This article describes a novel approach to realtime motion assessment for rehabilitation exercises based on the integration of comprehensive kinematic modeling with fuzzy inference. To facilitate the assessment of all important aspects of a rehabilitation exercise, a kinematic model is developed to capture the essential requirements for static poses, dynamic movements, as well as the invariance that must be observed during an exercise. The kinematic model is expressed in terms of a set of kinematic rules. During the actual execution of a rehabilitation exercise, the similarity between the measured motion data and the model is computed in terms of their distances, which are then used as inputs to a fuzzy interference system to derive the overall quality of the execution. The integrated approach provides both a detailed categorical assessment of the overall execution of the exercise and the degree of adherence to individual kinematic rules.

Wstęp. Kompleksowe leczenie zesztywniającego zapalenia stawów kręgosłupa powinno łączyć metody farmakologiczne i niefarmakologiczne. Celem pracy jest ocena wpływu nienadzorowanych ćwiczeń domowych na poziom funkcjonalny pacjentów z zesztywniającym zapaleniem stawów kręgosłupa. Dodatkowo przeprowadzono ocenę wpływu ćwiczeń realizowanych w domu na efektywność prowadzonej rehabilitacji szpitalnej. Materiał i metody. Badaniem zostało objętych 52 pacjentów (12 kobiet i 40 mężczyzn) z zesztywniającym zapaleniem stawów kręgosłupa w wieku średnio 44,1 lat. W oparciu o zebrane dane z ankiet, podzielono pacjentów na dwie grupy: grupa A (34 pacjentów) – pacjenci ćwiczący w domu, grupa B (18 pacjentów) – pacjenci niećwiczący w domu. W obu grupach dokonano dwukrotnie oceny stanu funkcjonalnego za pomocą kwestionariusza BASFI (na początku rehabilitacji oraz po 3 tygodniach). Wyniki. W obu grupach wykazano porównywalne wartości wskaźnika BASFI na początku rehabilitacji (BASFI I). Po 3 tygodniach prowadzonego leczenia szpitalnego odnotowano istotną statystycznie poprawę stanu funkcjonalnego (wskaźnik BASFI II) w całej grupie pacjentów. Stopień poprawy stanu funkcjonalnego był porównywalny w obu grupach. Wnioski. W przeprowadzonych badaniach wykazano brak wpływu nienadzorowanych ćwiczeń domowych na poziom funkcjonalny pacjentów z zesztywniającym zapaleniem stawów kręgosłupa. Stan funkcjonalny pacjentów po 3-tygodniowej rehabilitacji szpitalnej uległ znaczącej poprawie. Nie wykazano, by ćwiczenia realizowane w domu wpływały na zwiększenie efektywności prowadzonej rehabilitacji.
Background. Complex treatment of ankylosing spondylitis should combine pharmacologic and non-pharmacologic methods. The aim of the research was to estimate the influence of non-supervised home exercises on functional condition of patients with ankylosing spondylitis. Additionally, the assessment of the influence of home exercises on the efficiency of inpatient rehabilitation was performed. Material and methods. The study involved 52 patients (12 women, 40 men) approximately at the age of 44.1 with ankylosing spondylitis. The patients were divided into two groups: group A (34 patients) exercising at home, group B (18 patients) not exercising at home. The assessment of functional condition of patients in both groups was performed twice using the BASFI questionnaire at the beginning of the rehabilitation and 3 weeks after the treatment. Results. A comparative value of the BASFI index was found for both groups at the beginning of the rehabilitation, it was determined as BASFI I. After 3 weeks of rehabilitative treatment a statistically significant improvement was observed (index BASFI II) in all patients, as well as in group A and B separately. Improvement of functional condition was almost the same in both groups. Conclusion. The analysis of the collected research material proved that there was no influence of non-supervised home exercises on the functional condition of patients with ankylosing spondylitis. The functional condition of the patients after 3 weeks of rehabilitation improved significantly. The study did not demonstrate any influence of home exercises on the increase of effectiveness of inpatient rehabilitation. Aim of the study: To evaluate the effects of two different modes of improvement on the level of perceived pain, spinal mobility, lumbar spinal motion control and limitations in activities of daily living in women aged 51–62 with lumbar spine pain. Material and methods: The study included a group of 31 women between the ages of 51 and 62. The subjects were divided into 2 groups. Group I (FK) received physical therapy treatments and general gymnastics, while group II (FS) received physical therapy treatments and exercises using a PBU stabilizer. The project covered a period of 10 treatment days excluding Saturdays and Sundays. Before and immediately after the rehabilitation, tests of flexion and extension control in the L-spine, active knee joint flexion test, Schober test were performed, and the NRS scale was used scale and QBPDS scale. Results: After the applied rehabilitation, a significant reduction in the level of perceived pain and improvement in spinal mobility in the direction of flexion were observed in both groups. In the FS group, there was also a significant improvement in lumbar flexion control and better lumbar spine control during the active knee flexion test. In addition, a reduction in disability as measured by the QBPDS scale was noted. Conclusions: The use of comprehensive rehabilitation brings significant improvements in terms of pain reduction and improvement in lumbar spine mobility in the flexion direction. A two-week exercise program using a stabilizer appears to yield significantly better results in terms of lumbar flexion control as well as better lumbar control during the active flexion test of both knee joints. And this, in turn, may affect the results obtained using the QBPDS scale among FS subjects.

