Opis:
The implementation of a successful ergonomic and medical intervention program designed to reduce the number and severity of injuries and illnesses and the associated levels of discomfort in the workplace is presented. Because of the recent activity concerning the on-again-off-again Occupational Safety and Health Administration (OSHA) Ergonomic Program Standard questions have been raised as to the value and effectiveness of an organization’s ergonomics program. In light of these concerns, the immense cost associated with work-related injury and illness, and the related pain and suffering associated with such injuries and illnesses, it is important to present a workable and effective ergonomic and medical intervention program. The results of this applied study demonstrate that through the application of an ergonomic and medical intervention program, workplace-related injuries and illnesses can be reduced or eliminated.

Case-study research was carried out with a view to find the attributes of occupational injury among workers in the chemical industry and to enhance safety issues. Injury data were collected and processed in terms of different variables, such as age, gender, skills, type of hazard, etc. Pareto analysis was then applied to find a pattern of occupational injury among the workers. The study revealed that 79.52% of the injured workers were in the 40–59 age group; 57.14% of accidents occurred during the 1st shift; 73.26% of accidents caused injury to hands, feet, chest to thigh, arms and eyes; and 70.93% of injuries were caused by pumps, carrying and lifting, vehicles, pipelines, valves, and grinding. Surprisingly, no one was injured in the group of temporary workers. The paper also provides specific suggestions followed by some action plans.

The objective of the study was to describe the characteristics of agriculture and forestry related injury cases admitted to an Emergency Department (ED), and to asses factors related to injury severity and hospital admission. Retrospective analysis of ED case records in Teaching Hospital No 1 in Lublin, from January 2004 to December 2005 were utilized. Inclusion criteria: patient >14 years old, with agriculture and forestry related injuries. Univariate and bivariate descriptive analyses and multiple logistic regressions were performed. 3791 cases were included, 63.1% males, 53.3% cases younger than 30 years and 47.1% of the patients sustained injuries related to machines or falls. After adjusting for age, sex and the presence of multiple injuries, animal related injuries, followed by machine related injuries and falls, were at higher risk of a more severe injury (OR: 1.77, 1.61 and 1.50, respectively). This groups also showed a higher likelihood of hospital admission (OR: 2.03, 192 and 2.00, respectively). Patients attended to in the ED during night hours (OR: 2.06) were also at a higher risk of hospitalization. It was concluded that animal related injuries, agriculture machine operators, and falls, besides accounting for two thirds of analyzed cases, are the mechanism of injury with a greater risk of a more severe injury and higher likelihood of a hospitalization.

Objectives. To assess occupational injuries in the 1st versus 2nd half of a working shift in terms of (a) the likelihood of hospital referral following an injury event; (b) the external causes of injuries. Methods. Logistic regression analyses of data from a 16-year (1991–2007) experience of ongoing surveillance of occupational injuries in a synthetic fiber factory in Iran. Results. The likelihood of a hospital referral following an injury in the 1st half of a shift was higher than in the 2nd half (adjusted odds ratio [OR] 1.50, 95% confidence interval [CI] 1.10–2.00). Comparing the 2 halves of the shift, an injury occurring in the 2nd half was more likely to be due to exposure to smoke, fire and flames (OR 2.34, 95% CI 1.06–5.19) or transport accidents (OR 1.84, 95% CI 1.06–3.21). Conclusions. Time-specific safety interventions could be used in the risk management of occupational injuries. Further studies to investigate the effect of time-dependent interventions are recommended.

Introduction. The purpose of this study was to describe factors of possible importance for the occurrence of hand injury from powered wood splitters. Patients. Patients were identified by a computerized patient registry. Information was obtained from hospital records, a written questionnaire and a structured telephone interview. Results. Very few splitters were constructed according to European standards. Twenty-one percent of patients injured with wedge splitters thought that having more than one person at the machine was one cause of the accident. Seventy-nine percent of patients injured with screw splitters stated that glove use was one cause of the accident. Conclusions. The level of safety in wood splitters that cause hand injury is often poor. Having more than one person at the machine during work may contribute to wedge splitter injury. Glove use commonly contributes to screw splitter injury. Prevention should be directed towards unsafe machines and dangerous patterns of use.

