    Aviation is the youngest of the transport industries, yet despite its short history, it is considered one of the most important spheres of transport, both in terms of passenger and cargo transportation. Civil aviation is used by an increasing number of people, and the number of aircraft used by airlines around the world continues to grow. An inherent element that is a particularly important aspect of this mode of transportation is security. In civil aviation, there are numerous dangers associated with events occurring before the flight, during the flight, as well as those associated with the landing process. The events need to be controlled and their causes actively sought and ultimately prevented. The Polish Civil Aviation Authority, as part of the creation of the National Civil Aviation Safety Program, developed the National Safety Plan 2020-2023. The document covers threats identified in the Systemic, European, and National Areas. They are characterized and classified based on the materiality (significance) of the event. The aim of this article is to characterize and analyze selected factors (e.g. collisions with birds, helicopter events) that affect the number of safety incidents in civil aviation. The background of the study was the analysis and synthesis of the literature on the subject, while the main research method was the statistical analysis of historical data on aviation incidents. The data provided in Poland's National Security Plan 2020-2023 were used to distinguish the factors associated with the threats present and synthetically evaluate their impact. The analyses made it possible to identify areas of particular safety risks and form the basis for further detailed research.

    Safety in aviation has various connotations. According to the International Civil Aviation Organization (ICAO), it is a state in which the possibility of harm to people and property is minimised and maintained within a continuous process of identifying threats and managing safety risks at an acceptable level or below an acceptable level. Actions related to threat identification can be reactive, proactive, or predictive. Reactive actions aim to verify what happened, why, and how to prevent it from recurring. For this purpose, incidents that occurred in the past are analysed. By Polish law, the Commission for Investigating Aviation Accidents investigates accidents and serious incidents, while incidents mostly fall under the responsibility of the aviation organisation in which the incident occurred. Therefore, this article aims to identify threats causing incidents. Three thousand two hundred aviation incidents reported between 2017 and 2022 to the Civil Aviation Office as part of the mandatory and voluntary event reporting system were analysed. The identified causes, due to the diversity of their description, were divided into four groups. The first group consists of human factors, representing inadequate actions by pilots, crews, or individuals who caused a situation of danger. The second group comprises errors in the operation of technical objects, including aircraft. This group also includes situations where foreign objects violate airspace or minimum conditions are breached. The next group includes environmental causes such as wild animals, birds, and weather conditions. The last group consists of procedures related to flight phases.

    One of the primary sources of information and inspiration in the creation of new, more secure solutions in traffic organization are the occurrences with most serious consequences called accidents (air), serious accidents (rail) or catastrophes (road, sea). Accidents investigation is usually conducted in terms of searching for the reasons of these events and to make preventive recommendations aimed at elimination of these causes. In this paper, it is proposed to draw more attention to the events of somewhat less severity of the consequences - serious incidents (air), traffic conflicts (road) and incidents (rail, maritime). A model of real serious traffic incident, created with the use of stochastic, timed Petri nets is presented. Simulation experiments were carried out, which allowed for determination of the probability of transformation of the incident into the accident. The proposed method of analyzing serious traffic incidents also allows to determine the effectiveness of the barriers that have prevented an accident in a real occurrence.

W artykule opisano także działania i inicjatywy podejmowane na rzecz poprawy bezpieczeństwa.
    In the article the problem of the road safety in Bytom based on analysis of the period 2013-2017 is presented. The types of traffic incidents and their classification are specified. Selected issues that have a particular impact on the safety level of road users, such as the number of road accidents, the reasons for their occurrence, the perpetrators of accidents and their consequences, are described. Actions and initiatives taken to improve safety were discussed. In addition, the paper evaluates the effects of measures to improve road safety introduced in Bytom.

    National Road Safety Program 2013-2020 assumes to reduce fatalities by half and seriously injured by 40% from 2010 to 2020. It means that no more than 2,000 people per year should die on Polish roads and no more than 6,900 people should suffer serious injuries in traffic accidents. The experience of other European Union countries with improving road safety gives hope that these goals are still realistic, but there is a need to intensify the activities carried out in this area. In Poland, more and more institutions are taking part in the process of improving road safety: state institutions, nongovernmental organizations, local governments, enterprises, scientific and research institutions, the media and many others. The study presents the problem of road safety in the city of Bytom on the basis of its analysis in the years 2013-2017. It discusses the types of road accidents and their classification. Moreover, the paper describes the issues illustrating the situation on roads that have a particular impact on the safety level of road users, among others, the number of road accidents, reasons for their occurrence, the consequences and groups of perpetrators of accidents. It also describes actions and initiatives taken to improve road safety in the city of Bytom.

