    This case study uses three different frameworks of inquiry to examine Turtles All the Way Down by John Green (2017) with a disability lens. The analysis extends beyond the traditional medical/social dichotomy and considers how disability is tied to both agency and identity. Narratives and counter-narratives of disability are also investigated, as well as disability markers used in previous scholarship. The discussion concludes with an argument to include the novel in secondary English classes to create mental health allies.  A consideration for medical humanities scholars is also included to use Green’s text with patients with OCD, as a way for readers to find an identifiable protagonist.

    Opis:
    Objectives Obsessive-compulsive disorder (OCD) is an anxiety-spectrum disorder that affects 1–2% of the adult population. People with OCD are more likely to report impaired social and occupational functioning. Although effective treatments of the OCD exist, many sufferers from this disorder are continuously misdiagnosed. Therefore, improving the assessment of the OCD remains an important area of scientific research. The main goal of the study is the initial verification of psychometric properties in the Polish version of the Obsessive-Compulsive Inventory-Revised (OCI-R) in a college student sample. Material and Methods A group of students completed a battery of measures consisting of obsessive-compulsive symptoms (The OCI-R, The Yale-Brown Obsessive Compulsive Scale), depression (The Beck Depression Inventory) and anxiety trait (The State-Trait Anxiety Inventory). Results A confirmatory factor analysis, conducted on data from 334 university students, supported a solid and replicable 6-fold factor structure of the OCI-R. Further analyses on test-retest reliability (following a 1-month interval), convergent and divergent validity of the OCI-R were respectively conducted in a group of 137 students who had completed a battery of measures mentioned above. The results showed adequate testretest reliability for the full scale and subscales cores, high internal consistency and confirmed satisfactory convergent and divergent validity. Conclusions The study constitutes the first phase of work on a Polish version of measurement for obsessive-compulsive symptoms. Satisfactory results obtained in a non-clinical sample allow to recognize this method to be promising for further research. Int J Occup Med Environ Health 2016;29(6):1011–1021

    Opis:
    Selective serotonin reuptake inhibitors (SSRIs) are the frontline class of antidepressant drugs. They are widely used because of their safety, tolerability, and demonstrated efficacy. In this review, we are pointing at other indications of SSRIs – apart from depression.This article is based on a selective review of the relevant literature retrieved by a PubMed search and ScienceDirect search. The results of electronic searches show SSRIs to be of great importance in pharmacotherapy of many conditions outside depression. Medical literature supports the use of SSRIs for the treatment of premature ejaculation (especially short-acting SSRI - dapoxetine), eating disorders (especially bulimia nervosa and binge eating disorder), obsessive-compulsive disorder and social anxiety disorder.

    W artykule omówiono chorobę skrupułów i nerwicę eklezjogenną oraz przedstawiono ich podobieństwo do zaburzeń obsesyjno-kompulsyjnych opisanych w Międzynarodowej Klasyfikacji Chorób ICD-11. Jednocześnie ukazano najważniejsze różnice między chorobą skrupułów i nerwicą eklezjogenną a zaburzeniami obsesyjno-kompulsyjnymi, z których to różnic wynikają znaczące trudności, zarówno w terapii jak i z punktu widzenia chorego. We wstępie artykułu przedstawiony został opis zaburzeń obsesyjno-kompulsyjnych oraz zaproponowano autorski schemat dynamiki przebiegu objawów tego zaburzenia. W dalszej części w ten schemat zostają wpisane objawy typowe dla choroby skrupułów i nerwicy eklezjogennej oraz zostają omówione największe trudności w ich leczeniu.
    This article discusses scrupulosity disorder and ecclesiogenic neurosis and shows their similarity to obsessive-compulsive disorder described in the International Statistical Classification of Diseases ICD-11. It shows the most important differences between the scrupulosity disorder, ecclesiogenic neurosis and obsessive-compulsive disorder, the differences which result in difficulties in therapy and from the perspective of the patient. At the beginning, the article presents a description of obsessive-compulsive disorder and proposes a diagram of the dynamics of the symptoms of this disorder. Later, in this diagram the symptoms typical of the scrupulosity disorder and ecclesiogenic neurosis are included, and the difficulties in their therapy are discussed.

