Sialic acid and sialyl Lewisa/x are found on N- and O-glycans of many human malignant cells. Carbohydrate antigens can be used as tumor markers, and an increase of their levels in cancer cells is associated with tumor progression. The aim of this study was to assess the level of some Lewis blood group antigens on glycoproteins in tumor (cancer tissue), intermediate zone (adjacent to tumor tissue), and normal renal cortex/medulla (uninvolved by tumor). The study was performed on tissues taken from 30 patients. Relative amounts of sugar structures of proteins with molecular masses above 30 kDa were determined by ELISA-like test with biotinylated lectins: MAA (Maackia amurensis), SNA (Sambucus nigra), and monoclonal antibodies anti-sialyl Lewisa/x.. Higher expression of all examined structures was revealed in cancer tissues. Significant increases were observed for sialic acid linked α 2-3 in cancer tissues when compared to healthy ones and also among intermediate and healthy tissues. The sialic acid linked α 2-6 and sialyl Lewisx structures were significantly increased in cancerous cells when compared to normal and intermediate renal tissue. In case of sialyl Lewisa antigen, a significant difference was discovered between normal and intermediate tissue. Our results confirm that the examined Lewis antigens can be involved in tumor development. Their increase in cancer tissues can suggest their specific role in the process.

Renal cell carcinoma accounts for about 2-3% of all malignancies. It is characterized by a high frequency of metastases, which are most often observed in the lungs, bones, liver, brain and regional lymph nodes. Literature highlights the rare occurrence of renal cancer metastases to the head and neck, even though it is the third most common cancer that metastasizes to the head and neck. Below we present a case report of metastases of clear cell carcinoma from a kidney to the organs of the head and neck. The analysis was carried out retrospectively based on medical records of a patient hospitalized at the Department of Otorhinolaryngology, Head and Neck Surgery of the Medical University of Warsaw in the years 2009-2019. We describe an 81-year-old man after left-sided nephrectomy due to clear cell carcinoma, with numerous distant metastases, who was diagnosed with metastases to the left submandibular salivary gland 8 years after primary surgery, followed by a diagnosis of metastasis to the right submandibular salivary gland 3 years later. The patient underwent 2 radical procedures to remove the submandibular salivary glands. Due to the presence of lung metastases, treatment with a tyrosine kinase inhibitor was also implemented.

Introduction: The fourth edition of the WHO Classification of Tumours of the Urinary System and Male Genital Organs (2016) contains new renal tumour entities. These new subtypes of renal cell carcinoma (RCC) were introduced based on morphological criteria, some genetic features, and clinical characteristics with prognostic implications. We present three patients with rare renal tumours belonging to newly recognized or still emerging categories of RCC. All cases were diagnosed based on careful morphological examination with immunophenotyping, and patho- clinical correlation. The first case is an example of acquired cystic disease - associated renal cell carcinoma with heterogeneous architecture as well as specific intra- and intercytoplasmic microlumens. The second tumour - a tubulocystic renal cell carcinoma - was composed of multiple, various-sized cysts divided by fibrovascular septa and tubules lined focally with hobnail cells. The third case presents very rare sporadic eosinophilic, solid, and cystic RCC. This tumour contained macro et microcystic, areas intermixed with solid fields composed of large, in part multinucleated eosinophilic cells. Inflammatory infiltrations accompanied the neoplastic stroma. New subtypes of RCC, although rare, can be encountered in everyday practice. It is important to perform careful differential diagnosis and classify such tumours according to the recent guidelines.

We presented a 39-year-old male who developed progressive cancer disease 4 years after nephrectomy due to clear cell carcinoma. He was diagnosed with locally reccurence and metastases to the liver, spleen and abdominal muscles. The patient was treated with sunitinib and then after disease progression – with sorafenib. We observed 18 months of cancer control (TKI-TKI). After second progression everolimus was administered. Third line everolimus therapy helped to achieve durable stable disease with PFS 46 months till now (May 2015). The patient remains in very good performance status with minimal toxicity from the regimen. This case illustrates a long term survival for patients with metastatic renal cell carcinoma, a malignancy with historically poor prognosis. The use of three sequential targeted therapies (TKI – TKI – mTORi) helped to achieve over 5 years or disease control, with rarely seen long-term response to third line treatment (mTORi) – where stabilization is good enough. We discussed therapeutic strategies in metastatic renal cell carcinoma according to the literature and therapeutic possibilities in Poland.

