The dissection of the internal carotid artery, although rare, is in five to twenty-five percent a causative agent of acute ischemic stroke in patients up to the age of 45. The aim of this paper is to present the case of ischemic stroke within the right hemisphere with left-sided uncomplicated dissection of the internal carotid artery in the case of a young woman. A 35-year-old patient was admitted to the Department of Neurology on an emergency basis due to the sudden motor dysphasia and paresis of the right upper limb. During the anamnesis, the woman reported a head injury suffered two days earlier as a result of fainting. Diagnostic procedures toward ischemic stroke indicated a presence of hypodense area within the left hemisphere of the brain. Subsequent studies – MRI, CT angiography confirmed the existence of an ischemic lesion in the area of the left middle cerebral artery as a consequence of obstruction of the left internal carotid artery. In addition, a dissection of the right internal carotid artery was revealed. Further differential diagnosis, including systemic diseases, did not reveal the reasons for the coexistence of ischemic stroke and vessel dissection on the opposite side. Carotid dissection is a rare but dangerous condition. It either develops spontaneously or after an injury to the head or neck. Any kind of injury of the carotid artery can cause dissection and should lead to include this condition in the differential diagnosis- the more so since even small dissection of the internal carotid artery can be manifested with a delay.

Patients with ischemic brain injury may be affected by damage of brain centers responsible for speech, which is the cause of aphasia. Because the disintegration of linguistic and communication skills makes it difficult or even impossible to communicate with the environment effectively, every patient with aphasia requires speech therapy. In diagnosing aphasia, a qualitative approach is desirable. Presented case proves that the main goal of reeducation of a patient who has lost the ability to communicate with the environment is first to restore this contact in the simplest form, and then gradually rebuild various types of competences and improve implementation, using adequate methods, techniques and tools. All activities, taking into account the possibilitiesand limitations of the patient and adapted to the dynamics of her speech disorder, were aimed at improving the quality of patient’s life and becoming more independent, so that she could successfully participate in social life.

Stroke is the second most common cause of mortality. Ischemic stroke is approximately 10 times more common than haemorrhagic stroke. The strongest risk factor for ischemic stroke is hypertension; thus, reduction of blood pressure decreases the risk of ischemic stroke. However, the prognostic importance of blood pressure after is unclear. The problem is even more complex considering blood pressure variability, i.e. continuous changes of blood pressure values. The aim of this review is to discuss very short-term, short-term, mid-term, and long-term blood pressure variability in the context of clinical outcome in patients after acute ischemic stroke.

Opis:
Pro-inflammatory cytokines participate in the induction of ischemic stroke. So far, their participation in the cerebral ischemia was proven for the tumor necrosis factor TNF-α, interleukin-1 (IL-1), and interleukin-6 (IL-6). The release of the pro-inflammatory cytokines into the extracellular space causes the enlargement of the brain damage region, and consequently increases the neurological deficit and negatively affects the survival rate prognoses. That is confirmed by the increased concentration of pro-inflammatory cytokines in blood and the cerebrospinal fluid of patients with brain stroke, as well as by the research on the induced/experimental cerebral ischemia in animals. The pro-inflammatory cytokines participate in the migration of the reactive T lymphocytes to the regions of brain ischemia where they enhance the nerve tissue damage by down-regulation of microcirculation, induce the pro-thrombotic processes and release other neurotoxic cytokines. Also, in the early stage of cerebral ischemia, cytokines activate the axis hypothalamus-pituitary gland-adrenal cortex and increase the cortisol concentration in blood, what results in the decreased resistance to infectious diseases. Administration of the inhibitor of the interleukin-1 receptor (IL-1Ra) inhibits the inflammatory processes in the region of brain ischemia, and subsequently improves the prognosis for the size of the neurological deficit and the survival rate, as well as resistance to infectious diseases.

