Opis:
Introduction and aim. COVID-19 causes an uncontrolled and generalized inflammatory response of the host immune system. Early recognition of the disease and early prediction of the clinical course are of great importance. The aim of this study was to evaluate the predictive role of the C-reactive protein/albumin ratio (CAR) and the neutrophil/lymphocyte ratio (NLR) for mortality in patients hospitalized with the diagnosis of COVID-19. Material and methods. The patients, who were hospitalized for COVID-19 and whose CRP, albumin, neutrophil, and lymphocyte levels were documented within the first 24 hours after admission, were analyzed retrospectively. Patients were divided into survivors and non-survivors; the groups were compared. Univariate and multivariate Cox regression models were developed to evaluate the CAR and the NLR as risk factors for mortality in COVID-19 patients. Results. One hundred and thirty patients were included in this study. The mean age of the survivor group (n=114) was 60±16 and 52% were male. The mean age of the non-survivor group (n=16) was 75±13 and 56% were male. In the non-survivor group, the CAR detected at the time of admission to the hospital was significantly higher compared to patients in the survivor group (p=0.026). Conclusion. As a result, the CAR, the NLR and LDH are independent risk factor indicators of mortality in hospitalized patients.

Opis:
The aim was to evaluate inflammatory biomarkers as prognostic factors in patients with triple negativbreast cancer. We have collected data from 143 patients and evaluated using Chi-Squared test, Wilcoxon–Mann–Whitney test and Cox regression. We found a relationship between high neutrophil- to-lymphocyte ratio, platelet-to-lymphocyte ratio, and systemic immune inflammation index and local advancement features: tumor (T3/T4) (P = 0.0001, P = 0.0198, P = 0.0001), positive regional lymph nodes (P = 0.0014, P = 0.0075, P = 0.0206). In the multivariate analysis metastatic disease, worse performance status and high NLR (Hazard ratio: 4.48 [2.05–9.80], P=0.0002; 2.23 [1.24–4.03], P=0.0010; 2.23 [1.24–4.03], P=0.0075) were adverse prognostic factors. High neutrophil-to-lymphocyte ratio with worse performance status turned out an adverse independent prognostic factors.
Celem pracy było określenie wartości prognostycznej markerów stanu zapalnego u pacjentów z potrójnie ujemnym rakiem piersi. Zebrano dane dotyczące 143 chorych i oceniono używając testu chi-kwadrat, testu Wilcoxon–Mann–Whitney oraz regresji Coxa. Stwierdzono związek między wysokim poziomem stosunku neutrofilów do limfocytów, płytek do limfocytów, wskaźnikiem ogólnoustrojowej reakcji immunologiczno-zapalnej a miejscowym stopniem zaawansowania: guz (T3/T4) (P=0.0001, P=0.0198, P=0.0001), dodatnie regionalne węzły chłonne (P=0.0014, P=0.0075, P=0.0206). Wielowymiarowa analiza wykazała, że choroba rozsiana, gorszy stan sprawności oraz wysoki stosunek neutrofilów do limfocytów (Hazard ratio: 4.48 [2.05-9.80], P=0.0002; 2.23 [1.24-4.03], P=0.0010; 2.23 [1.24-4.03], P=0.0075) stanowią negatywne czynniki prognostyczne. Wysoki stosunek neutrofilów do limfocytów oraz gorszy stan sprawności stanowią niezależne negatywne czynniki prognostyczne.

Opis:
Introduction: Lung cancer is the most frequent malignancy in Poland and is the first cause of death in oncology. The aim of the study is to analyze selected parameters of inflammation in lung cancer. Material and methods: 48 patients with lung cancer were included in the study group and 30 persons in the control group. The group was divided according to the level of severity - 19 with low/moderate stage (I-IIIA), 29 with high (IIIB and IV) and to histopatho- logical type - 9 with small cell carcinomas, 39 with non-small cell carcinomas (therein 18 - adenocarcinomas and 21 squamous carcinomas). The concentrations of circulating immune complexes (CIC), C-reactive protein (CRP), neutrophils, lymphocytes and their associated indicators were examined: CRP/CIC, NLR (neutrophil/lymphocyte ratio), PLR (platelet/lymphocyte ratio). Results: The average parameter values were significantly higher among people with lung cancer. The median value of CIC was significantly higher in the group with advanced cancer (0.30 vs 0.23, p<0.01 compare to stage I-IIIA). The median PLR and platelet counts among patients with squamous cell carcinoma were significantly higher compared to adenocarcinomas (215 vs 138.5, p = 0.037, 388.0 thousand/ul vs 277.5 thousand/ul, p = 0.034). Conclusion: Based on the results of our work, we suggested the dominance of a specific humoral response in the more advanced stage of lung cancerogenesis. The process of carcinogenesis is associated with various activities of neutrophils and lymphocytes.
Mediany wskaźnika PLR oraz liczby płytek krwi wśród osób z rakiem płaskonabłonkowym były istotnie wyższe porównując do gruczolakoraka (215 vs 138,5, p = 0,037; 388,0 tys/ul vs 277,5 tys/ul, p = 0,034). Wnioski: Na podstawie wyników pracy zasugerowaliśmy dominację swoistej odpowiedzi humoralnej w bardziej zaawansowanym stadium kancerogenezy płuca. Proces kancerogenezy związany jest z różną aktywnością neutrofili i limfocytów.