Introduction: Medical students undertake clinical procedures which carry a risk of sharps injuries exposing them to bloodborne infections. Objectives: To study the prevalence and correlates of sharps injuries among 4th-year medical students in the Faculty of Medicine, University of Colombo, Sri Lanka. Materials and Methods: The survey was conducted among 4th-year medical students to find out the incidence of injuries during high-risk procedures, associated factors and practice and perceptions regarding standard precautions. A self-administered questionnaire was administered to a batch of 197 4th-year medical students. Results: A total of 168 medical students responded. One or more injury was experienced by 95% (N = 159) of the students. The majority (89%) occurred during suturing; 23% during venipuncture and 14% while assisting in deliveries. Most of the incidents (49%) occurred during Obstetrics and Gynecology attachments. Recapping needles led to 8.6% of the injuries. Thirty-five percent of students believed they were inadequately protected. In this group, adequate protection was not available in 21% of the incidences and 24% thought protection was not needed. Following the injury, 47% completely ignored the event and only 5.7% followed the accepted post-exposure management. Only 34% of the students knew about post-exposure management at the time of the incident. Only 15% stated that their knowledge regarding prevention and management was adequate. The majority (97%) believed that curriculum should put more emphasis on improving the knowledge and practice regarding sharps injuries. Conclusions: The incidence of sharps injuries was high in this setting. Safer methods of suturing should be taught and practiced. The practice of standard precautions and post-injury management should be taught.

Occupational injuries requiring admission to a trauma unit were examined to outline the events surrounding the injury and to examine the costs. Sixty-nine patients were admitted over a 12-month period, representing 4.30% of all work-related injuries attending the emergency department and 4.25% of all admissions to the trauma unit. Most were male (91%), working in skilled trade occupations (65%), with a mean age of 38.8 years. Personal protective equipment was used only by 46% of injured workers who should have been using it. Sixty-one percent of patients believed that their injury was preventable. Half of the injuries were to the upper limb, fall was the most frequent mechanism (25%) and the median duration of admission was 2 days. The direct hospital costs were estimated at over 300 000 GBP. Failure to use protective equipment and to follow health and safety guidelines suggests that opportunities exist for injury prevention.

This study used OWAS to analyze the working postures of construction workers on building the foundations of a log cabin. Three construction workers, with an average work experience of 40 years, participated in this study. Eight elementary jobs of building the foundations of a log cabin were videotaped at a construction site and analyzed later in the laboratory. For an overall distribution of trunk postures, OWAS identified that a bent and twisted trunk posture (34%), which fell into action category 3, was the major poor posture for construction workers. This study also identified that tying beams with steel bars, assembling column templates, and cement grouting of the ground were the 3 principal jobs in which workers building the foundations exhibited poor working posture. This article suggests ways to reduce and evaluate poor posture in a dynamic construction site.

Although under-reporting of work-related injuries by workers is recognized as a significant problem in construction and other industries, little is known about the specific reasons for such occurrences. Qualitative and quantitative methods were used in this study to (a) identify reasons why construction workers may choose not to report work-related injuries, and (b) to investigate the frequency of the identified reasons. Twenty-seven percent of a sample of construction workers (N = 135) indicated that they had failed to report a work-related injury. The most frequent reasons given were related to perceptions of injuries as "small" and "part of the job" as well as fear of negative consequences, which may follow injury reporting. These findings are discussed in terms of practical implications. Strategies to overcome these reasons are suggested to decrease the under-reporting of injuries in the construction industry.

Twenty-one risk factors affecting laborers in manual materials handling tasks were analyzed to determine what, if any, statistically significant relationships existed between the factors and the emergence of occupational back injury. The statistically significant risk factors (p ≤.05) in the univariate analysis were determined to be weight lifted per hour (work intensity), trunk twists per hour, weight lifted per day, frequency of lift, trunk motions per hour, and trunk flexions per hour, with odds ratios (ORs) of 1.28-2.88. In addition, self-reported discomfort in the neck, middle back, knees, and lower back was associated with the outcome of back injury (p ≤.05, OR 1.75-2.66). In the multivariate analysis, the statistically significant risk factors (p ≤.05) were weight lifted per hour (work intensity), average weight of lift, and number of trunk twists per hour, with ORs of 1.74-4.98.

Objective: The role of occupational hazards in occupational injury may be mediated by individual factors across various age groups. This study assessed the role of occupational hazards as well as contribution of individual factors to injuries among Indian and French coalminers. Material and Methods: We conducted a case-control study on 245 injured workers and on 330 controls without any injuries from Indian coal mines using face-to-face interviews, and a retrospective study on 516 French coalminers using a self-administered questionnaire including potential occupational and personal factors. Data were analyzed using logistic models. Results: The annual rate of injuries was 5.5% for Indian coalminers and 14.9% for the French ones. Logistic model including all occupational factors showed that major injury causes were: hand-tools, material handling, machines, and environment/work-geological/strata conditions among Indian miners (adjusted oddsratios 2.01 to 3.30) and biomechanical exposure score among French miners (adjusted odds-ratio 3.01 for score the 1-4, 3.47 for the score 5-7, and 7.26 for score ≥ 8, vs. score 0). Personal factors among Indian and French coalminers reduced/exacerbated the roles of various occupational hazards to a different extent depending on workers' age. Conclusion: We conclude that injury roles of occupational hazards were reduced or exacerbated by personal factors depending on workers' age in both populations. This knowledge is useful when designing prevention which should definitely consider workers' age.

