The work purpose: To develop, scientifically to prove and introduce medical-organizational model of protection of reproductive health of patients with inflammatory diseases of genitals. Materials and methods. The female population of fertile age with inflammatory diseases of the genitals, living in the Grodno area per 1954–2008 is studied. The system approach is used, following methods were applied: sanitary-statistical, economic-mathematical, expert estimations, sociological, historical, organizational experiment, modelling and forecasting. Results. On the basis of the carried out complex socially-hygienic research including studying of volumes and quality of medical aid, security of obstetric-gynecologic service material, financial and personnel resources, introductions of new organizational, medical and information technologies, studying of the reproductive equipments of women of fertile age and prospects of realisation by them of reproductive function, and also an expert estimation of the importance of the problems bound to disturbances in reproductive system, develop medical-organizational model of protection of reproductive health of women with inflammatory diseases of the genitals, based on state policy realisation in public health services. Model actions are realised in three stages: pregravidal improvement; medical maintenance in pregnancy; The organisation of medical aid to women in childbirth and newborns, and also aftertreatment after a failure of pregnancy and sorts.

Introduction and aim. In extraordinary situations, sexual/reproductive health services are very important for the health of the whole society, especially women and children. The aim of this study is to assess the impact of training provided on sexual health/reproductive health during extraordinary situations on the knowledge levels of nurses and midwives. Material and methods. The research was conducted using a single-group pre-test-post-test follow-up test measurement quasi-experimental design. It was completed between November 2021–June 2022 with 140 participants working in a province in Turkey. The data were collected with the “Descriptive Information Form” and the “Sexual Health and Reproductive Health Knowledge Assessment Form for Extraordinary Situations”. Training was given with the Sexual Health/Reproductive Health Training Booklet for Extraordinary Situations. Results. It was found that the participants scored 20.82±4.47 on the Sexual Health and Reproductive Health Knowledge Assessment Form for Extraordinary Situations in the pre-test, 27.63±2.67 in the post-test, and 27.07±3.46 in the follow-up test. As a result of the training, it was determined that the difference between the scores they got from the Sexual Health and Reproductive Health Knowledge Assessment Form for Extraordinary Situations was due to the pre-test (p<0.05). Conclusion. In the study, the participants scored above the average in the pre-test, while they achieved significantly higher scores in the post-test and follow-up test. This shows that the training given to nurses and midwives is effective.

Introduction and objective. The tendency towards postponement of maternity implies a greater exposure of female germ cells to damaging environmental effects, including ionizing radiation (IR). Progress in paediatric oncology, based on the use of radiotherapy, also implies the occurrence of gonadal dysfunctions and subsequent female fertility disorders. Therefore, it seems justifiable to systematize the state of knowledge concerning the effect of IR on the female reproductive system. Brief description of the state of knowledge. A considerable part of studies concerning the effect of IR on female germ cells have been conducted on animals. Their extrapolation to humans is hindered because in animal studies high acute exposures are applied, which do not reflect human environmental exposures characterized by chronic low dose exposure. Studies on animals provide a heterogenous image, which hinders the formulation of unequivocal conclusions and indicates that radiosensitivity depends, i.a. on IR dose, stage of development of oocytes, the applied marker of the effects of IR, or on the species. LD50 of human oocytes is estimated to be below 2 Gy. The effect of IR depends, i.a. on the dose fractionation and the age (older women are more radiosensitive). In females, the effective sterilizing dose is: at birth 20.3 Gy, at 10 years 18.4 Gy, at 20 years 16.5 Gy, whereas at 30 years 14.3 Gy, which is associated with the available pool of ovarian follicles. Conclusions. Within the range of low doses received as a result of environmental exposure to IR, there is no evidence for the occurrence of either adverse pregnancy outcomes, nor fertility disorders in females. These effects may be related to the cancer radiotherapy, or exposure to high IR doses during nuclear accidents.