Background: Coronary heart disease is one of the most common causes of hospitalization and premature deaths in Europe. ST-segment elevation myocardial infarction (STEMI) has been a clinical problem for many years, particularly in the aspect of choosing the optimal treatment method. The success of treatment is determined by many factors, including risk factors for ischemic heart disease, time between onset of symptoms and initiation of treatment, number and degree of coronary stenosis, and many more. Aim of the study: The aim of the study was to identify risk factors for ischemic heart disease affecting the success of STEMI patients treated with percutaneous coronary intervention (PCI). Material and methods: A retrospective analysis was carried out on data from medical records of patients treated in the Department of Acute Coronary Syndromes of St. Hedvig Provincial Hospital No. 2 in Rzeszow between 2009 and 2014. The research tool used in this paper was the author’s questionnaire. A total of 508 patients with STEMI myocardial infarction treated in the Department of Acute Coronary Syndromes (ACS) between 2009 and 2013 were included in the analysis. The inclusion criteria were the complete and clear files of patient treatment in the ACS department between 2009 and 2013 due to acute coronary syndrome treated invasively by the PCI method. Results: Majority of the study group, 334 subjects, (65.7%) had hypertension. The most common risk factors for ischemic heart disease were found to be dyslipidemia in 176 subjects (34.6%) and smoking in 163 subjects (32.1%). This paper presents the results of the analysis of the success of treatment in relation to risk factors for ischemic heart disease. There was a statistically significant relationship between hypertension and successful treatment (p=0.0425). More cases in which treatment was unsuccessful were observed in the group of patients who had no previous treatment for lipid disorders (20.2% vs. 4.0%) (p = 0.0000). Significantly more cases of treatment failure were found among people who denied smoking (17.4% vs. 8.6%; p = 0.0087). Conclusions: Among the analyzed behavioral and somatic risk factors for failure in patients subjected to treatment were untreated hypertension, hyperlipidemia and a negative history of cigarette smoking.

The mitochondrial ATP-sensitive K+ (mitoKATP) channel was discovered more than a decade ago. Since then, several pharmacological studies have identified agents that target this channel some of which selectively target mitoKATP. These and other studies have also suggested that mitoKATP plays a key role in the process of ischemic preconditioning (IPC) and prevention of apoptosis. The mechanism by which mitoKATP exerts its protective effects is unclear, however, changes in mitochondrial Ca2+ uptake and levels of reactive oxygen species, and mitochondrial matrix swelling are believed to be involved. Despite major advances, several important issues regarding mitoKATP remain unanswered. These questions include, but are not limited to: the molecular structure of mitoKATP, the downstream and upstream mechanisms that leads to IPC and cell death, and the pharmacological profile of the channel. This review attempts to provide an up-to-date overview of the role of mitoKATP in cardioprotection.

Stroke is one of the most devastating neurological conditions, with an approximate worldwide mortality of 5.5 million annually and loss of 44 million disability-adjusted life-years. The etiology of stroke is often unknown; it has been estimated that the etiology and pathophysiology remains unexplained in more than 40% of stroke cases. The conventional stroke risk factors, including hypertension, diabetes mellitus, smoking, and cardiac diseases, do not fully account for the risk of stroke, and stroke victims, especially young subjects, often do not have any of these factors. It is very likely that inflammation, specific genetic predispositions and traditional risk factors interact with each other and may together increase the risk of stroke. Inflammatory and immune responses play important roles in the course of ischemic stroke. Hyperhomocysteinemia (hcy) is considered a modifiable risk factor for stroke, possibly through an atherogenic and prothrombotic mechanism. Both genetic and environmental factors (e.g., dietary intake of folic acid and B vitamins) affect homocysteine level. Identification of the role of hcy as a modifiable risk factor for stroke and of HSPs as regulators of the immune response may lead to more effective prevention and treatment of stroke through dietary and pharmacological intervention. Dietary modification may also include supplementation with novel preventive compounds, such as the antioxidative isoflavones - genistein or daidzein.

