Introduction: General acceptance of the robotic platform in pancreatic surgery is poor. One of the main concerns regarding this technique is that the likelihood of complications is greater compared to other approaches. Material and Methods: We performed a retrospective analysis of our database on robotic pancreatic surgery. Results: A total of 22 patients (12 male) underwent robotic pancreatic surgery. 6 pancreatoduodenectomies (PD 27.3%), 12 distal pancreatectomies (DP 54.5%), 2 tumor enucleations (9.1%) and 2 pseudocyst-gastrostomy (9.1%) were performed. The overall operative time was 425 (390–620) min, the median blood loss was 150 ml (70–600). We observed 10/22 (45.4%) overall postoperative morbidity, with 4 grade III to V complications according to the Clavien-Dindo classifi cation system. The Clinically relevant pancreatic fi stula rate was 3/22 (13.6%): 2 in DP group, 1 in the PD group. The reoperation rate was 2/22, one in the PD group, the other in the PG group; while the readmission rate was 18.6%. There was no postoperative death during the 30 days post surgery. Conclusion: Robotic pancreatic surgery seems to be safe and feasible and it is associated with an acceptable risk of complications, low estimated blood loss and low conversion rate.
Wprowadzenie: Ogólny stopień akceptacji dla stosowania chirurgii robotowej w chirurgii trzustki jest niski. Jedną z podstawowych barier dla wprowadzania tej techniki jest obawa przed większym niż w przypadku innych technik operacyjnych ryzykiem wystąpienia powikłań pooperacyjnych. Materiał i metody: Przeprowadzono retrospektywną analizę danych szpitalnych dotyczących zabiegów operacyjnych trzustki z dostępu robotowego. Wyniki: Ogółem operowano 22 chorych (w tym 12 mężczyzn) z zastosowaniem systemu robotowego do operacji trzustki. Wykonano 6 pankreatoduodenektomii (27,3%), 12 pankreatektomii obwodowych (54,5%), 2 wyłuszczenia guza (9,1%) oraz 2 zespolenia pseudotorbieli trzustki ze światłem żołądka (9,1%). Czas operacji wyniósł średnio 425 min (390–620 min), a mediana utraty krwi – 150 ml (70–600 ml). Powikłania pooperacyjne stwierdzono u 10 z 22 chorych (45,4%) przy czym u 4 wystąpiły powikłania w stopniu III–V według skali Claviena-Dindo. Klinicznie istotna przetoka trzustkowa wystąpiła u 3 z 22 chorych (13,6%), w tym u 2 chorych po resekcji obwodowej trzustki i u 1 po pankreatoduodenektomii. Reoperacje były konieczne u 2 z 22 chorych: jedna po zabiegu pankreatoduodenektomii i jedna po zespoleniu pseudotorbieli ze światłem żołądka. Odsetek ponownych przyjęć do szpitala wyniósł 18,6%. Nie stwierdzono zgonów w okresie 30 dni po zabiegu operacyjnym. Wnioski: Robotowa chirurgia trzustki wydaje się być techniką bezpieczną i wykonalną przy akceptowalnym ryzyku powikłań pooperacyjnych, niskiej śródoperacyjnej utracie krwi oraz niskim ryzyku konwersji.

Insulinomas are rare tumors, accounting for 1-2% of all neoplasms of the pancreas. Usually their treatment is not associated with any problems; however there is a small subset of problematic clinical cases. The authors present their own clinical experience with surgical treatment of insulinomas of the pancreas. The aim of the study was to conduct a retrospective analysis of patients with insulinomas of the pancreas who underwent surgical treatment at Department of General and Transplant Surgery Medical University in Łódź. Material and methods. The study included all patients who underwent surgical treatment at the Department between 2007 and 2013 for the tumor of the pancreas. Further retrospective analysis included all patients with tumors of the insulinoma type. The data was obtained from medical records, surgical protocols and histopathology reports. Results. The analysis included 530 patients who underwent surgical treatment for the tumor of the pancreas. Insulinoma was found in 10 (1.88%) patients (8 females, 2 males). An average age of patients who underwent surgical treatment was 47.5±13.8 years. An average size of the tumor was 1.6±0.5 cm. Six patients underwent extirpation of the insulinoma, while the other patients underwent distal resection of the pancreas. All patients underwent “an open surgical procedure”. The average duration of the surgical procedure was 55±45 minutes. Duration of the hospitalization in the analyzed group of patients was 7±5 days. Incidence of postoperative pancreatic fistulas was 10%. All insulinomas were benign. Conclusions. Insulinomas were rare among patients who underwent surgical treatment at the Department. They were benign and their treatment was unproblematic. However, there is a small group of cases that can be associated with problematic clinical situations. Thus treatment of patients with insulinomas should be conducted at specialist centers. Correct diagnostic and therapeutic management, involving close cooperation between multiple medical specialists, results in complete curing of majority of patients.

