Background: Exposure to alcohol and tobacco during pregnancy may lead to developmental disorders in the foetus and can also adversely impact pregnancy outcomes and the newborn’s health status. Objective: Assessment of alcohol consumption and tobacco use during pregnancy as well as their impact on pregnancy outcomes and birth parameters of the newborn in a selected group of mothers with children aged ≤ 2 years. Material and methods: The study was conducted in 104 mothers with children aged ≤ 2 years living in the Mazowiecki Voivodeship. A questionnaire was used for assessing selected lifestyle parameters tobacco and alcohol use, dietary supplements use, physical activity during pregnancy, socio-demographic characteristics as well as children’s birth parameters (birth weight, length, head circumference, chest circumference). Data were analyzed statistically using logistic regression analysis, Spearman’s rang correlation, the Mann-Whitney U, Kruskal-Wallis, Student-t, Tukey and Chi2 tests (p≤0.05; STATISTICA 13.1 PL). Results: 22% of the participants solely smoked cigarettes, 13% solely drank alcohol while 7% smoked cigarettes and drank alcohol. Both cigarette smoking and alcohol use increased the risk of preterm birth (OR 9.6, 95% CI 2.9 – 31.4, p=0.00015; OR 5.3 95% CI 1.3 – 22.3, p=0.0199, respectively) as along with a low birth weight (OR 6.9, 95% CI 1.9 – 25.2, p=0.0028; OR 7.6, 95% CI 1.7 – 34.3, p=0.007). Cigarette smoking and alcohol use by the pregnant women also adversely impacted on children’s health status (8.7 ± 1.4 vs. 9.7 ± 0.8, p=0.001 and 8.5 ± 1.9 vs. 9.6 ± 0.8, p=0.027, respectively). Simultaneous use of alcohol and cigarettes exacerbated the adverse impact of such substances on birth parameters and health status (birth weight, body length, Apgar score) as well as on pregnancy duration. Conclusions: The introduction of educational programs for high risk groups as well as prenatal obligatory consultations for mothers about the negative effects of substance use on fetus and the development of the child are warranted. Conclusions: Introducing educational programs are thus warranted for high risk groups as well as prenatal obligatory consultations for mothers about the adverse effects of such substance use on foetal and child development.
Wprowadzenie: Ekspozycja na używki (alkohol, papierosy, kofeinę) w okresie ciąży może prowadzić do zaburzeń rozwoju płodowego, jak również może negatywnie wpływać na przebieg ciąży i stan urodzeniowy dziecka. Cel pracy: Ocena powszechności picia (spożywania) alkoholu i palenia tytoniu w ciąży oraz ich wpływu na przebieg ciąży i parametry urodzeniowe noworodka w wybranej grupie matek dzieci do 2 roku życia. Materiał i metody: Badanie przeprowadzono wśród 104 matek dzieci poniżej 2 roku życia zamieszkujących województwo mazowieckie. Do oceny wybranych parametrów stylu życia (palenie papierosów, spożywanie alkoholu, kawy oraz herbaty, aktywność fizyczna), charakterystyki socjo-demograficznej oraz parametrów urodzeniowych dziecka (wykorzystano kwestionariusz ankiety. Wyniki zinterpretowano za pomocą analizy regresji logistycznej, korelacji rang Spearmana, oraz testów U Manna-Whitneya, Kruskala-Wallisa, t-Studenta, Tukeya i Chi2 (p≤0.05; STATISTICA 13.1 PL). Wyniki: W badanej grupie 22% kobiet w trakcie ciąży paliło papierosy, 13% spożywało alkohol, natomiast jednoczesne stosowanie obu używek deklarowało około 7% badanych. Palenie tytoniu, jak również spożywanie alkoholu zwiększało ryzyko wystąpienia porodu przedwczesnego (odpowiednio: OR 9.6, 95%CI 2.9 – 31.4, p=0.00015; OR 5.3 95% CI 1.3 – 22.3, p=0.0199) oraz niskiej masy urodzeniowej (OR 6.9, 95%CI 1.9 – 25.2, p=0.0028; OR 7.6, 95% CI 1.7 – 34.3, p=0.007), jak również predysponowało do gorszego stanu urodzeniowego dziecka (odpowiednio 8.7 ± 1.4 vs. 9.7 ± 0.8, p=0.001 oraz 8.5 ± 1.9 vs. 9.6 ± 0.8, p=0.027). Jednoczesne spożywanie alkoholu i palenie tytoniu zwiększało negatywne oddziaływanie stosowanych używek zarówno na parametry urodzeniowe, jak i stan zdrowia dziecka (masa urodzeniowa, długość ciała, punkty w skali Apgar) oraz czas trwania ciąży. Wnioski: Konieczne jest wdrożenie programów edukacyjnych skierowanych do grup ryzyka, jak również wprowadzenie obowiązkowej rozmowy z matkami (przyszłymi rodzicami) na temat szkodliwego działania używek na płód i rozwój dziecka.

