As part of the European Public Health project IMCA II validity and practicability of “air pollution” as a respiratory health indicator were analyzed. The definitions of air quality as an indicator proposed by the WHO project ECOEHIS and by IMCA I were compared. The public availability of the necessary data was checked through access to web-based data-bases. Practicability and interpretation of the indicator were discussed with project partners and external experts. Air quality serves as a kind of benchmark for the good health-related environmental policy. In this sense, it is a relevant health indicator. Although air quality is not directly in the responsibility of health policy, its vital importance for the population’s health should not be neglected. In principle, data is available to calculate this IMCA indicator for any chosen area in Europe. The indicator is relevant and informative, but calculation and interpretation need input from local expert knowledge. The European health policy is well advised to take air quality into account. To that end, an interdisciplinary approach is warranted. The proposed definition of air quality as a (respiratory) health indicator is workable, but correct interpretation depends on expert and local knowledge.

Objective: A number of studies show an association between traffi c-related air pollution and adverse respiratory health effects in children. However, most evidence relates to the regions with low or moderate levels of ambient air pollution. The study was undertaken to assess the impact of traffi c-related air pollution on respiratory health status in children living in the area of high levels of industrial and municipal ambient air pollution. Materials and Methods: Analyses involved data obtained from cross-sectional study on respiratory health in children (N = 5733), conducted between 2003–2004 in Bytom, one of the largest cities of Silesian Metropolis (Poland). Exposure to traffic-related air pollution was assessed by means of geographic information system and expressed as several measures of potential exposure to traffic-related air pollution, involving residential distance to major road and traffic density in the residential area. Logistic regression was used to examin association between reported respiratory health and traffic measures. Results: Statistically signifi cant association was found between doctor-diagnosed asthma and residential proximity to traffic. Results of multivariate logistic regression (logOR; 95%CI) confi rmed the effect of living in an area of a city with high-traffic-density on childhood asthma: 1.60 (1.07–2.39). Similar effects were found in case of allergic rhinitis and rhinitis symptoms, but the observed associations were not statistically signifi cant. Conclusion: The study fi ndings suggest that even in an area with poor regional ambient air quality, adverse respiratory health outcomes are more frequent in children living in a proximity to the high vehicle traffic flow

Objectives: Studies have correlated second hand smoke (SHS) with many diseases, especially respiratory effects. The goal of this study was to measure the impact of SHS on the respiratory symptoms and exhaled carbon monoxide. Material and Methods: The study population consisted of 50 young workers in restaurants serving waterpipes, 48 university students who sit frequently in the university cafeteria where cigarette smoking is allowed and 49 university students spending time in places where smoking is not allowed. Subjects completed questionnaires on socio-demographic characteristics, respiratory symptoms and exposure to SHS. Exhaled carbon monoxide levels were measured. ANOVA and Chi-square tests were used when applicable as well as linear and logistic regression analysis. Results: Exposure to cigarette smoke in university (adjusted odds ratio (ORa) = 6.06) and occupational exposure to waterpipe smoke (ORa = 7.08) were predictors of chronic cough. Being married (ORa = 6.40), living near a heavy traffic road (ORa = 9.49) or near a local power generator (ORa = 7.54) appeared responsible for chronic sputum production. Moreover, predictors of chronic allergies were: being male (ORa = 7.81), living near a local power generator (ORa = 5.52) and having a family history of chronic respiratory diseases (ORa = 17.01). Carbon monoxide levels were augmented by the number of weekly hours of occupational exposure to waterpipe smoke (β = 1.46) and the number of daily hours of exposure to cigarette smoke (β = 1.14). Conclusions: In summary, young non-smoker subjects demonstrated more chronic cough and elevated carbon monoxide levels when exposed to SHS while the effect of waterpipe was even more evident.

