Integrative photodynamic therapy (IPDT) of tumors consists of combined use of different curable factors that extremely increase the effectiveness of IPDT. The leading curable effect of PDT is determined by developing of aggressive photochemical reactions in the tumor. The main destructive factor of these reactions is the generation of free radicals that kill tumor’s cells. For PDT efficacy increasing it is necessary to solve the following problems and assure: easiness of production or synthesis; accumulation high selectivity regarding tumor tissue; low toxicity in light and darkness; singled oxygen generation high quant output; promptly elimination from the organism after the treatment procedure; good absorption in spectral intervals most transparent for tissues (red and infrared intervals); optimal interconversion between quant output and fluorescence quant output. PDT effectiveness may be significantly increased by means of: increasing of photosentisizer selectivity and accumulation in tumor tissue; increasing of tumor tissue photochemical destruction caused by photosensitizer. Realization of integrative PDT using Cyber Laser may increase tumor therapy effectiveness to 94–96% and at the same time will establish a new direction in PDT.

Nevoid basal cell carcinoma syndrome (NBCCS) is a rare genetic disease that is manifested in a number of disorders concerning the skin, skeleton, cardiovascular and nervous systems. Various defects which can be observed at first contact with a patient visiting a dermatologist or dentist may help to diagnose this syndrome. Frequent problems with odontogenic cysts and metastatic basal-cell carcinomas result in patients being under the constant care of a specialist. This short study presents the case of 31-year-old patient with Gorlin-Goltz syndrome treated with combination therapy using CO2 laser and photodynamic therapy.

Termografię w podczerwieni zastosowano jako nieinwazyjną technikę do diagnostyki zmian nowotworowych skóry oraz zbadania efektów termicznych zachodzących podczas leczenia za pomocą terapii fotodynamicznej powierzchniowo zlokalizowanych nowotworów. Przeprowadzone wstępne badania termowizyjne pokazały zróżnicowanie termiczne powierzchni skóry występujące między zmianami nowotworowymi złośliwymi (rak podstawnokomórkowy), a zmianami łagodnymi (brodawki łojotokowe). Ponadto zaobserwowano pewne zmiany gradientu temperaturowego powierzchni skóry u pacjentów cierpiących na raka podstawnokomórkowego podczas terapii metodą fotodynamiczną. Wydaje się iż termowizja może być przydatna zarówno w diagnostyce, jak i monitorowaniu efektów leczenia raka podstawnokomórkowego za pomocą terapii fotodynamicznej.
The infrared thermography as a non-invasive diagnostic technique was used to differentiation between basal cell carcinoma (BCC) and benign skin tumors diagnosed as seborrhoeic keratosis. Additionally, for some patients thermal imaging was used for monitoring the photodynamic therapy (PDT) effects. All studies were performed by thermovision camera Flir Systems A40 (temperature sensitivity was 0,7K). The group of subjects was treated in the Chair and Clinic of Internal Diseases, Angiology and Physical Medicine in Bytom. It consisted of 10 patients (7 males and 3 females). 7 patients suffered from basal cell carcinoma treated by PDT and 3 patients were diagnosed as benign skin tumors. All skin tumors were confirmed by histopathology examination.The results of the studies showed differences in skin thermal map between patients with skin cancerous tumors - basal cell carcinoma and benign skin ones diagnosed as seborrhoeic keratosis. It seems that seborrhoeic keratosis are characterised with lower temperature than healthy skin around, in opposite to basal cell carcinoma where higher temperature was observed. In the case of patients suffered from basal cell carcinoma the temperature skin lesions were especially seen after application of photosensibilisator, which caused changes of the skin temperature gradient during photodynamic therapy. The skin temperature changes were observed probably due to some processes switched on, which might influence metabolism in tissues. However, the obtained effects required further studies. It seems that thermovision may be useful as a diagnostic technique and may give some additional information about photodynamic therapy effects.

Photosensitizing dyes are used in fluorescence diagnostics and photodynamic therapy (PDT). These usually hematoporphyrin derivatives (HpD) accumulate preferentially within neoplastic tissues. HpD is a mixture of ether and ester linked porphyrins. Its partially purified form is known under the commercial name of photofrin II (PII). PII emission spectra were studied in a hydrophilic (PBS) and a lipophilic (PC liposomes) environment. Red shift was observed in their emission maxima from 615 nm in buffer solution to 635 nm in lipid. Identical red shift was obtained when the intracellular fluorescence of two cancer cell lines, MCF 7 and Jurkat, incubated with PII was investigated. Thus, intramembrane localization of PII may be suggested. As determined from the total fluorescence intensity, the uptake of PII was only slightly higher for Jurkat than for MCF 7 cells. Nevertheless the kinetics of the uptake was found to be different for both cell lines.

