Purpose: To establish the relation between the reasons one registers to a primary care physician and a patient's expectations of and satisfaction from the medical services provided. Material and methods: A total of 422 patients from public and non-public health care centres participated in the research. The research was carried out in 20 institutions of public and non-public centres for primary care in the Swiętokrzyskie Voivodeship. A Patient Request Form was used as the research tool. The Polish version of the Patient Request Form (PRF) is composed of 18 statements concerning different reasons for the present contacting of a general practitioner. Comparative analysis of interval or ratio scale type variables was performed using variance analysis, which in the case of significance of the main effects or interactions was supplemented by post-hoc analysisResults: After a repeated measures analysis of variance was done, a significant variation was found in terms of the type of institution: a public and non-public health care centre (p=0.01955). The effect of the type of expectation also proved significant (p=0.00000). A significant interaction was also found between the type of institution and the kind of PRF (Patient Request Form) (p=0.00985). A significant effect (p=0.00805) was obtained using single-classification analysis of variance upon analysing the assessment of the treatment conditions at a primary care clinic (JUM2) in terms of the type of institution.Conclusions. Patients participating in the study indicated expectations related with an explanation of the illness and obtaining information about test results and further treatment as the reasons for the present registration to a primary care physician. As regards assessment of the treatment conditions (JUM2) at primary care clinics in view of the type of institution and area of services, the patients participating in the study assessed non-public health care centres more highly.

The paper is a part of a general mycological monitoring study of nosocomial infections. Differences in the species composition and drug resistance of fungi isolated from in-patients and out-patients prompted an analysis of this topic. The studies were conducted with fungi from the collection of the Department of Mycology, University of Warmia and Mazury in Olsztyn, that were isolated from the oral cavity, sputum, bronchoscopic fluid, anus and skin from 100 in-patients and 100 out-patients. Laboratory analyses, including species categorization of fungi, were performed according to routine mycological diagnostics. Drug sensitivity to fluconazole and nystatin was tested with the disc diffusion method. In the group of in-patients, a wider taxonomic diversity of fungi (12 species) was found in comparison with the out-patients (7 species) and 31 cases of multifocal infections were recorded, while in the second group the number of the latter was only six. In all patients, C. albicans were predominant, constituting the largest proportion in focal infections in all patients and in multifocal infections in the in-patients. In the latter, over a half of the examined individuals were resistant to fluconazole (C. glabrata and C. krusei – 80%, C. tropicalis and S. capsularis – 60%, C. guilliermondii – 50%) and nystatin (T. beigelii – 80%, C. krusei and C. tropicalis – 50%). Substantially lower drug resistance of fungi was recorded in the out-patients. The hospital environment is an abundant reservoir of different fungal species with significantly greater expansiveness and aggressiveness compared to the environment outside a hospital.

Objectives: The paper describes the case of a physical therapist with acute Low Back Pain (LBP) due to patient handling and the efficacy of Kinesio Taping (KT) around the trunk in the treatment of this occupational LBP. Materials and Methods: KT was applied around the trunk for 3 days, for an average of 10 h/day. Kinesio tape was applied with 130–140% stretch to the rectus abdominis, internal oblique, erector spinae, and latissimus dorsi muscles, which are activated in the process of lifting. Results: Following the KT application, the ‘Visual Analog Scale’ and ‘Oswestry Disability Questionnaire scores’ gradually decreased and active trunk range of motion limited by the LBP increased progressively. The physical therapist no longer complained of LBP and was able to handle patients without any pain. Conclusions: Hence, continuous application of KT around the trunk may be a supplementary treatment method for acute LBP in physical therapists and enable continuous patient handling without any loss of work time due to occupational LBP. In addition, KT may also be applicable for the prevention and treatment of occupational LBP in other professions involving lifting heavy objects.