The improvement of laparoscopic techniques that we witnessed over the last decade, also applied to pancreatic surgery. Both worldwide and in Poland, increasingly more patients with lesions located within the body and tail of the pancreas are treated using laparoscopic distal pancreatectomy. The aim of the study was to compare the outcomes of laparoscopic and open distal pancreatectomy. Material and method. Between January 2009 and March 2013, 107 patients underwent distal pancreatectomy at a single institution: 39 using laparoscopic technique and 68 using open technique. This was a retrospective study. Results. There were no significance differences with regard to duration of the surgical procedure, duration of hospitalization and complication rate between both groups. Statistically significant difference was found for the incidence of spleen preservation in patients undergoing laparoscopy, due to better visualization of the structures. In the group of patients with pancreatic cancer there was no significance difference in the incidence of malignancy found in the surgical margin. Our results are similar to that presented in the literature. Conclusions. Laparoscopic distal pancreatectomy is a safe method, with a higher rate of spleen preservation among the patients with the lesion located in the body and tail of the pancreas.

Introduction: Pancreatic cancer is a devastating disease, being the seventh cause of cancer-related deaths worldwide. Its aggressiveness is due to its specific biology and the late diagnosis of cancer. Therefore, the prognosis for patients suffering from this cancer is dismal, with 5-year overall survival rate of around 6–10%. Up to date, only a complete surgical resection of the cancerous entity warrants a significant improvement in patients’ survival. Nevertheless, the pancreatic cancer’s biology is still not fully elucidated, so that the accuracy of prognosis for certain patients is highly uncertain. Consequently, the importance of both clinical and basic research aiming to reveal the crucial molecular factors affecting long-term prognosis should be highlighted. There is a growing number of evidence that biomarkers of PC not only reflect the presence of tumor itself but also present a “hint” regarding its physiology. Thus the aim of this study was to assess the levels of commonly measured biomarkers and their influence on patients’ overall survival. Materials and methods: The retrospective analysis of data on 129 patients admitted to our Department due to the diagnosis of pancreatic cancer was carried out. On the day of admission all the patients had their levels of CA19-9, CA125, CEA and CA15-3 measured. The overall survival (OS) was defined as time elapsing from the day of admission to the day of death. The Kaplan- Meier curves were built for all potential factors, Cox regression model was applied to carry out a multivariate analysis. Results: We retrospectively analyzed 129 patients with a mean age of 62 years. As many as 95 of them had an unresectable lesion and 34 underwent curative operation. In total, the analyzed patient group was characterized by a median survival of 7 months and 12 days. Cumulative 1-year, 2-year and 4-year survival rates were 35%, 16% and 15%, respectively. In univariate analysis, factors such as age >= 60, inoperable lesion, CA19-9 >= 200, CA125 >= 20 and Neutrophile to Lymphocyte Ratio (NLR) >= 5 were associated with a lower median OS. In multivariate analysis, three factors, CA19-9 >= 200, CA125 >= 20 and age >= 60, were found to be statistically significant. Indeed, patients possessing all of them noted much poorer outcomes regarding OS factors: 89 days versus 235 days for the other patients (log rank test P = 0.02). Conclusions: Our study fortifies the evidence that preoperative levels of CA19-9 and CA125 have a direct influence on the longterm OS. Interestingly, in our patient group, the correlation of biomarkers with OS was higher than that of resectability. However, our study has some limitations regarding, for instance, the lack of data on chemotherapy, comorbidities etc. In the view of recent molecular studies on mucin involvement in PC development, it provides a strong clinical evidence to prove their importance.

Opis:
Pancreatic cancer is the second most common malignancy of the gastrointestinal tract. The only potentially curative treatment is complete surgical resection but it can be performed only in patients without metastatic disease and entire resection with negative surgically and microscopically margins (R0 resection). Complete resection of the tumour with vessels’ involvement is still controversial. However, many authors proved that venous as well as arterial resection and reconstruction did not influence on final outcome. Proper preoperative staging helps in relevant treatment (surgery, radiochemotherapy or combination of these methods). The most common diagnostic step is MDCT with CTA which improves high sensitivity and specificity. The purpose of the study was to assess vessels’ infiltration by pancreatic carcinoma on the basis of CTA and surgery. CTA showed 88,9% sensitivity in diagnosing vessel’s involvement, however in some cases surgery is the method of choice in assessing tumours as resectable or unresectable.