The paper summarizes 32 months (January 2014 – August 2016) of activity of the Replantation Service for hand amputation in Poland. Over this period a total of 568 cases of total and subtotal amputations as well as other complex injuries to the hand were referred. Of these, 354 referrals (62%) were accepted and 214 (38%) rejected. Among accepted, there were 167 total (47%) and 142 subtotal (40%) amputations; 45 patients (13%) had other severe hand injuries. Vast majority of the patients constituted males aged a mean of 39 years. The most common injury was amputation of several digits in one patient, and thumb amputation – a total of 229 cases (65%), followed by transmetacarpal and wrist amputations – 92 (30%) and forearm/arm amputations – 33 cases (9%). Replantation of amputated extremity was performed in 141 patients (40%), revascularization in 145 (41%) and in 29 (8%) primary repair of the complex injuries. In 27 cases (8%), a coverage of the tissue defects, and in 12 (3%) primary terminalization was performed. Survival rate was of 78% for replantation and revascularization. Comparing to the period 2010–2012, an increase in number of treated patients (of n = 64 cases), in number of amputations (of 96 cases) and in number of amputated digits (of 88 cases) were noted. The activity report shows importance of Replantation Service, an informal structure, in saving limbs of severely mutilated patients.

This article summarises activity of the Replantation Service for hand amputations in Poland in years 2017-2019. Over this period a total of 551 cases of total and subtotal amputations as well as other complex injuries to the hand were referred to the referenced centres. Of these, 330 referrals were accepted and 221 rejected, for various reasons. Among these accepted, there were 165 total (50%) and 131 subtotal (40%) amputations; 34 patients (10%) had other severe hand injuries. Vast majority of the patients constituted young and middle-age males. The most common was amputation of several digits and thumbs - a total of 251 cases (76%), followed by transmetacarpal - 30 (9%), forearm - 23 (7%) and wrist - 20 (6%) amputations. Replantation of amputated extremity was performed in 138 patients (42%), revascularization in 98 (30%) and in 45 (14%) primary repair of the complex injuries. In 26 cases (8%), coverage of tissue defects was performed, and in 23 (7%) primary terminalization. Survival rate was of a mean of 65% for replantations and 85% for revascularizations. Comparing to the previously reported period 2013-2017, the number of treated patients was similar, but structure of injuries differed: number of digital amputations increased (of 22 cases), whereas number of proximal amputations (hand, forearm and arm) significantly dropped (of 50 cases). The importance of Replantation Service, an informal structure, in saving limbs of severely mutilated patients was emphasised.

A permanent on-call service for hand amputation (Replantation Service) was established in 2010 as the initiative of the Council of Polish Society for Surgery of the Hand. It is run by three qualified hand centres in Trzebnica, Poznań and Szczecin. The aim of the study was to present a summary of the almost three-year activity of this service. Material and methods. Over this period, a total of 435 cases of total amputations, subtotal amputations and other severe injuries to the hand were referred. Of these, 290 referrals (67%) were accepted and 141 (33%) rejected. Among accepted, there were 100 total (34%) and 113 subtotal (39%) amputations; 81 patients had other, severe hand injuries, such as crush, degloving and extensive wounds involving all tissues. Results. Young and middle-age males constituted the majority of patients with the mean age of 42 years (range 2-82). The most common injury was amputation of several digits (including thumbs) in one patient - 141 cases (48%), followed by amputations from the metacarpal- to the proximal forearm level -115 (39%) and elbow/arm level - 9 cases (3%). Niniteen patients (6%) had multi-level amputation or injury of the involved extremity. Replantation of the completely amputated extremity was performed in 83 patients (28%), revascularization in 95 (32%) and in 59 (20%) primary repair of the complex injuries. In 23 cases, the repair of tissue defects was performed with flaps, mostly with greater omentum. Survival rate was of 84% for replantations and 88% for revascularizations. Conclusion. Establishing of the Replantation Service constituted a significant progress in organization of the management of the most severest upper limb injuries.

Stroke is the second most common cause of mortality. Ischemic stroke is approximately 10 times more common than haemorrhagic stroke. The strongest risk factor for ischemic stroke is hypertension; thus, reduction of blood pressure decreases the risk of ischemic stroke. However, the prognostic importance of blood pressure after is unclear. The problem is even more complex considering blood pressure variability, i.e. continuous changes of blood pressure values. The aim of this review is to discuss very short-term, short-term, mid-term, and long-term blood pressure variability in the context of clinical outcome in patients after acute ischemic stroke.

