Introduction and aim. Nursing fields begin to face a wide variety of challenges when they first enter clinical practice. The aim of this study is to determine the difficulties experienced in patient communication by nursing students taking the clinical practice course for the first time. Material and methods. Focus group interview design was used for this study. Focus group design is used to give information about the opinions and experiences of the sample group on any subject. It is reported as a convenient design to identify different perspectives on a subject within the scope of the sample. Focus group interviews are a widely used method as data collection technique. Results. The findings of the study continue on 4 main themes at level 3, by identifying the similarities and differences in the codes determined by the three experts (researchers consulted and the researcher conducting/reporting the study). The findings of the study were evaluated in comparison with the findings of the studies reached in the literature and conducted in regions such as Israel, Iran, Sri-Lanka, Kenya, and Africa. Conclusion. There are many problems in student-patient communication in many different dimensions. The difficulties arising due to these problems generally focus on unknown clinical environment, lack of knowledge, differences between theory and practice, and mentor interaction.

Canthaxanthin (β,β-carotene 4,4' dione) used widely as a drug or as a food and cosmetic colorant may have some undesirable effects on human health, caused mainly by the formation of crystals in the macula lutea membranes of the retina of an eye. Experiments show the exceptional molecular organization of canthaxanthin and a strong effect of this pigment on the physical properties of lipid membranes. The most striking difference between canthaxanthin and other macular pigments is that the effects of canthaxanthin at a molecular level are observed at much lower concentration of this pigment with respect to lipid (as low as 0.05 mol%). An analysis of the molecular interactions of canthaxanthin showed molecular mechanisms such as: strong van der Waals interactions between the canthaxanthin molecule and the acyl chains of lipids, restrictions to the segmental molecular motion of lipid molecules, modifications of the surface of the lipid membranes, effect on the membrane thermotropic properties and finally interactions based on the formation of the hydrogen bonds. Such interactions can lead to a destabilization of the membrane and loss of membrane compactness. In the case of the retinal vasculature, it can lead to an increase in the permeability of the retinal capillary walls and the development of retinopathy.

Troxerutin is a well and for a long time known substance, it is a semi-synthetic bioflavonoid derived from rutin. Original substance is supplied from flowers of Styphnolobium japonicum tree. The author discusses actual medical effects of troxerutin, including antioxidant, anti-inflammatory, antithrombosis and protecting endothelium of small vessels. Being water-soluble, it can be used as ophthalmic drops in treatment of subconjunctival haemorrhage, vitreous haemorrhage and early stages of diabetic retinopathy.

Diabetic retinopathy as a complication of diabetes is one of the most common causes of vision loss. It is a dangerous disease, which affects more and more people. It is important to know its different causes, background and to generate new, effective therapeutic threatments. One of the possible causes of diabetic retinopathy is the accumulation of advanced glycation products and over-expression of AGEs receptors (RAGE) leading to tissue damage, oxidative stress formation, and the promotion of other changes and disorders. There are many genetic variants that encode the AGEs receptor (RAGE). Some polymorphisms (Gly82Ser, 1704G/T, 2184A/G, -429T/C, -374T/A) may be associated with diabetic retinopathy. This paper aims to systematize knowledge in this field and to show the relationship between these compounds and diabetic retinopathy.

Objectives: The aim of this study was to analyze the level of physical activity in people with diabetes, depending on their characteristics. Methods: The sample consisted of 81 participants with type 1 and type 2 diabetes and belonging to a diabetes care association. First of all, the participants completed the short version of the International Physical Activity Questionnaire and the Mediterranean Diet Adherence Questionnaire online. Subsequently, they indicated their sex, age, type of diabetes, the presence of other complications and the years since diagnosis. In addition, data on consumption of tobacco and alcohol, glycosylated hemoglobin and fasting glucose were collected. For the analysis of the data, t-test for independent samples was used to compare means between categories, always with a level of significance of p <.05. Furthermore, effect size was calculated through Cohen’s d. Finally, the frequency and the percentage of the variables were calculated. Findings: The results showed in all the categories an average level of physical activity higher than the recommendations of physical activity for people with diabetes, being the level of physical activity predominantly moderate. Although women also complied with the recommendations for physical activity, their level of physical activity was significantly lower than the level of men Conclusions: It is recommended to expand the research on the barriers and motivations found by people with diabetes to practice physical activity, playing special attention to women with diabetes.

Objectives: The objective of the present study was to perform an analysis of the barriers to the practice of physical activity and diet in people with diabetes mellitus residing in Spain. Methods: Ninety participants (40 men and 50 women) with type 1 diabetes mellitus (71) and type 2 diabetes mellitus (19) residing in Spain, and aged between 12 and 80 years (average age 41.12 ± 17.04) were part of the present study. The participants were asked about the sociodemographic variables of sex and age, as well as the type of diabetes, the presence of other associated pathologies and the years elapsed since their diagnosis. The participants were also asked about the consumption of tobacco and alcohol. Afterwards, the barriers or difficulties perceived in relation to physical exercise were assessed through the Barriers Self-Report for the Practice of Physical Activity (BSPPA) and the diet with the Questionnaire of Adherence to the Mediterranean Diet (MEDAS-14). Then, the participants provided information regarding their last blood test with respect to the levels of glycosylated hemoglobin (HbA1c) and fasting glucose. A descriptive analysis was carried out to present the characteristics of the sample as well as the barriers for the practice of physical activity presented by the sample studied. Finally, a comparison analysis of means was carried out using the Student's t test for independent samples, showing the significant differences between the barriers for the practice of physical activity according to the characteristics of the sample. Findings: The data showed how the Obligations / Lack of time factor represents the main barrier to the practice of physical activity in this population, especially in women, in those under 65 and in people with type 1 diabetes. Conclusions: In general, it is observed that all those categories that may indicate a less healthy lifestyle have a greater degree of agreement with the perceived barriers.

