Epidemiological survey and antitobacco intervention in Polish 400 Cities ProjecCardiovascular diseases (CVD) represent the principal cause of death in Poland. The main reasons are high prevalence and low detectability of main risk factors: arterial hypertension, diabetes and hyprelipidaemia. Also smoking is one of the main CVD risk factor. Polish 400 Cities Project consists of the following modules: 1. social marketing preceding screening tests and education,2. medical intervention – screening tests,3. education for local leaders regarding health promotion and CVD prevention,4. antitobacco intervention,5. educational intervention for children: training program for teachershealth promotion among pupils,6. training programs for doctors and nurses,7. education for patients with newly-detected diseases.Antitobacco module contains the following interventions:1) social marketing via mass media,2) education among children in schools,3) education for smoking patients with newly-detected diseases,4) training program for doctors: treatment of nicotinism,5) training program for local representatives: elaboration of local antitobacco program,6) in 2006 new module – antitobacco intervention for pregnant women.In years 2003–2006 screening tests were performer among 92378 adults. Tobacco smoking was reported by 12.3% of woman and 21.9% of men, mainly in the age group 25–45 (W 21.1%; M 28.5%). Data from intervention among children show that among 24,005 ten years old boys and girls, 5.7% girls and 12.8% boys say “yes” for the question “have you ever smoke” in the questionnaire interview. In years 2006–2008 the main antitobacco module was antitobacco intervention for pregnant women. The purpose of the research was to measure phenomenon of active and passive smoking among pregnant women in the smallest cities with surrounding villages. The aim was also to trace social features associated with smoking and develop the strategy for future antitabacco interventions. The research was conducted along with intervention based on American College of Obstetricians and Gynecologists 5 A’s model tailored for pregnant women adjusted to Polish organizational limitations. The model include 1) asking about tobacco smoking during every contact with pregnant women, 2) advising not to smoke or avoid passive exposure, 3) assessing the determination to make a quit attempt, 4) assist with the trial, 5) arrange next meeting with the focus on the problem. During the research 919 pregnant women were questioned and examined with micro Co device. Among that number were 22% of active smokers and 31% exposed on passive smoking in their home or work. The prevalence of smoking were higher among lower educated – 46% of women with only primary school compared with 7% of women with university diploma. Smoking was more frequent among women with lower per capita personal income. 80% of smoking pregnant women were motivated to make a quit attempt.

Introduction. The epidemic of tobacco smoking remains one of the most important challenges in public health. It is associated with premature death. For this reason, the Act of 9 November 1995 on the Protection of Public Health against the Effects of Use of Tobacco and Tobacco Products, as amended on 15 November 2010, implemented significant additional restrictions on smoking in public places. Objective. The objective of this study is evaluation of the impact of smoking regulations on social behaviour and attitude to the legal forms of tobacco control, due to the new smoke-free policy. Materials and method. A secondary statistical analysis of third party data was used in the study. The material was obtained from the Public Opinion Research Centre. Polls are conducted among representative samples of about 1,000 adult inhabitants of Poland. The studies on cigarette smoking were carried out in 3 editions (2010, 2011, 2012). Results. The collected data on tobacco smoking before and after implementation of the Act show that the number of smokers remained at a comparable level – about 30% (p=0.891). Between 2010–2012, an increase was observed in the percentage of people supporting the smoking ban in public places. The total number of people who definitely or rather supported the smoking ban increased from 75.6% in 2010 to 85.0% in 2012 (p<0.001). Conclusions. The restrictive smoking ban in public places implemented in 2010 did not have an impact on the number of smokers in the studied time interval, although an increase in the number of people supporting the smoking ban in public places was observed.

Objectives Monitoring tobacco use is one of the most important tobacco control interventions recommended by the World Health Organization. This study aimed to assess the prevalence of cigarette smoking, e-cigarette use, and heated tobacco use in a nationwide sample of adults in Poland, as well as to identify factors associated with smoking, e-cigarette use, and heated tobacco use. Material and Methods This nationwide crosssectional survey was carried out in February 2–4, 2024 using the computer-assisted web interviewing method. The study questionnaire included 17 closed questions on smoking, e-cigarette use, and heated tobacco use. A sample of 1080 adults was selected using a non-probability quota sampling method. Results Data were collected from 1080 adults, age M±SD 48.4±15.5 years, 53% females. Past 30-day smoking was declared by 30.4% of adults in Poland, including 24.5% of daily smokers. Ever e-cigarette use was declared by 33% of respondents, and 20.6% had ever tried heated tobacco. Past 30-day e-cigarette use was declared by 15.2% of adults, wherein 5.9% were daily e-cigarette users. Past 30-day heated tobacco use was declared by 10.9% of respondents, wherein 4.9% were daily heated tobacco users. One-tenth of adults were dual users (cigarette and e-cigarette or heated tobacco), and 6.5% were past 30-day triple users. Age 40–59 years, having less than higher education, and bad economic status were significantly associated with daily smoking (p < 0.05). Age 18–39 years, having less than higher education, and active occupational status (employment/ self-employment) were associated with daily e-cigarette use (p < 0.05). Age 18–39 years and active occupational status were associated with daily heated tobacco use (p < 0.05). Conclusions This study provides comprehensive epidemiological data on the current prevalence of smoking, e-cigarette use, and heated tobacco use. The introduction of e-cigarettes and heated tobacco products evoked changes in social behaviors related to nicotine use.

