Stigmasterol has been shown exihbit anti-osteoarthritic properties in vitro studies. However, the in vivo effects of stigmasterol on cartilage are still unclear. This study investigated the anti-osteoarthritic properties of stigmasterol on cartilage degradation in a rabbit model of osteoarthritis (OA). Twenty rabbits underwent bilateral anterior cruciate ligament transection (ACLT) to induce OA. Five rabbits were used as normal control. Two weeks after operation, the rabbits were randomly divided into two groups. Each group of 10 rabbits received intra-articular injection with 0.3 ml of stigmasterol in left knees and vehicle in right knees, once weekly. Group 1 was killed 6 weeks after ACLT and 2 were sacrificed 9 weeks after ACLT. The knee joints were assessed by gross morphology, histology and gene expression analysis. We found that expression of genes encoding matrix metalloproteinases (MMPs) was significantly higher while tissue inhibitors of metalloproteinase (TIMP)-1 was significantly lower in the both joints of the two OA groups compared to normal contrals. Stigmasterol reduced the cartilage degradation as assessed by histological analysis and markedly suppressed MMPs expression both in group 1 and group 2. Our results suggest that stigmasterol may be considered as a possible therapeutical agent in the treatment of OA.

Osteoarthritis is the most frequently reported and the most onerous disease in middle-aged and elderly people. Degenerative changes constitute a heterogeneous set of pathological transformations of the structure and function of the elements forming the joint, which is the result of factors disturbing their balance. The loss of joint cartilage mass with its insufficient rebuilding or transformation of subchondral bone and other soft tissue structures is characteristic for this disease [1-3]. Nowadays, in addition to the development of invasive surgical methods aimed at cartilage reconstruction, it is possible to use ortobiological therapy in parallel, taking into account the careful qualification for this type of treatment. Ortobiological therapies use the vital potential of the unique tissue that is blood. The expected effects of ortobiological treatment in osteoarthritis is joint cartilage regeneration and acceleration of healing [1,2,4]. The effectiveness of each therapy depends on a number of factors that affect its results not only during or after its application, but also before the start of treatment. Therefore, it is important to properly prepare the tissues for the planned therapy. Targeted physical activity, taking into account the correctness of its performance is extremely important in the preparation of the tissue base for the application of autologous preparations. Physical exercises, preceding the application of injection within the musculoskeletal system, improve the effectiveness of ortobiological treatment. The combination of ortobiological therapy and subsequent physiotherapy support the rehabilitation process, leading to the achievement of the best possible end results [5-7]. The platelet rich plasma (PRP) is a more and more commonly used method of ortobiological therapy. Platelet rich plasma is a source of growth factors that is used to improve the function of tissues by remodeling their morphology and improving metabolism. This therapy distinguishes itself from conventional means of influencing tissue and contributes to establishing a new therapeutic direction in the treatment of osteoarthritis [8]. Another therapy used in ortobiology is autologous conditioned serum (Orthokine®). This modern method of treating inflammation and degenerative joints strengthen the integrity of joint cartilage by introducing antiinflammatory cytokines into the body. In addition, a significant increase in the concentration of growth factors during conditioning using Orthokine® has been demonstrated. These factors stimulate tissue repair and regeneration processes by migrating cells to the site of damage, their multiplication, differentiation and the formation of new blood vessels [9-11]. One of the methods used to treat osteoarthritis is stem cell therapy. The purpose of these cells is to replace the dead cells and rebuild the tissue. Due to the fact that stem cells present in the human body, as the body ages, they lose their ability to intensively regenerate, the method of stem cell proliferation is used for therapeutic purposes. Properly prepared stem cells are injected precisely to the affected area. Stem cell therapy is a method that is constantly improved both in the selection of the source of cells and the method of their preparation in order to isolate and concentrate the cells. The research carried out in this direction is aimed at optimizing the therapy and, consequently, effective assistance to patients with osteoarthritis [12]. An important problem associated with osteoarthritis is the occurrence of degenerative cysts. They appear due to the formation of bloody strokes and the conduct of destructive processes in the subchondral bone. Ongoing work on surgical methods to prevent the rapid progression of the disease. Currently, tissue scaffolds are used to fill defects. These are the resorbable material that fills the tissue defect, and through their structure they allow for the migration of cells and overgrowth of tissues, and thus its reconstruction [13-15]. In order to improve the properties of materials, research is conducted on hybrid implants that would combine individual therapies. The possibility of comprehensive treatment, consisting in filling cavities with an implant, with the use of an ortobiological preparation in the scaffolding space, will allow bone tissue remodeling to proceed under optimal conditions [16].