Despite improvements in neonatal care, retinopathy of prematurity is still leading cause of blindness in the world. The most treatment problems pose stage 4 and 5 of this disease. Performing vitrectomy in stage 5 is a subject of controversy among ophthalmologists due to unsatisfactory functional results after the surgery. Therefore, our aim was to present the results of 1000 vitrectomies performed by the author in years 1994–2019 in 1018 eyes of 692 children with stage 5 retinopathy of prematurity. In these children lens sparing vitrectomies and lensectomy-vitrectomy with limbal approach were performed. The results were compared with visual function of 127 untreated children with stage 5 retinopathy of prematurity. Visual acuity of treated patients ranged from 0.05 to no light perception in stage 5 and from no light perception to finger counting in the untreated group. Comparing the functional results of treated and untreated patients, it can be concluded that vitrectomy gives a better chance of gaining useful vision.

Objectiv. To evaluate the efficacy of subthreshold micropulse yellow (577 nm) laser photocoagulation in diffuse macular edema (DME) in rural and urban patients of south-eastern Poland. Materials and method. Seventy-five eyes of 75 patients with diffuse DME were treated with subthreshold micropulse yellow laser photocoagulation with a 5% duty cycle at an energy level. The laser exposure time was 20 ms and the spot diameter was 100 μm. Best corrected visual acuity (BCVA), reading vision (Snellen) and optical coherence tomography-determined central retinal thickness (CRT) were estimated before and 2, 4 and 6 months after laser treatment. There were no statistically important differences in: the advancement of DME, HbA1c (glycated hemoglobin) level, duration of diabetes mellitus (DM), the degree of vision damage between rural and urban patients. Results. The follow-up was 6 months later. The baseline BCVA was 0.20 and remained stable- 0.3 after 6 months. The Snellen at baseline was 1.0 and improved to 0.5 finally (p=0.0004). The CRT at baseline was 500 μm and changed to 346 μm (p=0.00000) at the final follow-up. Finally, no retinal damage was observed. Conclusions. Place of residence had no statistically significant effect on the demographics data, baseline visual acuity, reading visual acuity and central retinal thickness. Subthreshold micropulse yellow laser showed a highly significant efficiency in the treatment of DME. The effects of the treatment were more significant in rural patients than in urban ones.

Aldose reductase gene polymorphisms has been indicated to be associated with diabetic retinopathy (DR). The research data were from PubMed and EMBASE. We identified -106C > T single nucleotide polymorphism (SNP). Pool odds ratio (OR) with 95% CI were calculated. Nine studies were included. ALR2 106C > T gene polymorphisms was associated with the increased risk of DR in T1DM (C vs. T, OR = 2.07, p = 0.001; CC vs. CT + TT, OR = 2.56, p = 0.005). T allele and TT genotype were associated with decreased risk of DR in T1DM (OR = 0.48, p = 0.0001 and OR = 0.12, p = 0.0005). In conclusion, C allele and CC genotype may be a risk factor, while T allele and TT genotype may serve as protective factor for DR in T1DM patient.

Diabetes has been declared an epidemic of the XXIst century by the World Health Organisation. Diabetic retinopathy, one of its main complications, is a leading cause of vision impairment among professionally active adults. Diabetic retinopathy is a microangiopathy that affects mainly small vessels, due to their highest vulnerability to hyperglycemia. Preventive measures involve mainly strict diabetic follow-ups, balanced level of glycated hemoglobin, blood pressure values and analysis of lipids. Regular ophthalmological check-ups and imaging tests of the retina are also extremely significant. For many years scientists have searched for a therapy to suppress vascular changes in diabetes. In May 2021 a stance on sulodexide use as a supporting treatment in mild and intermediate diabetic retinopathy was published by the Polish Society of Ophthalmology. Sulodexide protects vascular endothelium, contributes to endothelial cells’ function and has profibrinolytic, anti-inflammatory and vasoregulatory properties. These features make it a promising protective agent in the early stages of diabetic retinopathy.
Cukrzyca została uznana przez Światową Organizację Zdrowia za epidemię XXI w. Retinopatia cukrzycowa należy do głównych komplikacji cukrzycy i wiodących przyczyn utraty wzroku wśród aktywnych zawodowo dorosłych. Jest to mikroangiopatia, w której dochodzi do uszkodzenia głównie drobnych naczyń, ponieważ są one najbardziej wrażliwe na hiperglikemię. W ramach działań prewencyjnych kluczowe są adekwatne monitorowanie metaboliczne cukrzycy, w tym wyrównane wartości hemoglobiny glikowanej, ciśnienia tętniczego oraz lipidogramu. Niezwykle ważne są również regularne kontrole okulistyczne i okresowe badania obrazowe siatkówki. Od lat naukowcy poszukują terapii umożliwiającej zahamowanie zmian naczyniowych w cukrzycy. W maju 2021 r. Polskie Towarzystwo Okulistyczne opublikowało stanowisko grupy eksperckiej w zakresie stosowania sulodeksydu jako leczenia wspomagającego w łagodnej oraz średnio zaawansowanej retinopatii cukrzycowej. Sulodeksyd chroni śródbłonek naczyń, wpływa na czynność jego komórek, ma właściwości profibrynolityczne, przeciwzapalne i wazoregulujące. Cechy te czynią go obiecującym czynnikiem protekcyjnym w pierwszych fazach retinopatii cukrzycowej.
