Opis:
Introduction. Alcohol consumption is the world’s third largest risk factor for disease and disability. According to the WHO report from 2011: 71% of urban respondents ty and 77% of rural respondents admit to alcohol consumption]. Lower socio-economic status and educational levels result in a greater risk of alcohol-related injury, disease and death. Alcohol is a common component of many medicines, as well as an ingredient in many oral hygiene home products. Mouthwashes containing alcohol are considered to inhibit wound healing in the oral cavity. Due to the fact that many different results are described for different concentrations of alcohol at different times, an attemptwas made to visualise the direct impact of 7.2% and 22% alcohol on human gingival fibroblasts. Materials and method. PANsystem 2000 was used for visualisation of the reaction of human gingival fibroblasts isolated from gingiva on ethanol in 2 different concentrations. PANsys 3000 is a multi-system fully-automated cell culture device used for in vitro culture and to study a variety of cell lines under conditions similar to in vivo. Observations were carried out for 48 hours since alcohol addition. Pictures were taken in a continuous process at 5 minute intervalds and combined into a film. Results. Both contamination of 7.2% and 22% ethyl alcohol negatively affected morphology and cell proliferation. Addition of ethanol at a concentration 7.2% enabled cells to regain their ability to divide and recover normal morphology after 10 hours; changes caused by 22% ethanol, however, were irreversible. Conclusions. The obtained results suggest that daily usage of 7.2% alcohol contained in mouthwashes is non-toxic for gingival fibroblasts, and could be recommended after periodontal surgery.

Opis:
Vitamin D3 (1,25(OH)2D3 (1,25-dihydroxyvitamin D3)) is a hormone playing a crucial role in numerous biological processes in the human body, including induction and control of cell proliferation and differentiation. Numerous data relate the vitamin D3 level with various types of cancer. It has been suggested that SNPs in the vitamin D3 receptor (VDR) gene might influence both the risk of cancer occurrence and cancer progression. The aim of this study was to search for genetic correlations between individual SNPs in the VDR gene and the risk of oral cavity carcinoma. Two SNPs were selected based on the literature and our previous results. Seventy-three patients with squamous cell carcinoma of the head and neck and one hundred control subjects were investigated. Two SNPs in the VDR gene were genotyped in minisequencing reactions followed by capillary electrophoresis. Hardy-Weinberg equilibrium (HWE), the χ2 test and logistic regression were used for statistical analysis. The SNP rs2238135 in the VDR gene displayed statistical differences in frequency between the tested groups (p=0,0007). Furthermore, the G/C genotype of the rs2238135 in the VDR gene was characterized by a 3.16 fold increased risk of oral cavity carcinoma. The obtained results provide evidence for a genetic association between rs2238135 in the VDR gene and the occurrence and risk of oral cavity cancer.

This paper concerns the frequency of occurrence and enzymatic activity of fungi of the genus Rhodotorula isolated from the oral cavity of patients with colorectal cancer. Swabs from the oral cavity were subjected to standard mycological diagnostic procedures; enzymatic activity of the fungi was evaluated using and API Zym test by BioMereux. The fungi of the genus Rhodotorula were noted in 22.5% positive isolates originating from women and 25.7% positive isolates originating from men. They included mono-species isolates of: Rh. glutinis, Rh. minuta, Rh. mucilaginosa and two-species isolates: Rh. minuta + Candida dubliniensis, Rh. mucilaginosa + C. albicans and Rh. mucilaginosa + Saccharomyces cerevisiae. The enzymatic activity of the isolated fungi was evaluated as high and very high in the case of acidic and alkaline phosphatase, esterase lipase, lipase, as well as leucine, valine and cysteine arylamidase. The highest values were determined in Rh. mucilaginosa with co-occurrence of C. albicans and Rh. minuta with co-occurrence of C. dubliniensis. In the ontosphere of oncologic patients, the increasingly frequent appearance of the species of fungi with high and very high enzymatic activity indicates the progressing risk posed by opportunistic forms, which includes Rhodotorula species.

