Opis:
The paper examines the features of segmentation of the upper respiratory tract to determine nasal air conduction. 2D and 3D illustrations of the segmentation process and the obtained results are given. When forming an analytical model of the aerodynamics of the nasal cavity, the main indicator that characterizes the configuration of the nasal canal is the equivalent diameter, which is determined at each intersection of the nasal cavity. It is calculated based on the area and perimeter of the corresponding section of the nasal canal. When segmenting the nasal cavity, it is first necessary to eliminate air structures that do not affect the aerodynamics of the upper respiratory tract - these are, first of all, intact spaces of the paranasal sinuses, in which diffuse air exchange prevails. In the automatic mode, this is possible by performing the elimination of unconnected isolated areas and finding the difference coefficients of the areas connected by confluences with the nasal canal in the next step. High coefficients of difference of sections between intersections will indicate the presence of separated areas and contribute to their elimination. The complex configuration and high individual variability of the structures of the nasal cavity does not allow segmentation to be fully automated, but this approach contributes to the absence of interactive correction in 80% of tomographic datasets. The proposed method, which takes into account the intensity of the image elements close to the contour ones, allows to reduce the averaging error from tomographic reconstruction up to 2 times due to artificial sub-resolution. The perspective of the work is the development of methods for fully automatic segmentation of the structures of the nasal cavity, taking into account the individual anatomical variability of the upper respiratory tract.
W pracy przeanalizowano cechy segmentacji górnych dróg oddechowych w celu określenia powietrznego przewodnictwa nosowego. Przedstawiono zdjęcia 2D i 3D procesu segmentacji oraz uzyskanych wyników. Podczas formowania analitycznego modelu aerodynamiki jamy nosowej głównym wskaźnikiem charakteryzującym konfigurację kanału nosowego jest ekwiwalentna średnica, którą wyznacza się na każdym skrzyżowaniu jam nosowych. Jest ona obliczana na podstawie pola powierzchni i obwodu odpowiedniego odcinka kanału nosowego. Podczas segmentacji jamy nosowej w pierwszej kolejności należy wyeliminować struktury powietrzne, które nie wpływają na aerodynamikę górnych dróg oddechowych – są to przede wszystkim nienaruszone przestrzenie zatok przynosowych, w których dominuje rozproszona wymiana powietrza. W trybie automatycznym jest to możliwe dzięki eliminacji niepołączonych izolowanych obszarów i znalezieniu, w kolejnym kroku, współczynników różnicy obszarów połączonych konfluencjami z przewodem nosowym. Wysokie współczynniki różnic przekrojów pomiędzy skrzyżowaniami będą wskazywały na obecność wydzielonych obszarów i przyczynią się do ich eliminacji. Złożona konfiguracja i duża zmienność osobnicza struktur jamy nosowej nie pozwala na pełną automatyzację segmentacji, jednak takie podejście przyczynia się do braku konieczności interaktywnej korekcji w 80% zestawów danych tomograficznych. Zaproponowana metoda, uwzględniająca intensywność elementów obrazu znajdujących się blisko konturu, pozwala na nawet 2-krotne zmniejszenie błędu uśredniania z rekonstrukcji tomograficznej, wynikającego ze sztucznej subrozdzielczości. Perspektywą pracy jest opracowanie metod w pełni automatycznej segmentacji struktur jamy nosowej z uwzględnieniem indywidualnej zmienności anatomicznej górnych dróg oddechowych.

This article presents selected physical diagnostic methods used in otorhinolaryngology and results of their application. In addition to the applications of methods using the capabilities of selective sensors, selected methods of hybrid diagnostics were also presented - for assessment of parameters of respiratory processes, with polysomnography as an example of using both typical diagnostic methods dedicated to otolaryngology, as well as standard EEG and ECG methods. It has been shown that in some special cases of respiratory disorders, measurements of the air flow in the respiratory tract can be supplemented with pressure measurements in selected positions within the airways. The presented optical methods and diagnostic systems are very often used in the diagnosis of diseases not specific for otolaryngology occurring in the area of the head and neck. The presented material is the second part of the study discussing both standard and widely used diagnostic methods. All presented methods are dedicated to otolaryngology. This text is a continuation of the material published in No 4 of 2021 [1].