Introduction and objective. The quality of a doctor-patient relationship plays a vital role in all fields of medicine. In the case of psychiatry, this role is special as it provides the foundation for the whole therapeutic process. The aim of this study was to investigate the patient’s perspective on psychiatric visits: patient’s attitudes towards the psychiatrist, patient’s view of the patient-psychiatrist relationship, and the patient’s needs and expectations from this relationship. Materials and method. 615 psychiatric outpatients responded to the anonymous questionnaires connected with their attitudes towards the psychiatrist, evaluation of the doctor, and expectations from psychiatric care. The study was conducted in 10 out of 30 public centres for psychiatric care in north-eastern Poland. Results. Generally, the patients liked and positively evaluated their psychiatrists. Patient’s liking for the doctor was connected with the feeling that the doctor also liked the patient, as well as with perceiving the doctor as competent and willing to meet the patient. The longer the treatment with a particular psychiatrist and the rarer need to consult the doctor, the more positive attitude and evaluation of the doctor patients had. According to the patients, the most significant expectations were associated with both conversation with the doctor and receiving emotional support. Conclusions. The key phase for forming the patient-psychiatrist relationship was the first stage of cooperation in which patients created their attitudes towards the doctor without modifying them at further stages. Thus, further studies on learning and developing the ability to establish the relationship with the patient, inspiring the patient’s trust and making psychiatric appointments comfortable from the first meeting, will be highly valuable.

Nursing care is limited not only to fulfil biological needs of a patient, but also to fulfil psychological, social and spiritual needs of the dying patient. It is necessary to get, at least for a while, to a certain resonance with the patient, which can carry their mutual commu- nication. Psychological approach to the patient has to include respect and natural reverence, regardless of his (her) age, status or profession. At each stage of dying patient behaves differently and a nurse has to accept his (her) behavior. The nurse can provide a psychosocial support by a permanent contact of a significant person with the patient.

The aim of the article is to introduce the issue of informed consent in the physician-patient relation- ship. I would like to point out the relevance of physician’s communication skills which are crucial in providing patients with information, especially in the first phase of the informed consent pro- cess when a health care professional performs patient instruction. This phase is of utmost impor- tance considering patient’s decision. Health care professional informs the patient about diagnostic and therapeutic approach, answers patient’s questions, explores the best possible treatment options in collaboration with the patient, encourages and gives reassurance in order to facilitate patient’s decision-making and managing his/her condition. Moreover, the manner and style used by physician to communicate with the patient must be taken into account. It mirrors the formation of their mutu- al relationship and their communication can significantly influence the course of treatment and its results.

This theoretical and research paper focuses on the physician-patient relationship, and more specifically on the meanings ascribed to the notion of “patient”. Authors of relevant literature indicate that particular models of the physician-patient relationship depend on the understanding of the notion of “patient” as well as on the understanding of two basic terms in medicine: health and illness. This assumption was the starting point for the presented analyses. Research was conducted within an interpretative paradigm framework. The study group consisted of physicians with different levels of experience and lengths of professional employment, as well as of various specializations. An analysis of collected data allowed for the identification of three categories of the notion of “patient”: personal, subjective-objective and objective.
Prezentowana praca ma charakter teoretyczno-badawczy i dotyczy relacji lekarz – pacjent ze szczególnym uwzględnieniem znaczeń przypisywanych pojęciu „pacjent”. W literaturze wskazywany jest fakt, że określony model relacji lekarz – pacjent zależy od rozumienia osoby pacjenta, a także od rozumienia podstawowych dla medycyny pojęć: zdrowia i choroby. Powyższe założenie stało się powodem podjęcia prezentowanych analiz. Badania prowadzono w paradygmacie interpretatywnym. Grupę badawczą stanowili lekarze o różnych specjalizacjach, z różnym doświadczeniem i stażem pracy. Analiza zebranego materiału pozwoliła wyłonić trzy podstawowe zakresy rozumienia pojęcia „pacjent”: osobowy, podmiotowo-przedmiotowy i przedmiotowy.

The aim of this article is to show the influence of witches, demons, ghosts, and gods on human health in Ancient Mesopotamia. Mesopotamian medicine was based on magic and considered illness to be the work of a supernatural power. Ashu and ashipu – doctors of body and soul – worked together to diagnose and help the patient. Sometimes prayers and magical rituals were necessary for a patient to get well; often, only a herbal mixture was needed. Mesopotamian magical and medical texts describe many kinds of diseases: somatic, mental, and others. Among these are: fever, headaches, leprosy, epilepsy, blindness, impotency, paralysis. This article is an effort to analyse Mesopotamian prescriptions, incantations, and medical interventions and to answer questions about the health of ancient mankind.