Introduction. Haemophilia A and B are congenital bleeding disorders caused by coagulation factor VIII or IX deficiency. Haemophilia A and B occur in men, like all qualities dependent on genes linked to gender, whereas women are asymptomatic carriers of the gene. The most dangerous manifestations of severe haemophilia include spontaneous haemorrhages into the joints, muscles and body cavities, haematuria and potentially very dangerous intracranial haemorrhages are also quite frequent. The most common manifestations in the region of the maxillofacial skeleton and oral cavity are recurring bleedings from the nose and the gums after dental extractions, as well as extensive haemorrhages from wounds after surgical procedures. Objective. The aim of the study was the analysis of epidemiologic case records of 22 patients affected by various types of haemophilia, admitted to Maxillofacial Surgery Department at Medical University in Lublin, Poland, during 2008–2013. Results. The most numerous group constituted patients diagnosed with Haemophilia A, followed by patients with von Willebrand disease and Haemophilia B, aged 21–40 years. The main cause of hospital admission was the need to perform complex oral cavity sanation. The patients were treated with factor VIII or Factor IX, FEIBA or Heamate P, depending on the type and severity of bleeding diathesis.

Overweight and obesity constitute a serious social problem. They are considered hazards of developed countries. Overweight and obesity affect both adults and children. Numerous researches on the negative impact of obesity on the condition of oral cavity have been conducted. The purpose of this paper is to provide a review of the most recent data published and noted in PubMed database between 2007 and 2014, on the association between overweight and obesity and oral diseases, including caries and periodontal diseases, as well as to discuss their mechanisms. The majority of studies discussed in this paper demonstrate the existence of an association between obesity and the health of the oral cavity. An association between overweight and obesity expressed as various anthropometric indices, and oral cavity conditions should be the subject of further prospective studies.

Introduction. Metastatic renal cell carcinoma can spread from the primary renal cell carcinoma to other organs, practically to all tissues. Distant metastases – to the tissue of the head and neck – are transferred through the blood and lymph vessels. A case of such a metastasis localized on the palate is relatively rare and therefore poses interesting material for description. Objective. Analysis of a case of cell renal carcinoma meastasis of the palate in a 67-year-old patient, based on clinical observations and medical publications. Conclusion. Clear cell carcinoma metastases to distant organs are often the first sign of a covertly progressing disease of which the primary tumour is localized in the kidney. The treatment is surgical, and prognosis is poor in the case of distant metastases since tumours recur even after a few years.

Introduction: The aim of the study was to analyse the risk factors for oral cavity and oropharynx cancer in peopled examined under the Head and Neck Cancer Awareness Week in 2016, Lodz. Material and methods. In Lodz, 21st September 2016, under the Head and Neck Cancer Awareness Week, 106 people, including 67 women aged 29–77 and 39 men aged 23–84, underwent preventive examinations in the hospital department. Prior to the laryngological examination, the patients were asked to answer questions that referred to their education, medical case history, symptoms, smoking habits with the number of cigarettes per day, alcohol intake, the number of lifetime sexual partners, oral sex engagement, incidents of head and neck cancer in the family history. Results: The major part of the examined patients were women and men with the secondary and high level of education, 47,76% and 35,82%, and 58,97% and 35,91% respectively. The patients were informed by mass media about the planned preventive medical examinations – 80,60% women and 79,49% men. The most common symptoms reported by women were: hoarse voice in 61,19% cases, dysphagia in 32,84% cases and burning sensation and/or pain in the oral cavity in 29,85% cases. The examined male patients mainly showed hoarse voice (46,15%), other symptoms (43,59%) and dysphagia (25,64%). 28,35% women and 28,20% men smoked cigarettes, while passive smokers were 22,38% and 25,64% respectively. Alcohol consumption was reported by 67,16% women and 82,05% men, rather occasionally. Having oral sex was noted in 25,37% women and 38,46% men, mostly with multiple sexual partners. Among the studied patients, 13,43% women and 5,12% men suffered from malignant cancer, including 2,98% women and 2,56% men who reported head and neck carcinoma in the medical interview. On the basis of the interview and ENT examination, 11,94% women and 17,94% men were qualified for the extended oncological diagnostics. Conclussion. The Fourth Head and Neck Cancer Awareness Week shows the increased interest in preventive screening, especially oncological screening, and thus the necessity of such preventive activities in the future.

