Metal-working fluids (MWFs) are used in machining and grinding operations to cool the tool and work, reduce the friction between the tool and work, improve the surface integrity of the work piece, and increase tool life and productivity. Health problems have been reported among workers exposed to MWFs, including incidences of respiratory, digestive and skin cancers, and increased rates of cough and phlegm. This paper reviews and discusses issues concerning health risks from exposure to MWFs in machining and grinding operations, the various factors that influence the degree of exposure, and control methods to reduce exposure to metal-working fluids.

Bladder cancer (BCa) and prostate cancer (PCa) are genitourinary cancers which constitute significant health problems in men and in which environmental factors play an important role. Understanding the genetic susceptibility to BCa or PCa and occupational exposure is paramount to improving cancer prevention and early detection. The aim of this review article was to address the scientific evidence on the genetic risk factors and occupational exposure associated with the occurrence of BCa and PCa. The authors identified relevant original articles that have been published between 1994 and 2023. Variations of the following search terms: “gene” and “occupational” combined with one of the following terms: “bladder cancer” or “prostate cancer” were applied for the search purpose. The authors found 342 publications of which 50 population studies met their requirements for gene-occupation interactions. In total, 34 full-text manuscripts were about BCa and 16 about PCa. These research examines the genes involved in detoxification processes of xenobiotics (glutathione S-transferase, N-acetyltransferase, cytochrome P450, UDP-glucuronosyltransferase), oxidative stress (glutathione peroxidase 1, manganese superoxide dismutase, catalase), altering DNA repair capacity (X-ray repair cross-complementing 1, base excision repair, nucleotide excision repair), tumour suppression (TP53 gene), and vitamin D pathway (vitamin D receptor gene). The role of genetic factors in the occupational exposure has not been conclusively established, but it appears the possibility of genetic involvement. Determination of environmentally responsive genes provides important mechanistic implications for the etiology of occupational cancers, and valuable input in occupational exposure limits set by taking genetic susceptibility into account. Introduction: The number of occupational diseases (OD) recorded in Poland in the 1990's rapidly increased, and the numer of recognized cases has steadily decreased until now. Hence, it was decided to demonstrate the trends of selected pathologies which in Poland are "underestimated" in comparison to other countries. The presented data may constitute a basis for further research into the dependence of OD on socio-economic factors. Materials and Methods: Occupational Disease Reporting Forms, completed and sent obligatorily by the state health inspectors to the Central Register of Occupational Diseases were used as source documents for analysis. This work analyzes changes in the incidence of chronic poisonings, asbestosis, voice organ diseases, cancers, viral hepatitis, asthma and the musculoskeletal disorders over the years 1998-2011. Results: In 1998, the total number of registered diseases reached the maximum - 12,017 cases, which fell in the subsequent years to 2,562 cases in 2011. During that period, the incidence rate decreased by 6 cases per year per 100,000 employees. A considerable decrease, exceeding 90% of cases, was observed in voice organ disorders, hearing loss, chronic poisonings and viral hepatitis. The abovementioned changes, as well as improved detection of asbestos-related diseases through implementing a medical examination program of former asbestos processing plant workers, are advantages of the current situation in the epidemiology of OD. However, the disadvantages include underestimation, in comparison to other countries, of asthma, cancer and pathologies of the musculoskeletal system. Conclusion: The reported data indicates the need to assess the occupational fraction of the underestimated pathologies present in the work environment in Poland, as well as the need for studies aimed at clarifying the effect of systemic factors on identifying their occupational background.

Objectives: The objective of this work was to identify the high-risk occupations in Iran and to re-inspect occupations that were related to bladder cancer. Materials and Methods: In the study, 300 patients suffering from bladder cancer and 500 control individuals were interviewed. Demographic information, occupational history, and history of exposure to chemical compounds such as aromatic amines for each participant were collected. ORs and 95% CIs were calculated using unconditional logistic regression for each occupation. Results: There was a significantly increased risk of bladder cancer among truck and bus drivers (OR = 11.3), skilled agricultural, forestry and fishery workers (OR = 6.0), metal industry workers (OR = 6.0), domestic housekeepers (OR = 5.9), and construction workers (OR = 3.8). Conclusions: The study showed a strong correlation between truck and bus drivers, skilled agricultural, forestry and fishery workers, metal industry workers, domestic housekeepers, as well as construction workers and the increased risk of bladder cancer in these occupations.

