Opis:
Anatomical and physiological conditions in the vagina do not facilitate easy application of the drug neither its maintenance on the site of application. Insufficient duration of the drug’s contact with vaginal mucosa does not provide adequate pH conditioning the physiological biocenosis of the organ. Gels containing lactic acid complexed with chitosan at a stechiometric ration 1:1 and 2:1 and 5 - 25% content of PEG-200 are able to move from 25 to 30 cm. Higher concentrations of the poloxamer 407: 23 and 25% result in the movability of 20 to 24 cm. The addition of 20 – 25% poloxamer 407 increases the pH to 4.55 to 5.30 for 1:1 gels and 4.00 to 4.85 for 2:1 gels. The investigations showed that the addition of a thermosensitive polymer increases the adhesive properties of the investigated gels. Rheological investigations revealed an increase in the dynamic viscosity of preparations containing poloxamer 407 in comparison to the reference gels.

Opis:
Gels containing lactic acid complexed with chitosan at a stoichiometric ratio 1:1 and 2:1 and 5 - 25% content of polyoxyethylene glycol 200 reveal pH from 2.5 to 4.6. The addition of 20 – 25% poloxamer 407 increases the pH to 4.55 to 5.30 for 1:1 gels and 4.00 to 4.85 for 2:1 gels. The addition of 0.5% polyvinyl alcohol decreases the pH of the investigated gels to 4.00 to 4.55 for 1:1 gels and 3.85 to 4.40 for 2:1 gels. The addition of 1.0% dextrane decreases the pH of investigated gels to 4.40 to 4.90 (1:1) and 4.00 to 4.48 (2:1). Rheological investigations revealed an increase in the dynamic viscosity of preparations containing poloxamer 407 with the addition of polyvinyl alcohol or dextrane in comparison to the reference gels. Results obtained in the experimental studies proved that it is possible to produce a preparation with optimal pharmaceutical and application properties.

Opis:
Commonly applied drug forms tend to leave the vagina when the patient assumes an upright position. Permanent contact of the drug form with the vaginal mucosa during daily activities of the patient is an indispensable condition for 24-hour therapy. This condition may be fulfilled by drug forms with high ability to adhere to vaginal mucosa. The investigations demonstrated that the thermosensitive polymer - poloxamer 407 increases significantly the adhesive properties of hydrophilic gels, but at the same time it increases their pH. The use of pectin resulted in a decrease of the pH. The addition of glycerol or 1,2-propylene glycol caused further decrease of the pH. Rheological investigations demonstrated positive pharmaceutical properties of the investigated preparations. The obtained parameters gels should provide positive application effects. The investigations revealed that it is possible to obtain gels with high adhesion properties to vaginal mucous membrane.

Gels containing lactic acid complexed with chitosan at a stoichiometric ration 1:1 and 2:1 and 5-25% content of polyoxyethylene glycol 200 are able to move from 25 to 30 cm. The addition of 4% hydroxypropylmethylcellulose besides glycerol increases the movability gels containing lactic acid complexed with chitosan in a stoichiometric ration 1:1 from 25 cm to 28 cm and at 2:1 ratio from 27 cm to 30 cm. Higher concentrations of the poloxamer 407: 23 and 25% result in the movability of 24 to 29 cm The addition of 5-25% glycerol decreases the pH of investigated gels from 3.95 to 4.45 (1:1) and from 3.63 to 4.30 (2:1). The addition of 5-25% 1,2-propylene glycol decreases the pH of investigated gels from 4.00 to 4.46 (1:1) and from 3.70 to 4.33 (2:1). The use of hydroxypropylmethylcellulose allowed to obtain physiological range. The addition of hydroxypropylmethylcellulose reduces the pH and maintains high adhesion.

Opis:
Treatment of esophageal and extraesophageal reflux syndromes is mainly focused on inhibiting the secretion of hydrochloric acid. In spite of the high efficacy of proton pump inhibitors, approx. 30–60% of GERD patients experience daily symptoms. Beside acid reflux, other factors such as abnormal esophageal peristalsis, visceral hypersensitivity, ineffective esophageal clearance mechanisms, and impaired mucosal barrier also play an important role in generating GERD symptoms. An additional therapeutic proposition is a procedure aimed at improving the defense mechanisms of esophageal mucosa rather than inhibiting the damage-inducing factors. The preparation consisting of hyaluronic acid (HA), chondroitin sulfate (SC) and poloxamer 407 protects against harmful factors (hydrochloric acid, pepsin) and accelerates mucosal healing and regeneration, constituting a substantial element of monotherapy or add-on therapy in patients with gastroesophageal reflux disease.

Gels containing lactic acid complexed with chitosan at a stoichiometric ration 1:1 and 2:1 and 5-25% content of polyoxyethylene glycol 200 are able to move from 25 to 30 cm. Measurements performed in a biopharmaceutical model revealed that the addition of 20% poloxamer 407 to gels containing lactic acid complexed with chitosan in a stoichiometric ration 1:1 decreases their movability from 20 to 25 cm and at 2:1 ratio, from 22 to 28 cm. Higher concentrations of the poloxamer 407: 23 and 25% result in the movability of 20 to 24 cm. The addition of 5% PVP K-30 besides 1,2-propylene glycol increases the movability gels containing lactic acid complexed with chitosan in a stoichiometric ration 1:1 from 24 cm to 28 cm and at 2:1 ratio from 26 cm to 30 cm. Higher concentrations of the poloxamer 407: 23 and 25% result in the movability of 24 to 27 cm

Opis:
Leczenie zespołów przełykowych i pozaprzełykowych choroby refluksowej przełyku (GERD) jest ukierunkowane przede wszystkim na hamowanie wydzielania kwasu solnego. Pomimo wysokiej skuteczności działania inhibitorów pompy protonowej (PPI) u około 30–60% pacjentów z GERD występują codzienne objawy choroby. Ważną rolę w generowaniu objawów GERD, poza refluksem kwaśnym, mają także inne czynniki, takie jak: nieprawidłowa perystaltyka przełyku, nadwrażliwość trzewna, zaburzone mechanizmy oczyszczania przełyku i zaburzona bariera śluzówkowa. Dodatkową propozycją terapeutyczną jest postępowanie, mające na celu poprawę mechanizmów obronnych błony śluzowej przełyku, a nie wpływ na hamowanie czynników uszkadzających. Preparat składający się z kwasu hialuronowego (HA), siarczanu chondroityny (CS) i poloxameru 407 chroni przed czynnikami uszkadzającymi (kwas solny, pepsyna) oraz przyspiesza proces gojenia i regeneracji błony śluzowej, stanowiąc ważny element monoterapii lub terapii dodanej u pacjentów z GERD.

The addition of 0.5% polyvinyl alcohol increases the dynamic viscosity from 590.20 to 720.63 for 1:1 and from 560.90 to 698.82 for 2:1 ratios. The addition of polyvinyl alcohol and auxiliary substances reduces the pH and maintains high adhesion.

Gels with addition 20-25% poloxamer 407 containing lactic acid complexed with chitosan and 5-25% content of glycerol reveal pH from 4.25 to 4.98 for 1:1 and from 3.60 to 4.45 for 2:1. The investigations showed that the addition of a thermosensitive polymer increases the adhesive properties of the investigated gels, but at the same time it increases their pH. The addition of glycerol or 1,2-propylene glycol reduces the pH and maintains high adhesion. The use of hydrofilizing substances allowed to obtain physiological range pH gynaecological gels.
