Opis:
Iceland moss Cetraria islandica (L.) Ach. is a lichen that belongs to the Parmeliaceae family. It occurs throughout Poland, mainly in dry pine forests, grasslands and heaths. A raw material which is used in health care is a thallus of Iceland moss, Lichen islandicus. The properties of the species have been well-known since the 18th century when it started to be used as a food, mostly jelly after soaking in water, or an admixture to flour for baking bread, as well as a cure for tuberculosis. Nowadays, this lichen is also used as an additive to pet food in Scandinavia. Thanks to the presence of biologically active substances such as lichen acids (cetraric acid, protocetraric acid, fumarprotocetraric acid, protolichesterinic acid, small amounts of usnic acid), polysaccharides and mucous compounds, it has antibacterial, anti-inflammatory, protective, reinforcing and antiemetic properties. Moreover animal research which has been conducted recently has confirmed the anticancer properties of the lichen after parenteral administration. Extracts from this lichen are used in treating infl ammation of the upper respiratory tract, laryngitis, as an antitussive agent, against inflammation of the esophagus and stomach, hypo-acidity, peptic ulcers, as well as externally in the form of rinses to improve the condition of damaged hair and wraps to control the excessive secretion of sebum.
Płucnica islandzka (tarczownica islandzka, porost islandzki) Cetraria islandica (L.) Ach. jest porostem z rodziny tarczownicowatych Parmeliaceae, spotykanym na terenie całego kraju, głównie w suchych borach sosnowych, murawach i na wrzosowiskach. W lecznictwie stosowana jest plecha płucnicy Lichen islandicus. Surowiec ten znany był już ludziom w XVIII w. Stosowano go jako środek spożywczy, głównie galaretkę po odgoryczeniu przez moczenie w wodzie, jako domieszkę do mąki przy wypieku chleba oraz jako lekarstwo na gruźlicę. Do dzisiejszych czasów w Skandynawii używa się go jako dodatek do karmy dla zwierząt domowych. Dzięki zawartości biologicznie czynnych związków chemicznych, głównie kwasów porostowych, takich jak kwasy cetrarowy, protocetrarowy, fumaroprotocetrarowy, protolichesterynowy, a w niewielkich ilościach także usninowy, oraz polisacharydów i związków śluzowych, wykazuje działanie przeciwbakteryjne, przeciwzapalne, osłaniające, wzmacniające, przeciwwymiotne. Ostatnio prowadzone badania na zwierzętach wykazały również, po podaniu pozajelitowym, właściwości przeciwnowotworowe. Wyciągi z płucnicy stosuje się w stanach zapalnych górnych dróg oddechowych, w nieżycie gardła, jako środek przeciwkaszlowy, w stanach zapalnych przełyku i żołądka, w niedokwaśności, chorobie wrzodowej, a także – zewnętrznie – w postaci płukanek do polepszenia kondycji zniszczonych włosów oraz okładów w celu regulacji nadmiernego wydzielania łoju.

INTRODUCTION: Lower respiratory tract (LRT) sampling is an aerosol generating procedure. In COVID 19 pandemic, guidelines have advocated caution against all aerosol generating procedures. However, microbial cultures on tracheobronchial aspirates are important to guide antibiotic usage in ventilator-associated pneumonia (VAP). MATERIAL AND METHODS: In our tertiary care COVID-19 intensive care unit (ICU), a protocol was set for using closed suction system for timely LRT sampling in VAP and to reduce the risk of exposure to respiratory secretions. Timing of sample collection was as per intensivist discretion following CDC VAP definition. This prospective study was conducted between June to November 2020, to assess the utility of this technique in diagnosis of suspected VAP. Microbiological and clinico-radiological parameters were documented. Heavy growth (>105 cfu/mL) on semiquantitative culture was taken as significant. RESULTS: Total 69 samples generated from 63 patients were analyzed. Mean age 54.48 years and 77.78% of patients had one or more comorbidities. Average duration of invasive ventilation prior to the first culture was 7.14 ± 4.36 days. Progressive radiological worsening at the time of sample collection was in 92.75% (64 of 69 episodes). Microbiological diagnosis of VAP was confirmed in 76.81%. Culture reports guided antibiotic change. Insignificant culture growth in 13.06%. The positivity rate for early and late (>4days) samples were 69.56% and 80.43% respectively. 95% of culture isolates were Gram negative microorganisms. Most common being Acinetobacter baumannii (41.67%) and Klebsiella pneumoniae (31.66%) in both early and late VAP. Around 85% were multidrug resistant organisms. There were no significant adverse events related to sampling technique. CONCLUSIONS: Lower respiratory tract sampling using closed suction system is easy to execute and minimizes procedure related risk to both patient and health care workers in COVID-19 ICU. Gram negative MDR pathogens are prevalent in both early and late VAP. Need further comparative study to understand effectiveness of this technique against other conventional techniques in VAP diagnosis.