Objectives The development of allergic conditions is largely dependent on the interactions between genetic (individual genetic predisposition) and environmental factors (exposure to risk factors). The aim of this study was an attempt to assess the influence of selected elements of the hygiene theory in the development of allergic diseases such as allergic rhinitis and asthma. Material and Methods The study group consisted of 5518 women and 3868 men. The method that was used was the European Community Respiratory Health Survey II and International Study of Asthma and Allergies in Childhood questionnaire validated and adapted to Central and Eastern European conditions. The project was conducted in 8 urban areas (Gdańsk, Wrocław, Poznań, Katowice, Kraków, Lublin, Białystok, Warsaw) and 1 rural area (Krasnystaw county). This study had 2 stages; the first stage involved grouping the 22 500 respondents based on their questionnaire responses with the use of a Personal Digital Assistant (PDA); the second stage involved 7000 subjects, who underwent additional assessments: skin prick tests (birch, grasses/cereals, Dermatophagoides pteronyssinus and Dermatophagoides farinae, molds [set I: Botrytis cinerea, Cladosporium herbarum, Alternaria tenuis, Curvularia lunata, Fusarium moniliforme, Helminthosporium], molds [set II: Aspergillus fumigatus, Mucor mucedo, Penicillium notatum, Pullularia pullulans, Rhizopus nigricans, Serpula lacrymans], cat, dog, molds Cladosporium herbarum, Alternaria tenuis) and spirometry tests. Results The age at which children attend the nursery school is critical to the development of allergic diseases; in allergic rhinitis, the risk of an IgE-dependent reaction is 2 times higher in the second than in the first year of life (p = 0.00147, p < 0.05), while in asthma, having a large number of siblings increases the risk of developing obstructive disease by almost 6 times (p = 0.00316, p < 0.05).The age at which children attend the nursery school is critical to the development of allergic diseases; in allergic rhinitis, the risk of an IgE-dependent reaction is 2 times higher in the second than in the first year of life (p = 0.00147, p < 0.05), while in asthma, having a large number of siblings increases the risk of developing obstructive disease by almost 6 times (p = 0.00316, p < 0.05). This article has investigated the considerations of healthcare facilities to utilize reusable respirators as an alternative to disposable respirators during the COVID-19 pandemic. The decision to choose specific equipment should be based on the protection factors and also on the overall analysis of given conditions. International scientific databases, such as Web of Science, PubMed and MedLine, were searched on May 5, 2020, with the following key words: COVID-19, respiratory protection, surgical masks, filtering facepiece respirators (FFRs) and disposable respirators. The differences between various respiratory protective equipment, i.e., surgical masks, respirators such as FFRs, elastomeric half-facepiece respirators, elastomeric full-facepiece respirators and powered air-purifying respirators (PAPRs), were compared. Reusable elastomeric respirators (RERs) may provide a better adaptation to the face and may be more stable when used by healthcare providers (HCPs). Protection factors were found to be higher in FFRs compared to surgical masks. While FFRs provide a one-tenth decrease in the inhaled aerosol concentration, PAPRs diminish the inhaled aerosol up to one-twenty-fifth. Even with some full-face PAPRs and helmets, the protection factor assigned by the Occupational Safety and Health Administration can reach a value up to 1000. For HCPs, the evidence shown in this article provides an additional support for the utilization of RERs. Such equipment might be less prone to leakages, can provide a better fit, and indicates a better stability compared to disposable FFRs (N95 and similar). By providing higher protection factors, reusable elastomeric respirators are recommended to be used by HCPs under controlled cleaning and disinfection protocols.

Opis:
Street cleaning is an integral part of the solid waste management system. There are different ways to achieve clean streets depending on the availability of equipment, the type and magnitude of dirt, the surface conditions encountered or traffic conditions. In general, hand sweeping by an individual worker or a group, hose flushing, or machine sweeping or flushing are applied. In order to obtain information about the occurrence and relevance of occupational health hazards of street cleaners, the current international literature, as well as corresponding German regulations, were reviewed and evaluated. Street cleaning includes a variety of health hazards for employees. These can be subdivided into effects of occupational tasks and effects of working conditions such as weather or road traffic. The hazards result from physical, chemical and biological exposures, but may also be due to physiological and psychological burden or inadequate safety aspects. The most commonly reported work-related complaints are musculoskeletal and respiratory disorders, cuts, slips, and road traffic accidents. In developing countries, street cleaners seem to be still heavily exposed to dust and, in most cases, no suitable protective measures are available. Especially in industrialized countries there exist a number of standards and recommendations for waste workers that aim to reduce their occupational health impacts.

