Opis:
A new standard of still image compression is characterised in the context of medical applications. Wide spectrum of JPEG2000 features is analysed with respect to its application potential to improve the performance of modern medical services (i.e. telemedicine, PACS, radiology information systems, wireless personal/home health care systems). Image data security techniques, error resilience technologies, client-side interactive Region of Interest (ROI) transmission and decoding (e.g. for teleconsultation with very large radiography exams), and storage of multiple image data sets are considered in detail. Selected tests of coders realized according to parts I and II of JPEG2000 for different modality test images are presented to evaluate the compression efficacy of this standard. Exemplary results of encoding process optimisation by wavelet transform and subband decomposition selection and screen-shots of software interfaces designed for these tests are also presented.

Health informatics is characterized by the need to securely store, process and transmit large amounts of sensitive medical data while ensuring interoperability with other systems. Among many standards used in such systems there are two which have gained interest in recent years and cover most of those needs: openEHR and HL7 FHIR. In this paper, both standards are discussed and compared with each other. The architecture of both systems, the similarities and differences, methods of data modeling and ensuring interoperability were presented.

Opis:
This study was based on observations of 117 patients suffering from motor disturbances. Among them 42 cases with hemiparetic syndrome, majority of them after cerebral stroke, 52 cases affected by acute sciatic neuralgia and 23 patients with recognition of Parkinson - disease symptoms. To the control group 16 healthy adults were selected, from medical staff of clinics. All subjects were examined using pedobarographic equipment - Parotec System for Windows (PSW) [1]. Based on these observations several pattern solutions have been introduced. They concern gait disturbances descriptions in three distinguished neurological diseases. These findings extracted a new data from the PSW records and options with new diagnostic techniques, based on the gait characteristics observation.

The paper presents selected aims of WROVASK project carried on by Regional Specialist Hospital in Wroclaw with cooperation with a number of academic and research centres. The project is of interdisciplinary kind and gathers researchers and experts of number different disciplines, like chemistry, agricultural, and technical sciences, especially mechanics and informatics. Some tasks aiming at development information systems are presented and described such as: data warehouse based reporting system for the hospital, the system supporting assignment management in teleradiology with optimization of its distribution, and intelligent decision support system for automated assessment of washing surgical tools quality.

The Medical information systems evolve constantly. The quality of the medical information system relies on competences, qualifications, and the organization of the participating partners. Those last ones belong to heterogeneous and autonomous information systems. So, it is necessary to assure a permanent cooperation. This paper shows a coherent architectural framework that allows the development of interoperable medical information systems in measure or these systems evolve. The essential idea is to use concepts of multi-agent systems to perform the different activities of medical information system and thus, to adapt the solutions provided by the agent paradigm to solve the different problems encountered while establishing a medical information system.

In the paper, the method of acoustic model complexity level selection for automatic speech recognition is proposed. Selection of the appropriate model complexity affects significantly the accuracy of speech recognition. For this reason the selection of the appropriate complexity level is crucial for practical speech recognition applications, where end user effort related to the implementation of speech recognition system is important. We investigated the correlation between speech recognition accuracy and two popular information criteria used in statistical model evaluation: Bayesian Information Criterion and Akaike Information Criterion computed for applied acoustic models. Experiments carried out for language models related to general medicine texts and radiology diagnostic reporting in CT and MR showed strong correlation of speech recognition accuracy and BIC criterion. Using this dependency, the procedure of Gaussian mixture count selection for acoustic model was proposed. Application of this procedure makes it possible to create the acoustic model maximizing the speech recognition accuracy without additional computational costs related to alternative cross-validation approach and without reduction of training set size, which is unavoidable in the case of cross-validation approach.

In the paper, the method of short word deletion errors correction in automatic speech recognition is described. Short word deletion errors appear to be a frequent error type in Polish speech recognition. The proposed speech recognition process consists of two stages. At the first stage the utterance is recognized by a typical speech recognizer based on forward bigram language model. At the second stage the word sequence recognized by the first stage recognizer is analyzed and such pairs of adjacent words in the recognized sequence are localized, which are likely to be separated by a short word like conjunction or preposition. The probability of short word appearance in context of found words is evaluated using centered trigrams and backward bigram language model for short words prone to deletion. The set of probabilistic language properties used to correct deletions is called here Local Bidirectional Language Model (in contrast to purely forward or backward model used typically in speech recognition). The decision of short word insertion is based on comparison of deletion error probability of the first stage recognizer and the error probability of the decision based only on centered trigrams and backward model. Despite its simplicity, the method proved to be effective in correcting deletion errors of most frequently appearing Polish prepositions. The method was tested in application to medical spoken reports recognition, where the overall short word deletion error rate was reduced by almost 45%.