Introduction and objective. αB-crystallin belongs to the ubiquitous family of small heat-shock proteins. It was discovered as a physiological protein of the eye lens, maintaining its liquid-like property. Furthermore, αB-crystallin was proved to playa bipolar role in both physiological and pathophysiological conditions. This review discusses current knowledge about the biology and genetics of αB-crystallin, and summarizes recent advances in understanding its role in ophthalmic and neurological disorders, as well as breast cancer, renal cancer and other malignancies. State of knowledge. α-crystallins are established as important elements of the protein quality control network, and consequently their defects are related to multiple human diseases. New studies highlight αB-crystallin’s involvement in proliferative diabetic retinopathy angiogenesis and point out its therapeutic potential in age-related macular degeneration. αB-crystallin is thought to be associated with the disease-causing protein aggregates, leading to its connection with such neurological disturbances as anaplastic astrocytoma, Parkinson disease, aging deficits in the peripheral nervous system and multiple sclerosis. In breast cancer, it was proven to be a marker of aggressive behaviur and cerebral metastases. Strong expression of αB-crystallin promoted growth and migration of clear cell renal cell carcinoma cells and was correlated with lower overall survival rate. Considering other malignancies, its various roles were established in colorectal and gastric cancers, head and neck squamous cell carcinomas and osteosarcomas. Conclusions. Further studies concerning αB-crystallin seem to be enormously promising, as they might improve our understanding of common human pathologies as well as contemporary diagnostics and treatment.

Introduction: Radio- and chemotherapy increase the risk of disorders in the hemostasis system. Pathologic activation of coagulation and fibrinolysis may lead to formation of clots in microcirculation of many organs and subsequently to their dysfunction. Presence of serum coagulation proteins in urine may indicate renal dysfunction. Aim of paper: 1) Evaluation of correlation between hemostatic parameters in urine of patients with stage IIB-IIIB cervical cancer and results of dynamic scintigraphy. 2) Assessment of influence of prophylactic administration of low-molecular-weight heparins (LMWH) on renal function in this patient population. Material and method: The study was randomized. Treatment protocol included radiotherapy (46- 65 Gy administered by the box technique) and cisplatin (40 mg/m2 q7d) in patients with normal serum creatinine level. Renal function was assessed by dynamic scintigraphy and glomerular filtration rate (GFR) was determined. Parameters of serum hemostatic system assessed included: D-dimers, PAP, PAI-1, tPA, F1+2 and TAT. Patients were subdivided into 2 groups: a study group – with reduced GFR and a control – with normal GFR. Half of the patients in the study group received 2850 I.U. aXa or 0.3 mL nadroparin during and 6 weeks after chemotherapy (randomization list). Results: Reduction of GFR in the control group (median -9.7%) and in the study group without nadroparin (median -9.9%) were noticed. In the study group receiving nadroparin, an increase of GFR (median 22.3%) was seen. Significant differences were noticed between alterations of GFR in the control and study groups receiving nadroparin and between study groups with and without nadroparin (p=0.0001). Assessment of parameters of hemostatic system in urine revealed activation of fibrinolysis in patients receiving nadroparin and its further inhibition in the group without nadroparin. Conclusions: 1) Alterations in GFR values correlate with changes in hemostasis parameters assessed in urine. An increase of GFR is associated with alterations consistent with inhibition of coagulation and activation of fibrinolysis. Inhibition of fibrinolysis and continuous activation of coagulation results in reduction of GFR values. 2) Prophylactic administration of LMWH enhance activation of fibrinolysis, resulting in an improvement of GFR values in this patient population.
Wstęp: Radioterapia i chemioterapia zwiększają prawdopodobieństwo zaburzeń w układzie hemostazy. Patologiczna aktywacja krzepnięcia i ograniczenie fibrynolizy mogą prowadzić do powstania zakrzepów w mi-krokrążeniu wielu narządów, upośledzając ich czynność. Pojawienie się osoczowych białek układu krzepnięcia w moczu świadczy o nieprawidłowej czynności nerek. Cele pracy: 1) Ocena korelacji wartości parametrów układu hemostazy oznaczanych w moczu chorych na raka szyjki macicy w stopniu IIB-IIIB z wynikami scyntygrafii dynamicznej. 2) Ocena wpływu profilaktyki heparynami drobnocząsteczkowymi na czynność nerek w badanej grupie chorych. Materiał i metoda: Badanie miało charakter randomizowany. Leczenie zakładało podanie dawki 46-65 Gy techniką box i cisplatyny w dawce 40 mg/m2 co 7 dni u chorych z prawidłowymi wartościami kreatyniny we krwi. Czynność nerek oceniano za pomocą scyntygrafii dynamicznej, oznaczając filtrację kłębkową (GFR). W osoczowym układzie hemostazy oznaczano stężenia D-dimerów, PAP, PAI-1, tPA, F1+2 i TAT. Pacjentki podzielono na dwie grupy: badaną - z obniżonym GFR i kontrolną - z prawidłowym GFR. W grupie badanej połowa pacjentek otrzymywała 2850 j.m. aXa/0,3 ml nadroparyny w trakcie i 6 tygodni po leczeniu (lista randomizacyjna). Wyniki: Stwierdzono spadki GFR w grupie kontrolnej (mediana -9,7%) i grupie badanej bez nadroparyny (mediana -9,9%). W grupie badanej otrzymującej nadroparynę zaobserwowano wzrost wartości GFR (mediana 22,3%). Stwierdzono znamienne różnice między zmianami GFR wgru-pie kontrolnej i badanej otrzymującej nadroparynę oraz między grupą badaną bez nadroparyny i grupą badaną otrzymującą nadroparynę (p=0,0001). Badania parametrów układu hemostazy oznaczanych w moczu wykazały aktywację fibrynolizy u chorych otrzymujących nadroparynę oraz dalsze jej blokowanie w grupie bez nadroparyny. Wnioski: 1) Zmiany wartości GFR korelują z wahaniami wartości parametrów hemostazy oznaczanych w moczu. Wzrost GFR następuje, gdy przeważają zmiany świadczące o zahamowaniu krzepnięcia z równoczesną aktywacją fibrynolizy. Zahamowanie fibrynolizy i ciągła aktywacja krzepnięcia pogłębiają obniżenie wartości GFR. 2) Profilaktyczne dawki heparyny drobnocząsteczkowej nasilają aktywację fi-brynolizy i powodują wzrost wartości GFR w badanej grupie chorych.

