Opis:
Introduction and aim. Computed tomography (CT) is the first imaging modality in the evaluation of children in case of patients with head injury in pediatric Emergency Departments (EDs). Radiological CT reports include not only lesions that are the main cause of the child’s complaints but also incidental findings. The objective of this study is to assess incidental findings observed in children who were admitted to the ED and had the head CT performed. Material and methods. This retrospective, cross-sectional study enrolled 644 children under the age of 18, from 1st January 2021 to 31st June 2021. Each child could have had one or more incidental findings in CT. Results. Among all CT studies, incidental findings were found in 279 out of 644 (43.32%) cases, of which 73 (11.34%) had both lesions detected incidentally and related to the trauma. Conclusion. Head CT is an incredibly useful tool in the assessment of some head emergencies. However, evaluation of the prevalence of incidental findings is difficult. Most of them require no specific further investigation. Pediatricians, who order CTs in children, must be prepared to interpret and communicate findings to families and introduce treatment in necessary situations.

Numerous studies of mitochondrial DNA (mtDNA) in cancer have shown differences between mtDNA sequences in tumor and normal tissue and at various stages of cancer treatment in the same patient. However, there is little data on acute lymphoblastic leukemia (ALL), the most common type of leukemia in children. In this study we compared mitochondrial sequence variation in the D-loop region and in 5 genes of mtDNA in bone marrow samples of 6 pediatric patients with ALL at various stages of therapy. We found several common polymorphisms and one variant at position 3688 whose level varied during leukemia treatment. Our results suggest that mitochondrial DNA mutations, whose levels change during patient treatment, could be potential biomarkers for monitoring treatment efficacy and disease progression.

Opis:
Introduction: Acute appendicitis is the most common surgical emergency in children. Appendicular mass is a relatively common complication in improperly treated patients. The management of appendicular mass remains controversial. Aim: This study aims at determining factors affecting the effectiveness of conservative management of appendicular mass. Material and methods: This was a retrospective study of 71 children younger than 15 years with appendicular mass managed at Basra Children’s Specialty Hospital during the period between 2015 and 2019. Factors like age of the patient, duration of symptoms prior to hospital admission, size of the mass, complications, hospital stay and outcome are reviewed. Results: Appendicular mass complicates 3.9% of all cases of acute appendicitis. Conservative management of appendicular mass was effective in 84.5%. Appendicular mass occurred most frequently in children aged 5–10 years (48%). Male is more frequently affected than female with a ratio of 1.4. Regarding age effect on the efficacy of conservative management of appendicular mass, there is a significant association with P-value = 0.017. Duration of symptoms or size of the mass has no significant association with the success of conservative management. No mortality or major surgical complications are encountered. Although effective, conservative management prolongs the length of hospital stay. Conclusion: Conservative management of appendicular mass in children should be revised particularly in children younger than 5 years old, wherein operative treatment may be the first option. The reasons for that are high rate of failure of conservative management with early progress to appendicular abscess. These could be explained by the underdevelopment of the greater omentum lacking its protective effect in limiting the spread of inflammation. In addition, early appendectomy has the following advantages: decreased risk of adhesive intestinal obstruction, shortening of hospital stay (i.e, less economic burden), and avoidance of second readmission for interval appendectomy

Opis:
Introduction: So far, very little data on pediatric obesity has been published in Poland, although the problem seems to be growing. Objective: To investigate present prevalence of overweight and obesity among schoolchildren using CDC (Center of Disease Control and Prevention) criteria in Krakow and to compare how it has changed between the years 2001 and 2013. Our aim was to establish the magnitude of this rising problem within the last decade.Materials and methods: The study was conducted in two phases. In 2001, height and weight of the group of schoolchildren were obtained. Twelve years later, children in the same age, attending the same schools, were measured and weighed. Using the collected data, BMI (kg/m2) was calculated in order to acquire BMI percentile. BMI cutoffs referential for Polish population were used. Results: Using the CDC criteria to diagnose children as obese or overweight, the authors determined that the prevalence of overweight, including obesity was 10.8% in 2001 and changed to 15.1% in 2013 (p=0.0054). The percentage of obese children increased from 2.0% to 4.8% (p=0.0012). The occurrence of overweight and obesity has increased among girls (p=0.0025; p=0.0112 respectively) while among the group of boys, it did not change. The factors associated with excess weight were: male sex (OR=1.48; 95%CI=1.13-1.95) and second phase of the study (OR= 1.48; 95%CI=1.13-1.95).Conclusion: Our study indicated that the prevalence of obesity and overweight among adolescents in Krakow rose within the last decade. The rising problem affected females more than males. Despite that, excess weight is still more frequent among males than females.