Background To improve the effectiveness of lung cancer screening using low-dose computed tomography (LDCT), the presence of smoking-related comorbidities that may significantly affect mortality in this group should be taken into account. Material and methods A questionnaire survey and spirometry tests were conducted in a group of 730 respondents as part of a lung cancer screening study between 2016 and 2018. People diagnosed with COPD underwent a three-year follow-up to assess the incidence of medical events. Results Our study confirmed that cardiovascular diseases (CVDs) were the most common comorbidities in patients who were diagnosed with COPD and participated in LDCT lung cancer screening. Among the CVDs, the most common were arterial hypertension (45.8%) and coronary artery disease (12.5%). Tobacco-related diseases (e.g. CVD, lung cancer, and exacerbations of COPD) were the leading causes of emergency department visits and hospitalizations. The number of visits due to COPD in specialized clinics more than doubled in the observed period. Conclusions Properly planned screening tests allow not only for the detection of the disease for which they were designed but also for the assessment of comorbidities. In patients undergoing lung cancer screening, it is justified to extend the diagnostics to include spirometry.

The rapidly developing measurement techniques and emerging new physical methods are frequently used in otolaryngological diagnostics. A wide range of applied diagnostic methods constituted the basis for the review study aimed at presenting selected modern diagnostic methods and achieved diagnostic results to a wider group of users. In this part, the methods based on measuring the respiratory parameters of patients were analysed. Respiration is the most important and necessary action to support life and its effective duration. It is an actual gas exchange in the respiratory system consisting of removing CO2 and supplying O2. Gas exchange occurs in the alveoli, and an efficient respiratory tract allows for effective ventilation. The disruption in the work of the respiratory system leads to measurable disturbances in blood saturation and, consequently, hypoxia. Frequent, even short-term, recurrent hypoxia in any part of the body leads to multiple complications. This process is largely related to its duration and the processes that accompany it. The causes of hypoxia resulting from impaired patency of the respiratory tract and/or the absence of neuronal respiratory drive can be divided into the following groups depending on the cause: peripheral, central and/or of mixed origin. Causes of the peripheral form of these disorders are largely due to the impaired patency of the upper and/or lower respiratory tract. Therefore, early diagnosis and location of these disorders can be considered reversible and not a cause of complications. Slow, gradually increasing obstruction of the upper respiratory tract (URT) is not noticeable and becomes a slow killer. Hypoxic individuals in a large percentage of cases have a shorter life expectancy and, above all, deal with the consequences of hypoxia much sooner.

Coronavirus disease (COVID-19) is caused by the SARS-CoV-2 virus and often presents with flu-like symptoms that can have varying degrees, which may subsequently lead to acute respiratory distress (ARDS). The genetic material of the virus in samples of respiratory secretions is identified by way of basic diagnostic tests. Due to the altered course of the respiratory tract, patients after total laryngectomy require special attention in the diagnosis of SARS-CoV-2 infection. We present a case of a patient after laryngectomy who obtained different results of COVID-19 tests depending on the site of sampling.
Choroba COVID-19 wywoływana jest przez wirusa SARS-CoV-2 i często manifestuje się objawami grypopodobnymi, które mogą przebiegać z różnym nasileniem. W dalszej kolejności może prowadzić do ostrej niewydolności oddechowej (ARDS). Podstawowe testy diagnostyczne identyfikują materiał genetyczny wirusa w próbkach wydzielin z dróg oddechowych. Chorzy po laryngektomii całkowitej z uwagi na zmieniony przebieg dróg oddechowych wymagają szczególnej uwagi pod względem diagnostyki w kierunku zakażenia SARS-CoV-2. W naszej pracy przedstawiamy przypadek pacjenta po laryngektomii, u którego uzyskano różne wyniki testów w kierunku COVID-19 w zależności od miejsca pobrania próbek. INTRODUCTION: Lower respiratory tract (LRT) sampling is an aerosol generating procedure. In COVID 19 pandemic, guidelines have advocated caution against all aerosol generating procedures. However, microbial cultures on tracheobronchial aspirates are important to guide antibiotic usage in ventilator-associated pneumonia (VAP). MATERIAL AND METHODS: In our tertiary care COVID-19 intensive care unit (ICU), a protocol was set for using closed suction system for timely LRT sampling in VAP and to reduce the risk of exposure to respiratory secretions. Timing of sample collection was as per intensivist discretion following CDC VAP definition. This prospective study was conducted between June to November 2020, to assess the utility of this technique in diagnosis of suspected VAP. Microbiological and clinico-radiological parameters were documented. Heavy growth (>105 cfu/mL) on semiquantitative culture was taken as significant. RESULTS: Total 69 samples generated from 63 patients were analyzed. Mean age 54.48 years and 77.78% of patients had one or more comorbidities. Average duration of invasive ventilation prior to the first culture was 7.14 ± 4.36 days. Progressive radiological worsening at the time of sample collection was in 92.75% (64 of 69 episodes). Microbiological diagnosis of VAP was confirmed in 76.81%. Culture reports guided antibiotic change. Insignificant culture growth in 13.06%. The positivity rate for early and late (>4days) samples were 69.56% and 80.43% respectively. 95% of culture isolates were Gram negative microorganisms. Most common being Acinetobacter baumannii (41.67%) and Klebsiella pneumoniae (31.66%) in both early and late VAP. Around 85% were multidrug resistant organisms. There were no significant adverse events related to sampling technique. CONCLUSIONS: Lower respiratory tract sampling using closed suction system is easy to execute and minimizes procedure related risk to both patient and health care workers in COVID-19 ICU. Gram negative MDR pathogens are prevalent in both early and late VAP. Need further comparative study to understand effectiveness of this technique against other conventional techniques in VAP diagnosis.