Antibiotic susceptibility of bacteria isolated from nasal swabs and lungs of pigs, to 16 commonly used antibiotics, was determined by disc diffusion test, ß-lactams showed the best activity against Streptococcus suis (S. suis) (> 99% of susceptible strains). The lowest sensitivity of S. suis was evidenced to: tylosin, tetracycline and neomycin (50%, 40% and 25%, respectively). Isolates of Escherichia coli (E. coli) demonstrated the highest susceptibility to cephalosporin (85% strains), gentamicin and norfloxacin (over 74%). The lowest susceptibility of E. coli was demonstrated to tiamulin and penicillin (11.3% and 1.9%, respectively). Over 80% of Actinobacillus pleuropneumoniae (App) strains were susceptible to all antibiotics tested. The highest resistance of App, but demonstrated by below 20% of tested isolates only, was evidenced to neomycin and LxS. Isolates of Pasteurella multocida (Pm), Haemophilus parasuis (Hps) and Arcanobacterium pyogenes (A. pyogenes) were highly susceptible to the most antibiotics included in the analysis. The comparison of the in vitro susceptibility of pathogens to the chemotherapeutics used on Polish farms for the therapy of bacterial infection of pigs within the last five years and the last 10 years, showed an increasing percent of E. coli and S. suis strains resistant to commonly used antibiotics. It is also shown that Pm, Hps, App and A. pyogenes isolates were continuously susceptible to the most chemotherapeutics applied.

ObjectivesThe physiological impact of wearing personal protective equipment (PPE), in particular filtering-face-piece 3 (FFP3) masks, has increasingly been gaining importance since the outbreak of coronavirus disease 2019 (COVID-19). So far, gas exchange has been examined using transcutaneously measured partial pressure of carbon dioxide (PaCO2), ergo-spirometry and impedance cardiography.Material and MethodsIn this structured investigation, arterial blood gas analysis in a 30-year-old female resident was carried out during a 13-hour day shift on the COVID-19 Intensive Care Unit of the University Hospital of Innsbruck, Austria. An FFP3 mask (3MTM AuraTM) with an exhalation valve was continuously worn, except for 1 break of 20 min. Arterial blood samples were obtained before putting on the PPE, and after 5 h, 9 h and 13 h of working in the contaminated area.ResultsDuring the multi-hour wearing time, an increase in PaCO2 (the baseline value: 29.3 mm Hg, the max. value: 38.9 mm Hg) and a continuous decrease in partial pressure of oxygen (PaO2, the baseline value: 102 mm Hg, the min. value: 80.8 mm Hg) was detectable.ConclusionsAll measured values were within the normal range, but a trend towards an insufficient gas exchange could be suspected.

Objectives Concentrations of particulate matter that contains particles with diameter ≤ 10 mm ($\text{PM}_\text{10}$) and diameter ≤ 2.5 mm ($\text{PM}_\text{2.5}$) as well as nitrogen dioxide ($\text{NO}_{2}$) have considerable impact on human mortality, especially in the cases when cardiovascular or respiratory causes are attributed. Additionally, they affect morbidity. An estimation of human mortality and morbidity due to the increased concentrations of $\text{PM}_\text{10}$, $\text{PM}_\text{2.5}$ and $\text{NO}_{2}$ between the years 2005–2013 was performed for the city of Kraków, Poland. For this purpose the Air Quality Health Impact Assessment Tool (AirQ) software was successfully applied. Material and Methods The Air Quality Health Impact Assessment Tool was used for the calculation of the total, cardiovascular and respiratory mortality as well as hospital admissions related to cardiovascular and respiratory diseases. Data on concentrations of $\text{PM}_\text{10}$, $\text{PM}_\text{2.5}$ and $\text{NO}_{2}$, which was obtained from the website of the Voivodeship Inspectorate for Environmental Protection (WIOS) in Kraków, was used in this study. Results Total mortality due to exposure to $\text{PM}_\text{10}$ in 2005 was found to be 41 deaths per 100 000 and dropped to 30 deaths per 100 000 in 2013. Cardiovascular mortality was 2 times lower than the total mortality. However, hospital admissions due to respiratory diseases were more than an order of magnitude higher than the respiratory mortality. Conclusions The calculated total mortality due to $\text{PM}_\text{2.5}$ was higher than that due to $\text{PM}_\text{10}$. Air pollution was determined to have a significant effect on human health. The values obtained by the use of the AirQ software for the city of Kraków imply that exposure to polluted air can result in serious health problems.

