The aim of this study was to present a case study of 59 years old men with external otitis complicated by subperiostal abscess with destruction of squamosus part of temporal bone. 61 years old men was admitted to the Clinic of Otolaryngology PUM because of severe pain of temporal region on the left side and for few days he was treated for external otitis. The patient complained of severe ear pain, hearing loss and fever 38°C. For a week he was receiving ciprofloxacin orally and as ear drops but symptoms worsened. On the day of addmition edema and pain in temporal region on the left side, trismus and severe edema in left external ear were stated, tympanic membrane was red, thick and there was no perforation in it. Pure tone audiometry showed air bone gap 50-60dB. In computer tomography an abscess in soft tissue of temporal region was stated with destruction of the squamosus part of temporal bone 18x10mm. Surgery was performed – incision of abscess and mastoidectomy. During surgery the place of destruction in the postero – superior part of external auditory canal was identified and it communicated with abscess. The bony destruction and dura were covered with cartilage. In the postoperative period fast general and local recovery appeared. The bacteriological specimen was negative. The patient was diagnosed for diabetes or others immunological impairment but results were negative. In the control pure tone audiometry there was complete closure of air bone gap. The patient was discharged in good general and local condition. In the follow up there was no other ear infections.

Introduction: Ossiculoplasty can be carried out in a number of ways, depending on the anatomical and functional conditions encountered during otosurgery and the experience of a given centre. The extent of damage to the ossicular chain determines the reconstruction method. Aim: The objective of the study was to analyse treatment effects in terms of postoperative hearing improvement in patients with chronic otitis media, with a particular emphasis on stapedial superstructure preservation. Material and method: The records of 294 consecutive patients undergoing their first ENT surgery due to chronic otitis media at the Department of Otolaryngology of Collegium Medicum, Jagiellonian University of Kraków in 2009–2013 were analysed. In order to assess the role of preserved stapedial superstructure, 96 patients were eligible for further analysis. Results: The analysis points to a significant hearing improvement after ossiculoplasty with the preserved stapedial superstructure as compared with the patients after footplate mobilisation. On the other hand, the research results point to significantly smaller hearing improvement in those patients, in whom only stapes was preserved, as compared with those, in whom a more extensive reconstruction of the ossicular chain was possible. Conclusions: The air-bone gap measured before otosurgery often fails to reflect the extent of abnormalities and cannot, therefore, be considered as the only prognostic factor for postoperative hearing improvement. Stapes preservation is crucial for hearing improvement after middle ear surgery.

Objective. To investigate a relationship between the etiological factor of external otitis and occurrence of particular signs/symptoms. Design. A special questionnaire was designed and completed by all patients covering personal details, medical history, results of otolaryngological examination and bacteriological and mycological investigations. Subjects. 249 patients of the Outpatient Department of Laryngology at the Regional Hospital in Bełchatów with symptoms of external otitis. For analysis of relationships between particular signs/symptoms and species/genera of microorganisms, statistical tests were used: χ2 test, χ2 test with Yate’s modification, C−Pearson index, and Fisher exact test for very small samples. Results. There is a statistical dependence between discharge, hearing loss, swelling of skin, scant, dried discharge with fetid odour and bacteria isolated from the external ear canal. Similar dependence exists between pain, hearing loss, no smelly discharge or wet, black plug of fetid odour and fungi. Also there is a statistical dependence between pruritus, red skin and grey, fetid discharge and mixed flora. Conclusions. Some of symptoms and signs are connected with definite etiological factors which is important not only for correct diagnosis but also for institution of appropriate and effective treatment. On the basis of some characteristic symptoms and signs it is possible to make a tentative diagnosis as to the etiological pathogen responsible for external otitis.

Introduction: The aim of the middle ear surgery is to eliminate abnormalities from the mucous, ensure the due airing of the postoperative cavity and reconstruct the sound-conducting system in the middle ear. Numerous reports can be found in literature on the changes to bone conduction as a consequence of middle ear surgery. Study objective: The aim of the work is to define the factors that affect bone conduction in the patients operated on due to the perforation of the tympanic membrane with the preserved and normal mobile ossicular chain. Material and method: A prospective analysis of patients operated on due to diseases of the middle ear in 2009–2012 was carried out. The cases of patients operated on for the first time due to chronic otitis media were taken into consideration. The analysis encompassed the patients who had undergone myringoplasty. The patients were divided into several groups taking into account the abnormalities of the mucous observed during otosurgery. Results: A significant improvement of bone conduction was observed in the patients with dry perforation, without other lesions in the middle ear. The appropriate pharmacological treatment before otosurgery in patients with permanent discharge from the ear resulted in significant bone conduction improvement. The elimination of granulation lesions turned out to be a positive factor for the future improvement of the function of the inner ear. Conclusions: The lack of abnormalities on the mucous of the middle ear (e.g. granulation,) and discharge has a positive impact on improvement of bone conduction after myringoplasty. Adhesions in the tympanic cavity, especially in the niche of the round window, have a negative impact on improvement of bone conduction in patients after myringoplasty.