Introduction and aim. Polycystic ovarian syndrome (PCOS) is one of the major reproductive health issues, thought to be multifactorial, needs serious attention as a dual burden (health and economic) mainly for developing countries like India, due to its rapid rise (30%) in the last couple of years. Therefore, widespread and liberal screening for this disorder towards prognosis, diagnosis and intervention seems to be an urgent area of research. In this background, the present study attempts to unravel the association of salivary microbial diversity and PCOS. Material and methods. To achieve the purpose 100 clinically diagnosed PCOS individuals and 110 age matched non-PCOS participants from Bengalee Hindu caste population, West Bengal, India was considered. Obtained salivary samples were identified with 16S rDNA amplification and microbial diversity were determined by Alu I restriction enzyme digestion. Results. The present study revealed an explicit pattern of DNA fragment lengths varied between 200 bp and 225 bp in PCOs in comparison to the non-PCOS group. Conclusion. The cardinal feature of the present study as the first attempt from India envisaged, utilization of salivary microbial diversity as an additional potential and economizing biomarker for PCOS that stimulate new horizon of research in 21st century’s anthropology – the anthropology of microbes.

Non-persistent endocrine-disrupting chemicals (EDCs) are exogenous, man-made substances present in the environment that may interfere with the natural human hormones and may exert adverse consequences on human organism. Endocrinedisrupting chemicals have been suspected to be associated with altered reproductive function in the case of males and females. Environmental endocrine-disrupting non-persistent chemicals like parabens, phthalates, bisphenol A (BPA), synthetic pyrethroids and organophosphate pesticides are found in various products such as metal food cans, plastic bottles, detergents, personal care products or chemicals used for fighting against insects. The widespread distribution of these chemicals causes that humans are permanently exposed through multiple sources. The aim of this review is to summarize data linking non-persistent endocrine-disrupting chemicals exposure, and human, male reproductive hormones levels. The included studies were selected by searched PubMed, Web of Science and MEDLINE, original papers published from 2006 to 2016 and referring to human data were included to the review. The results of reviewed studies were not consistent, however, majority of the studies indicated that non-persistent EDCs may affect male reproductive hormones levels. Most findings suggest that exposure to environmental EDCs is negatively related to the level of testosterone (except for exposure to BPA which is positively associated). In most of the studies negative association was found between exposure to examined EDCs and free androgen index, too. Considering the suggested health effect of exposure to EDCs, more epidemiological data is needed. Int J Occup Med Environ Health 2018;31(5):551–573

Objectives The purpose of this paper was to analyze the relationship between the selected chemical air pollutants found in the Lublin Province and the semen parameters of men seeking fertility treatment for the first time. Material and Methods The study involved an analysis of semen sample test results obtained from male patients first reporting for fertility treatment in reproductive health centers in the Lublin Province, Poland. The data set comprises semen parameters of 13 148 men, and the number of samples in the reference period was 255–769 annually. Data on air pollution were obtained from the website of the Polish General Environmental Inspectorate and included selected chemical air pollutant levels, i.e., NO₂, SO₂, O₃, and PM₁₀. Results The mean PM₁₀ levels in the air increased, on average, by 0.65 μg/m₃ annually in 2000–2015 (p = 0.029); the mean levels of O₃, NO₂, SO₂ did not change significantly in the analyzed period. There were increasing trends in the mean sperm density and total sperm count of the subjects in 1992–2015 (p < 0.001). The mean percentage of sperm with normal morphology significantly decreased in the subsequent analysis periods: 1992–1998 (p = 0.001); 1999–2009 (p < 0.001); 2010–2015 (p = 0.001). A significant negative correlation was found between the ozone levels in the air in the Lublin Province and the percentage of sperm with normal morphology (r = –0.8311, p = 0.040). Conclusions Exposure to ozone in the air contributes to decreased percentages of sperm with normal sperm morphology. Over the years, there was an increasing trend in sperm density in the men first reporting for fertility treatment, and a decreasing trend in the percentage of sperm with normal morphology. Many women are exposed to carbon disulfide (CS₂) hazards at work every day. Working with CS₂ may cause some women to experience abnormalities in their reproductive health. Until now obtained data is generally concentrated on the health effects of CS₂ observed in the viscose industry. To date, CS₂ has not been studied precisely for its potential to have damaging effects on female reproductive system, especially the frequency of menstrual disturbances and the course of menopause. The aim of the study was to sum up female reproductive