Acknowledging the immunostimulation effect of ethanol extract white oyster mushroom to animal experiments which were desensitized with red blood cell of lamb. This study is an experimental study, 14 healthy male swiss Webster mices were used. The subjects are randomly divided into two groups. The first group is a control group and the second group is a treatment group. The immunostimulation effect is assessed by specific immune response to desensitized of lamb’s RBC. Humoral immune response was assessed through a hemagglutination test. Cellular immune response was assessed with a delayed type hypersensitivity test by measuring the thickness of edema of the mice’s foot Specific immune response is assessed through total of blood lymphocyte using flowcytometry method. We found an increase of total blood lymphocyte level on the primary sensitization of placebo and Ethanol extract of white oyster mushroom (EEWOM). After secondary sensitization, the total blood lymphocyte level on placebo and EEWOM showed a significant difference. EEWOM increases the total antibody titre significantly compare to placebo. The level of serum gamma-globulin of treatment mice is significantly higher than placebo. On the 24th and 48th hour after the sensitization of lamb’s RBC, the increase of thickness of the mice's feet which were given placebo and EEWOM have significant difference. EEWOM has an immunostimulating effect towards specific immune response.

Opis:
The Background Delayed graft function (DGF) is a common complication following kidney transplantation and is associated with ischemia-reperfusion injury (IRI). Lymphocytes contribute to the pathogenesis of IRI and ischemia-reperfusion related to delayed graft function. Materials and Methods : 135 Caucasian patients received a kidney graft from deceased heart-beating organ donors. We divided patients into 2 groups - patients with the eGFR>=30 on the 21st day post-transplantation (n=36) and patients with the eGFR<30 on the 21st day post-transplantation (n=99) to assess kidney graft function. We measured serum creatinine levels on 1st and 2nd post-transplant day and preoperative levels of monocytes, lymphocytes, platelets, and neutrophils and their ratios. Results: We have found statistically significant differences between the eGFR<30 and the eGFR>=30 groups in the average lnLymphocytes (0.36 +/-0.6 vs. -0.016 +/-0.74, respectively p=0.004) lnNLR (1,27 +/-0.92 vs. 1.73+/-1.08 p=0.016) lnLMR (1.01 +/-0.57 vs. 0.73 +/-0.64 p=0.02), lnPLR (4.97 +/-0.55 vs. 5.26 +/- 0.67 p=0.023), and CCR 2% (-20.20 +/- 21.55 vs. -4.29 +/- 29.62 p=0.004. In the univariate analysis, factors of lnLymphocytes >=0.22 (OR=0.331 95%CI 0.151-0.728 p=0.006), lnLMR>=1.4 (OR=0.255 95%CI 0.072-0.903 p=0.034) were associated with a worse graft function, while lnNLR>=1.05 (OR=2.653 95%CI 1.158-6,078 p=0.021), lnPLR>=5.15 (OR=2.536 95%CI 1.155-5.566 p=0.02) and CRR2 (OR=3.286 95% CI 1.359-7.944 p=0.008) indicated a better graft function. Conclusion: A higher absolute lymphocyte count (lnLymphocytes) and lnLMR, as well as lower lnNLR and lnPLR were associated with a lower eGFR on the 21st day after kidney transplantation. In the multivariate analysis, CRR2 in combination with either lnLymphocytes, lnNLR or lnPLR improved the accuracy of detecting patients with poor graft function.

