. There was a decrease of CS PR in the years 1982–2008 in the general population (p=0.000003) and in M (p=0.0000004). No decrease of CS PR in F was found (p=0.127). CMR and SAMR among M with MS showed highly significant correlation to PR of CS in the period 1982–2008; r=0.673,p=0.0006; r=0.774, p=0.0001. No correlation was found between SAMR among F with MS and PR of CS: r=0.369, p=0.0910. Conclusion: Reduction of MS mortality was associated with a decreased incidence of CS in the general population and among males, but not among females.
Wprowadzenie: Długookresowe badania stwardnienia rozsianego (SM) udowodniły spadek umieralności (U) w Europie i USA. Celem badania jest wykazanie związku między zmniejszeniem U na SM i rozpowszechnieniem palenia papierosów (PP) w Polsce. Metoda: Badania oparto na 10 272 chorych na SM (M – 4367, K – 5905), którzy zmarli w latach 1982–2008. Przeciętne, roczne, surowe współczynniki U (SWU) oraz dostosowane do płci współczynniki U (DPWU) w SM na 100 000 ludności skorelowano z rocznymi współczynnikami rozpowszechnienia (WR) PP na 100 mieszkańców w ogólnej populacji i 2 podpopulacjach. Dane uzyskano z Głównego Urzędu Statystycznego oraz z Centrum Onkologii w Warszawie. Wyniki: SWU, DPWU wśród M i K na SM były istotnie wyższe w latach 1982–1995: 1,33 (SD 0,07), 1,19 (SD 0,13), 1,47 (SD 0,07) niż w latach 1996–2008: p=0,000006, p=0,0002, p=0,0008. WR PP obniżyły się w latach 1982–2008 w ogólnej populacji (p=0,000003) i wśród mężczyzn (p=0,0000004), lecz nie wśród kobiet (p=0,127). SWU i DPWU wśród M z SM wykazały wysoką korelację z WR PP w latach 1982–2008; r=0,673, p=0,0006 i r=0,774, p=0,0001. Nie stwierdzono korelacji DPWU wśród K z SM i WR PP: r=0,369, p=0,0910. Wniosek: Spadek U na SM miał związek z rzadszym PP w ogólnej populacji i wśród mężczyzn, lecz nie wykazał asocjacji w podpopulacji kobiet w latach 1982–2008 w Polsce.