Objective: To demonstrate an occupation exposure impact on incidence of cancer diseases among nickel smelter workers in Sereď, Slovakia. To analyze the occurrence of cancer diseases among smelter workers. To make a risk assessment and survival analyses in selected group of workers. Methods: Retrospective cohort study is based on data about outcomes of work-related exposures in the present and also on the date about known nickel exposure in the past among smelter workers. Observed historical cohort group were 4622 workers in the nickel smelter in Sereď (Galanta district) who were employed during the years 1963 - 1993. Odds ratio (OR), logistic regression and survival analyses were calculate. Results: There were 1348 non-exposed employees (29%) and 3274 exposed employees (71%). There were 4222 employees without recorded cancer (91%) and 400 employees had recorded cancer disease (9%). Among cancer cases we can assess that they mostly work on manufacture department (52% of cases). The most cases were employed in nickel smelter over the 20 years (42%). The 36% of cases were first time employed in nickel smelter at the age of 31 - 40 years old. With increasing age to start work is increasing chance to be sick (OR = 1,48, CI95% = 1,30 - 1,69, P<0,001). Furthermore, with increasing duration of employment is also higher chance to have cancer (OR = 2,23, CI95% = 2,05 - 2,41, P<0,001). Also the higher risk exposure has impact on higher chance to have cancer (OR = 1,46, CI95% = 1,22 - 1,75, P<0,001). Conclusions: Presented study was the first study, which discussed aspects of the possible occupational exposure in the nickel refinery in Sereď in the Slovak Republic.
Cel: Zaprezentowanie wpływu narażenia zawodowego na występowanie chorób nowotworowych u pracowników huty niklu w Sereď, Słowacja. Analiza występowania przypadków zachorowań na raka wśród pracowników huty. Ocena ryzyka zawodowego i analiza przeżycia wybranej grupy pracowników. Metodyka: Retrospektywne badanie kohortowe w oparciu o dane na temat wyników narażeń związanych z obecnym wykonywaniem pracy zawodowej jak i na podstawie rozpoznanych przypadków narażenia zawodowego pracowników huty w przeszłości. Historyczne badania cohortowe obejmowały grupę 4622 pracowników huty niklu w Sereď (okręg Galanta), zatrudnionych w latach 1963 - 1993. Obliczono iloraz szans, regresję logistyczną oraz wykonano analizę przeżycia. Wyniki: Badana grupa obejmowała 1348 (29%) pracowników nie narażonych oraz 3274 narażonych (71%). U 4222 (91%) pracowników nie rozpoznano choroby nowotworowej, raka stwierdzono natomiast u 400 pracowników (9%). Pośród pracowników z wykrytą chorobą nowotworową większość (52% przypadków) była zatrudniona w dziale wytwórczym, przy czym 42% przypadków to osoby o stażu zatrudnienia w hucie niklu przekraczającym 20 lat. 36% zatrudnionych ze stwierdzoną chorobą nowotworową rozpoczęło pracę w hucie w wieku 31 - 40 lat. Wraz ze wzrastającym wiekiem zatrudnienia w hucie wzrasta ryzyko zachorowania na raka (OR = 1,48, CI95% = 1,30 - 1,69, P<0,001). Ponadto, im dłuższy staż pracy tym większe ryzyko raka (OR = 2,23, CI95% = 2,05 - 2,41, P<0,001). Ryzyko to wzrasta też wraz z wydłużeniem okresu narażenia (OR = 1,46, CI95% = 1,22 - 1,75, P ? 0,001). Wnioski: Zaprezentowane badania stanowią pierwszą próbę omówienia aspektów potencjalnego narażenia zawodowego występującego w hucie niklu w Sereď, Słowacja.

