Opis:
ObjectivesIt is generally held that exposure to both high-pressure and long-lasting contact with water makes diving a potentially hazardous sports activity as far as the ears are concerned. There is a number of research investigating the condition of the middle ear in a short period following diving; however, the knowledge regarding the long-term effects of regularly repeated diving remains limited.Material and methodsThe aim of this study is to evaluate the function of the middle ear after a diving season in a group of 31 adults diving regularly (1–17 years) by means of the following methods: 1) interview, 2) otoscopy, 3) pure tone audiometry, 4) classic tympanometry, and 5) wideband tympanometry.ResultsPeriodic problems with pressure equalization in the middle ear were observed in 12 individuals (38.7%). In all the analyzed cases, the authors found a normal condition of the external auditory canal and the tympanic membrane in otoscopy, normal hearing in pure tone audiometry, curve type A, and normal gradient in both classic and wideband tympanometry. Conclusions: Safe diving (according to safety precautions) does not have any long-term negative effects on the condition of the middle ear. However, these observations should be verified in a larger group of divers.

Opis:
A new specimen of Lactodens sheni, the only known spalacolestine from the Early Cretaceous Jehol Biota, is reported from the Jiufotang Formation, Liaoning, China. The description focuses on the dental and mandibular morphologies from both the new specimen and the holotype, particularly those that were unknown or poorly known from the holotype when the taxon was established. As revealed primarily by high-resolution computed tomography, morphologies and size gradient of the lower molars and detailed features of the mandibles, such as the masseteric foramen, can be unequivo- cally described. The dental and mandibular morphologies of Lactodens are compared with those of Origolestes lii, also from the Jehol Biota; these two taxa represent by far the best specimens in Spalacotheriidae and Zhangheotheriidae, respectively, and could be used as the representatives of their own groups in future higher-level phylogenetic analysis of mammals. The two taxa display considerable differences in dental and mandibular features, probably indicating a deeper split of spalacotheriids and zhangheotheriids than previously thought. Absence of the Meckelian groove in Lactodens, contrasting to the distinct one that holds a sizable Meckel’s cartilage in adult Origolestes, suggests an independent evolution of the definitive mammalian middle ear within “symmetrodontans”. The morphological gradient in the tooth row and association of the upper and lower dentitions from the same individual animal are also instructive for interpreting molar variations and evolution in “symmetrodontans” and mammals.

This article describes kinematic analysis of tympanic membrane and the ossicles mechanism, which is responsible for transferring vibrations from outer ear to inner ear. The tympanic membrane was presented as a continuous system and analyzed by means of FE method. The ossciles were described by discrete three elements planar model. Velocities and accelerations of key points of the mechanism were calculated. Furthermore, the influence of ossicles length on functioning of the mechanism was tested.

Opis:
The aim of this work was to study the effect of middle ear disorder on round window (RW) stimulation, so as to provide references for the optimal design of RW stimulation type middle ear implants (MEIs). Methods: A human ear finite-element model was built by reverse engineering technique based on micro-computed tomography scanning images of human temporal bone, and was validated by three sets of comparisons with experimental data. Then, based on this model, typical disorders in otosclerosis and otitis media were simulated. Finally, their influences on the RW stimulation were analyzed by comparison of the displacements of the basilar membrane. Results: For the otosclerosis, the stapedial abnormal bone growth severely deteriorated the equivalent sound pressure of the RW stimulation at higher frequencies, while the hardening of ligaments and tendons prominently decreased the RW stimulation at lower frequencies. Besides, among the hardening of the studied tissues, the influence of the stapedial annular ligament’s hardening was much more significant. For the otitis media, the round window membrane (RWM)’s thickening mainly decreased the RW stimulation’s performance at lower frequencies. When the elastic modulus’ reduction of the RWM was considered at the same time especially for the acute otitis media, it would raise the lower-frequency performance of the RW stimulation. Conclusions: The influence of the middle ear disorder on the RW stimulation is considerable and variable, it should be considered during the design of the RW stimulation type MEIs.

Objective: The aim was to determine the clinical characteristics and analysis of surgical treatment of patients with glomus tympanicum hospitalized in the Department of Otorhinolaryngology. Material and methods: Between 2000 and 2015, 27 patients were hospitalized. Age ranged 28-79 years old. The study analyzed data from the clinical examination and the type of surgical technique. Results: The most common symptoms were tinnitus and hearing loss. Resection of the tumor was performed by anterior tympanotomy in 3 (11,1 %), CWU mastoidectomy in 21 (77,8 %), CWD mastoidectomy in 1 (3,7 %), lateral petrosectomy in 2 (7,4 %) patients. Conclusion: Glomus tympanicum have a quite characteristic clinical picture with dominant symptoms of tinnitus, hearing loss and reddening of the tympanic membrane in otoscopic examination. This type of tumors require surgical treatment. Surgical access depends on the size of the tumor in the tympanic cavity. Most of these tumors can be removed using antromastoidectomy, with posterior tympanotomy and hypotympanotomy.