health hazards amongst women chronically exposed to CS₂ in their workplace in the viscose industry. In order to study the effect of CS₂ in the contemporary viscose industry, exposure measurements should be collected in prospective or cross-sectional studies. In conclusion, reproductive health hazards for women chronically exposed to CS2 in the workplace in the viscose industry are the following: 1) menstrual disorders essentially are more frequent than in the case of the healthy women, 2) for women chronically exposed to CS₂ the average menopausal age is statistically earlier, as compared to healthy women, 3) complex disturbances in neurohormonal system for women exposed to CS2, resulting from toxic influences of CS₂, which cause the secretion of estrogens and progesterone in ovaries and dehydroepiandrosterone sulfate in the adrenal gland to diminish. The article is aimed at the analysis of literature, international documents, legal regulations and research reports on the implementation of formal sexuality education in selected countries. There are many contradictions among researchers about the definition of sexuality education, its scope, the age from which it should be taught and learnt, people who should be its providers, its content and forms. These discrepancies are also present in the public sphere, where two strongly polarized discourses (the conservative and liberal) exist. National policies regarding sexuality education vary in the European Union. In some countries sexuality education has the status of a compulsory subject, is regulated by relevant acts, similarly to teachers training for this subject. In others, sexuality education curricula lack solid legal and theoretical basis. The analysis of selected documents and acts shows that the most effective type is the holistic sexuality education, which being part of the cultural sensitivity approach, addresses the development of knowledge and skills of education recipients, as well as the elaboration of appropriate attitudes towards positive sexuality and sexual and reproductive health. The existing research indicates also that the availability of data on sexuality education should be improved and further analysis is needed to enable better assessment of the situation, to ensure the evidence-based policy creation and strengthen the best identification practices.
Celem artykułu jest analiza literatury, międzynarodowych dokumentów, przepisów prawnych i raportów z badań dotyczących realizacji formalnej edukacji seksualnej w wybranych krajach. Wśród badaczy edukacji seksualnej istnieją rozbieżności co do samej jej definicji, zakresu, wieku, od którego powinna być prowadzona, środowisk, w jakich ma przebiegać, osób, które mają się nią zajmować, oraz jej form i doboru treści. Te rozbieżności są obecne także w sferze publicznej, gdzie istnieją dwa silnie spolaryzowane i wzajemnie się wykluczające dyskursy (konserwatywny i liberalny). Polityki krajowe w Unii Europejskiej w odniesieniu do edukacji seksualnej są także zróżnicowane. W niektórych krajach edukacja seksualna ma status przedmiotu obowiązkowego, jest regulowana odpowiednimi przepisami, podobnie jest w przypadku kształcenia nauczycieli tego przedmiotu. W innych krajach programy edukacji seksualnej nie mają solidnych podstaw prawnych i teoretycznych. Analiza wybranych dokumentów pozwala stwierdzić, że najbardziej efektywny jest typ edukacji holistycznej, który wpisując się w podejście dotyczące wrażliwości kulturowej, odnosi się do rozwoju wiedzy i umiejętności odbiorców edukacji oraz wypracowania odpowiedniej postawy na rzecz pozytywnej seksualności, zdrowia seksualnego i reprodukcyjnego. Istniejące badania wskazują ponadto, że należy poprawić dostępność danych nt. edukacji seksualnej oraz prowadzić dalsze analizy, aby umożliwić lepszą ocenę sytuacji, zapewnić tworzenie polityki opartej na dowodach i wzmocnić identyfikację najlepszych praktyk.

Background. Modern hormonal contraceptives are effective remedies for the prevention of undesirable pregnancies, have a positive impact on the health of women, and serious complications at their using meet seldom. However, 30-50% of women refuse to hormonal contraception during the first year of using. The main reasons for the refusal are side effects and a fear of their appearing. Material and methods. Our objective was to study the influence of hormonal contraceptives on the health and quality of life of women. We carried out the anonymousquestioning of 145 patients at the age of 20-39 years which were on a treatment in obstetric and gynecological hospitals of the Grodno region and ever used hormonal contraceptives. Results. H ormonal c ontraceptives a re h ighly e ffective a t r egular u sing. S ide e ffects a re widespread and were found in 9 of 10 (90.4%) women. However in 73.3% of cases they disappear during the first 3 months. Some women admitted that new diseases were diagnosed for them in a usage time of hormonal contraception. Gastritis was almost at every fifth respondent (18.6%), that requires attention. Conclusions. The most of women are satisfied with hormonal contraception. The half of respondents are ready to continue using despite side effects, and every third – if other hormonal contraceptive will be suitable. The quality of life of the women using hormonal contraceptives can be estimated as close to average norm.