Objective. Evaluation of the neutrophil-lymphocyte ratio and mean platelet volume as predictive factors for liver fibrosis and steatosis in HBV patients qualified for antiviral treatment. Materials and method. The study comprised 38 CHB patients who had commenced antiviral treatment, and 20 healthy volunteers who constituted the clinical control group. All patients had their blood count taken and underwent hepatic assessment using transient elastography with CAP (controlled attenuation parameter). Results. It was found that the mean hepatic fibrosis was 8.7 kPa (±8.8) and the mean liver steatosis – 286 db/m (±64). Mean NLR – 2.78(±1.1), whereas in the control group the mean NLR value was 1.64(±0.98). A negative linear correlation (r= -0.34; p=0.035) was found between liver fibrosis and the NLR value in the study group. No correlation was observed between hepatic steatosis and the NLR. Mean MPV – 12.6fl (±3.1), which was considerably higher in the CBH patients than in the control group. A positive correlation (r= 0.79, p= 0.001) was found between MPV and disease severity evaluated with transient elastography. Conclusions. The NLR and the MPV were significantly higher in the CHB patients than in the healthy volunteers. Both the NLR and the MPV can be treated as predictive factors for liver fibrosis in this group of patients.

Opis:
Objective: Despite the progress in the treatment of burn injuries, the mortality rate among seriously ill patients still remains high nowadays. The main causes of fatal cases with extensive deep burns are generalized infectious complications, the major of which is sepsis. According to the authors, 25%–85% of those who died of burn injuries in later periods, died from sepsis. The frequency and severity of infectious complications in patients with burn injuries requires an accurate diagnosis of infection that will determine the tactics of therapeutic and surgical treatment of such patients (Di Lonardo A. et al., 1993). However, the clinical diagnosis of wound infection in patients with burn injuries is quite difficult. Materials and methods: The clinical material included 188 patients with burn injuries. The average age of those patients ranged from 20 to 59 years. Men - 98 (52.1%), women - 90 (47.9%). The survey was conducted from April 2014 to July 2016. Results: In our opinion, the strength of NLR (neutrophil-lymphocyte ratio) is the possibility of implementing this parameter simply by using already available biomarkers (neutrophil count and lymphocyte count). Therefore, this ratio is easy to integrate in clinical practice and cost effective. Conclusion: The application of NLR for the diagnosis of sepsis in patients with burn injuries is the option of choice, since its determination requires only a general blood test.

Wyniki: Stwierdzono statystycznie istotne różnice między grupami eGFR <30 i eGFR 30 w ilości limfocytów (0,36 +/- 0,6 vs -0,016 +/- 0,74; odpowiednio p = 0,004) lnNLR (stosunek neutrofili do limfocytów) (1,27 +/- 0,92 vs 1,73 +/- 1,08; p = 0,016) lnLMR (stosunek limfocytów do monocytów) (1,01 +/- 0,57 vs 0,73 +/- 0,64; p = 0,02), lnPLR (stosunek płytek do limfocytów) (4,97 +/- 0,55 vs 5,26 +/- 0,67; p = 0,023), i CCR 2% (-20,20 +/- 21,55 vs -4,29 +/- 29,62; p = 0,004). W analizie jednowymiarowej, czynniki, takie jak: ilość limfocytów 0,22 (OR = 0,331; 95% CI 0,151–0,728; p = 0,006), LnLMR1,4 (OR = 0,255; 95% CI 0,072–0,903; p = 0,034) były związane z gorszą funkcją przeszczepu, podczas gdy lnNLR 1,05 (OR = 2,653 95% CI 1,1158–6,078; p = 0,021), lnPLR 5,15 (OR = 2,536; 95% CI 1,155–5,566; p = 0,02) i CRR2 (współczynnik redukcji kreatyniny w drugim dniu) (OR = 3,286; 95% CI 1,359 –7,944; p = 0,008) wskazywało na lepszą funkcję przeszczepu.