In the past, 2-naphthylamine (2-NA) was used for the production of azo dyes, as an antioxidant in the cable industry and in the rubber industry. Despite the fact that 2-NA is not produced on an industrial scale, it is still used in small quantities as a model bladder carcinogen in laboratories, and also for sewage control, water analysis and oxytocinase assays. In addition, it is detected in the air in coke ovens, where it is formed as one of the pyrolysis products. The main aim of this work is to provide an actual literature review for health risk assessments related to 2-NA which is still used in laboratories. Occupational exposure to 2-NA is important for the respiratory tract, mucous membranes and the skin, and, to a lesser extent, for absorption from the gastrointestinal tract. It is absorbed into the body through the skin and by inhalation, and then undergoes metabolic changes. Most of the absorbed 2-NA dose is excreted in the urine, in the form of metabolites, metabolites conjugated to acids, and even in an unchanged form. Based on literature data, the effects of 2-NA toxicity in sub-chronic and chronic exposure include contact dermatitis, chronic cystitis and bladder cancer. The authors have concluded that it is recommended to determine the occupational exposure limit which will allow preparing the exposure assessment of people at work. Med Pr. 2020;71(2):205–20

ObjectivesEpidemiological data on cancer diseases are alarming. The workplace has become an increasingly important site for disseminating health information and implementing health promotion activities. Occupational medicine physicians (OMPs) have the opportunity to carry out primary and secondary preventive activities focused on civilization diseases, especially cancer. The aim of this study was to evaluate the potential of OMPs in cancer prevention, including the analysis of factors determining the implementation of preventive measures, as part of standard healthcare for employees.Material and MethodsThe study was conducted among 362 OMPs. The interviews were carried out by the computer assisted telephone interview (CATI) method.ResultsOver 60% of the surveyed OMPs are ready to implement cancer preventive activities among employees. The doctors with the longest seniority in occupational health services are more likely to declare unwillingness to implement cancer preventive activities. Patient’s consent, informing women about the program and adjusting the time of the medical visit are the most important conditions for introducing cancer prevention programs by OMPs. Neither seniority nor the number of examinations performed by a physician influenced the currently implemented cancer preventive activities as part of occupational health services (including the evaluation of cancer risk factors occurrence among employees).ConclusionsIn Poland, OMPs are willing to implement cancer preventive activities among employees, but their current activity in this area is limited and needs development. The most specific actions should be addressed to doctors with the longest seniority in occupational health services, who are frequently unwilling to implement cancer preventive activities. Strengthening the preventive potential of Polish occupational health services requires a systemic approach to the scope and way of action of healthcare professionals.

