Opis:
Surgical interventions in patients with peritoneal metastases combined with hyperthermic intraperitoneal chemotherapy (HIPEC) and systemic treatment are becoming more common and, when applied to selected patient groups, they reach 5-year survival rates of 32–52%. Good clinical outcomes require experienced and well-equipped healthcare centers, experienced surgical team and adequate patient qualification process. As a result of the discussion on the need for evaluation of quality of care and treatment outcomes and at the request of the Peritoneal Cancer Section of the Polish Society of Surgical Oncology, accreditation standards have been developed and the Accreditation Committee has been established for healthcare centers providing cytoreductive surgery and HIPEC for the management of primary and secondary peritoneal cancers.

Celem artykułu jest przedstawienie uwarunkowań polskich uzdrowisk oraz zmian zachodzących w ich przestrzeniach publicznych w ostatnich latach. Na przykładzie małopolskich kurortów obserwowano zmiany w zakresie poprawy jakości przestrzeni publicznych oraz poprawy ich infrastruktury technicznej, zachodzące w ostatnich dziesięciu latach. Przekształcenia przestrzeni dokonywane w wielu z nich w oparciu o skutecznie pozyskane środki UE, dają podstawę dla dalszego rozwoju gospodarczego tych miejscowości. Wzmacniają też pozycję małopolskich uzdrowisk wśród pozostałych polskich i europejskich miejscowości kuracyjnych.
The observed changes in Małopolska health resorts that have resulted in improving the quality of their public spaces and technical infrastructure over the last 10 years, are directly related to the effective financing with EU funds obtained by municipalities and health resort companies. The implementation of costly investments provides a basis for further economic development of these localities, as well as strengthens the position of Małopolska spas among other Polish and European spas. On the example of selected spas the author presents the improvements taking place, with special regard to investments co-financed from EU funds.

Celem artykułu jest analiza podejścia do usługi leczenia szpitalnego jako usługi systemowej. Zaprezentowane zostały teoretyczne koncepcje ujmowania tej usługi oraz elementy wchodzące w jej skład. Wskazano także znaczenie systemowego ujęcia tej usługi w procesie zarządzania jej jakością. W artykule zaprezentowano także metodykę oraz wnioski z badania jakości świadczonych usług leczenia szpitalnego zrealizowanego w jednym ze szpitali w Warszawie.
The purpose of the article is analysis of the approach to hospital treatment as a service system. Theoretical concepts of recognition of this service and elements within its composition have been presented. The importance of the systemic approach to this service in the process of management of its quality have been pointed out. The article also presents the methodology and conclusions of the study of the service quality of hospital treatment in one of the hospitals in Warsaw.

Introduction: Atrial fibrillation (AF) is one of the most frequent arrhythmias. In Poland, there is a risk of developing AF in about 400,000 people. Atrial fibrillation occurs most frequently as a result of disorders of physiological automatism and afterdepolarization and triggered activity. The treatment strategy of AF is based on pharmacological treatment and procedures such as ablation or cardioversion. The quality of life as defined by the WHO is an individual perception of one's well-being. On the other hand, the assessment of the quality of life in illness is modified regarding health problems. Purpose: The main goal of the study was to get to know the quality of life of people with atrial fibrillation. Materials and methods: The research was carried out using a diagnostic survey, based on a survey among 100 patients diagnosed with AF. The place of the research was the Regional Specialist Hospital in Biała Podlaska, in the Emergency Department. Results: Increased morbidity was observed among women and people over the age of 60. The majority of patients with AF declared the quality of life at the medium level. The respondents most often followed medical recommendations. The majority of respondents did not follow healthy lifestyle rules, because as many as 68% of respondents did not do any physical activity and a significant part of the respondents smoked cigarettes and had an inflated BMI. Conclusion: AF affects the quality life by causing sadness, irritation and insomnia.