: The effectiveness of inhaled drugs is strictly related to areas reachable by drug particles. Unless particles reach the desired part of the bronchial tree, their influence might not meet the expectations. Consequently, the disease progress might not be stopped or even slowed down. Therefore, the primary objective of this research was to analyze the airflow patterns and particle deposition of a standard inhaled drug using computational fluid dynamics. Methods: The study was devoted to the analysis of the particle diameter influence on their deposition areas within the entire respiratory tract. Two patient-specific respiratory tract models, for 6 and 12-year-old patients, were reconstructed based on the computed tomography examinations. Numerical analyses were carried out as stationary ones with the constant inflow of the particles of various diameters (within the range of 1–50 μm). It was proven that depending on the particle size, their deposition within the respiratory tract varies significantly. Results: The vast majority of the particles with diameters over 20 μm is gathered on the walls of the throat, whereas particles of diameters 5–15 μm are accumulated mainly on the trachea walls, leaving the alveoli insufficiently supplied with the drug particles. Conclusions: The inhaled drug size cannot be treated as negligible factor during the drug spraying. An improper distribution of the particles might not inhibit the symptoms of the asthma. Numerical simulations may improve drugs selection and visualize their distribution along the airways, which might accelerate asthma treatment personalization.

The mass size distribution of five fractions of particulate matter inside most popular types of closed sports facilities before and after athlete training were compared. In all the facilities, the concentrations of particles were higher during the training than before it. Their values depended on the type of flooring and cubic capacity of the rooms. Particle mass size distribution importantly affects the particle deposition rate in the respiratory tract. The results of the work indicate the necessity of further investigating the relations between the physical properties of particles and the effects of inhaling them during training in various sports facilities.