Human prostate cancer cells were evaluated for growth after photodynamic therapy, radiotherapy, and combined treatment. Indocyanine green was tested as a photosensitizer and radiosensitizer. Two human cell lines were used: PC-3 derived from prostate carcinoma, and EPN derived from normal prostate tissue. The light source used for the photoactivation experiments was a diode laser peaked at 805 nm. The light dose incident on cells was 108 J/cm2. Ionizing radiation was produced by a linear accelerator, and the dose was 2, 4 and 6 Gy. Cytotoxicity was evaluated by measuring the colony forming ability of cells. Our results show that indocyanine green induces cell death by photoactivation, but it does not act as a radiosensitizer if used with ionizing radiation. The combined treatment of photodynamic therapy and radiotherapy produces an additive effect which does not depend on the sequence of the two treatments. Combined treatments could be more useful since they allow the reduction of the ionizing radiation dose to obtain the same effect as one obtainable by radiotherapy alone.

We report the synthesis, photochemical and photophysical properties and preliminary studies on biological effect of a new tritolylporphyrin dimer (T-D). Absorption and emission properties of T-D suggest its possible use in photodynamic therapy. T-D is capable of singlet oxygen production with 0.8 quantum yield. It also has a high photostability. The photodynamic properties of the dimer were examined following the growth of SKMEL 188 (human melanoma) cells irradiated with red light (cut off <630 nm). The surviving fraction of the cells decreased about 3-fold (vs. non-irradiated cells) for an 81 J/cm2 dose. Our results suggest that tritolylporphyrine dimer T-D may be an interesting hydrophobic sensitizer for photodynamic therapy.

Opis:
Spectral properties (anisotropy coefficients calculated for absorption, emission and fluorescence decay time) of two stilbazolium merocyanine dyes have been determined to evaluate the applicability of these dyes as sensitizers in photodynamic therapy. The dyes were embedded in an anisotropic polymer matrix. Analysis of the emission decay components measured in polarized light provides information on the interactions of the dye molecules with the polymer matrix being a model of an anisotropic biological system. Different values of the emission anisotropies obtained from various polarized components of fluorescence decays have shown that the orientations of the dye molecules influence their interactions with the polymer. This means that differently oriented dye molecules located in biological systems should exhibit different interactions with membranes. The chain length and type of side groups attached as well as the salt form of the dye molecule were shown to influence the dye-polymer interactions and should be taken into account before the application of merocyanine dyes in medicine. These dyes seem to be promising optical sensors with spectral properties, including the calculated anisotropy coefficients, sensitive to the molecular environment, useful to study orientation and interaction with neighbouring molecules in biological membranes.

Introduction. Our understanding of photodynamic therapy (PDT) is crucial for the applications of this treatment. In this physical phenomenon occurs light absorption by the applied photosensitizer, which results in its excitation to higher electron levels. After activation, a series of complex physicochemical processes take place in the tissues. Aim. The aim of this study was to describe the basic naturally occurring photosensitizers used in PDT techniques. This study clarified applications of photosensitizers. Material and methods. This study was used basic information about PDT reaction and the selective destruction of the tumor by photooxidation: a photosensitizer, a light source and oxygen. The papers reviewed here are based on Medline/Pub. Analysis of the literature. Photodynamic therapy is an innovative form of treatment. This method is not a commonly used therapeutic and therapeutic tool, but a supplement to many already tested and analyzed techniques. Thanks to continuous research, this method has a wider and wider range of applications in medicine. Conclusion. The data indicates increased number of papers regarding applications of PDT in medicine.

Terapia fotodynamiczna znajduje zastosowanie w wielu obszarach onkologii. Połączenie fotouczulacza, światła i tlenu prowadzi do powstania reaktywnych form tlenu, które niszczą patologiczne komórki. Celem niniejszej pracy jest przedstawienie stanu badań przedklinicznych z wykorzystaniem naturalnego fotouczulacza –hiperycyny uzyskiwanej z dziurawca zwyczajnego. Pozytywne rezultaty uzyskano w przypadku nowotworów skóry, jamy nosowo-gardłowej, pęcherza moczowego, nerki, popromiennych włókniakomięsakach oraz w hematologii i pediatrii. Terapia fotodynamiczna może stać się bezpieczną metodą leczenia nowotworów oraz uzupełnieniem standardowego leczenia płynu.
Photodynamic therapy (PDT) is used in various oncological fields. The combination of photosensitizer, light, and oxygen leads to the production of reactive oxygen species (ROS), which destroy pathological cells. Presented review informs on preclinical studies with the use of the natural photosensitizer – hypericin, derived from Hypericum perforatum. Positive results were achieved in case of tumors of skin, nasopharyngeal, bladder, kidney neoplasms, radiation-induced fibrosarcoma, in haematology and paediatrics. Photodynamic therapy can become a safe treatment method for neoplasms, as well as an additional method in conventional protocols.