Opis:
The article undertakes an analysis of decision-making methods used in solving difficult-formalized problems of medical diagnostics and treatment. A formal model of a medical expert system is proposed, with the purpose of selecting medical pharmaceutical treatment systems. The important task at any stage of the medical evaluation is to determine the general condition of the patient, the depth of the pathological process and the operational control of the effec-tiveness of treatment for timely correction. These problems can be attributed to a large class of practical problems in the weakly formalized decision-making process in the treatment of patients.
W artykule przedstawiono analizę metod wspomagania podejmowania decyzji, które są prze-znaczone do rozwiązywania słabo sformalizowanych problemów diagnostyki medycznej i lecze-nia. Zaprezentowano formalny model systemu eksperckiego, którego celem jest usprawnienie sposobu farmaceutycznego leczenia pacjentów. Najważniejsze zadania na każdym etapie oceny medycznej to określenie ogólnego stanu pa-cjenta i głębokości procesu patologicznego oraz prowadzenie kontroli skuteczności leczenia i jego odpowiednia korekta. Zadania te są powiązane z szeroką klasą praktycznych problemów występujących w słabo sformalizowanym procesie podejmowania decyzji dotyczących leczenia pacjentów.

The article presents the method of building compact language model for speech recognition in devices with limited amount of memory. Most popularly used bigram word-based language models allow for highly accurate speech recognition but need large amount of memory to store, mainly due to the big number of word bigrams. The method proposed here ranks bigrams according to their importance in speech recognition and replaces explicit estimation of less important bigrams probabilities by probabilities derived from the class-based model. The class-based model is created by assigning words appearing in the corpus to classes corresponding to syntactic properties of words. The classes represent various combinations of part of speech inflectional features like number, case, tense, person etc. In order to maximally reduce the amount of memory necessary to store class-based model, a method that reduces the number of part-of-speech classes has been applied, that merges the classes appearing in stochastically similar contexts in the corpus. The experiments carried out with selected domains of medical speech show that the method allows for 75% reduction of model size without significant loss of speech recognition accuracy.

In the paper a method of optimal selection of utterances used as command entry-words for voice controlled application is presented. Voice controlled programs seem to be particularly useful in the area of medical informatics, where a physician interacts with a program by voice while operating the medical device or being involved in examinations requiring manual activities. The proposed method selects command words from sets of proposals defined for each command so as to minimize the overall probability of incorrect command recognition. First the entry-word dissimilarity matrix is calculated. The word dissimilarities are evaluated using HMM models consisting of appropriately trained acoustic models of the phonemes constituting words. The trained HMM is used as the sample utterance generator for the word. The artificially created utterance samples are then recognized by speech recognizers created for pairs of words. The estimation of correct recognition probability is used as the word dissimilarity measure. The word dissimilarities are then used to determine the average assessment of words selections that can be used as commands. Selection is created by choosing single word from sets of candidates defined for each command. Finally, suboptimal selection is found by using genetic algorithm. Experiments carried out prove that suboptimal selection of command entry-words can observably increase the accuracy of spoken commands recognition in many cases.

Opis:
The paper concerns the implementation of some diagnostic measures describing gait disturbances produced by neurological diseases. The discussed contribution explains various experiments provided by the authors' works on Parotec System for Windows development [1]. The subject of the investigations was defined by many experiments carried-out at clinics, with gait's characteristic features analysis, involving the fuzzy logic paradigms. The linguistic notations of diagnostic classes are computed dynamically in accordance with disease statistics, provided by the characteristics of the patient under investigation. The comprehensive system was recognised as a friendly user package, which insulates the user from the complex analysis of measures describing the physiological gait characteristic features. What is more, the application has the ability to be tuned more precisely after the data record size has grown. The software-units implementation provided the user with a selftuned system that enriches the application knowledge, during the data acquisition, while the set of the data records is still growing.