Wprowadzenie. Nowoczesna antykoncepcja hormonalna to skuteczna metoda zapobiegania niechcianym ciążom, mająca zarazem pozytywny wpływ na zdrowie kobiet, w przypadku której bardzo rzadko mamy do czynienia z poważnymi komplikacjami związanymi z jej stosowaniem. Jednakże, 30-50% kobiet odstępuje od stosowania antykoncepcji hormonalnej w ciągu pierwszego roku od rozpoczęcia jej stosowania. Głównymi powodami są działania uboczne, a także strach przed ich pojawieniem się. Materiał i metody. C elem n aszego b adania b yła a naliza w pływu a ntykoncepcji h ormonalnej na zdrowie oraz jakość życia kobiet. Przeprowadziliśmy anonimową ankietę, która objęła 145 pacjentek w wieku 20-39 lat, które były leczone w szpitalach ginekologiczno-położniczych w regionie Grodna, a które stosowały w dowolnym okresie swojego życia antykoncepcję hormonalną. Wyniki. Antykoncepcja hormonalna jest wysoce skuteczna przy regularnym jej stosowaniu. Efekty uboczne są bardzo częste, gdyż rozpoznano je w przypadku 9 na 10 kobiet (90.4%). Jednakże, w przypadku 73.3% zanikają one w trakcie pierwszych 3 miesięcy stosowania. Część kobiet przyznała, że wykryto u nich nowe choroby w trakcie stosowania antykoncepcji hormonalnej. Zapalenie błony śluzowej żołądka zdiagnozowano u co piątej badanej (18.6%), co jest zapewne warte dalszej uwagi. Wnioski. Większość kobiet jest zadowolona ze stosowania antykoncepcji hormonalnej. Połowa badanych ma zamiar nadal korzystać z tego środka pomimo efektów ubocznych, a co trzecia kobieta – także z innych hormonalnych środków antykoncepcyjnych, które będą możliwe dla niej do stosowania.

Phthalates are a family of industrial chemicals that have been used for a variety of purposes. As the potential consequences of human exposure to phthalates have raised concerns in the general population, they have been studied in susceptible subjects such as pregnant women, infants and children. This article aims at evaluating the impact of exposure to phthalates on reproductive outcomes and children health by reviewing most recent published literature. Epidemiological studies focusing on exposure to phthalates and pregnancy outcome, genital development, semen quality, precocious puberty, thyroid function, respiratory symptoms and neurodevelopment in children for the last ten years were identified by a search of the PubMed, Medline, Ebsco, Agricola and Toxnet literature bases. The results from the presented studies suggest that there are strong and rather consistent indications that phthalates increase the risk of allergy and asthma and have an adverse impact on children's neurodevelopment reflected by quality of alertness among girls, decreased (less masculine) composite score in boys and attention deficit hyperactivity disorder. Results of few studies demonstrate negative associations between phthalate levels commonly experienced by the public and impaired sperm quality (concentration, morphology, motility). Phthalates negatively impact also on gestational age and head circumference; however, the results of the studies were not consistent. In all the reviewed studies, exposure to phthalates adversely affected the level of reproductive hormones (luteinizing hormone, free testosterone, sex hormone-binding globulin), anogenital distance and thyroid function. The urinary levels of phthalates were significantly higher in the pubertal gynecomastia group, in serum in girls with premature thelarche and in girls with precocious puberty. Epidemiological studies, in spite of their limitations, suggest that phthalates may affect reproductive outcome and children health. Considering the suggested health effects, more epidemiologic data is urgently needed and, in the meantime, precautionary policies must be implemented.

