Cabozantinib, the latest available in Poland medication for the treatment of renal cell carcinoma, registered in this indication by the European Medicines Agency (EMA) in September 2016, has been available in several cancer centers in Poland since November 2016 as part of the expanded access program. Primary hypothyroidism is a common complication during thyrosine kinase inhibitors (TKI) treatment, although there are few reports of its occurrence during treatment with cabozantinib, which belongs to this medication group. We present a case of rapid development of clinically apparent hypothyroidism after cabozantinib treatment and report data on this complication in the group of our patients.

Objective: Results of second-line everolimus treatment in clear cell renal carcinoma. Material and methods: Clinical data of 32 patients of the Clinic of Systemic and Metastatic Malignancies, Centre of Oncology, Maria Sklodowska-Curie Memorial Institute (Krakow Branch) treated with second-line everolimus between 2011 and 2016 were analysed. Results: Median progression-free survival was 7 months. Stable disease was observed in 24 cases. Adverse events occurred in 59% of all patients, and the majority were mild and moderate (G1 and G2) according to CTCAE. The most frequent adverse events included anaemia, stomatitis, rash and fatigue. Conclusion: Molecularly targeted drugs offer the only effective and available therapy in Poland, which can be administered in patients with advanced renal cell carcinoma. Second-line everolimus is a valuable therapeutic option in the above mentioned group (following progression on first-line TKI therapy), especially when it is necessary to avoid the accumulation of similar adverse events after TKI treatment (different toxicity profiles).

Renal carcinoma metastases to the gastrointestinal tract are seldom reported in medical literature. The study presented a case of a 76-year old female patient who underwent nephrectomy, due to T2N0M0 clear cell renal carcinoma and was additionally diagnosed with two metachronous metastases to the gall-bladder and pancreas. Abdominal ultrasound performed 32 months after nephrectomy demonstrated the presence of cholelithiasis and gall-bladder polyp. Laparoscopic cholecystectomy was performed. Clear cell renal carcinoma metastasis restricted to the gall-bladder mucosa was diagnosed on the basis of the histopathological examination. After surgery the patient remained under follow-up. In December, 2009 (47 months after nephrectomy) abdominal computer tomography (CT) revealed the presence of a tumor located in the tail of the pancreas. Distal pancreatic resection and splenectomy was performed. The histopathological examination confirmed the presence of clear cell renal carcinoma metastasis. The patient is in good general condition, under follow-up at the Oncological Outpatient Clinic.

Introduction. Renal cell carcinoma (RCC) is the most common malignant epithelial tumour of the kidney, accounting for 85–90% of all solid renal tumours in adults and comprising 1–3% of all malignant visceral neoplasms. Recently, computed tomography (CT) has been considered the ‘gold standard’ in the diagnostic imaging of RCC; however, the use of CT is always associated with radiation exposure and consequently carries a significant increase in the risk of malignancy for patients with neoplastic processes. In recent years, magnetic resonance imaging (MRI) is increasingly attracting the attention of clinicians as the method of choice for the diagnosis and staging of RCC, due to several advantages over CT. Materials and method. The study involved 62 adult patients with a pathologically verified clear cell subtype of the renal cell carcinoma (ccRCC) and 15 healthy volunteers. All patients underwent renal MRI which included diffusion-weighted imaging (DWI) with subsequent apparent diffusion coefficient (ADC) measurement. Results. A significant difference was observed in the mean ADC value of the normal renal parenchyma and ccRCC – 1.82 ± 0.16 × 10– 3 mm2/s vs 2.15 ± 0.12 × 10– 3 mm2/s, respectively (р < 0,05). Additionally, statistically reliable differences in ADC values in patients with high and low ccRCC grades were obtained: in patients with the I grade, the mean ADC value was 1.92 ± 0.12 × 10– 3 mm2/s, in patients with the II grade, this value was 1.84 ± 0.14 × 10– 3 mm2/s, in patients with the III grade, the mean ADC value was 1.79 ± 0.12 × 10– 3 mm2/s, and in patients with the IV grade of nuclear polymorphism the mean ADC value was 1.72 ± 0.11 × 10– 3 mm2/s (p <0.05). Conclusions. The data obtained in the survey show a significant restriction in the diffusion of hydrogen molecules in tissues of ccRCC, compared to the healthy renal parenchyma due to the tumour’s greater density. A statistically significant difference was observed in the mean ADC values of ccRCC tumours with different Fuhrman nuclear grades: tumours with a low grade of differentiation demonstrated higher mean ADC values compared to highly differentiated tumours. Application of DWI modality of MR imaging with ADC calculation allows to obtain valuable information that is vital for the diagnosis of ccRCC and differentiation of its degree of malignancy.