Aim: This article presents an original analysis of inhalation poisoning data using the example of the Łuków County in 2015-2017. The data was obtained from cases to which Emergency Medical Services were dispatched. Most of such exposures were accidental or caused by negligence. and could be avoided by applying appropriate prevention and safety rules. Introduction: Inhalation poisonings represent a small percentage of all poisonings. They are mainly associated with the heating season and carbon monoxide. The number of inhalation poisonings is much smaller than that of food poisonings, alcohol poisonings or drug overdoses but they also constitute a significant health risk for the public. Methodology: The study was based on an analysis of medical documentation of the emergency medical services station in Łuków. The research material was composed of the information contained in the dispatch documentation of EMS teams, i.e. ambulance call records and medical rescue records for events related to inhalation poisoning. The selection of events from all those that occurred during the audited period was made on the basis of: – information provided by the reporting person to the emergency number 999 or 112, – ICD-10 code in the medical emergency record, i.e. diagnosis of the cause of the illness or event. The analysed factors included age, sex, place of intoxication, seasonality and circadian variation of poisoning. The analysis also considered environmental conditions and the influence of stimulants. Results: In the analysed period there were 80 events related to inhalation poisoning. There were 89 people exposed in 80 events (65% – men, 35% – women). Over 90% were cases of accidental poisoning. 60% of the events occurred at 7 am – 7 pm and 40% of the events occurred at 7 pm – 7 am. Of all exposures, 90% were single poisonings and the remaining 10% were multiple. Most poisonings (78%) occurred at the place of residence. Among all the victims, 56% were hospitalized in the county hospital in Łuków, 38% of patients remained at home (including due to death –20% or lack of symptoms requiring hospital treatment – 18%), and almost 6% required immediate specialist treatment. Most incidents related to inhalation poisoning in the Łuków County occurred in rural areas – 56 out of 80. Most inhalation poisonings were caused by CO.
Cel: W poniższym artykule przedstawiono autorską analizę danych dotyczących zatruć wziewnych powiatu Łukowskiego w latach 2015–2017, do których dysponowane były zespoły ratownictwa medycznego. Wprowadzenie: Zatrucia wziewne stanowią niewielki odsetek wszystkich zatruć. Głównie kojarzy się je z okresem grzewczym i tlenkiem węgla. Ekspozycje te najczęściej są przypadkowe, dochodzi do nich również z zaniedbania. W większości można ich uniknąć, stosując odpowiednią profilaktykę i zasady bezpieczeństwa. Metodologia: Badanie miało charakter analizy dokumentacji medycznej Stacji Ratownictwa Medycznego w Łukowie. Materiał pracy stanowiły informacje zawarte w dokumentacji wyjazdowej zespołów ratownictwa medycznego tj. kart zlecenia wyjazdu oraz kart medycznych czynności ratunkowych w zdarzeniach związanych z zatruciem wziewnym. Selekcji zdarzeń spośród wszystkich, które wystąpiły w badanym okresie czasu dokonano na podstawie: – informacji podanej przez osobę zgłaszającą na numer ratunkowy 999 lub 112, – kodu ICD10 w karcie medycznych czynności ratunkowych tj. rozpoznania przyczyny choroby lub zdarzenia. Analizie poddano czynniki takie jak: wiek, płeć, miejsce ekspozycji, sezonowość i cykl dobowy zatruć. W analizie uwzględniono też warunki środowiskowe oraz udział używek. Wyniki: W 80 zdarzeniach ekspozycji poddanych zostało 89 osób: w tym 65% to mężczyźni, 35% – kobiety. Ponad 90% stanowiły zatrucia przypadkowe. 60% zdarzeń przypada na okres pomiędzy godzinami 7 a 19, a 40% – między 19 a 7. Wśród wszystkich ekspozycji 90% to zatrucia pojedyncze, pozostałe 10% – mnogie. Zdecydowana większość zatruć (78%) wydarzyła się w miejscu zamieszkania. Wśród wszystkich poszkodowanych 56% było hospitalizowanych w łukowskim szpitalu powiatowym, 38% osób pozostało w domu (w tym zgon 20% i brak objawów wymagających leczenia szpitalnego 18% ), a prawie 6% wymagała szybkiego leczenia specjalistycznego. Przeważająca liczba zdarzeń związanych z zatruciami wziewnymi występuje na terenach wiejskich: 56 z 80. Większość zatruć wziewnych powodowana jest przez CO.

Introduction: Sarcoidosis is a multi-organ disease characterized by noncaseating granulomas. The lesions are usually localized within the lungs and hilar lymph nodes. The sarcoidosis of the upper respiratory tract (SURT) rate is estimated at 0.5–6%. The involvement of the larynx is extremely rare – it occurs in 1.2% of cases of SURT. Materials and methods. We present a case of a patient with laryngeal sarcoidosis. Results: A 48-year-old man had had a diagnosed of dysphonia for 5 years. A diagnosis of chronic hypertrophic laryngitis was made. Empirical course of methylprednisolone was attempted with a short-term improvement. Due to coexisting skin lesions, he was diagnosed with allergy. Chest CT showed moderately enlarged mediastinal and hilar lymph nodes as well as bilateral nodules around the hila. The bronchial biopsy showed no granulomas. Samples taken during microlaryngoscopy revealed sarcoid infiltrates. Conclusions and discussion: There is no unambiguous test confirming the diagnosis of sarcoidosis. It is a typical example of a diagnosis by exclusion. Laryngeal involvement is extremely rare and occurs in 1.2% of cases of SURT. The clinical course of the disease is often difficult to predict. In many patients, the lesion may undergo spontaneous regression. In patients poorly responding to steroid therapy or developing side effects, cytotoxic drugs may be considered.