The effect of feeding ration, enriched in synthetic and natural ß-carotene on the selectedindicators of health condition of the cows and on reproduction parameters. The studies were conducted on 40 Simmental cows, classified into 4 groups, 10 animals in each group. The experiment was performed since the 4th week before calving until 12 week of lactation. In group I (control), standard nutrition, based on grass silage and maize silage was employed. In group II 400 mg of synthetic ß-carotene/cow/day to the ration was introduced. In group III, a part of maize silage was replaced by pumpkin silage in such ratio as to receive by 400 mg higher intake of natural ß-carotene/cow/day as compared to group I. The rations in group IV were not balanced in respect of the content of ß-carotene; there was only the replacement of 60% DM of maize silage by the pumpkin silage in this group. During the studies, blood samples were collected: to 5 hours after calving and on 4 and 12 week of lactation. Milk samples were collected on 4, 8 and 12 week of lactation and on 23 day after the first insemination. Morphological parameters of blood and concentration of ß-hydroxy-butyric acid in blood serum were analysed. In milk, somatic cell count and the level of progesterone on 23 day after insemination were determined. In group I during the following measurements were found the lowest levels of part of the morphological parameters of blood: after calving the lowest level of RBC (significant difference compared to group II), at 4th week of lactation the lowest level of HCT, and at 12 week of lactation RBC and HCT. The significant differences in somatic cell count in the milk, were recorded in the 8th week of lactation –significantly higher in group I as compared to group III. The significant differences between the groups in respect of BHM acid concentration was found only after calving. The highest BHM level occurred in the cows from group I and it was significantly higher in relation to group II and IV. In the cows from groups II, III and IV, the first heat occurred earlier; these animals had also shorter inter-gestation period. Also, the progesterone level, as determined on 23 day after insemination, was higher in the milk of cows from groups II, III and IV as compared to the animals from group I.
Wpływ dawki pokarmowej wzbogaconej w syntetyczny lub naturalny β-karoten na wybrane wskaźniki stanu zdrowia krów oraz parametry rozrodu.Badania przeprowadzono na 40 krowach rasy simentale, podzielonych na 4 grupy po 10 osobników. Doświadczenie prowadzono od 4 tygodnia przed planowanym wycieleniem do 12 tygodnia laktacji. W grupie I (kontrolnej) stosowano standardowe żywienie oparte na kiszonce z traw i kiszonce z kukurydzy. W grupie II wprowadzono dodatek do dawki 400 mg syntetycznego ß-karotenu/krowę/dzień. W grupie III zastępowano część kiszonki z kukurydzy kiszonką z dyni, tak aby uzyskać w porównaniu do grupy I wyższe o 400 mg pobranie naturalnego ß-karotenu/krowę/dzień. Dawki w grupie IV nie bilansowano pod kątem zawartości β-karotenu, a przyjęto w niej zastąpienie 60% s.m. kiszonki z kukurydzy kiszonką z dyni. W trakcie badań od krów pobierano krew: do 5 godzin po wycieleniu w 4, 12 tygodniu laktacji. Próbki mleka pobierano w 4, 8 i 12 tygodniu laktacji oraz w 23 dniu po pierwszej inseminacji. Analizowano parametry morfologiczne krwi oraz w surowicy stężenie kwasu β-hydroksymasłowego. W mleku oznaczono liczbę komórek somatycznych oraz poziom progesteronu w 23 dniu po inseminacji. W grupie I podczas kolejnych pomiarów stwierdzono najniższe poziomy części parametrów morfologicznych krwi. Po wycieleniu najniższy poziom RBC (istotna różnica w stosunku do grupy II), w 4 tygodniu laktacji najniższy poziom HCT, a w 12 tygodniu laktacji RBC i HCT. Różnice między grupami w zawartości komórek somatycznych w mleku stwierdzono w 8 tygodniu laktacji - istotnie więcej w grupie I w porównaniu do III. Istotne różnice między grupami w stężeniu kwasu BHM w surowicy stwierdzono tylko po wycieleniu. Najwyższy poziom BHM występował u krów z grupy I i był istotnie wyższy w stosunku do grupy II i IV. U krów z grup II, III i IV w porównaniu do I wcześniej wystąpiła I ruja, a także miały one krótszy okres międzyciążowy. Również oznaczony w 23 dniu po inseminacji poziom progesteronu wyższy był w mleku krów z grup II, III i IV w porównaniu do krów z grupy I.