Infekcje wirusowe dróg oddechowych stanowią poważny problem zdrowotny i ekonomiczny na całym Świecie wysoka zakaźność tej grupy wirusów bezpośrednio wynika z łatwości transmisji, szczególnie w większych skupiskach ludzkich, takich jak placówki medyczne i oświatowe, duże zakłady pracy czy środki transportu. Ryzyko zakażenia dla tych grup zawodowych, a zwłaszcza pracowników ochrony zdrowia fest więc znacząco wyższe. Konieczne jest świadome stosowanie zasad profilaktyki czynnej (szczepienia ochronne), a w okresie wzrostu zapadalności również profilaktyki biernej (izolacja zakażonych, higiena). Niski poziom wyszczepialności przeciw grypie w populacji polskiej, w tym również wśród pracowników opieki zdrowotnej stanowi istotny problem, niosący znaczące ryzyko dla zdrowotności społeczeństwa i gospodarki kraju.
Respiratory viral infections pose a significant health and economic burden all over the world. Due to their easy transmission. respiratory viruses are highly infective, particularly in the larger human groupings, such as medical and educational facilities, larger industrial units and public transportation. Therefore, respective professional groups are regarded as the high risk population In those groups, in particular in medical staff, the premeditated proactive flu prophylaxis by means of regular immunization is strongly advised. A secondary prophylaxis, the strict isolation of infected patients must be introduced in health care facilities during flu season or epidemic. The unexceptionally low flu vaccination rate in Poland, both in general population and medical staff is of particular concern as a potential serious treat for society's state of health but also country economy.

An analysis was made of solid particulate matter suspended in the air in the city of Cordoba. Particles greater than 10 micra were collected using volumetric particle samplers (Lanzoni VPPS 2000, Bologna, Italy), and analysed by means of aerobiological methods enabling identifi cation of the source of biological particulate material (BPM). Particles smaller than 10 micra were collected using automatic high-volume air samplers. Subsequent analysis showed that traffi c, and particularly diesel engine exhaust emissions, were the main source of non-biological particulate matter (nBPM). The dynamics of airborne BPM and nBPM were also studied over one year: although distribution patterns differed – BPM displaying marked seasonality and non-BPM exhibiting dependence on human activity – the curves ran parallel at certain times of the year. Statistical results point to the possible presence of plant residues on smaller particles. The occurrence of simultaneous peaks in airborne pollen-grain and PM10 counts suggests potential coadjuvant acivity, which may lead to high-risk situations for people with respiratory disease.

ABSTRACT Diacetyl (2,3-butanedione) is a natural ingredient in foodstuffs which is not generally regarded health risk to consumers. Nevertheless, when manufactured for use as a synthetic flavouring/additive in processed foods (e.g. microwave popcorn), it poses a human health threat at the workplace. Its pneumotoxic action consists of inflammation, obstruction and restriction in the distal respiratory tract. One of the factors causing bronchiolitis obliterans is also recognised to be diacetyl. The scientific literature mostly describes human exposure to diacetyl in factory settings where functional disorders and structural changes of the respiratory system have been recorded, particularly bronchiolitis obliterans. Moreover, differential diagnosis shows pathological changes in the distal respiratory tract and in the pneumotoxic actions of diacetyl.
Diacetyl (2,3-butandion) jako naturalny składnik żywności nie wydaje się stwarzać zagrożenia dla zdrowia konsumentów. Związek ten będąc syntetycznym dodatkiem do żywności przetworzonej jest czynnikiem szkodliwym dla zdrowia pracowników zatrudnionych przy jego syntezie i stosowaniu w produkcji prażonej kukurydzy do mikrofalówek. Pneumotoksyczne działanie tego związku manifestuje się zmianami zapalnymi, obturacyjnymi i restrykcyjnymi, szczególnie w dystalnych drogach oddechowych. Diacetyl uznano za czynnik etiologiczny zarostowego zapalenia oskrzelików. Na podstawie piśmiennictwa przedstawiono narażenie na diacetyl w warunkach przemysłowych, zaburzenia czynnościowe i zmiany strukturalne w układzie oddechowym u osób narażonych, ze szczególnym uwzględnieniem zarostowego zapalenia oskrzelików. Ponadto zwrócono uwagę na diagnostykę różnicową zmian patologicznych w dystalnych drogach oddechowych oraz na mechanizmy pneumotoksycznego działania diacetylu.