Objectives The correlation between quartz dust concentrations in the Swiss enterprises and the incidence of silicosis and other related diseases acknowledged as occupational diseases (OD) was investigated. Material and Methods Quartz dust concentrations were obtained from Suva’s databases of occupational health surveillance measurements between 2005 and 2014. Information on quartz dust-related diseases was from medical dossiers of workers with OD acknowledged by Suva between 2005 and 2014. Results The median quartz dust concentration of the 2579 measurements between 2005 and 2014 was 0.09 mg/m³ (alveolar fraction). Out of all measurements, 28% were above the Swiss occupational exposure limit (OEL) of 0.15 mg/m³ (alveolar fraction). One hundred eighty-one individuals suffered from acknowledged quartz dust-related disease (179 silicosis and 2 chronic obstructive pulmonary disease (COPD)). Additionally, 8 out of these workers were diagnosed with lung cancer and 55 with COPD of a non-specified cause. Out of all workers, 46% were exposed to silica dust for the first time before 1975 when the current Swiss OEL was introduced. Out of the foreign workers, 63% began to work abroad, during which they could have at least partly acquired their silicosis. Out of all workers, 75% were ever-smokers. Conclusions The incidence of silicosis decreased drastically from approximately 300 cases/year in the 1970s to fewer than 20 cases/year 20 years ago. Several findings of this study that could help to interpret the ongoing occurrence of the disease include excessive exposure in or outside of Switzerland in former or current times, vulnerability to the development of silicosis due to cigarette smoke, or poor compliance with wearing breathing masks. Objectives Occupational exposure to asbestos is associated with increased mortality which, however, has not been thoroughly validated in a general population. We have aimed at exploring whether this association may be confirmed within a population-based setting after adjustment for confounders. Furthermore, the impact of tobacco consumption on the association between occupational exposure to asbestos and mortality is assessed. Material and Methods We used data from 2072 (224 exposed) male participants of the Study of Health in Pomerania. Information on exposure to asbestos is based on a selfreport. Median follow-up time was 11.3 years. All-cause mortality and cause-specific mortality of exposed and non-exposed men were compared using mortality rate ratios, Kaplan-Meier analyses and multivariable Cox regression. Results During the follow-up, 52 (23.2%) exposed and 320 (17.3%) non-exposed participants deceased. Exposed subjects had increased hazard ratios (HR) for all-cause mortality (HR=1.48, 95% CI: 1.1–2), benign lung disease mortality (HR=3, 95% CI: 1.18– 7.62) and stomach cancer mortality (HR=4.59, 95% CI: 1.53–13.76). The duration of exposure (per 10 years) was associated with all-cause (HR=1.21, 95% CI: 1.07–1.36) and benign lung disease mortality (HR=1.68, 95% CI: 1.26–2.22). Smokers occupationally exposed to asbestos had the highest risk for all-cause (HR=3.70, 95% CI: 2.19–6.27) and cancer mortality (HR=4.56, 95% CI: 1.99–10.48) as compared to non-asbestos exposed non-smokers. Conclusions Our results confirm associations of occupational exposure to asbestos with all-cause, benign lung disease, and stomach cancer mortality and underline the impact of joint effects of asbestos and smoking on mortality.

ObjectivesThe aim of this study is to conduct a quantitative assessment of the risk related to occupational exposure to wood dust and the occurrence of sinonasal epithelial cancer (SNEC) by histotype, depending on the duration of exposure, using mathematical models.Material and MethodsThe relationship between the occurrence of SNEC by histotype (adenocarcinoma [AR] and tumors of other histotypes) and exposure to wood dust has been studied using the values of the odds ratios for individual periods of employment involving exposure to wood dust, and their 95% confidence intervals. The dose-response curves were constructed (more precisely, the duration of exposure-response curves). The author attempted to match the linear, quadratic or exponential models.ResultsIn all SNEC cases, there is a relationship between the duration of occupational exposure to wood dust and the relative risk of developing cancer. The estimated relative risk of developing AR after 35 years of exposure to wood dust is about 300, and the estimated risk of developing SNEC is 50, compared to non-exposed people for whom the relative risk is equal to 1. However, the relative risk of developing other types of cancer is <20, also in comparison with non-exposed people.ConclusionsThe author has identified a relationship between the duration of occupational exposure to wood dust and the relative risk of developing cancer in all SNEC cases.

This sudden and accidental leakage of deadly poisonous methyl isocyanate (MIC) gas instigated research efforts to understand the nature, severity of health damage and sufferings of 570 000 ailing survivors of this tragedy. In a decade-long period, our systematic laboratory investigations coupled with long-term molecular surveillance studies have comprehensively demonstrated that the risk of developing an environmental associated aberrant disease phenotype, including cancer, involves complex interplay of genomic and epigenetic reprogramming. These findings poised us to translate this knowledge into an investigative framework of “molecular biodosimetry” in a strictly selected cohort of MIC exposed individuals. A pragmatic cancer risk-assessment strategy pursued in concert with a large-scale epidemiological study might unfold molecular underpinnings of host-susceptibility and exposureresponse relationship. The challenges are enormous, but we postulate that the study will be necessary to establish a direct initiation-promotion paradigm of environmental carcinogenesis. Given that mitochondrial retrograde signaling-induced epigenetic reprogramming is apparently linked to neoplasticity, a cutting-edge tailored approach by an expert pool of biomedical researchers will be fundamental to drive these strategies from planning to execution. Validating the epigenomic signatures will hopefully result in the development of biomarkers to better protect human lives in an overburdened ecosystem, such as India, which is continuously challenged to meet population demands. Besides, delineating the mechanistic links between MIC exposure and cancer morbidity, our investigative strategy might help to formulate suitable regulatory policies and measures to reduce the overall burden of occupational and environmental carcinogenesis.

