Opis:
Introduction: The laryngeal cancer is the most frequently diagnose malignancy in head and neck region. The highest morbidity is within male patients in the age range between 50 and 70 years. The multicenter study, coordinated by Oto-laryngology Department of Medical University of Warsaw, was designed to investigate the epidemiology of laryngeal cancer in Poland from 1980 and to analyze the evolution of diagnostic and therapeutical procedures over the years.Material and methods: There was performed retrospective analysis of the medical records of patients with laryn-geal cancer hospitalized and treated in 12 otolaryngology centers in Poland from 2001 to 2010. The Microsoft Access 2003 (SP 2) platform was used to collect the data and subsequent statistical analysis. Results: There were collected data from 4124 patients, 3682 men (89,3%) and 442 women (10,7%). The largest group consisted of patients in the age range between 50 and 60 years (41,5%) and the second large group was of those be-tween 60 and 70 years (29,6%). The history of heavy cigarettes smoking was obtained from 81,3% of patients. Re-garding the staging of laryngeal cancer, there were 1634 patients with cancer stage of I or II, including 5 patients with carcinoma in situ and 2490 patients with III or IV stage. The most frequent localization of the cancer was the glottis, followed with invasion of all three laryngeal levels and tumors occupying both the epiglottis and glottis. The major-ity of patients – 1367 (33%) – has the T3 tumor advancement. The lymph nodes metastases (N) were present in 1216 (29,5%) patients and among them the N2b and N2c advancement ( lymph node larger than 6 cm, multiple) was de-tected in 533 of patients (13%). The lymph nodes involvement occurred in majority within advanced tumors of T3 or T4a. Considering the treatment options they varied depending on the staging of laryngeal cancer. The sole surgical procedures were performed in 73,7% of patients with cancer stage of I or II and in only 28.6% of patients with the stage of III or IV. The advanced tumors were in majority treated with the combined therapy: surgery and radiotherapy. The overall survival in patients with cancer stage of I and II was 64% and 61% for those with stage III and IV.Conclusions: The rate of laryngeal cancer detection is quite high in Poland, however the period from the onset of symptoms until diagnosis should be reduced. The effectiveness of surgical treatment is definitely unsatisfactory. The project to create a multicenter permanent base for monitoring the course of diagnosis and treatment in patients with laryngeal cancer surely will verify the procedures and enable to achieve better results. Continuation of this project is a task for all otorhinolaryngologists.KEYWORDS:laryngeal carcinoma, epidemiology, clinical characteristics, multicenter study

Opis:
Background In the assessment of healthcare processes focusing on the quality of care provided, patient satisfaction is an important indicator that healthcare providers may use for future setting of healthcare and preventing adverse events. The study aimed to determine satisfaction with nursing care among hospital inpatients. Material and Methods The sample comprised 14 023 patients staying in medical and surgical wards of 14 acute care hospitals in the Czech Republic in 2019–2020. Data were collected using the Patient Satisfaction Scale, a standardized tool containing 11 items in 3 subscales. Data analysis included descriptive statistics and correlation analysis (Spearman’s rank correlation coefficient). Results Overall, patients reported high satisfaction with nursing care (M = 3.57). Patients were most satisfied with how their technical/rational needs were met (M = 3.57); the least satisfaction was identified in the domain of information needs (M = 3.53). Patients who perceived their health as good (47%) or very good (18%) showed high satisfaction scores (M = 3.77 and M = 3.73, respectively). High scores were also achieved for patients with secondary (M = 3.58) and tertiary (M = 3.59) education, those whose admission was planned (M = 3.59) and those staying in large hospitals (M = 3.60). There were no differences in satisfaction with regard to gender (p = 0.755) and the COVID-19 pandemic (p = 0.190). Conclusions Patients’ satisfaction with care provided is a highly significant parameter of healthcare quality. It is influenced by a number of aspects which, if adequately defined, may aid in improving the quality of care.

Opis:
Exact pretherapeutic staging is considered to be essential for decision-making in the therapeutic algorithm of gastric cancer. The aim of the study was to characterize the role and value of EUS in the diagnostic and therapeutic management of gastric cancer in daily surgical practice.Material and methods. Thousand one hundred thirty nine patients with primary gastric cancer from 80 hospitals of each profile of care were enrolled in this systematic clinical prospective multicenter observational study over a time period of 12 months. The characteristics of the diagnostic management, in particular, of EUS were documented. The preoperative EUS findings were compared with the T stage (T1 to T4) and the N category (N+ or N-) revealed by the histopathologic investigation of the surgical specimen. By the mean of X2 test, the impact of EUS on the therapeutic decision-making was determined.Results. Pretherapeutic EUS was only performed in 27.4% (n=312) of all patients. Overall, the diagnostic accuracy for the T stage was 42.6% in average. The subgroup analysis showed the following results: T1, 31.5%; T2, 42.6%; T3, 65.2%; T4, 17.6%. The correct predictive value of the N category was 71.3% reaching a sensitivity of 69.7% and a specificity of 73.3%. Overstaging was observed in 45.8%, understaging in only 10.8%. Additional diagnostic information by EUS was only provided in 4.7% of subjects.Conclusions. The present study indicates the variability, limited reliability and only moderate acceptance of EUS in diagnosing gastric cancer in daily practice. In particular, the prediction of the T stage does not reach the data reported in the literature, which were mostly achieved in specific EUS studies.