Introduction. Osteoarthritis (OA) is generally a progressive disease that affects synovial joints, resulting in abnormalities to articular cartilage subchondral bone, synovium, and adjacent soft tissues. Aim. The purpose of this work was to examine the histological changes in knee cartilage and bone following the administration of two different chondroitin sulfate products in two experimental OA models in rats. Material and methods. OA was induced in rats by either a single injection of mono-iodoacetate or four once-weekly injections of dexamethasone. 70 adult rats were included: 30 received mono-iodoacetate, 30 received dexamethasone and the 10 remaining controls received no injection. Samples of knee bone and cartilage were then analyzed histologically. Results. Animals with OA that received CS had significantly less inflammation, improved motor activity, and better analgesia compared with those that did not receive CS, with little difference between products. Histologically, both products reduced the signs of OA and resulted in the activation of regenerative processes of cartilage and bone and stimulation of proliferation and formation of amorphous material. Conclusion. These results substantiate the importance of using high-quality pharmaceutical-grade CS to ensure optimal efficacy and safety of the final product in patients with OA

Introduction: Osteoarthritis an inflammatory process in the synovial membrane. These changes cause pain and joint deformities. Pharmacists can assess patients’ quality of life, which can be a good criterion for assessing analgesics treatment effectiveness and prevent the growing phenomenon of polypharmacy and self-treatment. Purpose: To answer these questions: 1) Is there a relationship between the quality of life of patients with osteoarthritis taking analgesics and those with osteoarthritis taking analgesics and implemented rehabilitation? 2) Do patients using analgesics practice polypharmacy? 3) Are patients with osteoarthritis the main consumers of painkillers? and 4) Does the use of analgesics affect the health-related quality of life of patients with osteoarthritis? Materials and methods: The study was conducted on 240 people who were divided into three groups: osteoarthritis, reference (generally healthy, occasionally taking analgesics), and control (generally healthy, no intake of analgesics). To assess the rise in analgesics consumption by patients with osteoarthritis and the phenomenon of polypharmacy, an original questionnaire was used. Results: There is a relationship between the quality of life of patients with osteoarthritis taking analgesics. Statistical analysis showed that in the years 2013-2015 the purchase of prescription analgesics and without a prescription systematically, significantly increased. Conclusions: Patients using analgesics unknowingly practice polypharmacy. In Ars Medica Pharmacies, analgesics purchases by patients with osteoarthritis grew significantly in the analyzed period, but they are not the main analgesics consumers. Use of analgesics by patients with osteoarthritis improves their health-related quality of life.

Introduction. Osteoarthritis is the most common disease of the musculoskeletal system. Osteoarthritis progresses slowly, but with time it results in movement disability and chronic pain. Its progression is also significantly associated with personality functioning and the ability to make rational life choices regarding various aspects of human life, including the economic sphere. Aim. The aim of the research was to determine how personality correlates with a tendency towards indebtedness between patients with and without osteoarthritis. Material and methods. The researched sample consisted of two groups - 50 patients diagnosed with osteoarthritis hospitalized in the Department of General and Neuro Rehabilitation in the Institute of Rural Health in Lublin and 50 healthy people. The KOS-B Questionnaire, the IVE Questionnaire, the SES Scale, the SPP-25 Scale, the Delta Questionnaire and the APSZ questionnaire were used in the research. Results. In the group of patients with osteoarthritis, the leading correlates of indebtedness are: assessment of the stress situation related to the current economic situation seen as a threat, and perceiving it as harm or loss. In the healthy group, the leading correlates of incurring financial liabilities are impulsiveness, self-esteem, assessment of the stress situation related to the current economic situation seen as a threat, openness to new experiences and sense of humor, empathy, and the need for social approval. Conclusion. In the group of patients with osteoarthritis, the factor conducive to incurring financial liabilities is the assessment of the stressful situation related to the current economic situation seen as a threat, while perceiving it as harm or loss leads to abandoning reliance on credit. In the healthy group, the factors that favor this type of activity are impulsiveness, self-esteem, the assessment of the stress situation related to the current economic situation seen as a threat and the need for social approval; the factors protecting against indebtedness in healthy persons are openness to new experiences and sense of humor as well as empathy.