Opis:
Hematoma in different parts of the brain is one of the most important complications of head injury and is associated with high mortality and morbidity rate. The aim of this study was evaluation of the relationship between Computed Tomography (CT) and intraoperative findings with clinical symptoms in head trauma patients. In this study 95 patients with cerebral hemorrhage due to head trauma, referred to Taleghani Hospital in Kermanshah were studied. After an initial clinical examination, the level of consciousness determined according to the Glasgow Coma Scale (GCS) was recorded. All patients underwent brain CT scan and findings were recorded, including size and location of the hematoma. Patients in all treatment such as surgical procedures under the supervision, and the information on their status was recorded until discharge or death. It was found that most patients (38%) were between 40-20 years: 73% of patients were male, while 27% were female. The outcome of 35 patients (35.4%) were normal, 12 patients (12.3%) had moderate disability, 9 patients (9.2%) had severe disability, 11 patients (10.8%) vegetative state and 31 patients (32.3%) died. There was a significant association between location of the hematoma and hematoma in CT scan and outcome of patients with cerebral hemorrhage caused by trauma (P<0.05). We also found a significant association between size of the hematoma and midline shift in CT scan with outcome of patients with cerebral hemorrhage caused by trauma (P<0.05). The prognosis of patients with traumatic brain injury depends on location of the hematoma; volume of hematoma, midline shift in CT scan and length of trauma to surgery more than 4 hours.

Evaluating the results of programmes for perpetrators of domestic violence is essential for both policy mak- ers and practitioners, and a growing number of studies have addressed this issue. However, few studies have described the ongoing, day-to-day outcome measurements routinely undertaken by perpetrator programmes to monitor their results. As part of the IMPACT Evaluation of European Perpetrator Programmes project, in the present study 134 domestic violence perpetrator programmes from 22 European countries answered a survey on the designs, methods and instruments they use in their current day-to-day outcome monitoring practice and on the obstacles to and need for improvements in this practice. The main results include the findings that 20% of programmes do not measure outcomes at all, that there is a great diversity in methods and instruments used, and a low percentage of programmes make use of data from the victims/survivors and from follow-ups.

This paper aims to analyse the impact of expenditure to fund primary schools in big cities in Poland on pupils’ performance (measured with the results of 6th graders’ exam). Analysis of literature allowed us to hypothesize about the correlation of average 6th graders’ exam results as a test of competence, and the expenditure related to the education of students in public primary schools, and presents the results of public primary schools in a big city. Research conducted by the author in a big city among the primary schools, which were established by one unit of local government, suggests that, although a dependence can be observed, interestingly this is of a negative nature, not statistically significant. Thus there is no correlation between the 6th graders’ exam results and the amount of expenditure in public primary schools.

Introduction. Traumatic brain injuries (TBI) are a real social problem, with an upward trend worldwide. TBI is the leading cause of death and disability, especially among young men. Each year in Europe, and also in Poland, 243 per 100,000 individuals suffer from TBI. Assessing prognosis after traumatic brain injury is very important in order to help clinicians to make a decision about the implementation of specific methods of treatment, and to make communication with the patient and the patient’s family easier. Objective. The objective of this review was to present prognostic factors, to assess outcomes within a short time after a moderate to severe traumatic brain injury, as well as to predict functional outcome. The most important independent variables were: age, Glasgow Coma Scale (GCS) motor score, pupil response, Marshall CT classification and traumatic subarachnoid haemorrhage. Other important prognostic factors included hypotension, hypoxia, glucose, coagulopathy, haemoglobin and category of CT characteristic, such as midline shift, mass lesion, basal cistern. Conclusions. Gender and intraventricular haemorrhage did not have predictive value. This subject needs much more research in the area of new prognostic factors which would be better associated with outcome after traumatic brain injury.