Bronchoscopy was introduced into clinical practice over 100 years ago. Due to technological advances, diagnostic capabilities of current bronchoscopy are not limited to the trachea and proximal bronchi but also include the peripheral airways as well as various anatomical structures located outside the bronchi. A wide range of available techniques that include bronchoalveolar lavage, protected microbiological brush, transbronchial biopsy of the lungs and mediastinal lymph nodes makes bronchoscopy useful in diagnosing various lower respiratory tract infections. For example, the collection of high quality biological samples for microbiological examination plays a crucial role when treating patients with nosocomial pneumonia. Bronchoscopy may provide the samples directly from the site of infection. In immunocompromised hosts bronchoscopy is routinely used as the diagnostic tool in lower respiratory tract infections. Due to implementation of novel therapies, the number of immunocompromised patients is steadily increasing, hence there are growing needs for effective diagnosis of opportunistic pulmonary infections. In the specific group of lung transplant recipients, bronchoscopy play a crucial role in monitoring the rejection process and also in differentiating between the rejection and pulmonary infections. Bronchoscopy is also useful in diagnosing tuberculosis or nontuberculous pulmonary infections. This particularly refers to patients who are unable to produce sputum for microbiological examination.

This review article presents immunological issues in the course of the turkey rhinotracheitis (TRT) emphasizing local immunity mechanisms, both humoral and cell-mediated, in the upper respiratory system. Studies on the influence of the humoral immunity in the course of infection and vaccinations against TRT have revealed many times the absence of correlation between the titre of specific IgY anti-aMPV (avian Metapneumovirus) antibodies in the serum and in the upper respiratory washings and the immunity against the occurrence of the clinical form of the TRT. Considering the above, T cells are increasingly often regarded as the main factor involved in the upper respiratory immunity against the TRT. However, there have been just a few reports on the role of the T cells in the local immunity processes in the infection with aMPV in turkeys. Additionally, studies of the T-cell-associated immunity against the TRT have given ambiguous results. Immunoprophylaxis issues against the aMPV infections are a significant part of the work where the authors confront current vaccination programmes against the perspectives of use of the future vaccines against the TRT. Future vaccines should face the following criteria: absence of the risk of immunosuppressive effect and reversion of vaccine strains virulence, ease-of-use combined with the possibility of administration of the vaccine to the large numbers of turkeys. The leading role in future vaccination programs for birds against the TRT is likely to be played by the in ovo technique and the recombinant vaccines. Great hopes are also linked with the development of subunit vaccines against the aMPV.