Introduction: The training of core muscles is key in sports training to improve performance and reduce the risk of injuries. However the most effective method to improve core musculature is still controversial. Therefore the objective of this study was aimed at investigating the effectiveness of dynamic Swiss ball training in reinforcing the core stability of collegiate athletes. Materials and methods: This two-group, two-factor research involved 67, 18- to 28-year-old collegiate athletes (18 females, 49 males) who were divided into experimental and control groups. The experimental group was provided dynamic Swiss ball training, whereas the control group was instructed to perform floor exercises. The groups underwent training for six weeks (three days per week). Four core stability tests (the Biering-Sorenson trunk extension, Side Bridge, prone bridge, and double leg lowering tests) were administrated pre- and post-training. Results: The mean scores of the participants in the core stability tests significantly improved after six weeks of training (p<0.05), but the between-group comparisons revealed that the experimental group significantly outperformed the control group in terms of enhancement in core stability (p<0.05). Conclusion: The relatively high improvement in core stability parameters after Swiss ball training suggested that these exercises are favorable alternatives to traditional floor exercises in strengthening the core muscles. The findings are expected to help athletes, coaches, trainers, and other strength and conditioning specialists involved in athletic training decide on appropriate training methods.

Zespół cieśni kanału nadgarstka to najczęściej występująca neuropatia i główna przyczyna upośledzenia funkcji nerwu pośrodkowego. Rozwój cywilizacyjny i często niekorzystne warunki pracy powodują znaczny wzrost liczby zachorowań. Różnorodność czynników terapeutycznych wykorzystywanych w zachowawczym i pooperacyjnym leczeniu zckn narzuca konieczność oceny ich skuteczności w celu udoskonalenia stosowanych programów usprawniania. Celem pracy była ocena skuteczności fijoterapii z wykorzystaniem sonoterapii, masażu wirowego i kinezyterapii u pacjentów z zckn. W badaniach wzięło udział 30 osób skierowanych na zabiegi fizjoterapeutyczne zgodne z przyjętym programem badawczym z powodu jedno- lub obustronnego zespołu cieśni kanału nadgarstka. Łącznie analizie poddano 48 rąk. Oceniano subiektywne i obiektywne objawy kliniczne. Przy wykorzystaniu elektronicznego urządzenia do diagnostyki ręki mierzony był zakres ruchu w stawach ręki i siła chwytu globalnego ręki. Przy zastosowaniu tradycyjnych metod elektrodiagnostycznych oceniano pobudliwość nerwową mięśni kłębu kciuka. Przedstawione badania wykonywano przed wdrożeniem programu rehabilitacyjnego i po jego zakończeniu. Program obejmował: sonoterapię, masaż wirowy oraz neuromobilizacje nerwu pośrodkowego. Uzyskano zmniejszenie nasilenia objawów klinicznych, jak też całkowite ustąpienie niektórych objawów. Wyniki komputerowych pomiarów zakresu ruchu i siły mięśniowej potwierdziły poprawę funkcji ręki. Uzyskano istotną statystycznie poprawę jakości czucia dwupunktowego, natomiast zmiany parametrów elektrodiagnostycznych, takich jak reobaza, chronaksja i współczynnik akomodacji były statystycznie nieistotne. Wnioski. Skojarzona terapia w postaci sonoterapii połączonej z hydroterapią i kinezyterapią pozwala uzyskać zmniejszenie nasilenia objawów klinicznych i poprawę funkcji ręki u pacjentów z zespołem cieśni kanału nadgarstka.
Carpal tunnel syndrome is the most commonly occurring neuropathy and main cause of median nerve dysfunction. Advancements in civilization and poor working conditions have often resulted in a signifiant increase in the number of cases. The variety of therapeutic agents applied in conservative and post-surgery treatment of carpal tunnel syndrome necessitates the need to evaluate their efficy in order to improve applicable rehabilitation programs. The aim of the paper was to assess the physiotherapeutic effctiveness of sonotherapy, whirl massage and physiotherapy applied on patients with carpal tunnel syndrome. A total of 30 people referred for physiotherapy in accordance with approved research program because of a single or bilateral carpal tunnel syndrome were covered by the study. A total of 48 arms were analyzed in all. Both subjective and objective clinical syndromes were evaluated. With the use of electrodiagnostic testing device hand measurements were made in areas of joint motions as well as the power grip. The nerve excitability of thenar muscles was evaluated using the traditional electrodiagnostic methods. The studies were performed prior to the implementation of rehabilitation programs and after their completion. The program covered sonotherapy, whirl massage and neuro-mobilization of the median nerve. A reduction in the severity of clinical symptoms as well as complete relief from some others was obtained. Results of computer measurements in respect of motion and muscle strength confimed the improved hand function. A statistically significant improvement in the two-point discrimination sensory quality was obtained while changes in electro-diagnostic parameters such as rheobasis, chronaxie and accommodation rate were statistically insignificant. Conclusions. A combination of antiretroviral therapy in the form of sonotherapy together with hydro-therapy and kinesitherapy allows for reducing the severity of clinical symptoms as well as improve the hand function in patients with carpal tunnel syndrome (CTS). discrimination sensory quality was obtained while changes in electro-diagnostic parameters such as rheobasis, chronaxie and accommodation rate were statistically insignifiant.