In many medical systems a reliable energy sources play an important role. In the case of failure of external energy system, the local electrical supply has to be activated. Such electrical sources should be environmentally friendly, infallible and easy in usage. The fuel cells using hydrogen as an energy source fulfil these conditions. Numerical modelling of such electrical sources is the primary aim of this publication. Influence of the distribution of temperature on the current density and electric potential fields in electrode of the PEM fuel cell is thoroughly considered. The problem is stated as coupled field problem, where partial differential equations describing mass transport are coupled with Laplace equations describing electric potential and temperature distribution. Decision making systems with their ability to learn automatically seem to be very appropriate for performing such tasks. Decision trees provide high classification accuracy with simple representation of gathered knowledge. Those advantages cause that decision trees have been widely used in different areas of medical decision making. In this paper we present characteristic of univariate and multivariate decision tree. We apply those classifiers to the problem of acute abdominal pain diagnosis.

Background. Physicians make up the primary frontline healthcare practitioner group that must be engaged in the implementation of electronic health records (EHRs). This study aimed to explore physician attitudes and readiness towards the implementation of EHR systems in public hospitals. Material and methods. A total of 296 clinicians from four public hospitals in Iraqi Kurdistan with different job titles were included in the study. Results. The physicians reported that top hospital management would give importance to the EHR project (87.5%), include physicians in its implementation (69.6%) and provide adequate training (69.75%). EHRs may negatively affect physician attitudes due to increased control/monitoring of clinical practices by hospital administration (55.4%) or due to security, legal, and ethical concerns (50.0%), which may limit physician autonomy (51.8%). Moreover, physicians reported that EHR may diminish patient confidence in physicians (37.5%), may threaten physician credibility with patients (51.8%), and will likely interfere with doctor-patient interactions (55.4%). The analysis showed that autonomy could be limited due to the interference of EHRs in doctor-patient interactions (p=0.003). Physician concerns with respect to patient relationships included privacy concerns (p=0.008) and compromised physician autonomy (p=0.003). Conclusions. The physicians reported that while hospital management would support the implementation of an EHR system, the physicians had concerns about physician autonomy and potential negative impacts on doctor-patient relationships.
Wprowadzenie. Lekarze są główną grupą służby zdrowia, która musi być zaangażowana we wdrażanie elektronicznej dokumentacji medycznej (EDM). Celem niniejszej pracy było zbadanie postaw i gotowości lekarzy do wdrożenia EDM w szpitalach publicznych. Materiał i metody. W badaniu wzięło udział 296 lekarzy z czterech szpitali publicznych w irackim Kurdystanie, o różnej hierarchii zawodowej. Wyniki. Lekarze deklarowali, że najwyższe kierownictwo szpitala przywiązałoby wagę do projektu EDM (87,5%), zaangażowałoby lekarzy w jego wdrożenie (69,6%) oraz zapewniłoby im odpowiednie szkolenia (69,75%). EDM może negatywnie wpływać na postawy lekarzy z powodu zwiększonej kontroli/monitoringu praktyk klinicznych przez administrację szpitala (55,4%) lub z powodu obaw związanych z bezpieczeństwem, prawem i etyką (50,0%), co może ograniczać autonomię lekarzy (51,8%). Ponadto lekarze stwierdzili, że EDM może zmniejszyć zaufanie pacjentów do lekarzy (37,5%), może zagrozić wiarygodności lekarzy wobec pacjentów (51,8%), a także prawdopodobnie zakłóci relację pomiędzy lekarzem a pacjentem (55,4%). Analiza wykazała, że autonomia lekarzy będzie ograniczona z powodu ingerencji EDM w relację pomiędzy lekarzem a pacjentem (p=0,003). Niepokój lekarzy dotyczący ich relacji z pacjentami wynikał z obawy o zagrożenie prywatności (p=0,008) i ograniczenie autonomii lekarzy (p=0,003). Wnioski. Lekarze deklarowali, że pomimo, iż kierownictwo szpitala poparłoby wdrożenie systemu EDM, mają oni obawy dotyczące ich autonomii i potencjalnego negatywnego wpływu na relacje pomiędzy lekarzem a pacjentem.
