Opis:
Maritime casualty is an event of considerable economic and social impact. For this reason, implemented the reporting systems of accidents at sea, and the Administration was obligated to establish a Commission of Maritime Accidents. On the basis of casualty analysis and reports are developed proposals preventing similar casualties in the future. However, there is no uniform evaluation system which check references of existing regulations and recommendations to the occurred casualties. This paper presents a method to evaluate the used methods of casualty prediction with respect to the real incident and catastrophe.

Opis:
Study presents the analysis of accidents that took place in marine areas and on board of the sea units in 2017, including: vessel carrying passengers, cargo vessels and offshore drilling and production installations, as well as offshore support vessels. The aim of the article is to indicate a wide spectrum of events related to: total losses of vessels, accidents on boards of the vessels and other accidents which happened in marine areas. The study presents numerous data, which were presented in the form of graphs and tables, based on international reports and own experience gained during work experiences. The analysis also referred to data from previous years to show better spectrum of the present situation.
W opracowaniu przedstawiono analizę wypadków jakie miały w 2017 roku miejsce na obszarach morskich, włączając w to: statki przewożące pasażerów, jak i różnego typu towary oraz morskie platformy wiertnicze i wydobywcze, a także statki przeznaczone do obsługi morskich pól eksploatacyjnych. Celem artykułu jest pokazanie szerokiego spektrum czynników oraz zdarzeń związanych ze stratami całkowitymi statków, wypadkami na jednostkach morskich oraz zagrożeniami, które występują na obszarach morskich. W opracowaniu przedstawiono liczne dane, które zostały przedstawione w formie wykresów i tabel, opartych o międzynarodowe raporty oraz własne doświadczenia uzyskane podczas pracy zawodowej.

Opis:
Work shortly analyses statistics of maritime casualties registered by European Maritime Safety Agency (EMSA) for years 2014-2019 and gives short description of chosen collisions that took place in last ten years. Article points out main causes of navigational accidents and the need to keep improving navigators skills and operation of systems available on board to minimalize risk of collision or grounding due to human error. In this work very small sample is presented and author mainly focuses on European waters and incidents investigated by only one maritime authority – Danish Maritime Accidents Investigation Board (DMAIB).

The primary purpose of a marine casualty investigation is to seek to establish the causal factors of the casualty with a view to learning the hard lessons and avoiding a repetition. The broad questions of an investigation: ""who?, what?, when?, where?, why?, and how?"", all help to uncover the facts. The investigation sequence will cover a diverse range of fact-finding activities, amongst which, as often the case, may be a requirement for “conducting specialised studies”. Following the fact-finding stage the typical investigation progresses to analysis of the facts, reaches conclusions and makes recommendations. Keeping an open mind, to avoid premature conclusions, requires the separation of the fact-finding and analysis phases. But the analysis may well help to identify missing pieces of evidence, or different lines of enquiry that may otherwise have gone undetected. As an effective reconstruction tool, a full-mission bridge simulator offers an opportunity to examine a broad spectrum of environmental conditions and vessel characteristics, as well as equipment failures, human factors and operating procedures. A casualty incident can be reconstructed in a real-time simulated environment, to aid more detailed analysis. Within the usual confines of the legal process, comprehension of nautical ‘black magic’ is greatly simplified for non-mariners, by seeing the simulated casualty incident unfold, in real-time or in selected short-time segments.
