Objectives Malignant mesothelioma is closely associated to asbestos exposure. One such exposure may occur through contact with occupationally exposed household members and their belongings. This study examines the features of pleural mesothelioma attributable only to asbestos brought home by another family member. Material and Methods The data sources were 1063 mesothelioma cases diagnosed between 1995 and 2014, from the Friuli Venezia Giulia Mesothelioma Register. In all cases the diagnosis of mesothelioma was based on the pathology report. Exposure information and demographic data were acquired by an occupational medical standardized questionnaire/interview. Results Household-exposure mesothelioma cases included 33 women and 2 men. Relationships were: wives (N = 22), daughters (N = 9), sons (N = 2), and mothers (N = 2). Asbestos exposure in the workers predominantly occurred in shipyards. Out of the 35 pleural cases, 19 were epithelial, 9 biphasic, 3 sarcomatoid, and 4 not specified. The mean age at diagnosis was 77 years old. The mean latency was 59 years, with wives having a significant shorter latency than offspring. Latency was not significantly related to morphology and asbestosis. The overall mean survival was 16 months (median 11 months) but treatment was beneficial (mean 16 months vs. 7 months). Biphasic/sarcomatoid histology and presence of asbestosis were associated with a decreased survival, although not with statistical significance. Conclusions Our data confirms that household exposure increases the risk for pleural mesothelioma amongst women with no history of occupational asbestos exposure. This is an ongoing problem in many countries, as well as in Italy, where the evaluation of a framework for the compensation of these cases is under debate. Malignant pleural mesothelioma is a rare neoplasm with poor prognosis. Surgical complete resection, which is the only radical therapy available, is only possible in a minority of patients who suffer from a locally advanced disease. Radiotherapy can be considered as an adjuvant therapy after surgery or as supportive treatment in palliative care. Attempts are also made at combining it with chemotherapy. In cases of locally advanced, non-resectable or metastatic disease, chemotherapy remains the treatment of choice. The most effective palliative chemotherapy is the pemetrexed plus cisplatin regimen. We describe 3 cases of patients who underwent standard palliative chemotherapy with pemetrexed and cisplatin, manifesting good radiological and clinical response, followed by maintenance therapy with pemetrexed. The use of pemetrexed maintenance therapy helped achieve many years of disease control with acceptable toxicity. The consecutive stages of therapy were continuously discussed with our patients, and their informed consent was obtained. Pemetrexed maintenance therapy is not a standard procedure, but recent findings suggest it may be an efficacious option to consider in selected groups of patients. Further randomized prospective studies are needed, but a limiting factor is the rarity of the disease.

The aim of this study is to extract the hidden patterns by using data mining and artificial intelligence techniques. The concept of artificial neural network depends on the idea that we can imitate the working of human brain by making the right links. Artificial Intelligence has always helped in many research areas including medical diagnosis. One of the basic methodologies for training and testing a network by utilizing medical information is discussed here. We have used SAS for analyzing our data and applying feed forward and back propagation mechanism for our diagnosis. The feed forward neural network with back propagation algorithm can be used to identify the diseased ones among different set of admitted individuals. In this paper, we have used multi-layer neural network to achieve the best performance with the minimum epoch (training iterations) and training time.

Objectives Malignant mesothelioma (MM) is the most serious asbestos-related disease. Its increasing incidence is alarming, suggesting the need for as early diagnosis as possible. This 4.5-year prospective longitudinal study aimed at assessing the benefit of measuring serum mesothelin as a marker for diagnosing malignant mesothelioma in individuals with previous occupational exposure to asbestos, as a part of their clinical follow-up care. Material and Methods The study comprised 309 participants (235 males, 74 females) with a mean age of 58.9 years (standard deviation (SD) = 9.8) and a mean duration of exposure to asbestos dust of 13.4 years (SD = 9.3). From 2009 to June 2013, all subjects were followed at a department of occupational medicine in Olomouc. Apart from the standard parts of medical examination (history, physical examination, simple chest radiographs and spirometry), the patients’ serum mesothelin levels were determined by the Mesomark immunoenzymatic diagnostic assay. Statistical analysis of the validity of serum mesothelin level measurement was carried out with respect to the diagnosis of MM. Results Among the participants, 16 (5.2%) individuals (14 males and 2 females) were diagnosed with malignant mesothelioma. Based on the detected mesothelin levels, their validity for prediction of malignant mesothelioma was calculated as follows: sensitivity – 0.75, specificity – 0.962, positive predictive value – 0.706, negative predictive value – 0.969, positive and negative likelihood ratios – 19.95 and 0.26, respectively, and diagnostic odds ratio – 76.8, at a 95% confidence interval. Conclusions The high specificity was identified indicating the low false positivity as well. In the case of detecting elevated soluble mesothelin-related peptides (SMRP) levels in formerly asbestos-exposed individuals, the possibility of the presence of MM should be included into the clinical consideration. The high negative predictive value denotes a lower probability of the presence of MM in patients with normal SMRP levels but due to the limiting lower sensitivity this possibility cannot be entirely excluded.