The purpose of this study was to compare the effect of quadriceps isometric exercises along with hip abductors isometric exercises as integrated exercise with quadriceps exercises in reducing pain, disability in medial knee osteoarthritis. The Total of 54 patients (both male and female) were recruited in this study but on the basis of inclusion and exclusion criteria, only 32 were included in this study. Womac index was opted as outcome measures to collect the data of pain, disability and functional limitation. Participants were randomly assigned to integrated exercise group; (n = 16) (mean age 52) who performed exercises [Quadriceps isometric along with hip abductor isometric exercises] and quadriceps isometric exercises group (n = 16) (mean age 49) performed for 3 weeks. 32 patients were analyzed and the effect of integrated exercises [Quadriceps isometric along with hip abductor isometric exercises] showed a statistically significant difference in pain, disability and functional limitation (p < 0.000). Total duration to collect the pre and post data of pain, disability and functional limitation was 3 weeks. 3 weeks of exercise protocol was effective in both groups in terms of reducing pain and disability but integrated exercises were found to be more effective than quadriceps exercise in improving the pain, disability and functional limitation over the time at the level of significance.

Introduction. Osteoarthritis of the knee is the most common presentation of osteoarthritis with prevalence between 12% and 35% of general population and is considered the leading cause of musculoskeletal disability in the elderly population worldwide. Aim. The study compared efficacy of kneading massage and pulsed ultrasound on pain, joint stiffness and difficulty in knee osteoarthritis (KOA). Material and methods. Fifty subjects with radiological evidence of KOA participated in the study. They were randomly allocated into kneading massage group (KMG) (25) and Ultrasound group (USG) (25). KMG received kneading massage for 7 minutes while USG received pulsed mode ultrasound for 15 minutes. Treatment was twice in a week for six weeks. Pain intensity (PI), joint stiffness and difficulty were assessed pre, 3rd and 6th weeks of treatment session with semantic differential scale and WOMAC. The data were analyzed using descriptive and inferential statistics, alpha level was set at 0.05 Results. There was a significant difference in present PI (F=11.45,P=0.001) and stiffness (F= 11.32, P=0.003) in USG. There was a significant reduction in PI (F=7.95, P=0.001) and joint stiffness (F=8.86, P=0.003) in KMG. At the 6th week, there was a significant differences in PI (t=12.23,P=0.000) and stiffness (t=8.08, P=0.000) when USG (3.00+0.4, vs 7.14+ 1.49) was compared with KMG (3.16+0.5 vs 7.50+1.5). Conclusion. Ultrasound (US) and kneading massage (KM) reduced PI and joint stiffness of KOA effectively; however US reduced PI than KM while KM reduces joint stiffness than US.

Purpose: The tools for measuring objective quality of life include research regarding the conditions of a successful life and life satisfaction. Osteoarthritis (OA) is characterized by progressive degradation and loss of articular cartilage as well as abnormal bone growth and remodeling, located directly under the cartilage. The aim of this study is to answer the question on which level OA patients perceive their quality of life dependent on health. Material and methods: The Polish version of the HAQ questionnaire, assessing the quality of life over the past week, was used for overall assessment of quality of life. The Polish version of the AIMS-2 questionnaire was used as a specific questionnaire for assessing the health status of patients with osteoarthritis. Surveys were conducted in a group of 240 people. Results: The quality of life dependent on health is significantly reduced in the group of healthy people who take painkillers sporadically because of other reasons and in the OA pateints group compared to the control group, the same as in the OA pateints group compared to the group of healthy people who take painkillers sporadically for other reasons. Conclusions: According to the answers from the questionnaire, patients with OA seek different forms of improving their quality of life. It is important to change their lifestyle (weight reduction, using a bench-mounted bike, walking stick or walking frame), reducing the surface area of the joints, reducing the defective position of the limb axis. Exercises that increase muscle mass and exercises to improve joint mobility should be introduced. It is advisable to refer the OA patient for physical therapy and/or balneotherapy.