    Aim: The aim of this study was to evaluate the coexistence of obsessive-compulsive symptoms with bipolar disorder (during the manic phase, depressive phase and remission). Method: The subjects were 70 patients previously diagnosed with and treated for bipolar disorder. For the purposes of this study, three subgroups were created: patients in the manic phase, depressive phase and in remission. The Hamilton Depression Rating Scale, Young Mania Rating Scale and Yale-Brown Obsessive Compulsive Scale were diagnostic tools used for the evaluation of patients’ mental health. Results: The data indicate high likelihood of co-occurrence of obsessive-compulsive disorder (28.6%) and obsessive-compulsive syndromes (32.8%) with bipolar disorder. Obsessions and compulsions were observed irrespectively of the type of bipolar disorder (type 1 and 2) and phase of the illness (depression, mania, remission). The results in the three subgroups were similar. The severity of anankastic symptoms depended both on the severity of depression and mania. The subjects confirmed the presence of obsessive-compulsive symptoms in the interview, although they were usually undiagnosed and untreated. Conclusions: Obsessive-compulsive disorder symptoms often coexist with bipolar disorder, both in its two phases and in remission. The severity of obsessive-compulsive symptoms in the course of bipolar condition varies, ranging from mild to extremely severe forms. The obsessive-compulsive disorder presentation in the course of bipolar disorder increases with the severity of depressive and manic symptoms. Obsessive-compulsive disorder can be primary to bipolar disorder. Obsessive-compulsive disorder coexisting with bipolar disorder is not diagnosed or treated properly.
    Cel pracy: Zbadano współwystępowanie zaburzeń obsesyjno-kompulsyjnych w przebiegu choroby afektywnej dwubiegunowej (w fazie depresji, manii i w stanie remisji). Metoda: Badaniu zostało poddanych 70 chorych już wcześniej zdiagnozowanych i leczonych w kierunku choroby afektywnej dwubiegunowej. Z grupy wszystkich badanych wyodrębniono trzy podgrupy: badani będący w fazie maniakalnej, depresyjnej i remisji choroby afektywnej dwubiegunowej. Do oceny stanu chorobowego użyto skali depresji Hamiltona, skali manii Younga i skali zaburzenia obsesyjno-kompulsyjnego Yale-Brown. Wyniki: Stwierdzono istotną możliwość współwystępowania zaburzeń obsesyjno-kompulsyjnych (28,6%) i zaburzeń o łagodniejszym przebiegu (obsessive-compulsive syndromes) (32,8%) w przebiegu choroby afektywnej dwubiegunowej. Natręctwa występowały niezależnie od typu choroby afektywnej dwubiegunowej (I i II) i fazy chorobowej (depresja, mania, remisja), a wyniki w trzech badanych grupach były zbliżone. Intensywność objawów anankastycznych zależała zarówno od nasilenia objawów depresyjnych, jak i maniakalnych. Badani potwierdzili obecność natręctw w wywiadzie chorobowym, chociaż najczęściej nie były one zdiagnozowane i leczone. Wnioski: Natręctwa często współwystępują z chorobą afektywną dwubiegunową zarówno w fazach chorobowych, jak i w remisji. Stopień nasilenia zaburzeń obsesyjno-kompulsyjnych w przebiegu choroby afektywnej dwubiegunowej jest różny – od form łagodnych po skrajnie ciężkie. Prezentacja zaburzeń obsesyjno-kompulsyjnych w przebiegu choroby afektywnej dwubiegunowej zwiększa się zarówno z nasileniem objawów depresyjnych, jak i maniakalnych. Zaburzenia obsesyjno-kompulsyjne mogą poprzedzić rozwój choroby afektywnej dwubiegunowej. Zaburzenia obsesyjno-kompulsyjne nie są właściwie współdiagnozowane, a tym samym nie są leczone w przebiegu choroby afektywnej dwubiegunowej.