ObjectivesUniformed services such as police employees are exposed to acute and chronic stressful events at work that may lead to tobacco use. This study aimed to assess the prevalence of cigarette smoking, e-cigarette use and heated tobacco use among police employees in Poland, and to investigate personal characteristics associated with tobacco or e-cigarette use.Material and MethodsThis cross-sectional study was carried out in June–July 2020 on a randomly selected sample of 8789 police employees from the Mazowieckie Province, Poland.ResultsCompleted questionnaires were obtained from 5082 police employees (79.2% being police officers) with an overall response rate of 57.8%. Smoking ≥100 cigarettes or similar amounts of other tobacco products was declared by 54.6% of the respondents, with significant differences (p < 0.001) between males (56.8%) and females (50.3%). Daily cigarette smoking was declared by 19.5% of the respondents, and 13.4% were occasional cigarette smokers. Daily e-cigarette use was declared by 3.1% of the respondents, and 3.2% were occasional e-cigarette users. Daily heated tobacco use was declared by 2.6% of the respondents, and 2.9% were occasional heated tobacco users. Higher odds of occasional cigarette smoking were observed among men compared to women (OR = 1.254, 95% CI: 1.009–1.558), and among the participants aged 20–29 years (OR = 7.982, 95% CI: 3.066–20.775) or 30–44 years (OR = 3.730, 95% CI: 1.44–9.599) vs. those aged ≥60 years. Higher odds of occasional e-cigarette use were observed among the participants aged 20–29 years (OR = 4.554, 95% CI: 1.213–17.101) vs. those aged 60 years. Police employees with office-based work had lower odds of daily cigarette smoking vs. those with fieldwork (OR = 0.726, 95% CI: 0.55–0.946). Police officers had higher odds of daily heated tobacco use compared to civil workers (OR = 3.362, 95% CI: 1.325–8.534). Conclusions The authors observed a marked proportion of police employees who declared occasional tobacco or e-cigarette use, which may indicate the common social smoking phenomenon in this occupational group.

Objectives To determine the age and the most common circumstances for smoking initiation along with smoking rates and to evaluate smoking trends for secondary and high school students in Poland during 2009 and 2011. Material and Methods In 2009, a pilot study was conducted in districts of Poland on high school students and their parents. For statistical analysis, correctly completed questionnaires from 999 students and 667 parents were qualified for use. After the pilot study, a nationwide study of secondary school students and their parents was also conducted in 2009. For statistical analysis, correctly completed questionnaires were used from 9360 students and 6951 from their parents. The research tool was a questionnaire developed by the Chief Sanitary Inspectorate. These studies were then compared to the nationwide research study from 2011. Questionnaires were obtained from a survey of 3548 students from secondary schools and 4423 of those from high schools. Results Smoking initiation usually begins at ages 12–15 years. Rates of secondary school student smoking at least once in their lifetime were about the same level in the surveyed years (2009 – 9%, 2011 – 11%), whereas rates of high school student smoking increased (2009 – 15%, 2011 – 24%). Moreover, 34% of secondary school student smoked less than once a week, whereas in 2009, only 8% of students had done so. For high school students, a 1/2 smoked every day; similar to 2009. Students usually smoked in parks, on streets or any other open space areas. Conclusions From analyzing the smoking trends over the survey period it can be concluded that the problem of smoking increases with respondent age. Int J Occup Med Environ Health 2017;30(5):763–773

According to the age-period-cohort projection of lung cancer incidence (ICD-10 codes C33-C34), smoking prevention will be crucial for both men and women longevity. Lung cancer remains the main cancer cause of death among males in Poland. In 2014 the number of deaths in Poland caused by lung cancer among men was more than twice as high as for women. The study is focused on identification of attitudes influencing the propensity to smoke, which is performed by structural equation modelling (SEM) based on weighted least squares means and variance adjusted (WLSMV) estimation. Good fit of the presented models is demonstrated by RMSEA, NNFI and CFI values. Reliability of scales is assessed by ordinal alpha and ordinal theta coefficients. Commitment to learning in childhood exhibits the highest standardized effect on propensity to smoke, which is an argument for youth smoking prevention. Structural equation model was estimated for men and women separately because of gender differences in smoking determinants. A strong character, individualism and self-reliance are the factors, which increase smoking propensity among males. These can be treated as features of masculinity and are not statistically important determinants of being a smoker for females. Depressive disorders, being a victim of violence as adults are statistically significant determinants of smoking among women.