Introduction: Osteoarthritis (OA) is a widely prevalent joint disease leading to motor disability and pain. Appropriate indicators for identifying patients at risk for this progressive disease, identifying molecular events for detecting early phases of the disease, or biomarkers to screen for treatment responses, however, are lacking. Micro RNAs (miRNAs), which play crucial roles in OA, could be potential biomarkers of OA. Because circulating miRNA levels reflect the disease state, they may be useful for OA screening and as diagnostic tools, reducing the need for invasive procedures and minimizing the cost of current diagnostic methods. Materials and methods: The expression levels of 18 microRNAs (let-7e-5p, miR-21-5p, miR-93-5p, miR-101-3p, miR-103a-3p, miR-130a-3p miR-146a-5p, miR-16-5p, miR-193b-3p miR-199a-3p, miR-210-3p, miR-222-3p, miR-22-3p, miR-27a-3p, miR-27b-3p, miR-335-5p, miR-454-3p, and miR-98-5p) were analyzed by quantitative real-time polymerase chain reaction in the cartilage tissues and serum samples of 28 OA patients and were compared to those of 2 healthy controls. Results: Expression of microRNA-146a-5p was significantly upregulated in the cartilage (p=0.006) and serum (p=0.002) of OA patients. The expression levels of miR-146a-5p in the serum were positively correlated with those in the cartilage (Pearson correlation coefficient R=0.32; p=0.002). Conclusion: miR-146a-5p was significantly overexpressed in patients with OA, both in the articular cartilage tissue and serum, with a positive correlation between the levels in both types of samples. Therefore, the miR-146a-5p serum level could reflect the molecular processes in the cartilage, suggesting its clinical utility as a biomarker for OA management. Implementing noninvasive biomarker using serum miRNAs involves the analysis of the misregulated miRNAs linked to the cartilage pathology.

Chondrocyte extracellular matrix type II collagen and proteoglycans ensure an important compression-bearing structure in synovial joint. However, much more type I collagen is generated in osteoarthritis, which implies the presence of abnormal tensile strain in cartilage. We hypothesize that tensile stress influences chondrocyte phenotype at the cellular level, leading to potential osteoarthritis. Methods: Chondrocytes were stimulated with cyclic excessive tensile (10%) or mild tensile or compressive strain (5%) at 0.5 Hz, 3 h per day for 3 days. Chondrocyte morphology and matrix proteoglycans level was separately determined by Rhodamine phalloidin and toluidine blue staining. The expression of cartilage marker molecules was measured using quantitative reverse transcription polymerase chain reaction and enzyme-linked immunosorbent assays. Results: Chondrocytes demonstrated significant fibroblastic morphology, reduced proliferation and increased apoptosis following exposure to 10% tensile strain. The 10% tensile strain group induced the lowest matrix proteoglycans level. It observably reduced the expression of COL2A1, Acan and SOX9, and increased COL1A1 expression level. The 5% tensile (5% compression) group, maintained the chondrocyte phenotype. Conclusions: The findings identified the effects of different magnitudes of tensile stress on chondrocyte phenotype compared to compressive strain. Further studies on cartilage biomechanical micro-environment might benefit from this study.

Osteoarthritis (OA) of the knee joints is nowadays a significant medical and economic problem in the world. The percentage of people diagnosed with degeneration of the knee joint is increasing rapidly. Not only does it cause negative social consequences, but also generates large treatment costs. The treatment consists in taking preventative measures and educating patients, as well as pharmacological therapy, physiotherapy and, as a last resort – surgery. Suitable and comprehensive measures can decrease symptoms of gonarthrosis. The research was conducted among 110 patients diagnosed with OA of the knee joints in one of the rehabilitation clinics in Podkarpacie voivodeship. A self-construction questionnaire, the KOOS scale and the VAS scale were the research tools. The respondents were informed about the anonymity of the survey. The research was carried out twice before and after the rehabilitation. Among the respondents, the highest percentage of people with gonarthrosis involved women (74.5%). Overweight and obesity were important factors correlating with the occurrence of gonarthrosis. The cryotherapy (66.4%), exercise in relief (42.7%), followed by laser therapy, magnetronics and individual exercises were considered the most helpful treatments. Electrotherapy and massage were considered as the least effective. Having analysed the result of the KOOS questionnaire and the VAS scale provided to the patients before and after the treatment, the positive effects of physiotherapy have been noted. The patients have noticed improvement in daily activities, quality of life, severity of symptoms and pain relief (p <0.05). The research indicates that physiotherapy is an effective measure, therefore it should be included in the treatment of patients diagnosed with OA.