The relationship between asbestos exposure and malignant mesothelioma is no longer disputed, although it is not always easy to trace past occupational exposure. This report describes a case of uncommon asbestos exposure of a small furniture industry worker, who subsequently died of pleural malignant mesothelioma, to stress the crucial importance of a full reconstruction of the occupational history, both for legal and compensation purposes. Sarcomatoid pleural mesothelioma was diagnosed in a 70-year-old man, who was previously employed as a carpenter in a small furniture industry. He worked for about 6 years in the small factory, was exposed to asbestos during the assembly of the furniture inspired by classical architecture, in which asbestos cement tubes were used to reproduce classical columns. During this production process no specific work safety measures were applied, nor masks or local aspirators. No extra-professional exposure to asbestos was identified. This mesothelioma case was investigated by the Public Prosecutor’s assignment that commissioned expert evidence on the legal accountability for the disease. Despite its uncommon expositive circumstance, the length of latency (about 30 years), the duration of exposure, the clinical and histochemical features are all consistent with literature evidence, accounting for the occupational origin of this malignancy.

Międzybłoniak opłucnej należy do najczęściej występują- cych nowotworów opłucnej. Rozpoznaje się go przeważnie u mężczyzn od 50 do 74 roku życia. W Polsce w 2009 roku odnotowano 250 nowych zachorowań na międzybłoniaka. Najczęstszym i najlepiej poznanym czynnikiem wpływającym na rozwój międzybłoniaka opłucnej są włókna azbestu, a w szczególności włókna krokidolitu. Azbest to grupa 6 minerałów, które były dość powszechnie stosowane w przemyśle z uwagi na ich właściwości fiyczne i chemiczne. Wykazano, iż na rozwój międzybłoniaka opłucnej ma wpływ czas oraz intensywność narażenia na włókna azbestu. Szacuje się, że czas ten wynosi od 20 do 40 lat. Po ukazaniu się dowodów na związek między ekspozycją na azbest a rozwojem mię- dzybłoniaka opłucnej, Międzynarodowa Organizacja Pracy uchwaliła konwencję odnoszącą się do bezpieczeństwa przy stosowaniu i wykorzystywaniu wyrobów zawierających włókna azbestu. W Polsce od 2008 roku realizowane są programy dotyczące usuwania materiałów zawierających azbest, jak również profiaktykę oraz wczesne wykrywanie międzybłoniaka opłucnej u osób mających kontakt z azbestem. Innymi czynnikami o udowodnionym wpływie na rozwój międzybłoniaka opłucnej są infekcja Simon Virus 40, genetyczna podatność na szkodliwe działanie erionitu oraz narażenie na promieniowanie jonizujące. Międzybłoniak opłucnej charakteryzuje się stosunkowo niskim odsetkiem całkowitych wyleczeń. Radykalne leczenie chirurgiczne jest możliwe u około 10% pacjentów. Pozostali chorzy poddawani są leczeniu, które ma za zadanie przedłużyć i poprawić ich jakość życia.
Malignant pleural mesothelioma is the most frequently occurring type of pleural neoplasm. It is mainly diagnosed in men between 50 and 74 years of age. In Poland was recorded 250 new cases of this neoplasm in 2009. Asbestos fire, especially crocidolite fire, is the most common and the best known factor predisposing to the development of pleural mesothelioma. Asbestos is a set of 6 minerals, which due to its physical and chemical properties, used to be commonly implemented by industry. It has been proven, that the time and intensity of exposure to asbestos are what affct the formation of pleural mesothelioma. This exposure time ranges between 20 and 40 years. When the correlation between asbestos exposure and development of mesothelioma was fist published, the International Labour Organization adopted a convention regulating safety in the use of products containing asbestos. As of year 2008, Poland implemented programs on asbestos removal as well as prevention and early mesothelioma diagnosis for individuals exposed to asbestos. Other provable factors, that predispose to the development of pleural mesothelioma are infection with Simon Virus 40, genetic susceptibility to erionite and exposure to ionizing radiation. The malignant pleural mesothelioma is characterized by relative low percentage of full recovery. Radical surgical treatment is possible in about 10% cases. Other patients are subjected to treatment aiming to prolong and improve the quality of life.

Objectives Environmental pollution is associated with morbidity and mortality for chronic-degenerative diseases. Recent data points out a relationship between proximity to industrial plants and mortality due to neoplasms. The aim of this study has been to compare mortality due to chronic-degenerative diseases in the area of Tuscany (Bassa Val di Cecina), Italy, characterized by the presence of 2 neighboring municipalities similar in terms of size but with substantial differences in industrial activities: Rosignano (the site of chemical, energy production and waste processing industries) and Cecina (with no polluting activity). Material and Methods Standardized mortality rates for the 2001–2010 decade were calculated; the data of the whole Tuscany was assumed as reference. Environmental levels of pollutants were obtained by databases of the Environmental Protection Agency of Tuscany Region (Agenzia Regionale per la Protezione Ambientale della Toscana – ARPAT). Maximum tolerated pollutant levels set by national laws were assumed as reference. Results In the whole Bassa Val di Cecina, significantly elevated standardized mortality rates due to mesothelioma, ischemic heart diseases, cerebrovascular diseases and Alzheimer and other degenerative diseases of nervous system were observed. In the municipality of Rosignano, a significant excess of mortality for all these groups of diseases was confirmed. On the contrary, the municipality of Cecina showed only significantly higher mortality rates for ischemic heart diseases. Elevated levels of heavy metals in sea water and of particulate matter which contains particles of diameter ≤ 10 mm (PM₁₀) and ozone in air were detected in Rosignano. Conclusions This study shows an excess of mortality for chronic-degenerative diseases in the area with elevated concentration of polluting factories. Proximity to industrial plants seems to represent a risk factor for those diseases. Int J Occup Med Environ Health 2017;30(4):641–653