Opis:
The role of Helicobacter pylori (H. pylori) antigens in driving a specific immune response against the bacteria causing gastroduodenal disorders is poorly understood. Using a guinea pig model mimicking the natural history of H. pylori infection, we evaluated the effectiveness of immature and mature macrophages in promoting the blastogenesis of splenocytes from H. pylori infected and uninfected animals, in response to H. pylori antigens: glycine acid extract (GE), cytotoxin associated gene A protein (CagA), urease A (UreA) and lipopolysaccharide (LPS). Lymphocyte expansion was assessed in 72 h cell cultures, containing: immature or mature macrophages derived from bone marrow monocytes, unstimulated or stimulated with H. pylori antigens for 2 h. The proliferation was expressed as a ratio of [3H]-thymidine incorporation into DNA of antigen-stimulated to unstimulated cells and the DNA damage was determined by DAPI cell staining. TGF-β and IFN-γ were assessed immunoenzymatically in cell culture supernatants. Lymphocytes of control and H. pylori-infected animals proliferated intensively in response to phytohaemagglutinin (PHA) and in co-cultures with immature or mature macrophages treated with CagA or UreA (significantly) and GE (slightly) exluding the cultures containing H. pylori or E. coli LPS. This lymphocyte growth inhibition was related to DNA damage of monocytic cells in response to H. pylori or E. coli LPS and secretion of regulatory TGF-β, but not proinflammatory IFN-γ. Impaired homeostasis of monocytic cell function related to DNA damage and TGF-β release, in response to H. pylori LPS may lead to the suppression of adaptive immune response against the bacteria and development of chronic infection.

Introduction and objective. According to the cancer epidemiology databases, cancer is the second leading cause of death in developing countries. Moreover, the WHO predicts a continuing increase in the incidence of cancer, extending this trend well into the next several decades. Hence, it seems obvious that the prediction of cancer susceptibility and early diagnosis is an important goal for modern biomedical sciences. The aim of this study is to clarify the value of chromosomal damage, capacity for the repair of double-strand breaks (DSBs), polymorphisms in DNA repair genes, and apoptosis as prognostic markers for prostate and cervical cancer. Materials and methods. 30 prostate cancer patients and 30 cervical cancer patients were enrolled into the study. In addition, 30 healthy female donors and 30 healthy male donors served as controls. The following endpoints were investigated: frequency of micronuclei, gamma-H2AX fluorescence, XRCC1 194C>T, XRCC1 399G>A, XRCC3 IVS5–14 A>G, OGG1 326 Ser>Cys polymorphisms and apoptosis. Results. Among all tested factors, only the homozygous variant (Arg/Arg) in XRCC1 (399 Arg/Gln) was strongly associated with prostate cancer risk, and only a low apoptotic response was connected with cervical cancer risk. The presented study confirmed a positive association between the frequency of MN and increased prostate and cervical cancer risk. However, such a biomarker is not cancer specific. In addition, the information gained by analyzing the gamma-H2AX fluorescence, as well apoptosis, had no value for predicting the risk of prostate and cervical cancers. Conclusions. The final conclusion of the study is that cancer susceptibility is a complex phenotype not readily detectable in relatively small studies by functional assays or analysis of SNP in few, selected genes.

Opis:
Background. Current studies have indicated many environmental factors, such as pesticides, that cause immune system disorders through inducing changes in humoral and cellular responses thereby increasing the risk of contracting infectious diseases and cancer. The literature suggests that low exposures to certain organophosphorus pesticides stimulate the immune system, whilst high exposures result in decreased function. Precise mechanisms for the fall in immunocompetence are often unclear, however it can be predicted that the intimate interaction between the nervous and immune systems can potentially lead to toxicity. Objectives. To determine the effects of organophosphorus pesticide, chlorpyrifos that is often used in Poland, on selected immunological responses, such as immune-competent cell proportions formed experimentally in-vivo by cells of Wistar rats during subchronic exposures after 45 and 90 days. Materials and Methods. The test was carried out on ten male and ten female Wistar rats in each of three test groups, who received 3 chlorpyrifos doses for 90 days intragastrically, according to OECD guidelines (No. 401). Two control groups were given olive oil. After completion, the animals were deeply anaesthetised by a mixture of ketamine (Vetaketam) and xylazine (Vetaxym). Immuno-competent cells were profiled by a commercial monoclonal antibody method. In order to measure the dynamics of any changes, the aforementioned immunological responses were investigated after 45 days using the same procedures for obtaining the relevant biological test material. Results. Test animals exposed to chlorpyrifos had altered number of white bood cells which were either increased or decreased relative to controls after 45 and 90 days for all exposure levels used. Conclusions. The study demonstrated changes in white-blood cell (lymphocyte) response profiles, reflecting an immunomodulation although such changes were equivocal, where both suppression and stimulation were observed.
Wprowadzenie. Materiał i metody. Badanie wykonano na samcach (10 szt.) i samicach (10 szt.) szczurów rasy Wistar. Zwierzętom z grupy badanej podawano dożołądkowo chloropiryfos przez 90 dni, zgodnie z zaleceniami OECD (Guideline No 401). Grupa kontrolna otrzymywała oliwę z oliwek. Po zakończeniu eksperymentu zwierzęta poddawano głębokiej narkozie przy użyciu mieszaniny: ketaminy (Vetaketam) i ksylazyny (Vetaxym). W celu określenia dynamiki ewentualnych zmian zbadano wyżej wymienione parametry immunologiczne po 45 dniach doświadczenia przy zachowaniu tych samych procedur uzyskania materiału badawczego. Wyniki. Wnioski. Odnotowane w wyniku przeprowadzonych badań zmiany w profilu białokrwinkowym świadczące o działaniu immunomodulującym były niejednoznaczne przyjmując formę zarówno immunosupresji jak i immunostymulacji.