Opis:
The rising prevalence of overweight and obesity in children and teenagers is a major challenge for public health. Obesity is a complex and heterogeneous disorder, affected by many interacting genetic and non-genetic factors. The aim of this article was to focus on the environmental risk factors for childhood obesity. Among different factors contributing to an increase in BMI, we highlighted the role of exposure to cigarette smoke, DDT, bisphenol A, pesticides, and noise. The correlation between exposure to environmental toxins during prenatal period and obesity development in later life was underlined. According to obesogenic environment hypothesis, some features of distal and proximal neighbourhood also have a pivotal impact on children's behaviour and may contribute to increasing the risk for overweight. The area of residence (urban or rural) may affect access to sports facilities or other opportunities for physical activity. Therefore, for designing adequate prophylaxis, it is essential to take into account modifiable risk factors present in residential neighbourhood. Prevention of childhood obesity should integrate activities for both micro- and macro-environment surrounding the child.
Wzrastająca częstość występowania nadwagi lub otyłości u dzieci i nastolatków stanowi poważne wyzwanie dla zdrowia publicznego. Otyłość to złożone i heterogenne zaburzenie wynikające z wielu interakcji pomiędzy czynnikami genetycznymi i pozagenetycznymi.

Introduction and aim. The use of pacifier is a very common practice in the early childhood period in the world. In recent years, the harms of pacifiers have been discussed rather than their benefits. We aimed to determine the knowledge and behaviors of mothers with children aged 0-3 about the use of pacifiers. Material and methods. A cross-sectional study was conducted with 363 mothers between January 10th and November 31st, 2020 in Zonguldak, Turkey. The data were collected with a web-based questionnaire. This article was prepared following STROBE guidelines. Results. The mothers started using the pacifier for the first time when their children were at an average of 2.86±3.31 months old, and they used it for at an average of 12.06±9.13 months. Of the mothers, 36.4% were found to clean the pacifier every month and 30.6% to have the behavior of dipping the pacifier into a product such as sugar, honey, molasses, and jam. The mothers with undergraduate degrees had the behavior of cleaning pacifiers more than those with postgraduate degrees (p<0.001). Conclusion. Mothers preferred to give pacifiers to babies at a high rate and had misinformation about the use of pacifiers that may harm their children’s health.

Aim: The aim of the study is to assess the satisfaction of parents with nursing care at a pediatric surgery department.Material and method: The research was conducted among parents / caregivers of children discharged from the ward in the period from February to April 2019. The study involved 50 respondents. The test method was a diagnostic poll method in the form of a questionnaire. The research tool was a questionnaire of own authorship. Results: Factors determining satisfaction with child care on a surgical ward are: the age of the child, the mode of the child’s admission to the ward, accessibility, and the nurse’s support. Thirty-one parents were in favour of the multi-purpose nature of nursing care on the ward. The others felt that nursing care was task-oriented. Parents of the youngest children (mean age 4.9 years) expected the nurse to be kind or professional (mean age 5.5 years). Parents of early childhood children (mean age 7.2 years) were more likely to expect understanding, and parents of the oldest children (mean age 9.0 years) expected communicativeness. It was shown that throughout the entire period of hospitalization, caregivers of children under 3 years of age (n = 13; 100.0%) or aged 3-6 years (n = 15; 78.9%) stayed with their children more often. There was a correlation between the mode of admission and the emotions that parents felt during the admission of their child to hospital, as shown in Tab. 5. The level of significance (p < 0.001) in the hypothesis tested, was lower than the typical level of significance of 0.05. In the case of emergency admission (n = 13; 54%) and planned admission for surgery (n = 17; 74%), emotions related to helplessness and anxiety predominated. Conclusion: Parents rated the quality of nursing care on the ward well. Parents’ expectations of the nurse are not dependent on the age of the child. The mode of admission of the child to the ward did not affect the expectations towards the nurse. The age of the child affected the time the caregiver stayed with the child on the ward. The mode of admission of the child affected the emotional state of the parents of the caregivers.

Introduction and aim. Pediatric nurses play an important role in ensuring quality healthcare through job satisfaction and professional behaviors. This study aims to examine the relationship between job satisfaction and professional behaviors in pediatric nurses. Material and methods. This was a descriptive and cross-sectional study. The sample of the study consisted of 244 pediatric nurses who worked in tertiary university hospitals in Turkey and agreed to participate in the study. Data were obtained through face-to-face interviews using a personal information form, the job satisfaction scale for nurses (JSSN), and the behavioral inventory form for professionalism in nursing (BIPN). Data were analyzed using the SPSS 22.0 software and evaluated using descriptive statistics, independent t-tests, one-way ANOVA, Pearson correlation, and simple linear regression analysis. Results. The nurses’ JSSN and BIPN means scores were 3.42±0.59 and 5.69±4.05, respectively. A statistically significant moderate positive relationship was found between the pediatric nurses’ professional behaviors and job satisfaction (R=0.342; p<0.001). The regression analysis revealed that professional behaviors significantly and positively influenced job satisfaction in pediatric nurses (β=0.050; p<0.001). Conclusion. This study found that pediatric nurses had a moderate level of job satisfaction and a low level of professional behaviors, and as their professional behavior increased, their job satisfaction also increased.