W Polsce pracownicy podlegają obowiązkowym okresowym badaniom lekarskim. Stwarza to unikalną szansę wdrożenia działań nakierowanych na wczesne wykrywanie wielu schorzeń, w tym chorób nowotworowych, w system opieki profilaktycznej nad pracującymi. Dane epidemiologiczne dotyczące chorób nowotworowych są niepokojące – co więcej, prognozuje się dalszy wzrost zachorowań na nowotwory złośliwe w populacji generalnej. Najczęstszym nowotworem wśród polskich kobiet jest rak piersi odpowiadający za 21,7% zachorowań, podczas gdy nowotwory złośliwe trzonu macicy, jajnika i szyjki macicy stanowią, odpowiednio, 7,4%, 4,7% i 3,5% zachorowań. Celem niniejszej pracy było opracowanie algorytmu działań profilaktycznych zintegrowanych z opieką profilaktyczną nad pracownikami, bazującego na przeglądzie piśmiennictwa medycznego oraz obowiązujących wytycznych dotyczących zapobieganiu wybranym typom nowotworów. W publikacji przedstawiono funkcjonujące programy profilaktyczne chorób nowotworowych i ich czynniki ryzyka. Opracowano także praktyczne rozwiązania pozwalające na włączenie profesjonalistów zajmujących się ochroną zdrowia pracowników w profilaktykę niektórych typów nowotworów występujących u kobiet. Med. Periodical medical examinations are mandatory for employees in Poland. This rule makes a unique opportunity during occupational health services for implementation of prophylactic activities focused on early diagnosis of various diseases, including cancers. Epidemiological data about cancers is alarming and what is more, further increase in development of cancers is being predicted in population overall. The highest incidence of cancers in the case of Polish women belongs to breast cancer (21.7% of diagnosed cancers in general), while the morbidity rate for uterine cancer, ovarian cancer and cervical cancer amounts to 7.4%, 4.7% and 3.5%, respectively. The aim of this study was to elaborate an algorithm of prophylactic activities integrated with the occupational healthcare system, based on medical literature review and guidelines concerning prophylaxis of selected cancers. Polish cancers’ prophylaxis programs related to risk factors were presented in this publication and practical indications for occupational healthcare physicians were worked out. Objectives This study has researched the significance of histologically raised findings and lung dust analyses in the context of claiming the recognition of and thus compensation for an asbestos-associated occupational disease. Material and Methods For this approach, all findings from the German Mesothelioma Register in 2015 that included lung dust analyses were evaluated and were compared with information on asbestos fiber exposure at work based on fiber years, and with the results of radiological findings. Results For 68 insured persons, recognition of an asbestos-induced lung disease according to Section 4104 of the German Ordinance on Occupational Diseases (Berufskrankheitenverordnung – BKV) could be recommended solely on the basis of the histological examinations of lung tissues and complementary lung dust analyses. Neither did the calculation of the cumulative asbestos dust exposure at work yield 25 fiber years, nor could bridge findings (e.g., plaques) be identified. In addition, the autopsies of 12 patients revealed plaques that had not been diagnosed during radiological examinations. These results show that – irrespective of the prescribed working techniques and radiological diagnosis – pathological/anatomical and histological diagnostics are often the only way for the insureds to demonstrate the causal connection between asbestos and their disease. Even after long intervals of up to 40 years post last exposure, the asbestos fibers would still be easily detectable in the lung tissues evaluated. Conclusions Whenever suitable tissue is available, it should be examined for mild asbestosis with the aid of a lung dust analysis. Otherwise there is a risk that an occupational disease is wrongfully rejected. In the context of health insurance, the lung dust analysis and the resulting proof of the presence of asbestosis often constitute one option of providing evidence of an occupational disease.