The interactions of two metal-free phthalocyanines [(H2Pc) and Solar Pc (with four peripherical groups: SO2N(CH2CH2OH)2)] and of one metal substituted dye (CoPc) with resting and stimulated human peripheral blood mononuclear cells (PBMC) were compared. The absorption, fluorescence, photoacoustic and EPR spectra of both resting cells and cells stimulated by phytohaemagglutinin, incubated in dimethyl sulfoxide (DMSO) with very low or 95% water content and with or without dye addition, were measured. The fate of the light absorbed by the samples was investigated. It is known that singlet oxygen production is crucial for photodynamic action of dyes. Thermal deactivation and luminescence emission compete with this process, so investigation of these alternative paths of sensitizer deactivation provides information about photodynamic action. The incorporation of the investigated dyes into cells and the perturbation of the cell structure caused by the dyes, the incubation solvent and the activator were investigated by comparing the spectral properties of PBMC before and after stimulation and incubation. Incubation of the cells for 1 h in a solution of Solar Pc in 99.5% aqueous DMSO, resulted in an efficient dye incorporation which was highly selective. Solar Pc being introduced much more efficiently into stimulated cells than into resting cells.

Photodynamic therapy (PDT) is based on photosensitizers activated by light of appropriate wavelength. Their activation leads to generation of singlet oxygen and free radicals responsible for the cytotoxic effect. The aim of this project was to compare the bactericidal effect of PDT using different porphyrin photosensitizers against a methicillin-resistant Staphylococcus aureus strain. Exogenous sensitizers (protoporphyrin IX and newly synthesized derivative, protoporphyrin diarginate) induced a 3 log10-unit reduction in bacterial viable counts. With the use of endogenous, ALA-induced porphyrins, a 1.6 log10-unit reduction was obtained. The sensitizers tested executed their antibacterial activity with no essential change in the antibiotic resistance pattern of the studied strain.

Lung adenocarcinoma is a leading human malignancy with fatal prognosis. Ninety percent of the deaths, however, are caused by metastases. The model of subcutaneous tumor xenograft in nude mice was adopted to study the growth of control and photodynamically treated tumors derived from the human A549 lung adenocarcinoma cell line. As a side-result of the primary studies, observations on the metastasis of these tumors to the murine lungs were collected, and reported in the present paper. The metastasizing primary tumors were drained by a prominent number of lymphatic vessels. The metastatic tissue revealed the morphology of well-differentiated or trans-differentiated adenocarcinoma. Further histological and histochemical analyses demonstrated the presence of golden-brown granules in the metastatic tissue, similar to these found in the tumor tissue. In contrast to the primary tumors, the electron paramagnetic resonance spectroscopy revealed no nitric oxide - hemoglobin complexes (a source of intense paramagnetic signals), in the metastases. No metastases were found in other murine organs; however, white infarctions were identified in a single liver. Taken together, the A549-derived tumors growing subcutaneously in nude mice can metastasize and grow on site in the pulmonary tissue. Thus, they can represent an alternative for the model of induced metastatic nodule formation, following intravenous administration of the cancerous cells.

The aim of the study is to analyze photothermal and photochemical phenomena that occur during photodynamic therapy (PDT). In this type of therapy, under the influence of the laser, reactions take place related to the transformation of triplet oxygen form into its singlet form which is cytotoxic to the tissue. The increases in temperature resulting from the laser-tissue interaction during PDT are not big; however, they can lead to changes in tissue perfusion, which can affect oxygen delivery to the tissue. The proposed model uses optical diffusion equation, Pennes bioheat transfer equation, and reactions equations for PDT. The main findings of the analysis show the impact of temperature on the value of the perfusion coefficient and triplet oxygen distributions at the end of the treatment procedure.

The impact of thelaser on the tissue may also be accompanied by changes in the thermophysical parameters,e.g., perfusion, which can affect the supply of oxygen to the tissue and, consequently,the outcome of the therapy. The proposed model uses the optical diffusion equation,the Pennes bioheat transfer equation, and reactions equations for PDT. The connectionbetween bioheat transfer and PDT models is taken into account through the respectiverelationships between perfusion rate, capillary blood velocity, and the maximum oxygensupply rate. Furthermore, a method is proposed to model abnormal vascular patterns inthe tumor subdomain. The boundary element method and the finite difference methodwere used in the numerical implementation stage.

A rapidly growing pharmaceutical industry requires faster and more efficient ways to find and test new drugs. One of the new method for cell culture and examining the toxic effects of drugs is application of microfluidic systems. They provide new types of microenvironments and new methods for investigation of anticancer therapy. The use of microsystems is a solution that gives the opportunity to reduce not only cost and time, but also a number of tests on animals. In this paper we present designed and fabricated hybrid microfluidic systems which are applicable for cell culture, cell based cytotoxicity assays and photodynamic therapy procedures. Polydimethylsiloxane (PDMS) and sodium glass were used for fabrication of microdevices. The designed geometry of the microdevices includes cell culture microchambers and a concentration gradient generator (CGG). The CGG enables to obtain diff erent concentrations of tested drugs in a single step, which is a significant simplification of cytotoxicity assay procedure. In the designed microsystems three various cell lines (normal and carcinoma) were cultured and analyzed.