Introduction and aim. The period immediately after leaving the hospital is known as the transition home, which presents a critical time for parents to take on all the responsibilities and care of a child without the support of the hospital environment. The purpose of this study is to culturally adapt the Pediatric Transition Experience Measure (P-TEM) instrument into Turkish and test its validity and reliability. Material and methods. We conducted this methodological study with parents of children who were discharged from the hospital between May 2021 and May 2022. We evaluated linguistic, content, construct, convergent validity, and internal consistency. Results. The P-TEM exhibited a two-factor structure and accounted for 65% of the overall variability. The internal consistency reliability for the transition preparation and transition support subdimensions was 0.779 and 0.793, respectively, while the total measure had a reliability of 0.831. The item-total correlations for the P-TEM ranged from 0.493 to 0.671. Parents who received the highest P-TEM scores experienced a 5.1-point improvement (95% confidence interval: 1.7 to 8.6) in their satisfaction with healthcare services, which was greater than that of parents who reported lower P-TEM scores. Conclusion. It is worth noting that the P-TEM has been validated and has high reliability in Türkiye.

Objective: The aims of this systematic review of the literature were to investigate the uses of cone beam computed tomography (CBCT) in pediatric dentistry and, if possible, identify the indications. Material and methods: A literature search was conducted using the PubMed and Scopus electronic databases and the keywords "CBCT and pediatric dentistry". This search provided us with 1518 references. The selected publications were all clinical articles written in French or English and referring to a pediatric population. After screening, 461 eligible full text articles remained. Results: In total, there were 169 references that met the inclusion criteria. Different topics, mainly relating to orthodontics, anatomy, and cleft lips and palate, were discussed. There was large variability in the information concerning the technical parameters. The radiographic protocols that we analyzed showed a large heterogeneity. Conclusions: The level of evidence provided by our work is limited because only two randomized double-blind controlled studies are included. Two indications can be distinguished: for orthodontics and for the rehabilitation of cleft lips and palate. There are a multitude of radiographic protocols. More research is needed to identify other potential clinical indications as well as to determine a standard CBCT protocol for children and adolescents.

Electroretinography is a useful tool used in diagnosing retinal disorders. Refractive errors are a common and increasing group of abnormalities, which, if undiagnosed, may lead to complications. Physiologically, i.e., refraction of the child’s eye, evolves from myopic or hyperopic after birth, towards emmetropic. However, high refractive errors tend to present with retinal diseases. Early electroretinography is a great diagnostic test that allows its detection. Because of that, it can help avoid loss of eyesight and prevent future implications. Unfortunately, low accessibility and interpretational difficulties are main barriers in wider implementation of that method.

Long QT syndrome (LQTS) is a hereditary disease with significant mortality, which might be reduced with appropriate management. This cardiac disorder is regarded as rare, but its prevalence remains unknown. The clinical course of LQTS is variable and syncope is a common first manifestation of LQTS. Therefore in each patient after syncope an ECG should be carried out. However, there is no universal QT value applying to all patients (especially infants and children), because it varies depending on age and sex. Genetic testing can be of great importance for the management of families with LQTS and early identification of patient relatives at risk of developing disease. We aimed to show that a very important part of treatment is not only pharmacotherapy, especially beta-blockers, but change of lifestyle plays a significant role.

Introduction: Our times are characterized by the development of information technologies and modern advertising methods. Representatives of each field of the service sector face social pressure, engaging in the process of creation of the image and building the brand. Thanks to professional activity in this area, they can gain an advantage over their competition. This also applies to doctors and managers of health care entities. A good image affects the patients' trust in relation to the subjects of the therapeutic activity. So what seems to be an extremely interesting issue is the identification of the decisive factor influencing the positive perception of pediatricians and outpatient pediatric care by parents in Silesian voivodeship. Material and methods: The diagnostic survey method, and the survey technique were applied in the course of research conducted in eleven educational institutions in Silesian Voivodeship in the period between May 2016 and January 2017. The sample group consisted of 123 people – parents of children up to eighteen years old. Results: A vast majority of the respondents consider politeness of the staff and the ease of the registration procedure to be the crucial factors affecting their positive assessment of their visit to the pediatric clinic. The doctors’ good reputation results mainly from factors such as their attitude to patients and provision of clear and extensive feedback. When asked about the factors influencing their decision to choose a particular doctor, the most respondents mentioned a personal recommendation by a family member or friend. Najważniejsze czynniki wpływające na pozytywną ocenę lekarza pediatry oraz poradni… 198 Conclusions: Despite their common access to the Internet and the development of technology, a recommendation by a family member or friend constitutes the most popular choice quoted by the respondents. Just like in the case of other areas of the service sector, the most important factors in a positive assessment of the facility are the courtesy of the staff and the ease of completing the formalities

Currently, surgery methods of pediatric cataracts allow obtaining good visual acuity in operated children. The operation should be performed in infants over 2 months of age because of more frequent occurrence of glaucoma in earlier operated patients. In children under 10 years of age posterior capsulotomy and anterior vitrectomy are required during surgery to prevent opacification of the posterior lens capsule after surgery. Due to the frequent occurrence of opacities in the visual axis, it is recommended no to implant intraocular lenses in infants under 7th month of life (2nd year of life according to some recommendations).