The aim of this study was to determine the sensitivity of Aspergillus niger strains isolated from birds to available antifungal drugs using different in vitro assays - classical disk diffusion, Etest® and broth microdilution NCCLS/CLSI M 38-A. The study material consisted of about 2.000 swabs and samples from different species of birds. A. niger (n=10) was accounted for 6.81% of the total pool of strains isolated. Determinations were made for 13 antifungal drugs using the disk diffusion method. The A. niger exhibited high susceptibility to enilconazole, terbinafine, voriconazole, tioconazole and ketoconazole, low susceptibility to clotrimazole, miconazole and nystatin, and resistance to amphotericin B, itraconazole, pimaricin, fluconazole and 5-fluorocytosine. Minimum inhibitory concentration (MIC) was determined for 9 antifungal drugs using the micromethod of duplicate serial dilutions in a liquid medium. A. niger strains were most susceptible to enilconazole and voriconazole. MIC ranged from 0.0625 to 0.5 μg/ml for enilconazole, with MIC90-0.5 μg/ml and MIC50-0.125 μg/ml. The corresponding values for voriconazole were 0.25-1 μg/ml, 1 μg/ml and 0.5 μg/ml. MIC for amphotericin B and terbinafine ranged from 0.5 to 4 μg/ml, while the values for the remaining drugs were highly varied. MIC was measured by the gradient diffusion method using Etest® for 5 antifungal drugs: amphotericin B, fluconazole, itraconazole, ketoconazole and voriconazole. By far the highest susceptibility was obtained in the case of voriconazole, with MIC ranging from 0.0625 to 1 μg/ml. MIC for amphotericin B ranged from 0.25 to 4 μg/ml, for itraconazole and ketoconazole ranging from 0.5 to 16 μg/ml. Methods available for this purpose are not always applicable in field conditions. The present results indicate that the Etest® technique, due to its high percentage of agreement with the M 38-A microdilution method, should find application in medical and veterinary practice.

This paper reviews the results of an extensive monitoring study, spanning 20-years of observation from three medical centres in Olsztyn on the dynamics and species diversity of fungi most frequently colonizing the respiratory and digestive systems of humans. The experimental materials were swabs and specimens from the gastrointestinal tract, swabs from the oral cavity and pharynx, as well as sputum and bronchial fluid from the respiratory system. The biological material was subjected to routine mycological diagnostics, taxonomic determination and identification. In total, 41 species of yeast and yeast-like fungi were isolated, including 34 from the respiratory and 25 from the digestive system. In the last decade, a significant increase has been noticed in the counts of fungi, especially the gastrointestinal tract, reported from people. As many as 18 species were isolated from both systems – they were predominated by fungi of the genus Candida and their perfect forms. Worthy of notice are also frequent isolations of yeast (Saccharomyces spp.) and detection of an endemic species, Paracoccidioides brasiliensis, in the respiratory system, and of the sexual stages of Rhodosporidium diobovatum and Rhodosporidium kratochvilovae from the gastrointestinal tract.

A diet, individually customized to the needs of sportsmen and sportswomen prepares them better for competition and achievement of better sports results. However, disorders of the gastrointestinal tract and frequently recurrent upper respiratory tract infections pose a common problem observed among athletes of disciplines such as triathlon, cycling and marathon. Diarrhea, splashing in the intestines or gastrointestinal bleeding make it difficult to start and win in the race. Recently researchers have paid special attention to the therapeutic effect of probiotic strains on the human body. Various probiotic strains may have a beneficial effect on elimination of disorders mentioned above among athletes of these disciplines. Still, researchers continue looking for answers to the question how a specific probiotic strain is able to reduce the risk of the gastrointestinal tract and the respiratory system disorders appearing during training or competition. Attention is also drawn to the possible impact of probiotics on the physical capacity athletes and their athletic performance. Probiotic strains properly applied may have a positive influence on the athletes’ bodies, but still randomized controlled trials are required to prove this thesis.
Sposób żywienia, indywidualnie dobrany do potrzeb zawodników różnych dyscyplin sportowych pozwala na lepsze przygotowanie sportowców do startów oraz osiąganie lepszych wyników sportowych. Jednak częstym problemem obserwowanym wśród zawodników dyscyplin takich jak triatlon, kolarstwo czy maraton, są zaburzenia ze strony przewodu pokarmowego oraz często nawracające infekcje górnych dróg oddechowych. Biegunki, przelewanie w jelitach czy krwawienie z przewodu pokarmowego, uniemożliwiają start oraz zdobywanie zwycięstw. Badacze w ostatnich latach zwracają szczególną uwagę na wpływ probiotykoterapii na organizm człowieka. Poszczególne szczepy probiotyczne mogą mieć korzystny wpływ na niwelowanie wspomnianych zaburzeń pojawiających się w grupach sportowców. Nadal poszukuje się odpowiedzi na pytanie jaki szczep probiotyczny jest w stanie zmniejszyć ryzyko występowania zaburzeń ze strony przewodu pokarmowego oraz układu oddechowego, pojawiających się w trakcie treningów czy zawodów. Zwraca się również uwagę na możliwy wpływ probiotyków na wydolność fizyczną zawodników oraz ich wyniki sportowe. Prawidłowo zastosowana probiotykoterapia może wpływać korzystnie na organizm zawodników, jednak nadal potrzeba randomizowanych badań potwierdzających tę tezę.