Selection of the treatment method in breast cancer patients and its consequences may affect their quality of life through somatic, psychical, and social factors. The aim of the study was early evaluation of the quality of life of women after mastectomy vs. breast conserving surgery. Material and methods. The study included 100 women aged 31 to 79 years (mean: 57) who underwent surgery due to breast cancer (amputation: 52; breast conserving surgery: 48 women) at the Cancer Centre in Bydgoszcz in 2014. The QLQ C-30 and QLQ BR-23 questionnaires were used to evaluate the quality of life of the patients 3 months after surgery. Results. In the Global Health Status/QoL domain, the mean score for women after amputation and breast conserving surgery was 49 and 53, respectively; for Physical Functioning, the scores were 70 and 75, and for Role Functioning, 62 and 68, respectively. For Cognitive Functioning, the mean score was 74 and 73; for Emotional Functioning - 62 and 68, and for Social Functioning 64 and 60, respectively. The difference in the arm symptoms domain was significant at 46 and 33 points, respectively (p = 0.004). The patients treated with breast conserving surgery had a better body image than women after amputation - the mean score was 52 and 66, respectively (p = 0.01). Conclusions. With respect to Global Health Status/QoL and Physical Functioning, the quality of life of women in the early postoperative period was similar in women after breast amputation and those who underwent breast conserving surgery. Patients treated with breast conserving surgery had a better score for body image, while those who underwent amputation more often suffered from arm symptoms, such as pain, oedema, and problems with raising of the limb.

This paper presents the analysis and evaluation of the impact of clean sewage on the water in the Mordarka river, in the period 2012–2015. The analysis was conducted based on the collected and statistical research: the amount and composition of the purified sewage discharged to the receiver. The following pollution indices were tested: total suspended solids, BOD5, COD, total nitrogen and total phosphorus. On the basis of the obtained results it was concluded that the amount of purified wastewater discharge to the Mordarka river, did not contribute to the degradation of the water quality in that period of time. Moreover, the level of indicators: total suspended solid, BOD5 and COD did not exceed the limit values from the regulation, which confirmed the effective protection of water quality in the Mordarka river. However, because of the high concentration of biogenic compounds in the purified wastewater effluents, the river is prone to eutrophication process.

Yoga exists in the context of health care, complementary therapy, movement training techniques as well as religious philosophy. Many studies suggest yoga is a non-demanding activity that can effectively reduce anxiety, depression, and pain in the adult population with acute and chronic health conditions without adverse effects. Based on the results of numerous studies, yoga intervention can improve mental health and improve the quality of life of cancer patients or survivors. This paper focuses on yoga and its impact on alleviating the symptoms of cancer and the treatment. Based on the reviewed literature, the results of the studies suggest the problem of heterogeneous study design, lack of quantitative findings and lack of evidence regarding the long-term effects of yoga on the personal well-being of cancer patients. Future research should focus on systematic reviews or meta-analyses to provide further evidence on the type and effectiveness of yoga during or after cancer treatment for specific cancers.

The work concerns the modernization of the Water Treatment Plant, the objective of which was to improve the quality of recipients; life by counteracting water deficits and ensuring a stable supply of water with appropriate pressure and quality, in accordance with the requirements of applicable regulations. Installing modern control and measuring equipment, as well as modern devices during the modernization of the facility, will significantly reduce electricity consumption, which will realistically minimize the operating costs. The article describes in detail what the modernization of the water treatment plant was like and what effects of water treatment from the quaternary aquifer were obtained. After the completed modernization of the plant, the County Sanitary and Epidemiological Station gave a positive opinion on all materials and devices used in the modernization of the WTP and agreed to supply the existing water supply with purified water.

The paper presents the assessment of the influence of wind direction and speed on the transverse fall of sprayed liquid for selected types of flat-stream nozzles. The measurements were carried out in laboratory conditions on an automated station equipped with a radial fan, speed and wind direction sensors, and a computer system for automatic acquisition and processing of measurement results. As criteria for the evaluation of the results of the conducted research on the quality of work of the tested nozzles, the values of the transverse nonuniformity index of CV liquid fall were assumed (with a maximum admissible value of 10%). The obtained results of laboratory tests were presented in the form of tables, and on their basis the dependencies of the CV index value on wind direction and wind speed were determined. On the basis of the obtained dependences, the assessment of individual nozzles was made due to the accepted criterion in the scope of the studied changes in wind speed and direction.