Background The study aimed to investigate the influence of extremely low-frequency electromagnetic fields (ELF-EMF) on clear cell renal cell carcinoma (ccRCC) by assessing alterations in gene expression and the secretion of cytokines and chemokines. Material and Methods Three ccRCC cell lines (786-O, 769-P, and CAKI-1) and a healthy HEK293 cell line were subjected to ELF-EMF exposure (frequency 50 Hz, magnetic field strength 4.5 mT) for 30 min daily for 5 days. The study examined the expression of ADAM28, NCAM1, and VEGFC genes, along with the secretion of 30 cytokines and chemokines. Results Notably, primary tumor-derived cell lines, but not those from metastatic sites, exhibited ADAM28 gene expression, which increased following ELF-EMF exposure. A statistically significant reduction in VEGFC gene expression was observed in 769-P cells after ELF-EMF exposure. Additionally, NCAM1 gene expression was upregulated in HEK293, 769-P, and 786-O cells, representing normal embryonic kidney cells and primary tumor cells, but not in CAKI-1 cells, which model metastatic sites. After EMF exposure, there was a statistically significant decrease in transforming growth factor β1 (TGF-β1) concentration in the cell culture supernatants of HEK293 and CAKI-1 cell lines, with no other significant changes in the secretion of tested cytokines. Conclusions Given the study’s findings and available research, caution is warranted when drawing conclusions about the potential inhibitory effect of ELF-EMF on ccRCC progression. Standardization of experimental models is imperative when assessing the effects of EMF in a human context.

We present a case of a patient with metastatic renal cell carcinoma who was treated with pazopanib in the first-line treatment. Although nephrectomy was not performed, there was a positive reaction to the therapy with multi-targeted tyrosine kinase inhibitor. After 18 months of the palliative treatment, the cancerous kidney was surgically removed and pazopanib was restarted with the effect of further disease remission. The development of hypertension, complete hair discoloration and isolated hyperbilirubinemia occurred, and there was occasionally hypokalaemia in laboratory findings within 48 months of therapy. No serious adverse events were reported.

Recombinant retroviral vectors are still the most common gene delivery vehicles for gene therapy purposes, especially for construction of genetically modified tumor vaccines (GMTV). However, these vehicles are characterized by relatively low titre and in the case of many tumor cell lines, low transduction efficiency. We constructed bicistronic retroviral vector pseudotypes of amphotropic murine leukemia virus (A-MuLV) and gibbon ape leukemia virus (GaLV), encoding enhanced green fluorescent protein (EGFP) as a rapid and easily detectable reporter gene. Transduction of five different human melanoma and four renal carcinoma cell lines by these two virus pseudotypes revealed differences in transduction efficiency, which wase markedly lower for the renal carcinoma cell lines. Stimulation of retroviral receptor expression (PiT1 and PiT2) by phosphate depletion induced a limited increase of receptor mRNA levels, but did not improve the gene transfer efficiency. In contrast, simultaneous transduction with both vector pseudotypes markedly increased the transduction efficiency, compared to GaLV or A-MuLV alone. The same effect could be achieved by several repeated exposures of target cells to fresh vector preparation. Overexpression of GaLV receptor (PiT1) in target cells significantly increased the transduction rate and enabled retrovirus mediated gene transfer into the cells which normally are not transducible by GaLV pseudotypes. We demonstrated that, using different transduction strategies, the relatively inefficient, widely used retroviral vector systems could be significantly improved.