Objective. The aim of this study is to evaluate neurological scales, as well as biochemical and radiological parameters measured on day 10 after ischemic stroke (IS), according to their value as predictors of the long-term outcome. Materials and method. 45 patients were assessed according to the Barthel Index (BI) and National Institute of Health Stroke Scale (NIHSS) on day 10, and according to Modified Rankin Scale (mRS) 3 months after the onset of IS. On day 10 of IS, the serum level of C-reactive protein (CRP), albumin, D-dimers (DD), S100BB and Tau proteins was measured and the volume of ischemic focus assessed with the use of Computed Tomography (CT). The patients were divided into groups with good outcome (GO) and mRS 0–2, and with bad outcome (BO) and mRS 3–6. Results. NIHSS and BI scores (p<0.001), the volume of ischemic focus (p<0.01), CRP (p<0.01) and albumin level (p<0.05), but not DD, S100BB and Tau protein levels evaluated on day 10, correlated with mRS after 3 months since IS onset. Patients from the BO group were observed to have lower BI (p=0.001), higher NIHSS (p<0.01) and CRP levels (p<0.05), and bigger volume of ischemic focus (p<0.05) measured on day 10 of IS. In the GO group, there were more patients with atherosclerotic etiology (p=0.02 x2=7.856). Regression analysis showed that only the BI score assessed on day 10 of IS can predict the outcome after 3 months assessed by mRS (OR=1.102, 95%, CI:1.01–1.203; p=0.001). Conclusions. BI assessed on day 10 has a predictive value for the outcome evaluated by mRS 3 months after the onset of IS.

INTRODUCTION: Stroke is a widespread disease in the community. For this reason, its treatment is a major challenge for medical staff. Currently one of the most effective methods of treatment of ischemic stroke is thrombolysis. MATERIALS AND METHODS: The medical records of 91 patients diagnosed with acute ischemic stroke treated with thrombolysis and hospitalized in the Lower Silesian Specialist Hospital in Wroclaw in the years 2007–2013 have been analyzed for the study. Statistical analysis of the material was performed. RESULTS: The analysis of the studied material showed that: ‒ only a small number of patients with ischemic stroke are treated with thrombolysis ‒ the largest group of patients were individuals who at the stage of qualification for the treatment scored 6 points in The National Institutes of Health Stroke Scale, or NIH Stroke Scale (NIHSS) ‒ neurological deficits were resolved in 46% of patients within 24 hours after thrombolytic therapy ‒ an average point value according to the modified Rankin scale assessing the patients' level of disability was 1.08 after completion of hospitalization. CONCLUSIONS: 1. The neurological condition of most patients during thrombolytic treatment improved significantly during the ad-ministration of rt-PA. 2. After the hospitalization process, 6 out of 10 patients demonstrated full self-reliance. 3. Active involvement of the patient and the family in the therapeutic process enhances patient autonomy. 4. There was no record of a worse health condition following the treatment. 5. Thrombolytic therapy is an effective treatment for ischemic stroke.
WSTĘP: Udar mózgu jest szeroko rozpowszechnionym schorzeniem w społeczeństwie, dlatego jego leczenie stanowi poważne wyzwanie dla personelu medycznego. Obecnie jedną z najskuteczniejszych metod leczenia ostrej fazy udaru niedokrwiennego jest tromboliza. MATERIAŁ I METODY: Analizie poddano dokumentację medyczną 91 pacjentów ze zdiagnozowanym udarem niedo-krwiennym mózgu, leczonych z wykorzystaniem trombolizy, hospitalizowanych w Dolnośląskim Szpitalu Specjali-stycznym we Wrocławiu w latach 2007–2013. Materiał poddano analizie statystycznej. WYNIKI: Analiza materiału badawczego wykazała, iż: ‒ tylko niewielki odsetek chorych z udarem niedokrwiennym jest leczony za pomocą trombolizy, ‒ wśród badanych najliczniejszą grupę stanowiły osoby, które na etapie kwalifikacji do leczenia uzyskały 6 pkt w skali NIHSS, ‒ w pierwszej dobie po leczeniu trombolitycznym stwierdzono u 46% pacjentów całkowite wycofanie się deficytów neurologicznych, ‒ po zakończonej hospitalizacji średnia liczba punktów według zmodyfikowanej skali Rankina, oceniająca stopień niesprawność pacjentów, wyniosła 1.08. WNIOSKI: 1. Stan neurologiczny większości pacjentów leczonych trobolitycznie uległ znacznej poprawie w trakcie podawania leku Actylise. 2. Po zakończonej hospitalizacji pełny zakres samodzielności odzyskało 6 na 10 pacjentów. 3. Aktywny udział chorego i rodziny w procesie terapeutycznym zwiększa samodzielność pacjenta. 4. Nie stwierdzono przypadków pogorszenia stanu zdrowia w wyniku leczenia. 5. Leczenie trombolityczne jest skuteczną metodą leczenia udaru niedokrwiennego mózgu.