Opis:
The paper presents the possibilities of quantitative analysis of results obtained from CT examination of organs and anatomical structures of the upper respiratory tract. The presented results of the analysis were obtained using proprietary software developed in the MATLAB 2018b environment (Image Processing toolbox). The software enables to visualize the original results of CT scan and, after evaluating the visible structures, enables to select the area to be subjected to quantitative analysis. After the initial identification of an area of interest requiring detailed diagnostics, its volume and the surface areas of individual cross-sections are calculated in the area separated for examinations. A graphical presentation of the analysis results - the surface areas of selected cross-sections possible to visualize in two- and three-dimensional space - enables quick analysis of changes in the examined region.

Bacterial lysates stimulate the general immunity of the body in a non-specific way. They act on non-specific defense mechanisms, leading to an increase in type A antibody in mucous membranes, phagocytic activity and INF-ƴ production. They can also stimulate the production of specific antibodies against the bacterial antigens that make up the preparation. The oral immunomodulatory preparations with the best documented clinical efficacy available on the Polish market are Ismigen, Broncho-Vaxom, Ribomunyl and Luivac. They are all lysates of bacterial strains that most often cause respiratory tract infections. In many clinical trials, oral bacterial lysates have been shown to minimize the risk of recurrent respiratory infections in children and adults and reduce the need for antibiotics.

Opis:
Acute infections of the upper respiratory tract and upper gastrointestinal tract (mouth and throat) are still the most common reason for reporting to the general practitioner or ENT specialist. Despite the fact that the most common causative agent of these diseases is a virus, antibiotics are still flavored in about 60-80% of patients who report to the doctor. In consensuses such as EPOS, WHO reports and other local/ / national reports, there is a need to limit antibacterial treatment for and to replace it with symptomatic and anti-inflammatory drugs. In addition to intranasal steroids and mucocutaneous decongestants, supplementary treatment should be considered which resolves persistent symptoms. Plant preparations are used here. The growing role of rational phytotherapy is emphasized, especially due to the most frequent the etiological factor of these infections, i.e. viruses. Modern phytotherapy is a part of pharmacology, and all preparations based on plants meet the standards for of medicines and are subject to standard registration procedures. The article discusses the use and mechanism of action of selected herbs, among others: sorrel, elderberry, yarrow, and horsetail in the treatment of upper respiratory tract infections and upper gastrointestinal tract. The phytotherapy shows good efficacy and tolerance and can be used both in children and adults in recurrent infections of the upper respiratory tract. It is worth paying attention to the combined preparations, the components of which have the effects confirmed by randomized trials and long-term registration for use in treatment, both during the first signs of cold and its further duration, and supportive as support in chronic inflammation especially of the paranasal sinuses.

Self-medication is a procedure that is increasingly eagerly chosen by people all over the world, especially in common conditions (e.g. infections of upper respiratory tract). The main objective of this study was to determine the prevalence of self-medication and identify the sources of knowledge used by Polish society in searching for information on OTC medicines. The study was conducted in the group of 252 Internet users at working age (18-64 years) living in Poland (women n=208; 82.5%, men m=44; 17.5%) and used an original questionnaire consisting of 29 questions available on Internet from December 2016 to February 2017. According to the obtained results self-treatment was undertaken by 90.08% of Poles. Using OTC products was most prevalent among people with university education (p=0.01) and without children (p=0.03). Most popular source of knowledge on self-medication was own experience (n=149; 59.1%) and pharmacists’ advice (n=148; 58.7%). TV commercials were considered as unimportant for most of respondents (54.3%). Young women (18-26 years) most often used Internet to search for information about OTC drugs (p=0.03), young male trusted own experience (p=0.03) and men from older age groups (p=0.04) advice of a pharmacist. Self-medication can characterized as prevalent in Poland. People with lower levels of education used OTC medicines less often and therefore might benefit from stronger support of professionals.