    W artykule omówiono stan bezpieczeństwa ruchu drogowego na sieci dróg krajowych województwa kujawsko-pomorskiego za lata 2015-2016 będących w zarządzaniu GDDKiA Oddział w Bydgoszczy. Scharakteryzowano stan bezpieczeństwa ruchu drogowego na tych drogach na tle kraju. Przedstawiono strukturę rodzajową zdarzeń drogowych, wypadki drogowe a: rodzaj zagospodarowania obszaru do nich przyległego, rodzaj elementu sieci drogowej, czas ich powstania i okoliczności, stan nawierzchni spowodowany warunkami pogodowymi, a także przyczyny powstawania wypadków drogowych.
    The article presents the state of road safety for the years 2015-2016 on the national road network of the Kujawsko-Pomorskie voivodship managed by GDDKiA Department in Bydgoszcz. Traffic safety on these roads has been assessed against the background of the whole country. In the article is presented: generic structure of road accidents, the type of development of the area near to them, type of road network element, their time and circumstances, pavement condition caused by weather conditions and also causes of road accidents.

    W pracy, na przykładzie kompozytu stomatologicznego przedstawiono problem oceny procesu i wyrobu medycznego z punktu widzenia finalnego niepożądanego zdarzenia określanego jako incydent medyczny oraz zaproponowano opartą o model metodę analizy takich zdarzeń. Rozpoznano cztery niepomijalnie prawdopodobne typy incydentów medycznych związanych ze stosowaniem kompozytu stomatologicznego. Są to: alergie, skaleczenia, zadławienia, zakrztuszenia oraz uszkodzenia tkanki miękkiej. Zidentyfikowan ciągi zdarzeń prowadzących do ich powstania. Wyniki tej identyfikacji zobrazowano za pomocą drzewa zdarzeń, które następnie przekształcono w sieć bayesowską. Taka reprezentacja wiedzy umożliwia, poprzez zastosowanie algorytmów uczenia maszynowego i automatycznego wnioskowania, uzyskiwanie praktycznie użytecznych odpowiedzi na szereg pytań dotyczących poziomu ryzyka i jego dystrybucji po stronie producenta i użytkownika wyrobu medycznego.
    On an example of a dental composite there was shown the problem of medical device evaluation from the viewpoint of finaladverse event known as medical incident. We proposed the model based method of description and analysis of such events. There are four likely types of medical incidents concerned with the application of dental composite: allergies, scars, throttlings, chokings, and soft tissue damage. Cause-consequence chains of events resulted with medical incidents were modeled using event tree method. resulted event tree was then converted into bayesian network. Such representation of knowledge allows for application of machine learning algorithms and automatic inference, getting practically useful answers to several questions about the level of risk and its distribution on the side of the product manufacturer and its user.

    Error is a part of human. Despite that organizations are trying to reduce error to the zero level, this goal is unachievable. As far as human operation is taking place in a complicated environment, error will occurred, and its possibility would be increased under the conditions of stress, extra loading work, and fatigue. One of the most important transportation modes is marine transportations. The sea is an unsafe place that kills many by a simple event. Every year there are thousands of marine accidents that result in injuries, casualties, marine pollutions and also massive financial loss. To reduce the accidents, there should be more attention to the factors such as suitable training of human resource, proper implementation of national and international laws and regulations, vessels and the equipment on board them, port facilities, and also the utilities for marine search and rescue. In this research 1816 marine accidents have been studied in five Iranian shipping companies. 17 factors are known to be effective in occurrence of human error in these accidents. Four factors of the most influence are negligence, poor training, inadequate tools, and lack of skill and experience.

    The article is an attempt to find a connection between the system of mandatory cyclic technical inspection of vehicles and their actual technical condition. How to perform technical research and verification identified in the course of possible faults, it has a significant influence on the operational reliability of vehicles and road traffic safety.