Opis:
A 44-year-old male presented with a facial schwannoma extending into both the middle cranial fossa and mastoid processus. Due to hearing loss, facial nerve palsy and inflammatory changes in CT scan, this patient was misdiagnosed as chronic otitis media. Audiogram showed a right mixed hearing loss with 30–40 dB air-bone gap. In MR, features of the facial nerve neuroma were found. The patient was qualified for surgery to remove the tumor via middle fossa approach, with possible conversion to the retroauricular approach. Ossiculoplasty LC was performed. Diagnostic problems and methods of treatment are discussed.

Aim: Aim of the study was retrospective analysis of the patients documentation treated for acute mastoiditis (AM) in Department of Pediatric Otolaryngology in Bialystok and available literature on this subject to determine the optimal rules of treatment in AM. Material and methods: A retrospective analysis of 40 patients treated for AM in 2001-2017. We have adopted Anthonsen et al. [15] AM diagnosis criteria to include patients to this study. Results: The mean age of the respondents was 46 months, 37% of children were less than 2 years old. 2/3 of the cases were not preceded by previous acute otitis media(AOM) episodes. 69% of patients received antibiotics before admission to the hospital. In laboratory studies, 95% of patients had elevated indices of inflammation (CRP, leukocytosis), and their value was higher among patients qualified for surgery. The indications for computed tomography (CT) were the lack of improvement after conservative treatment for 48 hours or symptoms of the presence of subperiosteal abscess. CT was performed in 35% of patients. 24 patients (60%) were treated only conservatively: myringotomy without / with the insertion of a ventilation tubes and intravenous antibiotic therapy. The most commonly used antibiotic was ceftriaxone - 75% of patients. 16 patients required mastoidectomy. The most common indication (30%) for mastoidectomy was the presence of subperiosteal abscess. Conclusion: In uncomplicated AM cases CT does not have to be routinely performed, a myringotomy should be performed with or without drains insertion and an empiric intravenous antibiotic therapy should be started. In presence of a subperiosteal abscess, it is recommended to perform CT with contrast and mastoidectomy. Deterioration of the patient's condition or lack of improvement after 48 hours of conservative treatment obliges us to make a CT and on the basis of the decision on mastoidectomy.

Objective: We reviewed functional outcomes of tympanoplasty. Study design: The results of tympanoplastic surgery are changing in time. We present late treatment outcomes among different types of tympanoplasty. Methods: Eighty-six patients who underwent tympanoplasty were enrolled in the study. The results of pure tone audiometry performed 7 days before, then at 3 months, 1 year, and 3 years after the surgery were assessed. Type II tympanoplasty involved implantation of a partial ossicular replacement prosthesis and type III tympanoplasty involved reconstruction with a total ossicular replacement prosthesis and the use of autogenous homogenous material. Statistical analysis was performed. Results: With all four types of tympanoplasty, hearing improvement was achieved at 3 months and 1 year after surgery based on the magnitude of the mean ABG reduction (p<0.001). In patients who underwent type I, type III, and type IV tympanoplasty, the ABG reduction at 3 years after surgery was maintained at the level reported at 3 months and 1 year after surgery (p<0.001). In patients who underwent type II tympanoplasty, however, the mean ABG value was increased at all tested frequencies (p<0.05). The mean ABG values reported 3 years after type II and type III tympanoplasty were similar. Conclusions: Based on the results over time, hearing improvement seems to be less durable after reconstruction with the partial ossicular replacement prosthesis procedure compared to the total ossicular replacement prosthesis procedure.

Authors presented a case of a 15-year-old girl diagnosed with otitis media with effusion complicated by petrositis. Otitis media with effusion is not a common cause of petrositis. The condition occurs mostly in adults, rarely in the pediatric population. Patients with suspected petrositis require magnetic resonance and computed tomography imaging. Conservative treatment is not sufficient when symptoms appear. Surgical treatment is essential to complete removal of inflamed tissues. A few surgical approaches are recommended (e.g., middle fossa approach).

Objective: A pure tone audiomety analysis of patients with extracranial complications of chronic suppurative otitis media (ECCSOM). Material and methods: We retrospectively analyzed audiometric data performed before treatment from 63 patients with ECCSOM (56 single, 7 multiple complications) including groups of frequencies. Results: The greatest levels of hearing loss were noted for 6 and 8 kHz (79.0 and 75.7 dBHL) and for the frequency groups high tone average (76.1 dBHL). As regards the severity of hearing impairment in pure tone average the prevalence of complications was as follows: labyrinthitis (77.8±33.6 dBHL), facial palsy (57.1±14.3 dBHL), perilymphatic fistula (53.9±19.9 dBHL) and mastoiditis (42.2±9.5 dBHL) (p=0.023). Conclusions: Hearing loss in ECCSOM is dominated by mixed, high-tone, moderate type of hearing loss, most profound in labyrinthitis. In 11% of patients the complication causes total deafness.