Respiratory tract infections are the most common communicable diseases, which include pharyngitis, otitis media, rhinosinusitis, laryngitis, bronchitis and pneumonia. Although the majority of pathogenic agents responsible for these conditions are cosmopolitan, the differences in frequency and severity of these diseases associated with climate and living conditions may appear. The aim of the study is to assess if there are significant differences in frequency and severity of respiratory tract infections in different countries. A survey, consisting of 49 questions involving the frequency of respiratory tract infections, antibiotics’ usage and demographic data was delivered to the users of a social network used to finding penfriends from abroad. Collected data was statistically analyzed and compared with climate conditions and economic indices in different countries. 462 respondents, aged 12-79 (mean age 25,15 ± 9,43, median 23) from 119 countries submitted their answers. First raw, preliminary data describe a prevalence of different types of respiratory tract infections and commonness of antibiotic therapy. This initial report with preliminary results of the survey should be a first step towards determining the differences in frequency and severity of respiratory tract infections, as well as in antibiotic treatment. Data gained during the survey need further analysis.

Self-medication is a procedure that is increasingly eagerly chosen by people all over the world, especially in common conditions (e.g. infections of upper respiratory tract). The main objective of this study was to determine the prevalence of self-medication and identify the sources of knowledge used by Polish society in searching for information on OTC medicines. The study was conducted in the group of 252 Internet users at working age (18-64 years) living in Poland (women n=208; 82.5%, men m=44; 17.5%) and used an original questionnaire consisting of 29 questions available on Internet from December 2016 to February 2017. According to the obtained results self-treatment was undertaken by 90.08% of Poles. Using OTC products was most prevalent among people with university education (p=0.01) and without children (p=0.03). Most popular source of knowledge on self-medication was own experience (n=149; 59.1%) and pharmacists’ advice (n=148; 58.7%). TV commercials were considered as unimportant for most of respondents (54.3%). Young women (18-26 years) most often used Internet to search for information about OTC drugs (p=0.03), young male trusted own experience (p=0.03) and men from older age groups (p=0.04) advice of a pharmacist. Self-medication can characterized as prevalent in Poland. People with lower levels of education used OTC medicines less often and therefore might benefit from stronger support of professionals.

Opis:
Acute infections of the upper respiratory tract and upper gastrointestinal tract (mouth and throat) are still the most common reason for reporting to the general practitioner or ENT specialist. Despite the fact that the most common causative agent of these diseases is a virus, antibiotics are still flavored in about 60-80% of patients who report to the doctor. In consensuses such as EPOS, WHO reports and other local/ / national reports, there is a need to limit antibacterial treatment for and to replace it with symptomatic and anti-inflammatory drugs. In addition to intranasal steroids and mucocutaneous decongestants, supplementary treatment should be considered which resolves persistent symptoms. Plant preparations are used here. The growing role of rational phytotherapy is emphasized, especially due to the most frequent the etiological factor of these infections, i.e. viruses. Modern phytotherapy is a part of pharmacology, and all preparations based on plants meet the standards for of medicines and are subject to standard registration procedures. The article discusses the use and mechanism of action of selected herbs, among others: sorrel, elderberry, yarrow, and horsetail in the treatment of upper respiratory tract infections and upper gastrointestinal tract. The phytotherapy shows good efficacy and tolerance and can be used both in children and adults in recurrent infections of the upper respiratory tract. It is worth paying attention to the combined preparations, the components of which have the effects confirmed by randomized trials and long-term registration for use in treatment, both during the first signs of cold and its further duration, and supportive as support in chronic inflammation especially of the paranasal sinuses.