The mitochondrial ATP-sensitive potassium channel (mKATP) is important in cardioprotection, although the channel remains molecularly undefined. Several studies have demonstrated that mitochondrial complex II inhibitors activate the mKATP, suggesting a potential role for complex II in channel composition or regulation. However, these inhibitors activate mKATP at concentrations which do not affect bulk complex II activity. Using the potent complex II inhibitor Atpenin A5, this relationship was investigated using tight-binding inhibitor theory, to demonstrate that only 0.4 % of total complex II molecules are necessary to activate the mKATP. These results estimate the mKATP content at 15 channels per mitochondrion.

Background: Questions regarding functional markers characterizing injured intestines remain unanswered. Brush border disaccharidases are crucial for the functioning of the intestines. Aims: The study was designed to assess changes in disaccharidase activity (DA) following intestinal injury and to compare them with morphological changes. Methods: Wistar rats, randomly divided into six experimental groups (each n = 6), were subjected to different ischemic/reperfusion injury. One-hour mesenteric ischemia followed by reperfusion for 0, 1, 2, 4, 12 or 24 hours was induced. As a control group sham-operated animals were used (n = 6). Intestine morphology was evaluated using histopathological injury index (HII) and goblet cell (GC) detection. DA (sucrase and maltase) was studied in mucosal scrape or in entire intestinal wall samples. Results: Moderate morphological damage (HII, GC) after mesenteric ischemia was detected. Deepening of the injury was found during reperfusion with a maximum after two hours. Improved morphology with longer reperfusion confirmed reversible damage with almost normal mucosal structure after 24 hours of reperfusion. Similar pattern was observed when DA was measured. The lowest activity was detected after 2 hours of reperfusion followed by increasing activity in the subsequent reperfusion periods. Physiological values after 24 hours of reperfusion were seen only in samples of entire intestinal wall. Conclusions: Significant changes in intestinal DA were observed after intestinal ischemia/reperfusion injury. A similar pattern was seen for morphological characteristics. Although based on microscopic survey the intestine seems to be fairly regenerated, some functional limitation is expected to persist.

Thrombolysis with the use of intravenously administrated alteplase is the only approved thrombolytic treatment of acute ischemic stroke. Tenecteplase is a fibrinolytic protein bioengineered from human tissue plasminogen activator with higher fibrin specificity, enhanced affinity to fibrin-rich clots, faster clot lysis and prolonged plasma half-life. The aim of this study was to determine which thrombolytic therapy (tenecteplase versus alteplase) provides better efficacy and safety outcomes. Eligible studies for meta-analysis published from their inception to May 2018 were identified through a search of PubMed database and two clinical trial registries websites: ClinicalTrials.gov and EU Clinical Trials Register. A meta-analysis was conducted with the use of Statistica software version 13.1. Five studies comprising 1529 patients were included. Tenecteplase 0,25mg/kg administration resulted in significantly higher number of patients with excellent score (0-1) in modified Rankin Scale at 90 days compared to alteplase group (RR 1,30, p=0,0215, 95% CI 1,04–1,62). Additionally, more patients treated with tenecteplase 0,25 mg/kg tended to develop favorable score changes in NIHSS at 24 hours after stroke onset (>8 point improvement, scale quantifying stroke severity) compared to alteplase treatment (RR 1,60, p=0,0545, 95% CI 0,99–2,58). No above correlation were observed in higher dose of tenecteplase group (0,4 mg/kg). There were no significant differences in the frequency of symptomatic intracerebral hemorrhage (ICH) and the mortality rates within 90 days after stroke onset in comparison of tenecteplase and alteplase groups. Tenecteplase 0,25 mg/kg administration in acute ischemic stroke resulted in better functional outcome at 90 days after ischemic stroke onset and tended to restrict the severity of stroke at 24 hours in NIHSS compared to alteplase 0,9 mg/kg. The frequency of symptomatic ICH occurrence and mortality within 3 months between tenecteplase and alteplase were comparable.