Background Occupational risk of blood-borne infections is investigated mostly among nurses and doctors, studies concerning non-clinical health personnel (nCHP) being rare. The analysis of the occupational exposure to the hepatitis B virus (HBV) infection and the history of vaccination against the HBV in the nCHP group has been the aim of the study. Material and Methods A retrospective analysis of 458 cases of the occupational exposure to biological agents was conducted: group I – doctors (N = 121, 28%), group II – nursing staff (N = 251, 55%), group III – nCHP (N = 86, 19%). Results In the group III the source was usually unknown (group: I – 0.83%, II – 11.16%, III – 86.05%, p < 0.001), and the proportion of individuals vaccinated against hepatitis B before the exposure was the lowest (group: I – 98.35%, II – 97.19%, III – 77.91%, p < 0.001). In this group most exposures resulted from injuries caused by needles/sharps deposited in waste sacks (60%) or anywhere outside of the medical waste container (5%). The possibility of the HBV infection risk during the exposure was found in 25 cases and was significantly more frequent in the group III. The qualification for the HBV post-exposure prophylaxis was also significantly more frequent in the group III. Conclusions The exposure to the occupational risk of the HBV infection also concerns the non-clinical healthcare personnel. The non-clinical healthcare personnel comprises one of the main groups of the HBV post-exposure recipients. It is essential to determine the causes of the low hepatitis B vaccination coverage in the nCHP and consider introduction of mandatory vaccination in this group in Poland. Med Pr 2016;67(3):301–310
Wstęp Narażenie zawodowe na zakażenia krwiopochodne bada się przede wszystkim u pielęgniarek i lekarzy, rzadko u innych grup zawodowych pracujących w placówkach opieki zdrowotnej. Celem badania była ocena ryzyka zawodowego narażenia na zakażenie wirusem zapalenia wątroby typu B (WZW B) i realizacji szczepień przeciw WZW B u pracowników pomocniczych zatrudnionych w Zespołach Opieki Zdrowotnej (ZOZ). Materiał i metody Retrospektywną analizą objęto 458 osób eksponowanych zawodowo na materiał biologiczny: lekarzy – grupa I (N = 121, 28%), personel pielęgniarski – grupa II (N = 251, 55%) i personel pomocniczy – grupa III (N = 86, 19%). Wyniki W grupie III pacjent będący źródłem zakażenia personelu (pacjent źródłowy) był najczęściej nieznany (grupa: I – 0,83%, II – 11,16%, III – 86,05%, p < 0,001), a odsetek wykonanych szczepień przedekspozycyjnych najniższy (grupa: I – 98,35%, II – 97,19%, III – 77,91%, p < 0,001). Badani z grupy III byli narażeni na zakażenie WZW B najczęściej poprzez skaleczenie/zakłucie ostrym narzędziem znajdującym się w worku ze śmieciami (60%) lub innych miejscach poza pojemnikiem na odpady medyczne (5%). Ryzyko zakażenia WZW B stwierdzono łącznie u 25 osób, jednak istotnie częściej występowało ono w grupie III. Także osoby z tej grupy badanej najczęściej kwalifikowano do profilaktyki poekspozycyjnej zakażenia WZW B. Wnioski Pracownicy pomocniczy zatrudnieni w służbie zdrowia również są zawodowo narażeni na zakażenie WZW B. Stanowią oni jedną z głównych grup pacjentów, u których stosuje się profilaktykę poekspozycyjną zakażenia WZW B. Konieczna jest ocena przyczyn niższego odsetka szczepień przeciw WZW B u pracowników pomocniczych niż u lekarzy i personelu pielęgniarskiego i wprowadzenie obowiązku szczepień w tej grupie w Polsce. Introduction. The study examines the problem of dominant arm function loss in rural adult patients due to work-related accidents. The types of risks involved in farmyard work include falling from a height, manually moving loads, overturning/ accident whilst driving an agricultural tractor, noise and vibration, use of pesticides, and the risk of being cut or injured. The study focuses on adaptation of the non-dominant arm. Objective. The main aim of the study was evaluation of visual-motor coordination on the basis of performance of the nondominant hand in patients after the loss of function of the dominant arm. Materials and method. The research sample consisted of 52 patients with a permanent or temporary loss of function or severely limited function of the dominant arm. The subjects were patients with arm amputations due to various occupational injuries sustained while operating agricultural and construction machinery and forestry equipment, following traumas or complicated medical surgeries of the arm, or due to car accidents. The following tests were applied in the analysis: I) Dufour cross-shaped apparatus test for assessing visual motor-coordination; II) paper-and-pencil tests and the Relay Baton motor fitness test; III) anthropometric measurements; IV) Edinburgh Handedness Inventory; and V) a questionnaire survey. Results. The results of the apparatus and motor tests indicate the same tendency: reaction to stimuli measured on the basis of performance of the non-dominant arm is longer in shorter and older patients. Conclusions. Visual-motor coordination, as measured by the performance of the non-dominant arm, is significantly affected by the subject’s body height and arm length.