Opis:
Coltsfoot leaves (Farfarae folium) are used in the European medicine in respiratory tract diseases, for cough, bronchitis and asthmatic disorders, while in the traditional Chinese medicine only flower buds (Farfarae flos) have been recognized as a medicine. A short literature review shows that most data concern the chemical composition of the coltsfoot flowers. During the carried out studies we have isolated and identified (UV, 1H and 13C NMR, analysis of acid and enzymatic hydrolyze products) six known flavonols from the coltsfoot leaves: kaempferol and its 3-O-β-glucopyranoside and 3-O-α-rhamnopyranosyl(1→6)- β-glucopyranoside, along with quercetin derivatives: 3-O-β-arabinopyranoside, 3-O-β- glucopyranoside and 3-O-α-rhamnopyranosyl(1→6)-β-glucopyranoside. Moreover, we have detected the presence of three phenolic acids.
Liście podbiału (Farfarae folium) są stosowane w medycynie europejskiej w chorobach dróg oddechowych, w kaszlu, w stanach zapalnych i skurczowych oskrzeli. W medycynie chińskiej są wykorzystywane jedynie kwiaty podbiału (Farfarae flos), dlatego większość danych literaturowych dotyczy składu chemicznego kwiatów. Z liści podbiału wyizolowaliśmy i zidentyfikowaliśmy (UV, 1H i 13C NMR, hydroliza kwasowa i enzymatyczna) 6 związków flawonoidowych: kemferol i jego glikozydy: 3-O-β-glukopiranozyd i 3-O-α-ramnopiranozylo(1→6)-β-glukopiranozyd, oraz glikozydy kwercetyny: 3-O-β- arabinopiranozyd, 3-O-β-glukopiranozyd i 3-O-α-ramnopiranozylo(1→6)-β-glukopiranozyd. Ponadto w surowcu stwierdzono obecność trzech kwasów fenolowych.

Respiratory infections are a major health and economic problems in every country. Most infections are viral, bacterial in-fection but also are not rare. It should be emphasized that the respiratory tract infections are the cause of 20% of all med-ical consultations and 75% antibiotic treatment. To reduce the need for antibiotics, more and more attention is paid to the preparation increases the body’s resistance to bacterial infection. Non-specific multivalent vaccines containing killed at high temperature, bacterial cells or lysates. These preparations are non-specifically stimulate the overall immunity. They operate on the non-specific defense mechanisms, leading to increased antibody type IgA in secretions of the mu-cous membranes, phagocytic activity and IFN-γ. They may also stimulate the production of specific antibodies against bacterial antigens included in the formulation. Available on the Polish market oral preparations immunomodulatory of the best documented clinical efficacy are Luivac, Ribomunyl, Broncho-Vaxom and Ismigen. All lysates of bacterial strains are most often causing respiratory tract infections. Many clinical studies have shown that oral bacterial lysates reduce the risk of recurrent respiratory tract infections in adults and children and reduce the need for antibiotics.

Despite the high availability and effectiveness of synthetic drugs, the interest in dietary supplements, including vitamin and plant preparations, is growing. Recurring respiratory tract infections, increasing drug resistance of microorganisms cause that we are more and more eager to choose them, especially during seasonal respiratory tract infections. In the winter, agents that improve the function of the immune system, containing vitamin D and C, preparations of echinacea purpurea or aloe arborescens are very popular.