Breast cancer is currently one of the most frequent malignant neoplasms in Polish women. Since the ‘80s of past century, breast cancer is the leading cause of mortality in women aged 40-59. Patients are usually admitted to the stationary palliative care facilities in a far advanced stage of neoplastic disease originating primarily in the breast and are mostly in a poor general condition. Their overall condition is a resultant of development and severity of the tumour itself with possible distant metastases, coexisting diseases, as well as late complications of cause-oriented or palliative treatment implemented. In the setting of palliative treatment, the primary goal is relief of pain, according to the recommendations of the so-called “analgetic ladder”, conforming with general principles of analgetic management. This consists in determination of cause of pain, evaluation of its type and severity, regular pace of administration of analgetic drugs, careful titration of dose of strong analgetics, oral administration of drugs, as this is the most physiologic and recommended route of administration, if only the patient can tolerate it. An important issue is also to relieve other bothersome ailments, both somatic, mental, spiritual and social. According to the WHO definition, palliative care ensures the patients a comprehensive and active care, improves their quality of life and, when justified, includes all oncologic therapeutic modalities. In the setting of palliative care, oncologic treatment aims at arresting or slowing-down of development of primary tumour and metastatic foci, thus improving the patient’s comfort and quality of life, obviously taking into account current state of the patient and toxicity of drugs.
Rak piersi należy obecnie do najczęstszych nowotworów złośliwych u kobiet w Polsce. Począwszy od lat 80. ubiegłego wieku rak piersi stanowi pierwszą co do częstości przyczynę zgonów kobiet w wieku między 40. a 59. rokiem życia. Chore przyjmowane na stacjonarny oddział opieki paliatywnej w zaawansowanym stadium choroby nowotworowej wywodzącej się pierwotnie z obszaru piersi są zazwyczaj w złym stanie ogólnym. Jest to spowodowane rozwojem i nasileniem objawów choroby, z możliwością występowania przerzutów do odległych narządów, a także chorobami współistniejącymi, jak również powikłaniami późnymi po stosowanym leczeniu przyczynowym lub paliatywnym. W oddziale opieki paliatywnej stosuje się w pierwszej kolejności leczenie przeciwbólowe, zgodnie z zaleceniami drabiny analgetycznej oraz uwzględnieniem głównych zasad postępowania przeciwbólowego. Leczenie to polega na ustaleniu przyczyny bólu, ocenie rodzaju i jego natężenia, regularności podawanego leku, miareczkowaniu dawki silnego analgetyku, podawaniu leków doustnie, gdyż jest to droga fizjologiczna i zalecana, o ile chory może w ten sposób przyjmować preparaty. Istotne jest także łagodzenie innych uciążliwych dolegliwości - zarówno somatycznych, psychicznych, duchowych, jak i socjalnych. Zgodnie z definicją Światowej Organizacji Zdrowia (WHO) w ramach opieki paliatywnej zapewnia się chorym całościową, czynną opiekę, poprawiającą ich jakość życia, a także stosuje w uzasadnionych przypadkach wszystkie onkologiczne metody leczenia. Terapia onkologiczna w ramach opieki paliatywnej ma na celu zahamowanie lub spowolnienie rozwoju guzów nowotworowych lub przerzutowych, a co za tym idzie poprawę komfortu i jakości życia chorego, oczywiście z uwzględnieniem jego aktualnego stanu i toksyczności ewentualnego leczenia.

The aim of the study was to analyze the results of patients’ subjective self-assessment of surgical treatment of prognathism. Materials and methods. The study included 35 patients who underwent extraoral vertical ramus osteotomy. Each patient responded to 12 questions contained in the questionnaire created by the authors. Results. All respondents positively assessed their appearance after surgery. In most cases they pointed out the positive impact of treatment on their self-confidence (82.9% of responses). The vast majority of respondents felt improvement in biting off food (91.4%). Conclusions. Patients positively evaluate the outcome of surgical treatment with extraoral vertical ramus osteotomy. The subjective improvement of the facial appearance, as well as chewing and articulation functions took place after the treatment.

Opis:
The thermally hardened 12 mm sheets of S620Q steel underwent research. The sheets weldability was examined during semi-automatic welding by MAG method, at the welding conditions usually applied in industry for welding normalized sheets of this steel. The technology of welding was presented. The achievement of metallically continuous joints was confirmed by X-ray examination. For the purpose of usefulness assessment, samples were taken from test joints then the joints hardness distribution measurements were performed. The microstructure was examined and the static tensile tests as well as technological bend tests were carried out. Hardness measurements were performed in three measurement lines for each joint according to PN-EN 1043-1:2000 requirements. Joints microstructures were examined in the joints axis, in SWC and in native materials. Steel microstructures were classified as low carbon tempered martensite of layered construction. Mean values of the selected joints strengths were also presented. The tensile tests values of native materials of metallurgic certificates were given for the sake of comparison. The research results presented in the paper made it possible to determine the weldability of S620Q steel.