Tympanostomy tube insertion and tympanic membrane incision are two the most frequently performed surgical procedures in otolaryngology, especially in children. The tympanic membrane incision - paracentesis, or myringotomy – is an incision of the tympanic membrane for diagnostic purposes or to allow drainage of pathological secretion from the tympanic cavity. Tympanostomy tube insertion involves incision made in the tympanic membrane and insertion of a ventilation tube (various types and for various periods of time) to improve hearing and aeration of the tympanic cavity. Procedures are performed through the ear canal (transcanal approach), under local or general anesthesia. Complications may occur in some cases of paracentesis and tympanostomy tube insertion.

Opis:
Introduction: Knowledge about the physiology of a healthy middle ear is essential for understanding the activity and mechanics of the ear as well as the basics of ossiculoplasty. Trauma of the epithelial lining of the tympanic cavity as well as the ossicular chain may be the result of chronic inflammation and surgery. Depending on the observed changes of the middle ear lining, there are several types of distinguished chronic inflammatory changes: simple, with cholesteatoma, with the formation of inflammatory granulation tissue, in course of specific diseases. Purpose: The aim of the article is presentation of the microstructure and vasculature of the ossicular chain in the Scanning Electron Microscope. Particular attention is drawn to the anatomical aspects of the structure and connections of auditory ossicles as vital elements for reconstruction of the conduction system of the middle ear. Material and method: The analysis covered auditory ossicles standardly removed in accordance with the methodology of the investigated surgical procedures. The preparations were evaluated in a scanning electron microscope. Results: The exposure of bone surface promotes deep erosion. The advanced process of destruction of bone surface in the case of chronic otitis media correlates with a significant degree of damage to both the lining covering the auditory ossicles and that surrounding articular surfaces. Conclusions: (1) The ossicles in the image of the Scanning Electron Microscope are covered with lining. It passes from the surface of the ossicles to the vascular bundles, forming vascular sheaths; (2) Damage to lining continuity on the surface of the auditory ossicles promotes the rapid destruction of bone tissue in the inflammatory process; (3) The dimensions of the individual ossicles are respectively: malleus – 8.36 +/- 0.01, incus – 8.14 +/- 0.0, stapes – 3.23 +/- 0.01 mm. Behavior of the anatomical length of ossicular chain during tympanoplasty appears to be essential to maintaining adequate vibration amplitude of the conductive system of the middle ear.

Opis:
This manuscript intends to review types, pathogenesis, associated risk factors, and potential methods of prevention and treatment of the retraction pockets in adults and children. The importance of retraction pockets (RP) lies in loss of original histological and anatomical structure which is associated with development of ossicular chain erosion, cholesteatoma formation and potentially life threatening complications of cholesteatoma. The trans-mucosal exchange each gas in the middle ear (ME) is towards equalizing its partial pressures with the partial pressure in the environment. MEs that have abnormalities in the volume and ventilation pathways in the epitympanic may be more susceptible to retraction pockets. Sustained pressure differences and/or inflammation leads to destruction of collagen fibers in the lamina propria. Inflammatory mediators and cytokines lead to release of collagenases result in viscoelastic properties of the lamina propria. The process of changes in the tympanic membrane structure may evolve to the cholesteatoma formation. There are many different staging systems that clinicians prioritize in their decision making in the management of RP. The authors discuss the management possibilities in different clinical situations: RP without and with ongoing or intermittent evidence of Eustachian Tube Dysfunction (ETD), presence of adenoid hypertrophy or re-growth of adenoids, presence or absence of effusion, invisible depth of RP without effusion. invisible depth of RP with effusion, ongoing RP after VT insertion, and finally suspicion of cholesteatoma in a deep RP with ME effusion. A decision algorithm regarding the management of TM retraction and retraction pockets is provided.

Purpose: In this work, a finite element study is proposed to evaluate the effects of the transducer and its coupling layer on the performance of round window (RW) stimulation. Methods: Based on a set of micro-computer tomography images of a healthy adult’s right ear and reverse engineering technique, a coupled finite-element model of the human ear and the transducer was constructed and verified. Then, the effect of the cross-section of the transducer, the elastic modulus of the coupling layer, the mass of the transducer, and the preload of the transducer were studied. Results: The increase of the transducer’s cross-section area deteriorates the RW stimulation, especially at the lower frequencies. This adverse effect of the cross-section area’s increase of the transducer can be reduced by adding a coupling layer between the transducer and the RW. However, the coupling layer’s improvement on the RW stimulation is reduced with the increase of its elastic modulus. Moreover, the mass loading of the transducer decreases the RW stimulation’s performance mainly at higher frequencies and applying a static preload on the transducer enhances its hearing compensating performance at higher frequencies. Conclusions: The influence of the transducer’s mass, the mass of the transducer, the applied static preload and the properties of the coupling layer must be taken into account in the design of the RW stimulation type implantable middle ear hearing device.