Opis:
Renal cell carcinoma (RCC) and bladder cancer (BC) are among the most frequently diagnosed urinary system cancers worldwide. They are characterized by high mortality and recurrence rates. In response to the rising incidence and mortality rates, scientists are exploring innovative diagnostic and therapeutic methods. Metabolomics, which analyzes metabolite levels, may enable early diagnosis and monitoring of therapy progress. Compared to other omics technologies, it focuses on the outcomes of metabolite activity, providing a unique perspective on processes occurring in cancer cells. Metabolomic analyses utilize techniques such as mass spectrometry. These methods allow the identification of biomarkers and precise determination of the chemical composition of biological samples. However, the most commonly used method is liquid chromatography-mass spectrometry (LC-MS), which enables the most comprehensive screening of cancer metabolomes. Recent studies show significant progress in recognizing characteristic metabolites associated with urological cancers, although this area remains partially unexplored. Research on circulating metabolites, especially those present in easily accessible samples like blood or urine, demonstrates promising potential in clinical practice. Study results reveal differences in metabolic profiles between various stages of cancer development, which may have clinical significance. The future of this field involves an increasing number of clinical cohorts, standardization of sample preparation, and further improvements in instrument sensitivity and speed. LC-MS-based metabolomics has the potential to contribute to the improvement of diagnostics, therapy, and the quality of life of patients with some urological cancers. However, challenges, such as the lack of uniform methodologies and understanding of metabolite determinants, require further research and innovation.

Colorectal cancer and its diagnosis remain a significant clinical problem. Renal ectopia is one of the major developmental anomalies of the urinary system, although it is relatively rare in the general population. It is most often asymptomatic and is revealed by chance. This study presents a case report of a 43-year-old man, in whom colorectal cancer was suspected due to the suggestive clinical picture (abdominal pain, constipation, weight loss, a family history of colorectal cancer) and the suspicion of a proliferative process described in an outpatient ultrasound examination of the abdominal cavity. As a result of the performed diagnostics, no proliferative process was found in the patient, and the previously diagnosed lesion that raised the suspicion of cancer turned out to be the right kidney located in the pelvis. The presented case report shows that renal ectopia may cause diagnostic difficulties, among others in patients diagnosed with a neoplastic process.
Rak jelita grubego i jego diagnostyka pozostają istotnym problemem klinicznym. Ektopia nerki jest jedną z głównych anomalii rozwojowych układu moczowego, chociaż występuje stosunkowo rzadko w populacji ogólnej. Najczęściej przebiega bez objawów i jest wykrywana przypadkowo. W pracy przedstawiono opis przypadku 43-letniego mężczyzny, u którego podejrzewano raka jelita grubego z powodu sugestywnego obrazu klinicznego (ból brzucha, zaparcie, spadek masy ciała, obciążający wywiad rodzinny w kierunku raka jelita grubego) oraz podejrzenia procesu rozrostowego na podstawie wykonanego ambulatoryjnie badania ultrasonograficznego jamy brzusznej. W wyniku przeprowadzonej diagnostyki nie stwierdzono u chorego procesu rozrostowego, rozpoznawana zaś uprzednio zmiana budząca podejrzenie nowotworu okazała się położoną w miednicy prawą nerką. Przedstawiony opis przypadku pokazuje, że ektopia nerki może powodować trudności diagnostyczne, m.in. u chorych diagnozowanych w kierunku procesu rozrostowego jelita.