Purpose: According to the available international recommendations, at least one independent verifi cation of the calculations of number of monitor unit (MU) is required for every patient treated by teleradiotherapy. The aim of this study was to estimate the differences of dose distributions calculated with two treatment planning systems: Eclipse (Varian) and Oncentra MasterPlan (Elekta). Materials and methods: The analysis was performed for 280 three-dimensional conformal radiotherapy treatment (3D-CRT) plans with photon beams from Varian accelerators: CL 600C/D X6 MV (109 plans), CL 2300C/D X6 MV (43 plans), and CL 2300C/D X15 MV (128 plans). The mean doses in the planning target volume (PTV) and doses at the isocenter point obtained with Eclipse and Oncentra MasterPlan (OMP) were compared with Wilcoxon matched-pairs signed rank test. Additionally, the treatment planning system (TPS) calculations were compared with dosimetric measurements performed in the inhomogeneous phantom. Results: Data were analysed for 6 MV plans and for 15 MV plans separately, independently of the treatment machine. The dose values calculated in Eclipse were significantly (p <0.001) higher compared to calculations of OMP system. The average difference of the mean dose to PTV was (1.4 ± 1.0)% for X6 MV and (2.5 ± 0.6)% for X15 MV. Average dose disparities at the isocenter point were (1.3 ± 1.9)% and (2.1 ± 1.0)% for X6 MV and X15 MV beams, respectively. The largest differences were observed in lungs, air cavities, and bone structures. Moreover the variation in dosimetric measurements was less as compared to Eclipse calculations. Conclusions: OMP calculations were introduced as the independent MU verification tool with the first action level range equal to 3.5%.

The conventional heat treatment is being replaced by thermal-plastic treatment. The process of steel hardening is carried out on quenching press with a double-sided water spray at the temperatures ranging from 600 to 700°C. The steel hardening process is conducted at the temperatures of 600-700°C with cooling in the air. After this, thermal hardening the steel sheets obtain the structure of a layered tempered matrensite with carbide and cyanide disperse separations The thermally hardened S540Q steel sheets 12 mm thick underwent examinations. The welding technology was presented. The sheets were welded semi automatically by means of MAG method. The usual welding conditions normally applied in industry were used after steel standardisations. The achievement of joints metallically continuous was confirmed by X-ray tests. The static tensile test was conducted to assess the applicability of this steel. The average values of the welded joints resistance to tensile strength were given. After conducting technological bend, test good ductility was noted. Steel microstructure was defined as low carbon tempered matrensite of layered construction. Microstructure observation was carried out in the joint axis, in heat affected zone and in native material. Hardness measurements were performed according to PN-EN 1043-1:2000 requirements. Impact test was conducted on the samples with a V cut notch at the temperatures of -20, -40, -60 Celsius degrees.

Opis:
S620Q steel sheets 12 mm thick that were heat treated were subjected to research. The sheets weldability was examined during semi-automatic welding by MAG method, at the welding conditions usually applied in industry for welding normalized sheets of this steel. The achievement of metallically continuous joints was confirmed by X-ray examination, the correctness of making test joints was confirmed by the measurement of transverse warp and edges of sample sections. Moreover the following tests were carried out: static tensile tests, technological bending tests and impact tests on the samples with V-notch cut in positions S, P, C2 and C5. The joints microstructure was observed in the weld axis, in SWC and in native materials with the use of Neophot-2 optical microscope. Steel microstructure was classified as low-carbon, tempered martensite of layered construction. The bend tests were carried out by means of bending arbour 36mm in diameter. The impact energy values for test temperatures -60°C, -40°C and -20°C marked in the drawings are the mean values of breaking three samples at a given test temperature. The places ofnotches cuts applied on welded joints samples are required by Det Norske Veritas regulations for welded joints. Mechanical properties ofS620Q steel were given as reference.