Phthalates are a family of industrial chemicals that have been used for a variety of purposes. As the potential consequences of human exposure to phthalates have raised concerns in the general population, they have been studied in susceptible subjects such as pregnant women, infants and children. This article aims at evaluating the impact of exposure to phthalates on reproductive outcomes and children health by reviewing most recent published literature. Epidemiological studies focusing on exposure to phthalates and pregnancy outcome, genital development, semen quality, precocious puberty, thyroid function, respiratory symptoms and neurodevelopment in children for the last ten years were identified by a search of the PubMed, Medline, Ebsco, Agricola and Toxnet literature bases. The results from the presented studies suggest that there are strong and rather consistent indications that phthalates increase the risk of allergy and asthma and have an adverse impact on children's neurodevelopment reflected by quality of alertness among girls, decreased (less masculine) composite score in boys and attention deficit hyperactivity disorder. Results of few studies demonstrate negative associations between phthalate levels commonly experienced by the public and impaired sperm quality (concentration, morphology, motility). Phthalates negatively impact also on gestational age and head circumference; however, the results of the studies were not consistent. In all the reviewed studies, exposure to phthalates adversely affected the level of reproductive hormones (luteinizing hormone, free testosterone, sex hormone-binding globulin), anogenital distance and thyroid function. The urinary levels of phthalates were significantly higher in the pubertal gynecomastia group, in serum in girls with premature thelarche and in girls with precocious puberty. Epidemiological studies, in spite of their limitations, suggest that phthalates may affect reproductive outcome and children health. Considering the suggested health effects, more epidemiologic data is urgently needed and, in the meantime, precautionary policies must be implemented.

Objective. The aim of this study is to evaluate neurological scales, as well as biochemical and radiological parameters measured on day 10 after ischemic stroke (IS), according to their value as predictors of the long-term outcome. Materials and method. 45 patients were assessed according to the Barthel Index (BI) and National Institute of Health Stroke Scale (NIHSS) on day 10, and according to Modified Rankin Scale (mRS) 3 months after the onset of IS. On day 10 of IS, the serum level of C-reactive protein (CRP), albumin, D-dimers (DD), S100BB and Tau proteins was measured and the volume of ischemic focus assessed with the use of Computed Tomography (CT). The patients were divided into groups with good outcome (GO) and mRS 0–2, and with bad outcome (BO) and mRS 3–6. Results. NIHSS and BI scores (p<0.001), the volume of ischemic focus (p<0.01), CRP (p<0.01) and albumin level (p<0.05), but not DD, S100BB and Tau protein levels evaluated on day 10, correlated with mRS after 3 months since IS onset. Patients from the BO group were observed to have lower BI (p=0.001), higher NIHSS (p<0.01) and CRP levels (p<0.05), and bigger volume of ischemic focus (p<0.05) measured on day 10 of IS. In the GO group, there were more patients with atherosclerotic etiology (p=0.02 x2=7.856). Regression analysis showed that only the BI score assessed on day 10 of IS can predict the outcome after 3 months assessed by mRS (OR=1.102, 95%, CI:1.01–1.203; p=0.001). Conclusions. BI assessed on day 10 has a predictive value for the outcome evaluated by mRS 3 months after the onset of IS.

Opis:
The objective of this study was to examine which educational experiences in general education impact on the student’s learning outcomes. The survey included 1,201 students in South Korea. Hierarchical multiple regressions were used. The result showed that educational experiences in general education demonstrated greater predictive power for the student’s learning outcomes than individual and institutional characteristics. However, not all educational experiences positively predicted learning outcomes. Educators are advised to improve general education towards better educational experiences in a way that encourages teaching higherorder thinking, evaluation and feedback, and active class participation.

Opis:
Need for closure is a construct that describes a motivational tendency to quickly select and prioritize information in the environment. Such tendencies can affect the process of negotiations, and so the quality of their outcome. The rigidity that accompanies high need for closure can lead to less openness to proposals that benefit one’s partner, and to solutions that are less optimal. We conducted a study in which 34 pairs of individuals negotiated. Pairs were matched in terms of need for closure (high vs. low) and gender. We found that need for closure affected subjective evaluations of certain aspects of the negotiation process. Participants with low need for closure were more likely to indicate that they and their partners sought win-win solutions during the negotiation. This led to a greater sense of process fairness for the negotiation. These results can be taken into consideration when teaching negotiations, and when planning real-life negotiations.

The aim of the study was to determine the role of self-efficacy, outcome expectancies, and risk perception (including consequences of mastectomy) in formulating the intention to undergo breast reconstruction in 178 women after total mastectomy. The social-cognitive variables were measured in the context of breast reconstruction, while depression was assessed using the Beck Depression Inventory. The structural equation modeling revealed that among the predictors there were only two that accounted for the intention to undergo breast reconstruction, i.e. self-efficacy and outcome expectancies (R2 = .67). Subsequent analyses of the related moderators, i.e. depression, age, and duration of the disease indicated a good fit to the data. Nevertheless, in subgroups with poorer resources (older age, depression, and longer duration of the disease) the direct effects of self-efficacy on intention were less noticeable or non-existent. The results suggest that self-efficacy may play the regulating role in making a breast reconstruction decision if individual resources are taken into account.