Renal cell carcinoma accounts for about 2-3% of all malignancies. It is characterized by a high frequency of metastases, which are most often observed in the lungs, bones, liver, brain and regional lymph nodes. Literature highlights the rare occurrence of renal cancer metastases to the head and neck, even though it is the third most common cancer that metastasizes to the head and neck. Below we present a case report of metastases of clear cell carcinoma from a kidney to the organs of the head and neck. The analysis was carried out retrospectively based on medical records of a patient hospitalized at the Department of Otorhinolaryngology, Head and Neck Surgery of the Medical University of Warsaw in the years 2009-2019. We describe an 81-year-old man after left-sided nephrectomy due to clear cell carcinoma, with numerous distant metastases, who was diagnosed with metastases to the left submandibular salivary gland 8 years after primary surgery, followed by a diagnosis of metastasis to the right submandibular salivary gland 3 years later. The patient underwent 2 radical procedures to remove the submandibular salivary glands. Due to the presence of lung metastases, treatment with a tyrosine kinase inhibitor was also implemented.

Introduction. Renal cell carcinoma (RCC) is the most common malignant renal tumor in adults accounting for 80-90% of primary malignant adult renal neoplasms. RCC represents a very heterogenous groups of tumors with a number of distinct histological varieties, of which the major 3 subtypes are clear cell RCC (70-80%), papillary RCC (13-20%), and chromophobe RCC (5%). Imaging features are varied from solid and relatively homogenous appearance to markedly heterogenous appearance with cystic changes, hemorrhage and necrosis. The use of diffusion weighted imaging (DWI) for RCC subtyping and also for differentiation of high grade and low grade tumors has been showed to be useful in many studies in the literature. Aim. In this study, we aimed to determine the comparative contribution of DWI in differentiation of the clear cell RCC from the major non-clear cell RCC subtypes at standard high b-value (1000 s/mm2) versus low b-value (500 s/mm2). In addition, we also aimed to assess the diagnostic performance of DWI for differentiating high grade clear cell RCC from low grade clear cell RCC based on Fuhrman grades in our patients. Material and methods. 62 cases with a prediagnosis of RCC according to MRI findings including DWI sequence with histological verification and subtyping of renal cortical tumor following a total or partial nephrectomy were included in the study. Results. Among 62 cases, 46 were male and 16 were female, with mean age of 59.5±15.7. Pathological diagnoses of 62 cases were as follows, clear cell RCC, (44) papillary cell RCC (14) and chromophobe cell RCC (4). They were divided into two groups as clear cell RCC group (44 cases) and non-clear cell RCC group (18 cases). There was no statistically significant difference between the mean ADC values of clear cell and non-clear cell groups at b-value of 1000 s/mm2 (p>0.05). However, the mean ADC level for clear cell RCC group at b-value of 500 s/mm2 were significantly higher than for non-clear cell RCC group (p<0.05). When a value of 0.99x10-3 mm2/s was set as cut-off for ADC at b-factor of 500 s/mm2, differentiation was achieved with a high sensitivity (91%) and specificity (56%). Regarding the diagnostic performance of DWI for differentiating high from low Fuhrman grades clear cell RCCs, there was no statistically significant difference between the ADC values of Grade I-II clear cell RCC cases and Grade III-IV clear cell RCC cases at b-factor of 1000 s/mm2 (p>0.05). However, ADC values for grade III-IV group was statistically significantly lower than ADC values for Grade I-II group at b-factor of 500 s/mm2 level. Conclusion. ADC measurements at moderate b-value of 500 s/mm2 were more sensitive in subtyping and grading of RCC cases. This technique can be used in clinical practice as a fast and additional sequence in abdominal MRI.