Opis:
Liver resection is essential part of colorectal cancer liver metastases (CLM) treatment. Mean 5-year overall survival after resection achieves 30-45%. There are many factors influencing long-term outcomes, and among them the inflammatory response to tumor plays an important role. The aim of the study was evaluation of outcomes and treatment safety of patients with metastatic colorectal cancer to the liver with estimation of prognostic factors. Material and methods. 130 consecutive patients (70 men and 60 women) operated in MSC Institute and Cancer Center in Gliwice from 2001 to 2009 due to colorectal liver metastases were analysed. Age of the patients ranged from 33 to 82 years (median 60 years). 96 (74%) patients underwent potentially radical resection, and in remaining 34 (26%) was performed radiofrequency ablation (RFA) alone or combined with the resection. In the resection group 37 right hepatectomies, 11 left hepatectomies, 28 segmentectomies and 20 metastasectomies were performed. Disease-free survival (DFS) and overall survival (OS) were statistically analysed using the Kaplan-Meier method. Factors determining DFS and OS were analysed using Cox regression model. Results. In the resection group the 3- and 5-years OS was 64,5% and 46,6% respectively, and the 3- and 5-years DFS was 32% and 30,5% respectively. In the RFA group the 3- and 5-years OS was 33% and 9,5%. Statistically significant prognostic factors in the resection group in uni- and multivariate analysis were: grade and nodal involvement of the primary tumor, diameter of metastatic focus, positive and narrow (<1 mm) resection margins, preoperative fibrinogen level, preoperative neutrophil to lymphocyte ratio and leukocyte amount of the peripheral blood. The perioperative mortality rate was 3%. Conclusions. Liver resection due to colorectal liver metastases is a safe and effective method resulting in high survival rates. We confirmed some generally accepted prognostic factors influencing longterm outcomes and shown the impact of inflammatory response. We also confirmed the hypothesis that preoperative plasma fibrinogen level influences outcomes after liver resection due to CLM.

Opis:
Cyanobacteria (Cyanophyta, Cyanoprocaryota, Cyanobacteria) (blue-green algae) are procaryotic phototrophic microorganisms playing an important ecological role in the freshwater and marine environment as primary producers. However, as a consequence of water eutrophication observed in many reservoirs in different parts of the world, these microorganisms form massive scums, known as water blooms, releasing cyanotoxins hazardous to fish and other aquatic organisms. Cyanotoxins are cyanobacterial secondary metabolites of various chemical structures harmful to humans, terrestial and aquatic animals such as fish. The most abundant cyanotoxins are microcystins and hepatotoxins inducing toxic changes in fish liver, kidney, gills, digestive tract and immune system. Very little is known on the effects of alkaloid neurotoxic anatoxin-a on fish and their immunity. The aim of this study was to assess the in vitro influence of anatoxin-a on immune cells isolated from the common carp (Cyprinus carpio L.). The leukocyte intracellular level of ATP was reduced only at the highest concentration of anatoxin-a. Apoptotic and necrotic leukocytes were observed at the lower and the highest concentrations of anatoxin-a, respectively. Elevated activity of caspases 3/7 after 2 hours and a concentration-dependent decrease in the proliferative ability of T and B lymphocytes was also observed. The results suggest that anatoxin-a could be a possible immunotoxic agent in the aquatic environment and may increase the susceptibility of fish to infectious and neoplastic diseases. Therefore, constant monitoring of anatoxin-a and its producers in lakes and fish ponds should be performed.