    The aim of this article is to discuss possible directions of interdisciplinary studies on obsessive-compulsive disorder. It is proposed that while examining the cognitive basis of the disorder one’s beliefs about the way cognitive processes work and the actual functioning of these processes should be taken into consideration, with executive functions in particular. Interdisciplinary studies combining psychiatry, clinical psychology and cognitive sciences may not only improve the understanding of the nature of cognitive processes, but also provide important data on how they are associated with specific symptoms of mental disorders. From a broader perspective such an approach may be potentially useful in developing new therapeutic techniques.

    Introduction. Researchers who study the problems of Internet addiction point out that this dependence is often co-morbid with symptoms of a variety of pathological disorders, including anxiety, depressive, somatization, and obsessive-compulsive disorders. The goal of this study was to compare the severity of psychopathological symptoms in individuals at risk of Internet addiction (according to Young’s criteria) and those not at risk of developing this addiction with respect to gender and place of residence (urban vs. rural). Materials and method. The study included a group of 692 respondents (485 females and 207 males). The average age of the participants was 20.8 years. 56.06% of them lived in urban areas and 43.94% in rural areas. The following instruments were used: a sociodemographic questionnaire designed by the authors, Young’s 20-item Internet Addiction Test (IAT, Polish translation by Majchrzak and Ogińska-Bulik), and the “O” Symptom Checklist (Kwestionariusz Objawowy “O”, in Polish) by Aleksandrowicz. Results. Individuals at risk of Internet addiction showed significantly more severe pathological symptoms than the individuals who were not at risk of this addiction. There were differences in the severity of psychopathological symptoms between people at risk of Internet dependence living in urban and rural areas. Conclusions. Individuals at risk of Internet addiction were found to be characterized by a significantly higher severity of obsessive-compulsive, conversion, anxiety, and depressive symptoms. Persons at risk of Internet addiction who lived in rural areas had significantly more severe psychopathological symptoms, mainly obsessive-compulsive, hypochondriac and phobic, compared to their urban peers.

    The article presents the correlations between eating disorders (anorexia nervosa and bulimia nervosa) and the obsessive-compulsive disorder. Basing on a survey of literature, the comorbidity was discussed along with the spectrum of obsessive-compulsive disorders to which some authors include also the eating disorders. Mutual characteristics of eating disorders and other mental disorders classified among the spectrum of obsessivecompulsive disorders are presented. These include: approximate age when the first symptoms occur, chronic course of the disorders, sometimes similar personality traits, as well as certain disorders within neurotransmitters, similar changes found in neuropicture tests, maybe also genetic conditions, and a positive response to psychotropic drugs affecting, e.g. the serotoninergic system. However, basing on the current knowledge we cannot state explicitly that the most important mutual characteristic, i.e. the mutual genetic-structural-biochemic pathology, indicates clearly that the given disorders belong to one spectrum of mental disorders. The problem is still open and we can expect that it can be solved by further studies of biological conditions.
    W artykule przedstawiono związki między zaburzeniami jedzenia (jadłowstrętem psychicznym i bulimią psychiczną) a zaburzeniem obsesyjno-kompulsyjnym. Omówiono na podstawie przeglądu piśmiennictwa współchorobowość oraz koncepcję spektrum zaburzeń obsesyjno-kompulsyjnych, do którego niektórzy autorzy kwalifikują również zaburzenia odżywiania się. Przedstawiono cechy wspólne dla zaburzeń odżywiania i innych zaburzeń psychicznych zaliczanych do spektrum zaburzenia obsesyjno-kompulsyjnego. Należą do nich: zbliżony wiek wystąpienia pierwszych objawów, przewlekły przebieg, czasem podobne cechy osobowości, a także niektóre zaburzenia w zakresie neuroprzekaźników, podobne zmiany stwierdzane w badaniach neuroobrazowych, być może również uwarunkowania genetyczne, jak i pozytywna odpowiedź na leki psychotropowe, działające na przykąłd na układ serotoninergiczny. Na podstawie wiedzy, którą dysponujemy aktualnie, nie można jednak z całą pewnością stwierdzić, że najważniejsza cecha, jaką jest wspólna patologia genetyczno-strukturalno-biochemiczna, wskazuje bez wątpliwości, że dane zaburzenia należą do jednego spektrum zaburzeń psychicznych. Problem pozostaje otwarty i oczekiwać można, że dopiero kolejne badania uwarunkowań biologicznych mogą go rozstrzygnąć.

