Background: With estimated prevalence of 1% of the general population, visceral artery aneurysms are not a frequent pathology, however, their rupture is a life-threatening occurrence with mortality rate ranging from 10% to 25%, up to 75% in case of pregnant women. Therefore, the identification and treatment of visceral artery aneurysms is of great importance in the prevention of early rupture. Purpose: To assess the effectiveness and safety of treatment of visceral artery aneurysms with various endovascular methods focusing on technical aspects, complications and long-term clinical outcome. Materials and Methods: 60 patients with visceral artery aneurysms were treated percutaneously. Various techniques were used: coiling (23), covered stents (15), thrombin (7), and other mixed techniques. Aneurysm characteristics, size and localization, procedural data, peri-procedural complications, technical success, 90- and 180-day clinical success and follow-up period (aneurysm exclusion, patency of the artery and complications) were prospectively analyzed. Results: The procedure was performed in 57 of 60 patients. In 56 of 57 patients aneurysms ware effectively excluded. The success rate was 93.3% (56 of 60 enrolled patients). Technical success after the intervention was 82.4%, after second 98.2%. Embolization with covered stents was characterized by the highest efficiency. Serious complications occurred in 1.8% of cases. Conclusions: The minimally invasive endovascular treatment of visceral artery aneurysms is characterized by high efficacy and safety. Good treatment results depend on proper assessment of the aneurysm’s morphology as well as on selection of the appropriate vascular approach and endovascular technique.

Opis:
The aim of the study was to present the first long-term results on the clinical use of compression anastomosis clips (CAC) in upper and lower gastrointestinal tract anastomoses. Material and methods. The study included 50 patients who underwent anastomosis of the upper (n = 32) or lower GI tract (n = 18) with the use of CAC. In the period of 6‑7 months after the surgery, patients underwent endoscopic examination and computed tomography evaluation of the anastomosis. Each anastomosis was evaluated macro and microscopically. The width of anastomoses was evaluated using a 4-point-scale for grading stenosis. Results. Of the 50 patients who underwent anastomosis with compression anastomosis clip, 28 (56%) patients reported to the follow-up examination within 190‑209 days of the execution of the anastomosis. Among the 22 patients who did not report to the study, 18 (36%) patients died within 91‑154 days from the execution of the anastomosis (mean 122 days), 4 (8%) patients were impossible to contact after discharge from hospital. Two mild stenoses (I0) were diagnosed; 1 of them was found in the gastroenterostomy and 1 in Braun enteroenterostomy. Microscopic changes were diagnosed in 4 anastomoses (3 gastroenterostomies, 1 Braun enteroenterostomy). Anastomoses were well-formed and wide, scars in the line of anastomoses were thin. Conclusions. During the period of 6 months after the anastomoses performed using CAC have been formed, they were evaluated as unobstructed and functioning properly; therefore, they can be safely performed within the upper and lower gastrointestinal tract.

Aim: We assessed long-term outcomes of vocal fold augmentation with calcium hydroxylapatyte performed under local anesthesia. Material and methods: We enrolled 17 patients with glottic insuf ficiency due to unilateral laryngeal paralysis or insuf ficiency of internal laryngeal muscles (10 women, 17 men; mean age, 57.6±17.7 years; median age, 61 years; age range, 32-83 years). All patients underwent laryngeal augmentation under local anesthesia, through the oral cavity, with calcium hydroxylapatite (Radiesse) injected laterally to the vocal folds, unilaterally or bilaterally. We assessed voice quality before laryngeal augmentation and at 3 and 12 months. To that end, we performed videolaryngostroboscopy, perceptual assessment of voice, acoustic analyses, and aerodynamic assessments; moreover, participants completed the Voice-Related Quality of Life (VRQoL) questionnaire. Results: Af ter 3 months, we observed a statistically significant improvement on the perceptual assessment of voice with regard to the G and A parameters, and good outcomes were also observed at 12 months. On the acoustic analyses, MDVP_Jitt, MDVP_Shim, and MDVP_NHR improved to 2.5, 5, and 0.1, respectively, at 3 months, and to 1.9, 3.6, and 0.1, respectively, at 12 months. MPTa was prolonged to 12.2s and 11.9s at 3 and 12 months, respectively. Voice intensity improved from 67dB(A) before augmentation to 68dB(A) and 71dB(A) at 3 and 12 months, respectively. VRQoL scores improved to 19.5 and 20 at 3 and 12 months, respectively. Discussion: Laryngeal augmentation with calcium hydroxylapatite performed under local anesthesia is associated with good long-term outcomes.
