In a lung ultrasound examination, interstitial lung lesions are visible as numerous B-line artifacts, and are best recorded with the use of a convex probe. Interstitial lung lesions may result from many conditions, including cardiogenic pulmonary oedema, non-cardiogenic pulmonary oedema, or interstitial lung disease. Hence difficulties in the differential diagnostics of the above clinical conditions. This article presents cases of patients suffering from interstitial lung lesions discovered in the course of lung ultrasound examination. The patients were examined with a 3.5–5.0 MHz convex probe and a 7.0–11.0 MHz linear probe. Ultrasound images have been analysed, and differences in the imaging with both probes in patients with interstitial lung lesions have been detailed. The use of a linear probe in patients with interstitial lung lesions (discovered with a convex or a micro-convex probe) provides additional information on the source of the origin of the lesions.
Zmiany śródmiąższowe płuc w badaniu ultrasonograficznym prezentują się jako liczne artefakty linii B, rejestrowane najlepiej przy pomocy sondy konweksowej. Przyczyn zmian śródmiąższowych płuc może być wiele: kardiogenny obrzęk płuc, niekardiogenny obrzęk płuc oraz zmiany w przebiegu śródmiąższowych chorób płuc. Powoduje to trudności w diagnostyce różnicowej powyższych stanów klinicznych. W artykule przedstawiono serię przypadków pacjentów, u których w badaniu ultrasonograficznym płuc stwierdzono zmiany śródmiąższowe płuc. Badania wykonano przy pomocy sondy konweksowej (3,5–5,0 MHz) oraz liniowej (7,0–11,0 MHz). Dokonano analizy obrazów ultrasonograficznych oraz wskazano różnice w obrazowaniu obiema głowicami w grupie pacjentów ze zmianami śródmiąższowymi w płucach. Zastosowanie sondy liniowej u pacjentów ze stwierdzonymi (przy pomocy sondy konweksowej lub mikrokonweksowej) zmianami śródmiąższowymi płuc wnosi nowe informacje na temat źródła ich pochodzenia. Opis:
Pulmonary lymphangitic carcinomatosis is a special type of diffuse metastasis of carcinoma in the lymphatic vessels of the lungs. Lymphangitic carcinomatosis is commonly observed in malignancies of the breast, lung, pancreas, colon and cervix as a strong marker for poor prognosis. Presenting with common respiratory symptoms, it may be easily misdiagnosed as other pulmonary interstitial diseases. Pulmonary lymphangitic carcinomatosis is a rare manifestation of metastatic gastric cancer. The presented case describes a patient with pulmonary lymphangitic carcinomatosis in the course of stomach cancer. The primary diagnosis was established based on the exclusion of other interstitial lung diseases, with the use of imaging techniques as well as biochemical, microbiological and cytological findings. The patient’s general condition was very severe, rendering him bedridden and therefore not eligible for any treatment, including chemotherapy. He died suddenly before final verification of the diagnosis. Pulmonary lymphangitic carcinomatosis should be suspected in patients with advanced gastric cancer, presenting with symptoms and signs of respiratory disease. Imaging techniques are mostly helpful to establish the diagnosis.

We present a case of a 70-year-old man with metastatic renal cell carcinoma treated with second-line everolimus after rapid progression on first-line tyrosine kinase inhibitor, in whom a side effect of the therapy occurred in the form of a drug-induced interstitial lung disease. We also provide a review of the literature concerning opinions on incidence, clinical picture, consequences and management of the complication discussed.

Atmospheric pollution suspended in humid air is popularly known as ‘smog’. It is composed of dust particles of different sizes, as well as non-metal oxides, organic compounds, and heavy metals. Exposure to harmful substances suspended in the air – apart from, for example – smoking cigarettes, one of the modifiable factors leading to the development of respiratory diseases. There are six types of substances present in the air that have a negative impact on public health and result in significant consequences: ozone, particulate matter (PM) of different diameters – PM2.5µ, PM2.5–10 µ, PM10 µ, nitrogen dioxide, sulphur dioxide, carbon monoxide and lead. Particular attention is given to small dust particles (PM10 and PM2.5) because they can penetrate into the lower respiratory tract. Apart from describing the composition of smog and sources of air pollution, the article also discusses the impact of atmospheric pollutants on both development and aggravation of the symptoms of such respiratory tract diseases as asthma, chronic obstructive pulmonary disease, respiratory infections and lung cancer. Some of legal measures applied in different countries aimed at reducing exposure to noxious air pollutants are reviewed. The authors believe that the increased focus on risks arising from inhaling toxic air pollution may be a first step for developing systemic solutions aimed at resolving or, at least, decreasing those risks.