Celem badań była ocena wpływu programu rehabilitacji pacjentów po udarze na funkcjonalność, motorykę, zakresy ruchomości oraz wartości siły kończyny górnej, uzupełnionego o ćwiczenia w przestrzeni wirtualnej oraz porównanie osiągniętych wyników z grupą kontrolną. Do oceny stanu funkcjonalnego pacjentów użyto testów Fugl-Mayer Motor Assessment, Frenchy Arm Test oraz skali Lovetta. Dokonano pomiarów zakresu ruchomości w stawach barkowym, promieniowo-nadgarstkowym (płaszczyzny: czołowa i strzałkowa), łokciowym, jak również zmierzono wartości siły chwytów: cylindrycznego, szczypcowego, bocznego i trójpunktowego.
The aim of the study was to assess the impact of post-stroke rehabilitation program on functionality, motor skills, mobility ranges and upper limb strength values, supplemented with VR exercises and comparison of the results obtained with the control group. The Fugl-Mayer Motor Assessment, the Frenchy Arm Test and the Lovett scale were used to assess the functional status of the patients. Measurements of the range of motion in shoulder, radial-wrist joints (planes: frontal and sagittal) and elbow were measured, as well as the values of grip strength: cylindrical, pincer, lateral and three-point.

The aim of the study is to present the problems arising during general anesthesia in patients with cerebrovascular diseases, taking into account the appropriate perioperative preparation. When planning the anesthetic procedure strategies aimed at the prevention of cerebral ischemia or hemorrhage into the brain must be taken into account. Material and research methods: a descriptive analysis was applied on the basis of the literature collected from the years 1992 to 2013. Conclusions: Preoperative evaluation of patients with cerebrovascular disease requires a thorough neurological examination, assessment of cardiovascular complications, including the risk of cerebrovascular, respiratory, renal, and endocrine system complications. During general anesthesia one should take care of haemodynamic stability and proper ventilation parameters. The choice of anesthetic agents so that during the induction of anesthesia, throughout anesthesia, during the recovery from the anesthesia there is no risk of hemodynamic instability and an appropriate level of blood perfusion in the brain is maintained. In the postoperative period the neurological status of the patient should be assessed, in order to ensure that he does not demonstrate symptoms of delirium and that he does not experience a recurrence of previous neurological deficits. Each surgery and anesthesia should be discussed with the operator in terms of the risks and benefits of surgery. Scheduled surgery in patients with acute cerebrovascular incidents need to be postponed until their performance is reasonably safe for the patient.

Objectives Non-adherence and non-compliance to pharmaceutical treatment is one of the most common causes of not effective management of patients suffering from ischemic heart disease (IHD). It is crucial to understand the reasons behind it but studies on this subject performed in the Polish population are still lacking. Material and Methods The 329 patients (160 male and 169 female) diagnosed with IHD who reported for follow-up appointments are examined. The following standardized questionnaires were used: Treatment Satisfaction with Medicines Questionnaire (SATMED-Q) and Adherence to Refills and Medication Scale (ARMS), which evaluates the patient’s compliance and adherence level, respectively. Results Patients with IHD showed moderate compliance with pharmacological recommendations and average satisfaction with treatment. Anemia, drugs side effects, and SATMED-Q total score were significant predictors of the overall ARMS score in the univariate analysis, whereas the male gender and satisfaction with treatment improves this results. In multivariate analysis, significant predictors of lower adherence included family history of IHD, anemia and drugs side effects, while higher education and SATMED-Q overall score increased adherence. Conclusions Treatment satisfaction is a significant predictor of increased overall treatment adherence as well as adherence in terms of drug intake and drug and prescription refills. Raising patient awareness should be an important goal of future educational activities.