The aim of the date was to determine the sensitivity to chitosan ascorbate microaerophilic bacteria isolated from pathological pockets, root canals and apical abscesses. Chitosan was obtained from Antarctic krill. The experiments included 70 clinical isolates and 4 standards strains. The susceptibility of microaerophilic bacteria was performed by means of plate dilution technique in Brucella agar supplemented with 5% defibrinated sheep blood. Inoculum contained 105 CFU per spot. Incubation inoculated agar was performed at 37°C for 48 hrs in microaerophilic conditions. MIC was defined as the least concentrations of chitosan ascorbate that inhibited growth of tested bacteria. The results indicated that 47% of all date bacteria was susceptible to chitosan ascorbate in ranges ≤ 0.01 – 0.5 mg/ml. The most susceptible were the strains of Corynebacterium matruchotii (100%, MIC ≤ 0.01 – 0.5 mg/ml) and the least sensitive strains of Rothia dentocariosa (MIC in ranges 1.0 ˗ ≥ 4.0 mg/ml ).

Besides their structural catalytic and diverse regulatory functions, proteins are also precursors of many important biological compounds essential for normal functioning of humans. Many of these compounds may be used as markers for identification of specific pathological states. A comprehensive knowledge about the metabolism of salivary proteins and the mechanisms of action of their metabolites allowed the development of effective treatment for many disorders. However, it should not be forgotten that in some pathological conditions, these compounds not only could be involved in the pathogenesis but also could be used as tool in the prediction of many diseases. This paper is a review of the published literature on selected salivary proteins in the context of the physiological processes of the human body and chosen chronic disorders, such as diabetes, diabetic nephropathy, mucositis, oral mycoses and caries.

Helicobacter pylori – Gram-negative rod, discovered more than 30 years ago, has a proven influence on inflammation of gastric and duodenal mucosa. The worldwide prevalence of H. pylori infection in the human population is estimated at 50% and is considered to be one of the most frequent bacterial infections in people. Many studies suggest that infection takes place in the early childhood within the family. The gastric mucosa is its natural habitat. In last time more and more findings about existence of this bacteria in another places of gastrointestinal tract and correlation with many diseases, especially an inflammation of oral cavity. The percentage of H. pylori detectability in the oral cavity ranges from 0 to 100%. Thus, more studies aimed at final determination of the bacterium reservoir in the oral cavity seem to be necessary.

Opis:
Approximately 555,000 new cases of head and neck cancer (HNC) are diagnosed annually worldwide. Moreover, second primary malignancy (SPM) occurs more commonly in the head and neck than in any other body region. A 67-year-old female patient was admitted to our Department for removal of an upper lip tumor. The patient underwent excision of the lip tumor under general anesthesia. Postoperative histopathology revealed a polymorphic, low-grade adenocarcinoma. Three weeks later, the patient was re-admitted to extend the margin of excision and perform lymphadenectomy. On physical examination, a second primary synchronous lesion was found on the left side of the hard palate. Lymphoscintigraphy was performed to determine the lymph node field and the sentinel lymph node. On ultrasonography, the lymph nodes were not enlarged. Extended excision of the upper lip was performed, and a 5-mm, hard palate tumor was removed with a margin of unchanged tissue. The patient was scheduled for adjuvant treatment. The criteria for a synchronous malignancy are as follows: both tumors must be malignant on histopathology, there must be a distance of at least 2 cm of unchanged mucosa between the index tumor and the primary tumor, the possibility that the second tumor is the metastasis of the index tumor needs to be excluded. In our patient, all these criteria were met. A thorough workup should include medical history, physical evaluation, and imaging, and it should be carried out on every step of treatment and during long-term follow-up in patients after cancer treatment. This is crucial, because not only is there a possibility of recurrence or metastases, but also a considerable percentage of patients may have a second primary lesion.