Introduction: Sarcomatoid renal-cell carcinoma is a very rare cancer characterised with aggressive course of disease and poor prognosis. At present there are no standards of care for this histologic subtype of renal cell carcinoma resistant to various forms of systemic treatment. Methods: The study describes a case of 58 year old woman after left nephrectomy for clear cell carcinoma with sarcomatoid component and after resection of right-kidney tumour for synchronous clear cell carcinoma who received first-line bevacizumab and temsirolimus under the clinical trial, and then second-line chemotherapy based on gemcitabine and doxorubicin and ifosfamide-based third-line chemotherapy. The patient underwent pulmonary metastasectomy twice, and once a metastasectomy for liver metastases. Conclusions: Surgery (including metastases treatment) followed by the systemic chemotherapy seems to be correct option of treatment in patients with renal cell carcinoma with sarcomatoid features. The development of optimum method of systemic treatment requires further prospective randomised trials.

Cancer cells are characterized by an aberrant increase in protein N-glycosylation and by disruption of E-cadherin-mediated adherens junctions. However, the relationship between alterations in N-glycosylation process and loss of E-cadherin adhesion in cancer remains unclear. The mechanisms of altered expression of adhesive glycoproteins in cancer cells have not been fully elucidated. Thus, the aim of this study was to examine the expression of E-cadherin and sialyl Lewisa/x, NeuAcα2-3Gal, NeuAcα2-6Gal/GalNAc structures in the normal renal tissue and intermediate and cancerous tissues from patients with clear cell RCC. Moreover, we attempted to correlate the E-cadherin expression with some specific sugar residues of renal cancer tissue glycoproteins. The expression of E-cadherin was analysed using ELISA test and immunoblotting. Oligosaccharide structures and sialylation level were detected with ELISA test using specific biotinylated lectins or antibodies. A significant decrease of E-cadherin expression as well as a significant increase in sialylated oligosaccharides level in intermediate zone and renal cancer tissue in comparison to normal renal tissue are reported. Significant decrease in expression of cadherins and increase in sialylation of oligosaccharide structures in renal cancer tissue in comparison to normal renal tissue, and in renal cancer tissue in comparison to intermediate zone of renal tissue, are important for the future research concerning detection and quantification of cadherins and sialylated oligosaccharide structures in urine and cells of urinary sediment as possible non-invasive marker of early RCC.

Introduction and objective. αB-crystallin belongs to the ubiquitous family of small heat-shock proteins. It was discovered as a physiological protein of the eye lens, maintaining its liquid-like property. Furthermore, αB-crystallin was proved to playa bipolar role in both physiological and pathophysiological conditions. This review discusses current knowledge about the biology and genetics of αB-crystallin, and summarizes recent advances in understanding its role in ophthalmic and neurological disorders, as well as breast cancer, renal cancer and other malignancies. State of knowledge. α-crystallins are established as important elements of the protein quality control network, and consequently their defects are related to multiple human diseases. New studies highlight αB-crystallin’s involvement in proliferative diabetic retinopathy angiogenesis and point out its therapeutic potential in age-related macular degeneration. αB-crystallin is thought to be associated with the disease-causing protein aggregates, leading to its connection with such neurological disturbances as anaplastic astrocytoma, Parkinson disease, aging deficits in the peripheral nervous system and multiple sclerosis. In breast cancer, it was proven to be a marker of aggressive behaviur and cerebral metastases. Strong expression of αB-crystallin promoted growth and migration of clear cell renal cell carcinoma cells and was correlated with lower overall survival rate. Considering other malignancies, its various roles were established in colorectal and gastric cancers, head and neck squamous cell carcinomas and osteosarcomas. Conclusions. Further studies concerning αB-crystallin seem to be enormously promising, as they might improve our understanding of common human pathologies as well as contemporary diagnostics and treatment.