    Perfekcjonizm rozumiany jako cecha osobowości, defi niowany jest najczęściej w ramach modeli unidymensjonalnych, dychotomicznych lub wieloczynnikowych. Dominujące podejścia teoretyczne koncentrują się nie tylko na dysfunkcjonalnych, negatywnych aspektach perfekcjonizmu związanych z symptomami psychopatologii, ale też wskazują na istnienie pozytywnego, adaptacyjnego wymiaru perfekcjonizmu. Celem artykułu jest omówienie i analiza koncepcji teoretycznych oraz dotychczasowych wyników badań empirycznych poświęconych obydwu formom perfekcjonizmu. Szczególną uwagę poświęcono analizie czynników modyfi kujących związek perfekcjonizmu z symptomami psychopatologii.
    Perfectionism as a personality trait is defined in the frame of unidimensional, bidimensional or multifaceted models. The dominating theoretical approaches not only concentrate on the dysfunctional, negative aspects of perfectionism related to the symptoms of psychopathoplolgy, but they also indicate the presence of a positive, adaptative dimansion of perfectionism. The aim of the article is to discuss and analyze theoretical concepts and the results of previous empirical research concerning both forms of perfectionism. Special attention was focused on the analysis of the factors modifying the relation between perfectionism and symptoms of psychopathology.

    The article presents the obsessive-compulsive disorder (OCD) as it is understood by the particular therapeutic approach – the cognitive-behavioural therapy (CBT). The disorder involves the appearance of recurring, persistent thoughts and acts (physical and mental ones) performed in response, with an aim of minimizing the anxiety triggered by these thoughts. The first part of the article presents the DSM-5 diagnostic criteria for the OCD. The second part introduces the general model of cognitive-behavioural therapy. The third part explains the disorder and its development using the CBT models. After the commentary on the effectiveness of the OCD treatment using the CBT, some specific techniques and the model of clinical practice are introduced. The article briefly deals with the topic of pharmacotherapy. It focuses on the CBT techniques based on the exposure to aversive triggers and holding back of reactions (E/RP), the gradual desensitization, behavioural experiments as well as the cognitive techniques focused on changing the dysfunctional interpretation.
    Artykuł przedstawia problem zaburzenia obsesyjno-kompulsyjnego (OCD) w kontekście terapii poznawczo-behawioralnej (TPB). Zaburzenie to polega na występowaniu nawracających, uporczywych myśli i wykonywanych w reakcji na nie czynnościach (fizycznych lub mentalnych) minimalizujących niepokój związany z tymi myślami. W pierwszej części artykułu zaprezentowane są kryteria diagnostyczne zaburzenia według klasyfikacji DSM 5. Druga część wprowadza w model terapii poznawczo-behawioralnej. Kolejna zaś ujmuje zaburzenie i jego rozwój w modelach poznawczo-behawioralnych. Po odniesieniu się do skuteczności leczenia zaburzenia obsesyjno-kompulsyjnego w opisywanym nurcie, przedstawione są specyficzne techniki terapeutyczne i model postępowania klinicznego. Artykuł sygnalizacyjnie ujmuje zagadnienie farmakoterapii, nie rozwijając go jednak.