    The principles of civil aviation safety are considered. At present time, aviation is an ultra-safe system, (i.e. a system that experiences less than one catastrophic safety breakdown every one million production cycles). But, unfortunately, we can never state that aviation activities will be absolutely safe. In current conditions the increase of the civil aviation safety level is one of the principal objectives of the world air transport development. The paper pays special attention to the civil aviation threats and risks in the context of globalization processes. The investigation deals with problems of global implementation of Annex 19 to the Convention of the International Civil Aviation. Present research paper discusses new theoretical approaches to searching for methods to assess the “safety space” of civil aviation activity. Special attention is paid to an effective test, proposed by A. Wald.

    WstępCelem głównym badań było opisanie hierarchii wartości kierowców ze szczególnym uwzględnieniem wartości istotnych z punktu widzenia bezpieczeństwa na drodze, czyli bezpieczeństwa osobistego, społecznego oraz pokory i przystosowania do reguł, a także relacji między nimi a liczbą wypadków i kolizji, w których uczestniczyli. Celem dodatkowym było określenie znaczenia zmiennych socjometrycznych (wieku, płci, stażu w kierowaniu pojazdem, wykształcenia, pozostawania w związku, liczby dzieci) dla hierarchii wartości kierowców.Materiał i metodyZastosowano Portretowy kwestionariusz wartości (Portrait Value Questionnaire) autorstwa Schwartza, w polskiej adaptacji Cieciucha. Badania przeprowadzono w grupie 704 kierowców (356 kobiet i 348 mężczyzn; w wieku 18–77 lat), z których 303 w drugim etapie badań pytano o liczbę wypadków oraz kolizji, w których brali udział lub które spowodowali.WynikiBadani kierowcy w znacznym stopniu cenili życzliwość i bezpieczeństwo, nad wartością dyscypliny przeważała jednak potrzeba samostanowienia, zwłaszcza u mężczyzn i młodych kierowców. Liczba wypadków i kolizji okazała się istotna dla wartościowania bezpieczeństwa w kontekście społecznym, przy czym obserwowana współzależność mogła być dwukierunkowa. Spośród zmiennych kontrolowanych dla hierarchii wartości szczególne znaczenie miały wiek i płeć kierowców.WnioskiHierarchia wartości badanych kierowców wskazuje na cenienie w znacznym stopniu życzliwości i bezpieczeństwa, nad wartością dyscypliny (pokora, poszanowanie reguł) przeważa jednak potrzeba samostanowienia (kierowanie sobą w działaniu), zwłaszcza u mężczyzn i młodych kierowców. Głównym ograniczeniem badań był sposób gromadzenia danych wyłącznie na podstawie samoopisu – z tego powodu uzyskane wyniki mogą być obciążone potrzebą aprobaty społecznej.
    BackgroundThe main objective of the research was to describe the hierarchy of values of drivers, with particular emphasis on the values relevant to road safety – personal safety, social security, humility and adaptation to rules, as well as the relationship between them and the number of accidents and collisions in which drivers participated. The additional goal was to determine the significance of variables (age, sex, seniority in driving, education, staying in a relationship, the number of children) for the hierarchy of drivers’ values.Material and MethodsThe Portrait Value Questionnaire (PVQ) by Shalom Schwartz, in the Polish adaptation by Cieciuch, was applied. The research was conducted on a group of 704 drivers of both sexes (356 women and 348 men), aged: 18–77 years, of whom 303 people were asked about the number of accidents and collisions in which they had participated or which they had caused.ResultsThe drivers that took part in the research valued kindness and safety to a large extent, but the need for self-determination, especially in men and young drivers, prevailed over the value of discipline. The number of accidents and collisions turned out to be important for the valuation of security in the social context, with the observed interdependence being bi-directional. Among the variables controlled for the hierarchy of values, drivers’ age and gender of were of particular importance.ConclusionsThe hierarchy of values of the examined drivers indicates the appreciation for kindness and safety, to a large extent, and the value of discipline (humility and respect for rules), but the need for self-determination (self-management), especially in men and young drivers, was found to prevail. The main limitation of this research was the data collection method based solely on self-reports, as a result of which the obtained results may be burdened with the need to gain social approval.