ObjectivesVestibular rehabilitation leads to a gradual diminution of the subjective and objective symptoms that accompany the vestibular disorders. The aim of the study was to compare the impact of 2 different types of vestibular rehabilitation on vestibular compensation in patients with chronic unilateral vestibular dysfunction.Material and MethodsThe study was conducted on a group of 58 subjects (43 females and 15 males) aged 40–64 years, who presented with chronic unilateral vestibular dysfunction and were hospitalized. The patients were randomly assigned to either of the 2 groups established. The study was conducted in a 6-week period. Group 1 consisted of patients who underwent customized group vestibular rehabilitation in an outpatient setting. The program was performed once a week for 1 h 30 min, under the supervision of a physiotherapist and a physiatrist. Group 2 was instructed to perform Cawthorne–Cooksey exercises and simple balance exercises twice a day for 15 min.ResultsAn improvement in the outcomes of the Dynamic Gait Index as well as the Berg Balance Scale was statistically significant for group 1. The time for fulfilling the task in the Timed Up and Go Test improved in both groups (p < 0.05). The subjective estimation of the symptoms evaluated with the use of the Dizziness Handicap Inventory and the Visual Analogue Scale revealed a statistically significant improvement in both groups, yet it was higher in group 1.ConclusionsThe compensation achieved after 6 weeks of the customized, supervised outpatient rehabilitation program in group 1 was superior to the results of the home-based unsupervised Cawthorne–Cooksey and balance exercises.

The system of human locomotion is very complex. Generally we can divide it into the skeletal and muscle systems. The muscle system (muscles) enable to move the body which is formed by the skeletal system (bones). Electromyography is a technique of measurement the electrical activity of muscles which is related with shortening or lengthening of muscle fibers (muscle contraction). In the paper analysis of the electromyographic signal (EMG) recorded during knee rehabilitation exercises was conducted. The records of EMG from person after knee injury were compared with records of EMG from healthy person.

According to the statistical data, there is being observed a definite increase in percentage of children affected by posture defects, particularly scolioses. The aquatic environment facilitates treatment of scolioses, since it provides relief conditions used in order to perform prophylactic, corrective and therapeutic tasks. Hydrokinetic therapy is one of the many rehabilitation methods. It seems, however, that corrective and therapeutic swimming is far too rarely used as an auxiliary measure for treatment of scolioses. This paper presents the concept of treatment of scolioses employing asymmetrical exercises in aquatic environment, the author of which in Poland is Iwanowski. In the study desk research of literature studies was applied along with the analysis of publications, including available study reports, articles, documents and also own video and photographic materials. The author suggests the concept of conservative treatment of scolioses through the application of asymmetrical swimming exercises in aquatic environment. Such procedure is efficient in preventing significant spinal deformations, which may protect against surgical intervention.