Introduction and objective. Occupational asbestos exposure is one of the major public health issues. Although asbestos use in Poland has decreased, asbestosis continues to remain an important health concern in the area of occupational medicine. The aim of the study is to perform a descriptive analysis of hospitalization cases in patients suffering from asbestosis in Poland. Materials and method. The authors used hospital discharge records to conduct a retrospective, population-based study. To estimate the asbestosis hospitalization rate, data from the Nationwide General Hospital Morbidity Study conducted by the National Institute of Public Health were used. Events were defined as in-patient hospital discharges during 2006–2016. 1,101 hospitalization records of 764 patients were included in the study. Results. In the study group of first-time hospitalized patients, the mean and median ages were 68.1 and 69 years, respectively; CI: 67.3–68.9; SD: 11.1, min-max:16–99 years. In this group, significant gender differences were observed (560 males vs. 204 female; P<0.001). No statistical differences were observed regarding the place of residence. During the observation period, 61 in-hospital deaths were reported (8% of all patients), 40 of whom (5.2 % of all patients) took place during the first-time hospitalizations. Asbestosis hospitalizations were mainly clustered in the south-western region, whereas the highest hospitalization rates were clustered in several areas of Poland. Conclusions. The authors believe this study to be the first evaluation of asbestosis in Poland made on the basis of the hospital morbidity database. The data presented may be helpful in comparative studies on the epidemiology of asbestosis across European countries.

Neutrophils are cells of the immune system which freely circulate in blood vessels and are recruited to the inflammation sites when the human organism responds to microbial infections. One of the mechanisms of neutrophil action is the formation of neutrophil extracellular traps (NETs) The process of NET generation, called netosis, is a specific type of cell death, different from necrosis and apoptosis. NETs are formed by neutrophils upon contact with various bacteria or fungi as well as with activated platelets or under the influence of numerous inflammatory stimuli, and this process is associated with dramatic changes in the morphology of the cells. The main components of NETs, DNA and granular antimicrobial proteins, determine their antimicrobial properties. The pathogens trapped in NETs are killed by oxidative and non-oxidative mechanisms. On the other hand, it was also discovered that chromatin and proteases released into the circulatory system during NET formation can regulate procoagulant and prothrombotic factors and take part in clot formation in blood vessels. NETs have also been detected in lungs where they are involved in chronic inflammation processes in ALI/ARDS patients. Moreover, DNA-proteins complexes have been found in the airway fluids of cystic fibrosis patients where they can increase the viscosity of the sputum and have a negative impact on the lung functions. The DNA-complexed granular proteins and other proteins released by neutrophils during netosis lead to autoimmunity syndromes such as systemic lupus erythematosus (SLE), small-vessel vasculitis (SVV) or autoimmune diseases associated with the formation of autoantibodies against chromatin and neutrophil components. A possible involvement of NETs in metastasis is also considered.

Described in this paper are different application of exhaled air analysis. Pulmonary alveolar proteinosis (PAP) is a rarely seen disease of the alveoli, characterized by accumulation of proteinous material, which stains positive with periodic acid Schiff, in the alveoli. Secondary PAP may develop as a result of occupational exposure to materials such as silica and indium. In the paper, together with a review of the relevant literature, we present an uncommon case of a 47-year old male, marble worker who was diagnosed with PAP associated with a 12-year history of exposure to marble dust. The case of preterm, male infant (27 Hbd) with extremely low birth weight (868 g) and Respiratory Distress Syndrom is presented. Despite prolonged artificial ventilation (with High Frequency Oscillation Ventilation and FiO2 =100%), dopamine continuous iv infusion) on 30th day of life he started to deteriorate dramatically and the risk of chronic lung disease appeared. On the 32nd day of life Dexamethasone therapy was started. Post initial therapy his condition began improving clearly. This resulted in a successful extubation on the seventh day of Dexamethasone treatment. Breathing with C-PAP (FiO2=34%, Sa02 = 100%) was started. Dexamethasone was gradually reduced, its total cumulative dose was 4.52 mg/kg. The revision of bibliography related with corticosteroids application in preterm infants with chronic lung disease is presented.
Zaprezentowano przypadek wcześniaka płci męskiej (Hbd 27) z niską masą urodzeniową (868 g), u którego wystąpiły objawy zespołu zaburzeń oddechowych. Stosowano przedłużoną wentylację mechaniczną ( również z zastosowaniem High Frequency Oscillation Ventilation z FiO2100%, ciągłą dożylną infuzją dopaminy), w 30 dniu życia jego stan zaczął się dramatycznie pogarszać i pojawiło się podejrzenie przewlekłej choroby płuc. W 32 dniu życia rozpoczęto stosowanie Dexametazonu. Już po rozpoczęciu leczenia zaobserwowano bardzo istotną poprawę w stanie zdrowia dziecka, co umożliwiło jego ekstubację w 7-mym dniu terapii i zastosowanie CPAP z FiO2 =34% uzyskując SaO2=100%. Dexametazon stopniowo odstawiono a jego całkowita dawka wyniosła 4,52 mg/kg. W pracy przedstawiono przegląd piśmiennictwa dotyczący zastosowania sterydoterapii u wcześniaków z przewlekłą chorobą płuc.