Wstęp: Nowotwory mające swoją etiologię w gruczołach ślinowych występują rzadko i stanowią ok. 3–10% wszystkich guzów w obrębie głowy i szyi. Materiał i metody: W latach 2006–2016 w Klinicznym Oddziale Chirurgii Czaszkowo-Szczękowo-Twarzowej Wojskowego Instytutu Medycznego w Warszawie z powodu nowotworów ślinianek przyusznych hospitalizowano i leczono 149 pacjentów. Podstawą do analizy były historie chorób pacjentów, wyniki badań histopatologicznych oraz protokoły zabiegów operacyjnych. Wyniki: W 126 przypadkach guzy były łagodne, natomiast w 26 guzy złośliwe. Analizując wyniki można stwierdzić, że nowotwory - zarówno łagodne, jak i złośliwe – częściej występowały u mężczyzn. Średni wiek całej grupy badanych wynosił 58,3 lat, dla zmian złośliwych – 65,5 lat, a dla zmian łagodnych był nieznacznie niższy i wynosił 56,9 lat. Wśród pacjentów objętych analizą szczyt zachorowalności przypadał na przedział wiekowy 60–69 lat. Ponadto zwraca uwagę, iż przedział ten był taki sam dla ogólnej liczby guzów, jak i nowotworów łagodnych, natomiast dla nowotworów złośliwych był nieznacznie wyższy i wynosił 70–79 lat. Z analizy zebranego materiału wynika, że guzy łagodne występowały znamiennie częściej, stanowiąc 84,56% wszystkich guzów. Najczęstszymi rozpoznaniami wśród nowotworów łagodnych były gruczolak wielopostaciowy (adenoma pleomorphicum) oraz gruczolak limfatyczny (adenolymphoma), które stanowiły łącznie 93,66%. Rezultaty badań wskazują, że wśród nowotworów złośliwych dominowały: gruczolakorak polimorficzny o niskiej złośliwości (polymorphous low-grade adenocarcinoma) występujący w 26,07% przypadków, gruczolakorak inaczej nieokreślony (adenocarcinoma NOS) oraz rak zrazikowokomórkowy (acinic cell carcinoma). Każdy z nich stanowił po 13,04% rozpoznań. Jak wynika z przeprowadzonej analizy najczęstszą metodą leczenia było leczenie chirurgiczne jako metoda stosowana samodzielnie. Wnioski: Uzyskane z retrospektywnej analizy dane korelują z piśmiennictwem krajowym i światowym.

Introduction: The treatment and rehabilitation of hypoacusis with the use of cochlear implants is a safe and reliable method suitable for both children and adults. In people affected by chronic otitis media cholesteatomatosa or such who have previously undergone open repair of the ear, we use a special surgical technique known as lateral/subtotal petrosectomy. Material and methods: The study group consisted of patients with profound bilateral sensorineural hearing loss, in which otitis media with and without cholesteatoma has been diagnosed or after open repair of the middle ear. A retrospective analysis of patient data, as well as radiological and audiological results, was conducted. Results: In the Clinic of Otolaryngology and Laryngological Oncology of the Pomeranian Medical University in the years 2008–2018 we performed 90 cochlear implant surgeries, including a petrosectomy in 1 child with cholesteatoma (5 years) and in 2 adults after open repair (62 and 73 years). In all cases the procedure was done in a single stage. Healing proceeded correctly in all patients undergoing petrosectomy. The observation period ranges from 26 to 32 months, computed tomography examinations revealed no indirect characteristics of recurrent cholesteatoma. The patients remain under constant ENT supervision. The child has risk factors for autism and mental retardation, he displays good auditory responses and speech understanding; he has not developed active speech. As regarding free field pure tone audiometry, in adults hearing in the cochlear implant remains at 35 and 40 dB, and speech understanding at 80%. Discussion: Patients with chronic otitis media can be treated efficiently and safely with a cochlear implant using lateral petrosectomy. Lateral/subtotal petrosectomy is the access of choice when deep sensorimotor hearing loss coexists with chronic inflammation in the middle ear.

The purpose of this study was to evaluate in detail both the in vivo and in vitro efficacy of the enzyme agents, ZYMOX® Plus Otic (ZYMOX-P), in the treatment of canine otitis externa (OE). Eight dogs with a diagnosis of non-seasonal severe chronic OE were recruited for the study. ZYMOX-P was administered for 2-4 weeks. The Otitis Index Score (OTIS3) and bacteria or yeast colony growth were measured. Also, minimum biofilm (BF) formation inhibition concentration (MBIC) and BF bactericidal concentration (BBC) were measured in vitro. OTIS3 showed a statistically significant reduction after treatment (88.2%, p<0.001; pre-treatment = 11.0 ± 0.9; post-treatment = 1.3 ± 0.4, mean ± SEM). The individual OTIS scores, erythema, edema, erosions/ ulcerations, exudate and pruritus showed significant reduction (85.7%, 95.7%, 83.3%, 80.0%, and 89.3%, respectively). Microscopic examination revealed the presence of BF exopolysaccharide in all 8 ear samples when stained with alcian blue. Seven of the 8 dogs (87.5%) showed a reduction in colony growth. ZYMOX-P was effective at 34-fold and 16-fold dilutions on MBIC and BBC, respectively. These findings indicate that ZYMOX-P has efficacy against BF-related infection and is beneficial when used for the management of canine OE.
